Effectiveness of a Clinical Pathway for Breast Cancer Patients Undergoing Surgical Operation on Clinical Outcomes and Costs

  • Purpose: This study aimed to evaluate the impact of implementing a clinical pathways (CPs) on the clinical outcomes and costs of patients undergoing breast cancer surgery. Methods: This retrospective cohort study included patients who were newly diagnosed with primary breast cancer at the Samsung Medical Center between 2014 and 2019 (N=8482; 2931 patients in the pre-path and 5551 patients in the post-path). Clinical outcomes included reoperation during hospitalization, readmission, and emergency room visits within 30 days of discharge. The cost data for each unit were obtained from an activity-based management accounting system. We performed an interrupted time series analysis. Results: The post-path period showed a significantly shorter hospital length of stay (LOS) than the pre-path period (6.3 days in pre-path vs. 5.0 days in post-path; -1.3 days' difference; p=.001), and fewer reoperations during hospitalization and within 30 days after discharge than the pre-path period. After adjusting for inflation rates and relative value scores, the model demonstrated savings of $146 per patient in the post-path for total costs, and $537 per patient for patient out-of-pocket costs (p=.001). Conclusion: CPs can help reduce costs without compromising the quality of care by reducing the number of reoperations, readmissions, and complications.

  • Background: Assessment of the clinical outcomes after rotator cuff repair is essential for their effectiveness on treatment. The Korean Shoulder and Elbow Society devised the Korean Shoulder Scoring System (KSS) for patients with rotator cuff disorder. The purpose of this study was to evaluate the availability of the KSS for assessment of clinical outcomes in patients after arthroscopic rotator cuff repair, and for comparison with other appraisal scoring systems. Methods: A total of 130 patients with partial-thickness or full-thickness rotator cuff tear who underwent arthroscopic repair using a single row or double row suture bridge technique were enrolled. The average follow-up period was 25.9 months. All patients were classified according to various factors. Comparison within corresponding categories was performed, and the correlation between the KSS and other shoulder assessment methods including University of California Los Angeles (UCLA), Constant and American Shoulder and Elbow Surgeons (ASES) score was analyzed. Results: Total score of the KSS response had increased from 59.6 preoperatively to 88.96 at last follow-up. All KSS domains, including function, pain, satisfaction, range of motion, and muscle power had improved up to 24 months postoperatively. Statistical significance was observed mainly in preoperative measurements with number and size of torn tendons, and greater than or equal to grade 3 of fatty infiltration. The KSS was best correlated with the UCLA scoring system in both preoperative (r=0.785) and postoperative (r=0.951) measurements. Conclusions: The KSS was highly reliable and valid as a discriminative instrument, and it showed strong correlation with ASES and UCLA scoring systems.

  • We proposed the objective ROC analysis method based on the setting of threshold value for evaluation of single photon emission computed tomography (SPECT) image. This proposed ROC analysis method uses the quantification computational threshold value to each signal on the SPECT image. The SPECT images for this study were acquired by using Monte Carlo n-particle extended simulation code (MCNPX, Ver. 2.6.0, Los Alamos National Laboratory, USA). The basic SPECT detectors and specific water phantom were realized in the simulation, and we could get the simulation results by the simulation operation. We tried to analyze the reconstructed images using threshold value application based objective ROC method. We can get the accuracy information of reconstructed region in the image. This proposed ROC technique can be helpful when we have to evaluate the weak signal for the NM image. In this study, the proposed threshold value based computational ROC analysis method can provide better objectivity than the conventional ROC analysis method.

  • For decades, many traffic flow studies on the analysis and determination of level of service (LOS) for the weaving sections have been made to Provide several regression equations. Weaving and non-weaving speeds were dependent variables for the equations, with independent variables being weaving length, number of lanes, and weaving ratios. One of the difficulties in developing the equations was that the weaving areas were rare in Korea, so the statistical analyses for calibrating the equation parameter could not be performed in a desirable manner. In this regard, a new and stochastic methodology for predicting the weaving and non-weaving speeds within the weaving sections was required. In this study the following design variables were developed; influence area of the weaving section. headway distribution within the weaving section, maximum weaving volume of the weaving section, length of the ideal weaving section, and speed estimations for the weaving and non-weaving flows. The evaluation of the new model was made comparing the delay in the weaving section with the one in the freeway basic section.

Evaluation of Antioxidant Effects of Vitamins C and E Alone and in Combination with Sorghum Bran in a Cooked and Stored Chicken Sausage

  • To evaluate the antioxidant effects of vitamin C, vitamin E and sorghum bran, alone or in combination on chicken sausages, 9 kg of chicken thigh meat was prepared. All thigh meat was divided into seven different batches as follows; no antioxidant (CON); vitamin C (VTC), vitamin E (VTE) or sorghum bran (SOR) at 0.02%; or three different combination ratios of vitamin C, vitamin E and sorghum bran at 0.02% (VT2, 2:1:1; VT4, 4:1:1; VT6, 6:1:1). All cooked sausages were stored at $4^{\circ}C$, and six sausages per treatment were used for chemical analysis on five different storage days. As the addition of vitamin E was increased, sausages stored for 10 d had decreased redness; thereby, VTE showed the lowest CIE $a^*$ (p<0.05). Sausages mixed with vitamins and sorghum bran combinations had lower peroxide and free fatty acid values (p<0.05) when compared to sausages without antioxidants. The TBARS were the lowest in sausages containing vitamin C, vitamin E and sorghum bran at 6:1:1 ratio, and they significantly differed to CON, VTC and SOR treatments (p<0.05). Therefore, our results suggest that meat mixed with vitamins and sorghum bran had more antioxidant activity than the meat mixed with only antioxidant vitamins or without antioxidants.

