  • Purpose : Insulin resistance is the most important risk factor linked to the development of impaired glucose tolerance(IGT), diabetes mellitus and cardiovascular diseases in childhood and adolescent obesity, The purpose of this study was to see whether insulin resistance of obese adolescent is higher than that of obese children. and to analyze gender difference and affecting factors of insulin resistance. Methods : Of the 9,837 school children from 5 to 16 tears old, 92 obese children and 187 adolescent, underwent a two-hour oral glucose tolerance test and plasma glucose, insulin, lipid profiles, leptin and high sensitive C-reactive protein(hs-CRP) were measure. Results : Plasma insulin levels of female were higher compared to those of males during oral glucose tolerance test(P<0.05). Four(4.3%) in obese children and twenty five(13.3%) in obese adolescents met the criteria of IGT. Female, leptin, adiponectin and triglyceride concentrations were strongly correlated with homeostatic model assessment insulin-resistance(HOMA-IR) by multiple linear regression analysis(P<0.05). Conclusion : Obese adolescents might have higher insulin concentrations compared to obese children and obese girls higher insulin concentrations than obese boys. Obese boys and children with impaired glucose tolerance have higher insulin concentrations than those with normal glucose tolerance. HOMA-IR was significantly correlated with female, plasma leptin, adiponectin and triglyceride concentrations.

Methods 40 Male Sprague-Dawley rats were fed on high fat diet for 8 weeks and 32 rats (above 400 g) were randomly divided into 4 groups (8 mice in each group) : control group, 100 mg/Kg CGHDH group, 200 mg/Kg CGHDH group, 300 mg/Kg CGHDH group. We fed a control group of rats a basal diet and administered normal saline(100 mg/kg, 1 time/1 day) for 4 weeks. And We fed each experimental group of rats basal diet and administered an extract of Cheunggihwadamhwan extracts (100 mg/kg, 200 mg/kg, 300 mg/kg, 1 time/1 day) for 4 weeks. At the end of the experiment, the rats were sacrificed to determine their chemical composition. We measured lipid in plasma and liver, concentration of proinflammatory cytokines, antioxidative activity and gene expression. The gene expression level was investigated by the way of reverse transcription-polymerase chain reaction (RT-PCR). Results 1. Concentration of plasma FFA, plasma TG, plasma total cholesterol and plasma LDL-cholesterol showed a significant decrement in Cheunggihwadamhwan groups. However, concentration of plasma HDL-cholesterol showed a significant increment in 200, 300 mg/kg Cheunggihwadamhwan groups. 2. Concentration of liver total cholesterol and liver TG showed a significant decrement in Cheunggihwadamhwan groups. 3. Concentration of plasma TBARS showed a significant decrement in all Cheunggihwadamhwan groups. Concentration of liver TBARS showed a significant decrement in 200, 300 mg/kg Cheunggihwadamhwan groups. Concentration of liver GSH-Px, SOD and CAT showed a tendency to decrease in all Cheunggihwadamhwan groups. 4. Concentration of plasma IL-$1{\beta}$, plasma IL-6, TNF-$\alpha$ and NO, showed a tendency to decrease in all Cheunggihwadamhwan groups. Concentration of plasma IL-10 showed a tendency to increase in all Cheunggihwadamhwan groups. 5. In the analysis of reverse transcription-polymerase chain reaction (RT-PCR), the gene expression of Apo-B and Apo-E in the Cheunggihwadamhwan groups showed a low expression than that of control group. The ratio of Apo-B expression per $\beta$-actin expression in the showed a significant decrement in all Cheunggihwadamhwan groups. The ratio of Apo-E expression per $\beta$-actin expression in the showed a significant decrement in 300 mg/kg Cheunggihwadamhwan groups. Conclusions According to this study, the extract of Cheunggihwadamhwan showed a positive effect of lowering lipid, antioxidation and a control of producing proinflammatory cytokines.

