The prevalence of Extended Spectrum β-Lactamase type produced by Clinical Isolates of Escherichia coli from ChungCheong Area (충청 지역에서 분리된 대장균이 생성하는 Extended Spectrum β-Lactamase 유형의 검출 빈도)

  • The study aims primarily to evaluate the resistance of antibiotics and the prevalence of these enzymes among Escherichia coli the most frequent isolate of Enterobacteriaceae producing Extended-Spectrum ${\beta}$-Lactamase(ESBLs), to differentiate the types of enzymes in these isolates. Total 74(26.2%) Strains of producing ESBLs among the 282 E. coli isolates were isolated from hospitals of Chungcheong area (Daejeon, Chungnam, and Chungbuk) during a 6 month-period from February to July, 2013. 282 E. coli isolates including ESBL shown resistance rates of aztreonam 30.8%, Cefotaxim 30.9%, and Ceftazidime 32.2%, 74 isolates producing ESBLs in E. coli were resistant rates to Aztreonam 58.1%, Cefotaxim 100%, and Ceftazidime 63.5% of ${\beta}$-lactam antibiotics. CTX-M-2 (48 isolates) was the most prevalent type of ESBLs identified. Followed the order of frequency by PER-1 (28 isolates), VEB-1 (26 isolates) and CTX-M-8 (20 isolates), of the 74 isolates, 2 isolates only showed GES-1 in Chungnam province. Accurate identification type of ESBLs would aid in hospital infection control. This would give aid to the physician to prescribe more appropriate antibiotics.

Secondary Adrenal Insufficiency Initially Misdiagnosed as Depression : A Case Report (우울증으로 오진되었던 이차성 부신기능저하 : 증례 보고)

  • The abnormalities in Hypothalamic-pituitary-adrenal(HPA) axis are associated with many psychiatric symptoms including depression. We present a report of a 71 year old man who was admitted to the psychiatric department presenting symptoms of headache, avolition, loss of energy, psychomotor retardation, poor appetite, insomnia, anxiety resulting from adrenal insufficiency and hypopituitarism. Hypothyroidism and electrolyte disturbance were managed and headache, insomnia, anxiety, GI symptoms were improved. But he remained in anergic state. After discharge, he was readmitted to infection department with high fever and drowsy mentality. Adrenal insufficiency was recognized and he was treated with corticosteroid replacement therapy. Finally his diagnosis was made as panhypopituitarism and overall symptoms were resolved. In this case, we showed how the atypical symptoms resulting from hypopituitarism develop and progress. Hypothyroidism, adrenal insufficiency, and growth hormone deficiency resulting secondarily from panhypopituitarism were associated with various nonspecific symptoms such as loss of energy, fatigue, insomnia, weight loss, decreased appetite etc. In clinical situation, differential diagnosis with depression is needed when clinicians were met a patient with these nonspecific symptoms. It is important that laboratory tests and differential diagnosis with endocrine diseases should be conducted, especially in geriatric patients with nonspecific symptoms like anergia, fatigue, poor appetite and so on.

Declining Incidence of Nasopharyngeal Carcinoma in Brunei Darussalam: a Three Decade Study (1986-2014)

  • Background: Nasopharyngeal carcinoma (NPC) is linked to Epstein Barr virus infection and is particularly common in the Far East, particularly among some Chinese groups. Certain ethnicities have been reported to have low incidence of NPC. This study looked at NPC in Brunei Darussalam over a three decade period. Materials and Methods: The cancer registry from 1986 to 2014 maintained by the State Laboratory was retrospectively reviewed. The age standardized rates (ASR) and the age specific incidence rates (ASIR) were calculated. Non NPC tumors were excluded from the study. Results: Altogether, there were a total of 450 NPC cases diagnosed accounting for 4.4% of all total cancer cases over the study period, declining from 10.3% in 1986-1990 to 2.3% in 2011-2014. The most common tumor type was the undifferentiated carcinoma (96.4%). The case characteristics were mean age $50.4{\pm}14.4$ years old, male 69%, and predominately Malays 74.4%, followed by Chinese 16.7%. The mean age of diagnosis increased over the study period from $45.6{\pm}17.1$ years (1986-1989) to $54.1{\pm}12.5$ years (ANOVA, p<0.01 for trend). There were no differences in the mean age of diagnosis between the ethnic groups or genders. The ASR showed a declining trend from 11.1 per 100,000 in 1986-1990 to 5.95 per 100,000 in 2011-2014, similar trends been observedfor both genders. Among the age groups, declining trends were seen in all the other age groups apart from the >70 years group. The overall ASRs for the Malays and Chinese were 7.92/100,000 and 8.83/100,000 respectively, both showing declining trends. Conclusions: The incidence of NPC in Brunei Darussalam is comparable to rates reported from Singapore and Malaysia, but higher than rates reported from the other Southeast Asian nations. Unlike higher rates reported for Chinese compared to the Malays in other countries, the rates between the Malays and Chinese in our study was comparable. Importantly, the ASR is declining overall and for both genders and ethnic groups.

