고관절 신전시 요통환자와 정상인의 슬괵극, 대둔근, 요추기립근의 근 수축 개시시간 비교 (The Comparison of the Onset Time of Hamstring, Gluteus Maximus, and Lumbar Erector Spinae Muscle Activity During Hip Extension Between Subjects With Low Back Pain and Healthy Subjects)

  • The purpose of this study was to compare the onset times of muscle activities and the order of muscle firing in hamstring gluteus maximus, and lumbar erector spinae muscle during active hip extension between subjects with low back pain (LBP) and healthy subjects. Thirty subjects, 15 with LBP and 15 healthy subjects, participated in this study. Electromyographic activity was recorded during active hip extension in prone and standing position. Relative onset times of these muscle activities were determined. Similar muscle firing order in hamstring, gluteus maximus, and lumbar erector spinae muscle showed in both groups and positions. However, the onset time of gluteus maximus was significantly later in prone and standing active hip extension in subjects with LBP than in healthy subjects. The onset time of lumber erector spinae muscle activity was significantly delayed in subjects with LBP in standing active hip extension, The delayed onset times of gluteus maximus and lumbar erector spinae muscles' activities were probably related to LBP. Further studies are needed to identify whether the delayed onset times of gluteus maximus and lumbar erector spinae muscle activities are the contributing factors to LBP.

만성요통환자의 반복체간운동 시 요부신전근 피로 분석 (A Fatigue Analysis on Lumbar Extension Muscle during Repeated Trunk Extention Exercise for Chronic Lumbar Pain)

  • The purpose of this study were analysed fatigue character of lumbar extension Muscle during repeated trunk flexion-extension motion we used EMG multi-telemeter (WEB 5000, Nihon Koden, Japan), Medex lumbar extension machine(Ocala, USA). We evaluated 20 persons on chronic low back pain group and 20 persons on control group. We analysed the quantitative variables IMF, MF, decreased ratios of MF to investigate fatigue. As a result, the following conclusion was drawn : 1. Each lumbar IMF of CLBP was L5> L3> L1 in the order of their size. CON group was similar quantitative and same order. But the relationship of two groups were not significantly. 2 The IMF within lumbar position(L1-L3, L1-L5, L3-L5) of two groups was significantly all positions(p<.05). 3. The MF shifting at lumbar position of two groups during repeated trunk flexion-extension motion was decrease shifting pattern with increase repeating motion, especially CLBP group was suddenly dropping than CON at all lumbar positions(L1, L3, L5) on early stage motion(34 set). 4. The lumbar fatigue of CLBP was higher than CON during repeated trunk flexion-extention motion, so CLBP supposed more tired than CON. And it was significantly within two groups at lumbar positions(L1, L3 : p<.05, L5 : P<.01).

Effects of Posterior-Anterior Mobilization of Lumbar Spine on Muscle Tone and Stiffness of Superficial Back Muscles and Lumbar Mobility

  • Background: Previous researchers have investigated the mechanical and neurophysiological effects of manual mobilization, however little research has been done on muscle tone and muscle stiffness. Objective: To compare the effects of posterior-anterior (PA) mobilization with weight bearing on sling and conventional PA mobilization on the bed. Design: Randomized controlled trial (single blind) Methods: The subjects were 16 male university students and randomized to sling mobilization group (SMG, n=8) or conventional mobilization group (CMG, n=8). SMG received PA mobilization using a sling and CMG received traditional mobilization on the bed during lumbar mobilization. Results: Both left and right muscle tones of SMG increased, but left muscle tone of SMG were increased and right muscle tone was decreased after intervention. In addition, both left and right muscle stiffness of SMG were also increased, however left muscle stiffness of SMG was increased and right muscle stiffness was decreased. The muscle tone and muscle stiffness of SMG were higher than those of DMG, especially the right side was statistically significantly higher. Extension of SMG, extension and flexion of CMG were increased statistically significantly except for Flexion of SMG (p<.05). There were no significant differences between the groups in Extension and Flexion. Conclusions: This study suggests that lumbar spine PA mobilization using sling is beneficial in improving muscle tone, muscle stiffness, and trunk movement.