  • Mobile IP provides hand-held devices with mobility which allows the user to do work over the network. However, handover time due transfer between access routers causes network delays and data loss. L2Trigger Handover expects this handover to take place, and executes L3 handover before L2 handover takes place, thereby reducing overall handover latency, although it still is an issue since handover latency between AR is not completely eliminated in L2 trigger handover. In this paper took into consideration where MIH is used in MIPv4 and using MIH Table when handover is about to occur in MN(Mobile Node), thereby pre-fetching data needed by Handover. In this way, when the handover is estimated, it improves the init time that L2trigger had. Furthermore we can find that we can execute the handover with shorten init time in smaller and narrow overlap length

  • This study develops composite indicators that is capable of estimating QOS (Quality of Service) of pedestrians by enhancing estimation for Level of Service in present transporting spaces for pedestrians that merely concerns quantitative indicators such as pedestrian flow rate, pedestrian stoppage. This paper conducts survey questionnaire to collect data. It utilizes AHP(Analytic Hierarchy Process) and ANP(Analytic Network Process). The main results show that: first, ANP is better analytic methods than AHP due to the fact that pedestrians feel repetition when they judge level of service. Second, qualitative factors such as pedestrian behaviors, maintenance, scenary for pedestrians and environments should be considered. Importance of qualitative factors is higher in residential area than other areas. I expect that using qualitative indicators and quantitative indicators is appropriate in order to estimate pedestrians' QOS.

Clinical and Structural Outcomes of Arthroscopic Intraarticular Knotless Fixation for Upper Subscapularis Tendon Tears: A Preliminary Report

  • Background: A novel technique for the repair of tears of the upper subscapularis tendon-intraarticular knotless fixation-has been introduced recently. The purpose of this study was to evaluate the clinical and structural outcomes of arthroscopic intraarticular knotless fixation for the treatment of upper subscapularis tendon tears. Methods: We retrospectively analyzed the clinical and radiological outcomes of 27 patients who underwent arthroscopic intraarticular knotless fixation for upper subscapularis tendon tears. Finally, a total of 10 patients who could participate in at least a 6 month follow-up of magnetic resonance imaging evaluation and in a least 1-year follow-up on an outpatient basis were enrolled in our study. The mean age at the time of operation was 60.7 years, and the mean duration of follow-up was 14.7 months. Two patients had concomitant tears of the supraspinatus tendon and 8 patients had concomitant tears of the supraspinatus and the infraspinatus tendons. Results: The clinical and radiological outcomes improved after the patients had undertaken arthroscopic intraarticular knotless fixation. The mean visual analogue scale score for pain during motion improved from 6.7 preoperatively to 1.4 at the final follow-up (p<0.001). The mean Constant score improved from 59.3 preoperatively to 79.6 at the final follow-up, and the mean the University of California at Los Angeles score, from 21.7 to 30.2, respectively (p<0.001 and p<0.001). The upper subscapularis tendon tear was healed in every patient except one (90%), for the patient had suffered from a postoperative trauma that resulted in a retear. Conclusions: We found that arthroscopic intraarticular knotless fixation gives good clinical and structural outcomes for the repair of upper subscapularis tendon tears. Arthroscopic intraarticular knotless fixation provided such a reliable and efficient restoration of the subscapularis tendon footprint that we anticipate it will become a widely-used procedure for upper subscapularis tendon tears.

A Study on the Performance Evaluation of Polarization Diversity in Heavy Density Urban (고밀집 도심에서의 편파 다이버시티 성능 분석 연구)

  • In this paper, we analyzed the propagation characteristics of polarization diversity in the view of diversity gain, cross correlation coefficient and average received signal strength, and compared it with those of space diversity. From the results, we could confirm that in the care of line of sight(LOS), space diversity gain is bigger than polarization diversity gain by 2 dB and the diversity gain of mobile transmitting with 90 degree is bigger than that of mobile transmitting with 45 degree by 1 dB. However, in the area of non-line of sight(NLOS), the diversity gains were all most the same in two diversity schemes and in specular cases, polarization diversity showed better performances. Also it was verified that under the NLOS conditions, diversity gain of mobile transmitting with 45 degree was at least 1 dB bigger than that of mobile transmitting with 90 degree.

Location Information Reliability-Based Precision Locating System Using NLOS Condition Estimation (NLOS 상태 추정을 이용한 위치 정보 신뢰성 기반의 정밀 위치 측정 시스템)

  • Recently, mobile devices were increased and there was a sharp rise in demand. To exploit the location information of each device, many researcher was studying locating systems. The favorite locating or positioning systems were a GPS using satellites and a RTLS using wireless communication between devices. If some obstacle existed nearby the target device, The system have difference of performance. The obstacles near targets were caused signal disconnection and reflection because of NLOS condition. As the result, the NLOS condition degrade the locating performance. In this paper, we propose a locating system which is cooperated two systems using information reliability estimates from LOS/NLOS condition. We developed proposed system. In addition, we performed fields test and simulation tests at various environment for performance evaluation. As the result, the test showed 97% success rate to estimate NLOS condition. Furthermore, the simulation result of our locating system was increased to 89% compared with a single system.