Detection of Viral Antigens in Stool Using EIA in Hospitalized Children and Clinical Implication (간접 효소 면역측정법을 이용한 입원 환아의 대변에서 바이러스 항원의 검출과 임상적 의의)

  • Purpose: The purpose of this study is to detect viral coproantigens in children who were hospitalized with acute diarrhea and to compare its association with clinical symptoms. Methods: Seventy-four stool samples were collected from children admitted to Ewha Mokdong Hospital from March 1996 to December 1999. The samples were frozen and analyzed for rotavirus, adenovirus, enterovirus, astrovirus, and calicivirus by enzyme immunoassay (EIA) with monoclonal antibody. 53 stool samples were collected from patients with diarrhea (diarrheal group) and 21 stool samples from patients hospitalized for reasons other than diarrhea (control group). Clinical features and laboratory findings were reviewed in both groups. Results: Among 74 stool samples, virus antigens were detected in 60 samples. Of the 60 virus-positive stool samples, 47 enterovirus, 26 rotavirus, 16 adenovirus, 11 astrovirus, and 11 calicivirus antigens were detected by EIA. Of the 60 virus-positive stool samples, 28 samples have one viral antigen, 30 samples have 2 or more viral antigens, and 2 samples showed a simultaneous infection of Salmonella group B and enterovirus. There was no relationship between the detected virus and clinical features. Conclusion: In this study, viral coproantigen and clinical symptoms were not associated. In the future, further larger scale studies are necessary.

Evaluation of Cumulative and Conditional Antibiotic Release from Vancomycin-Embedded Fibrin Sealant and Its Antibacterial Activity : An In Vitro Study

  • Objective : Fibrin sealants have been used for hemostasis, sealant for cerebrospinal fluid leakage, and adhesive barrier in neurosurgery. Further, as its clinical use and role of an effective drug delivery vehicle have been proposed. This study was performed to measure antibacterial activity and continuous local antibiotic release from different concentrations of vancomycin-impregnated fibrin sealant in vitro. Methods : Antibacterial activity was investigated by disk diffusion test by measuring the diameter of the growth inhibition zone of bacteria (methicillin-resistant Staphylococcus aureus, ATCC29213) from vancomycin-embedded fibrin sealant disc diluted at five different concentrations (C1-C5; 8.33, 4.167, 0.83, 0.083, and 0.0083 mg/disc, respectively). Continuous and conditioned release of vancomycin concentration (for 2 weeks and for 5 days, respectively) were also measured using high-performance liquid chromatography (HPLC) method. To mimic the physiologic wound conditions with in vitro, conditioned vancomycin release in phosphate buffer solution (PBS) was measured and replaced PBS for five consecutive days, half a day or completely daily. Results : In the disk diffusion test, the mean diameters of bacterial inhibition zone were 2.54±0.07 cm, 2.61±0.12 cm, and 2.13±0.15 cm (C1, C2, and C3 respectively) but 1.67±0.06 cm and 1.23±0.15 cm in C4 and C5, respectively. Continuous elution test elicited the peak release of vancomycin from the fibrin sealant at 48 hours, with continued release until 2 weeks. However, conditioned vancomycin release decreased to half or more on day 2, however, the sustainable release was measured over the therapeutic dose (10-20 ㎍/mL) for 5 days and 4 days in assays of half and total exchange of PBS. Conclusion : This study suggests that fibrin sealant can provide an efficient vehicle for antibiotic drug release in a wide range of neurosurgical procedures and the safe and effective therapeutic dose will be at the concentration embedded of 4.167 mg/disc or more of vancomycin.

Immune response and efficacy of pigeon pox virus vaccine and fowl pox virus vaccine in chickens

  • The humoral immune response of chicken vaccinated with fowl and pigeon pox virus vaccines was determined with the protective potentiality of the two vaccines in field condition of Bangladesh. Different aged Fayoumi chicks were subjected for the study. To assess the relationship with better immune response among experimental groups, the average percentage of 'take reaction' was examined and recorded to 97.77% in group A, 93.33% in group B and 100.0% in group C. The level of immune status induced by different vaccinated group was measured by passive hemagglutination (PHA) microplate test method. The mean PHA titer levels after primary vaccination were $33.06{\pm}14.13$ in group A, $32.0{\pm}14.81$ in group B, and $33.0{\pm}13.66$ in group C. Following booster vaccination, the mean PHA titer levels in prior of challenge were increased to $55.46{\pm}14.64$ in groups A and C, and $46.93{\pm}16.52$ in group B. The recorded PHA titer levels of each group at two weeks after challenge were significantly increased to $106.66{\pm}31.22$, $93.86{\pm}33.04$ and $110.93{\pm}29.29$, respectively. The PHA titer levels after vaccination and challenge were significantly increased compared to pre-vaccination titer levels (P<0.01). Although the PHA titer levels among three groups administrated different vaccine combinations in prior of challenge were significantly varied (P<0.01), it was observed that all of the vaccinated chicks were highly protected against challenge infection.