요부신전운동에 의한 효과에 관한 연구 (A Study on Effect by Lumbar Extension Exercise)

  • The purpose of this study is to improve the effect of Lumbar extensor exercise program and develop the Lumbar extensor exercise program which will be suitable to the patients characteristics. In this study the experimental group was made up of 38 subjects. They are the patients with low back pain using the Lumbar extension exercise program in C hospital. The lumbar extension exercise program was given two times a week for 8 weeks. The results were estimated by Lumbar extensor strength by diagnosis agent. The results were measured three times, one time pre-treatment and two times post-treatment at 4weeks and 8weeks by lumbar extensor curve angle(0, 12, 24, 36, 48, 60, 72). The results were compared at pre-test 4weeks and 8weeks. The muscle strength measured at both 4weeks and 8weeks with the greater strength was shown at 4weeks. The muscle strength of patients with M.strain, Laminectomy and HNP was increased at all angles except for patients with stenosis. The results of this study indicated that diagnosis influenced the muscle strength in Lumbar extensor exercise program.

만성요통환자와 정상인의 체간 운동시 요추 신전근의 근활성도와 관절운동범위의 변화 (The Changes of Range of Motion of Lumbar Region and Muscle Activities of Lumbar Extensor During Trunk Motions Between Subjects With Low Back Pain and Healthy Subjects)

  • The purpose of this study was to compare the electromyography (EMG) activities of the lumbar extensor muscles during motion of trunk flexion-extension and compare range of motion (ROM) with a 3-dimensional motion analysis system of the lumbar region between subjects with chronic low back pain (CLBP) and healthy subjects during the trunk flexion-extension, trunk rotation and trunk lateral flexion cycle. Thirty CLBP subjects and thirty healthy subjects were included. We measured the root mean square (RMS) value of the lumbar extensor muscles from resting, standing, lumbar flexion and return position. The RMS ratio was normalized from maximal EMG activity of the lumbar extensor muscles during trunk motion. The results of this study showed that the RMS ratio of the lumbar extensor was significantly higher in CLBP subjects than healthy subjects during all of trunk motion (p<.05). The ratio of the highest RMS value during flexion and extension was higher in CLBP subjects than in healthy subjects (p<.05). The ROM of the lumbar region was significantly lower in CLBP subjects than healthy subjects during trunk flexion-extension, trunk rotation and lateral flexion cycle. The relationship between the RMS ratio for full lumbar flexion and the ROM of lumbar flexion was not correlated significantly. CLBP subjects have both decreased ROM of the lumbar region and higher muscle activities of the lumbar extensor muscle than healthy subjects.

The Effect of Proprioceptive Position Sense by Lumbar Flexors and Extensors

  • Purpose: Muscle fatigue affects proprioception, and it causes problems in spinal stability. The purpose of this study was to examine the effect on the accuracy of reproducing the lumbar angles before lumbar exercise and after fatiguing isokinetic lumbar exercise. Methods: Thirty healthy adults participated in this study. Before induction of fatigue by exercise, the proprioception was measured by Biodex. Lumbar positions were passively maintained on stimulation position ($25^{\circ}$ flexion and $25^{\circ}$ extension), and back to the starting position. Subjects actively repositioned the remembered stimulation position, and error degrees between the stimulation position and reposition were measured. Using an isokinetic device at $120^{\circ}$/sec of velocity of angle lumbar flexion/extension exercise resulted in muscle fatigue. The post-fatigue proprioceptive position sense was used in the same way as in pre-fatigue measurement. Results: Means of position sense of pre-fatigue were $2.19{\pm}1.97$ on flexion angle, and $5.04{\pm}2.84$ on extension angle. After exercise induced fatigue, means of position sense were $2.37{\pm}1.83$ on flexion angle, and $4.93{\pm}2.57$ on extension angle. Results of this study showed significant differences of lumbar proprioceptive position sense between pre- and post-fatigue. Conclusion: Lumbar proprioception sense in active repositioning in flexion and extension was affected in the presence of muscle fatigue. Therefore, it should be noted that therapeutic exercise for patients with abnormal proprioceptive sense or elderly people must be performed with care because muscle fatigue can cause secondary damage.

만성 요통환자의 요추부 신전강화에 의한 효과 (The Effect on Chronic Low Back Pain Patient Through Lumbar Extension Muscular Strength Training)

  • The number of the subjects of this study were all sixteen including 8 male and 8 female. They were chronic low back pain patients and treated at hospital out patient. The patients took lumbar extension strength training. The results of examination and analysis isometric lumbar extension muscular strength before and after the training are as follows; 1. TFT increased after lumbar extension muscular strength exercise than before the exercise. Inspire of difference of each angle. The reciprocal action didn't happen between training and angle. 2. NMT increased after lumbar extension muscular strength exercise than the before the exercise and the difference of each angle didn't happen. The reciprocal action also didn't happen between the training and angle. 3. Comparing presented standard point with individual point, 13persons' muscle strength of first step increased, on the other and 3 person's muscle strength a little increased in a same category. 4. The grade of low back pain decreased for all subjects. Concluding this study, lumbar extension muscular strength exercise through isokinetic device decreases the grade of low back pain and increase lumbar extension muscular strength. It is that equal exercise effect happen in full range of motion, for the exercise effect doesn't show the difference of each angle.

Effects of Lumbar Stabilization Exercise with Kinesio Taping on Pain, Muscle Strength, and Oswestry Disability Index in Patients with Chronic Low Back Pain

  • Objective: The purpose of this study was to examine the effects lumbar stabilization exercise with kinesio taping on pain, muscle strength, and oswestry disability index (ODI) in patients with chronic low back pain. Design: Two groups pre-post randomized controlled design Methods: Thirty-two subjects were randomly divided in two groups; 1) lumbar stabilization exercise with kinesio taping group (Experimental group, n=16), 2) lumbar stabilization exercise with sham kinesio taping group (Control group, n=16). The intervention was conducted in each group for thirty minutes a day, 5 times a week, for 4 weeks. Both group did 30 minutes of lumbar stabilization application. Evaluations were performed before the commencement of training and again 4 weeks after training was initiated. Visual analog scale (VAS) was used to evaluate pain level of patients with chronic low back pain. Distal muscle test was used to evaluate muscle strength of trunk extension. In addition, ODI was used to evaluate activity daily life of low back pain. Results: After training, the VAS, muscle strength of trunk extension and ODI were significantly more improvement in Experimental group than in the control group (p<0.05). Conclusions: We confirmed that the effects of lumbar stabilization exercise with kinesio taping group on pain, muscle strength, and ODI in patients with chronic low back pain.

요부안정화 운동이 정상인의 요부 신전력 증진에 미치는 영향 (The Effects of Lumbar Stabilizing Exercise on Increasing of the Strength of Lumbar Extensor Muscle in Normal Subjects)

  • The purpose of this study was to find the effects of the lumbar stabilizing exercise on the isometric lumbar extension strength and the range of motion of lumbar extension. 16 healthy subjects were recruited (9 males, 7 females, mean age 25${\pm}$3.2). They performed 5 types of exercises 5 times per week for 4 weeks. We evaluated isometric trunk extension strength (0, 12, 24, 36, 48, 60, $72^{\circ}$) by MedX(Ocala, FL) and measured range of motion of lumbar extension. All measurements were measured at pre-exercise and 4 weeks post-exercise. The results of this study were summarized as follows; After 4 weeks of exercise, isometric lumbar extension strength was generally increased but there were only significant differences at $12^{\circ},\;24^{\circ},\;48^{\circ}$. The range of motion of lumbar extension was increased (p<.05). The isometric lumbar extension strength correlated with the range of motion of lumbar extension. In conclusion, our results have shown that lumbar stabilizing exercise influenced both lumbar extension strength and range of motion of lumbar extension.

요추가동술과 요부안정화운동이 요부근력에 미치는 효과 (The Effects of Lumbar Vertebrae Mobilization and Lumbar Stabilizing Exercise on Lumbar Muscle Strength)

  • Purpose: The purpose of this study was to evaluate effects of lumbar vertebrae mobilization and lumbar stabilizing exercises on the muscle strength. Methods: The subject were sixty healthy adult(30 females. 30 males) two decade from 21 to 35. All subjects randomly assigned the control group, lumbar vertebrae mobilization group, lumbar stabilizing exercises group. lumbar stabilizing exercises group received lumbar stabilizing exercises for 30 minutes, lumbar vertbrae mobilization group received lumbar vertbrae mobilization for 4-5 minutes per day and three times a week during 3 weeks period. BIODEX was used to measure muscle strength. All measurements of each subjects were measured at pre-experiment, after 10 day, and post-experiment. Results: The results of this study were summarized as follows; 1. The flexion strength test of control group, lumbar vertebrae mobilization group, lumbar stabilizing exercises group were no significantly differences at pre-experiment and after 10 days(p> .05), however significantly increased post-experiment(p <.05). The result of analyzed effects of flexion strength test was significantly increased accordining to experiment type(p <.05). 2. The extension strength test of control group, lumbar vertebrae mobilization group, lumbar stabilizing exercises group were no significantly differences at pre-experiment and after 10 days(p> .05), however significantly increased post-experiment(p <.05). The result of analyzed effects of extension strength test was significantly increased accordining to experiment type(p <.05). Conclusion: conclusionally these data suggest that a 3-week lumbar stabilizing exercises improved muscle strength. Additional randomized controlled trials to more fully investigate trement effects and factors that may mediate these effect are needed.