Enhancement of bacterial disease resistance in rockish(Sebastes schlegeli) by $\beta$-glucan administration ($\beta$-Glucan 투여에 의한 조피볼락(Sebastes schlegeli)의 세균성 질병에 대한 저항성 향상)

  • The effect of $\beta$-glucan as an immunostimulant to increase resistance to bacterial diseases by enhancing non-specific defense mechanism in rockfish (Sebastes schlegeli) was examined by oral and bath administration. After oral or bath administration with $\beta$-glucan, the injection challenges with Vibro ordalii, Staphylococcus epidermidis and Edwardsiella tarda were performed to assess $\beta$-glucan efficacy. After injection of V. ordalii, oral administration for 30 days with 1% $\beta$-glucan showed 25% of survival rate. But all control fish died within 3 days after the injection. After injection of S, epidermidis, oral administration group for 20 and 30 days showed a remarkably increased survival rate of 95%. But oral administration of $\beta$-glucan to rockfish did not induce protection against experimental E. tarda infection. $\beta$-Glucan bath administration with or without formalin-killed V. ordalii showed that no protection was observed at 10 days after challenge. The results show that $\beta$-glucan to rockfish was effective to increase survival rate of bacterial infections of S. epidermidis and V. ordalii but not against E. tarda.

The Comparative Study of Clinical Manifestations in Acute Poststreptococcal Glomerulonephritis with Gross Hematuria or Microscopic Hematuria (연쇄상구균 감염후 급성 사구체신염에서 육안적 혈뇨와 현미경적 혈뇨에 따른 임상양상의 비교)

  • Purpose: This study aimed to compare the clinical outcome of acute poststreptocaccal glomerulonephritis (APSGN) between patients who had presented with gross hematuria and those with microscopic hematuria. Methods: Thirty-nine patients with acute poststreptococcal glomerulonephritis, who were diagnosed from January 2000 to April 2003 were enrolled. Results: The mean age was 8.85 years and the male to female ratio was 1.1:1 Seventeen patients presented with gross hematuria at diagnosis(group A) and twenty-two patients had microscopic hematuria only(group B). There were no significant differences between the two groups in the incidence of edema, fever or history of respiratory infection and oliguria. But hypertension was more frequent in group B. Laboratory data showed decreased C3 and C4 level in group B. Spot urine protein/creatinine ratio and 24hr urine protein showed higher levels in group A. Conclusion; Patients with gross hematuria at diagnosis had lower incidence of hypertension and a higher rate of nephrotic range of proteinuna than patients with microscopic hematuria. However, no difference in the duration of admission or complication rate was observed. All patients had clinical improvement during follow-up. We conclude that gross hematuria is not a significant prognostic factor for poststreptococcal glomerulonephritis.

Effective High-Throughput Blood Pooling Strategy before DNA Extraction for Detection of Malaria in Low-Transmission Settings

  • In the era of (pre) elimination setting, the prevalence of malaria has been decreasing in most of the previously endemic areas. Therefore, effective cost- and time-saving validated pooling strategy is needed for detection of malaria in low transmission settings. In this study, optimal pooling numbers and lowest detection limit were assessed using known density samples prepared systematically, followed by genomic DNA extraction and nested PCR. Pooling strategy that composed of 10 samples in 1 pool, $20{\mu}l$ in 1 sample, was optimal, and the parasite density as low as $2p/{\mu}l$ for both falciparum and vivax infection was enough for detection of malaria. This pooling method showed effectiveness for handling of a huge number of samples in low transmission settings (<9% positive rate). The results indicated that pooling of the blood samples before DNA extraction followed by usual nested PCR is useful and effective for detection of malaria in screening of hidden cases in low-transmission settings.

A Clinical Study of Aseptic Meningitis from 1996 to 1998 (입원 치료한 무균성 뇌막염 환아의 임상적 검토 : 1996년~1998년)

  • Purpose : There was an epidemic of aseptic meningitis in Korea for 3 years from 1996 to 1998. In this study, we reported the time of outbreak, age and sex distribution, clinical manifestations, laboratory data, and complications of the children with aseptic meningitis. Methods : We reviewed clinical records of 192 aseptic meningitis patients who had been admitted to Pohang St. Mary's Hospital from 1996 to 1998. Results : Aseptic meningitis occurred most frequently in July and August. The ratio of male to female was 1.91 : 1, and the most prevalent age was 1 to 5 years of age. The main symptoms were fever, vomiting, headache in this order of frequency. Peripheral blood showed leukocytosis(WBC > $10,000/mm^3$) in 44.3% of the cases. CSF examination showed WBC count were less than $500/mm^3$ in 88.0%, protein levels less than 40 mg/dL in 61.5%, and glucose levels more than 40 mg/dL in 94.3% of the cases. We did not isolate the causative viral agents. Conclusion : Aseptic meningitis was prevalent in children in the Pohang area from 1996 to 1998. Clinical manifestations and CSF profiles of aseptic meningitis patients showed no great difference compared to other reported.