  • Objectives Advanced researches on the relationship between obesity and heart rate variability (HRV), heretofore, focused on characteristics of HRV depending on the state of obesity. However, the previous researches have not quantified predictive power of HRV toward the obesity-related variables, which is rather more meaningful for clinicians who regularly treat obese patients. Hence, we designed a research to investigate whether HRV could predict serum levels of obesity-related metabolites. Methods Ninety obese premenopausal women meeting the inclusion criteria were recruited. The HRV test, blood sampling, and measurement of physical traits were conducted. Multiple regression analysis of the measurement data was carried out, putting obesity-related metabolites (insulin, glucose, triglyceride, hs-CRP, HDL, LDL, total cholesterol) as outcome variables and the others as predictors. To select appropriate predictive variables, the Akaike's Information Criterion (AIC) was applied. Normality and homoskedasticity of residuals for each model were tested to identify if there were any violations of the regression analysis's basic assumption. Logarithm transformation was used for the values of the concentration of metabolites and the HRV. Results The regression model including Total Power (TP) value and BMI had significant predictive power for serum insulin concentration (F(2, 88)=835.7, p<0.001, $R^2=0.95$). The regression coefficient of ln (TP) was -0.1002. However, it was not sure if the HRV could predict concentrations of other metabolites. Conclusions The results suggest that the Total Power (TP) value of the HRV can predict the level of serum insulin. If the BMI could be assumed as being constant, when the TP value is multiplied by n, the predicted change of insulin could be drawn by multiplying $n^{-0.1002}$. The uncertainty of this model can be assumed as approximately 5%.

  • The decreased body, liver, kidney, spleen and testis weights of guinea pigs by TCDD (2,3,7,8-tetracholorodibenzo-${\rho}$-dioxin) treatment (TT) were statistically significantly increased after treating red ginseng saponin fraction (SF) (p>0.01). After treated with SF, the decreased hematocrits values and numbers of RBC and platelet, activity of amylase and lactate dehydrogenase, levels of uric acid, total protein and albumin by TT were increased, and the increased numbers of WBC, levels of triglyceride, total cholesterol (TC), LDL-cholesterol, HDL-cholesterol, creatinine, blood urea nitrogen, calcium and phosphorus, activities of creatinine kinase, aspartate aminotransferase, alanine aminotransferase and alkaline phosphatase were decreased after treated with SF. And they all had a statistical significance (p>0.01) except for RBC, WBC, platelet, blood glucose, TC, calcium and albumin. From these results, we knew that SF mollified the acute toxicity induced by TCDD in guinea pigs.

  • This study investigated the effects of mulberry leaf extract on lipid metabolism in rats fed a high cholesterol diet. Sprague-Dawley male rats weighing $100{\pm}10g$ were randomly assigned either to one of two normal diet groups, with (NE group) or without (N group) mulberry extract, or one of four high cholesterol groups containing 1% cholesterol and various levels of dietary mulberry leaf extract. The rats fed high cholesterol diets were subdivided into 4 groups according to level of mulberry extract; Mulberry extract free group (HC group), 0.8% mulberry leaf extract group (HCL group), 1.6% mulberry leaf extract (HCM group) and 3.2% mulberry leaf extract (HCH group). The rats were fed their respective diets ad libitum for 4 weeks. The levels of serum triglyceride, total cholesterol and LDL-cholesterol of the HC group were higher than mulberry leaf extract supplemented groups. In contrast, the levels of serum HDL-cholesterol in groups supplemented with mulberry leaf extract were significantly lower than that of HC group. Hepatic total lipids, triglycerides, and cholesterol were significantly higher in the high cholesterol groups compared to those of the normal group, but were lower in the HCL, HCM and HCH groups than in the HC group. HMG-CoA reductase activity was significantly decreased in the HC and HCL groups compared to the normal and NE groups. However, the activities in the HCM and HCH group were similar to that of the normal group. The activity of acyl-CoA-cholesterol acyl transferase (ACAT) was increased in high cholesterol groups compared to the normal group. However, the activity was lower for all of the high cholesterol groups fed mulberry leaf extracts, and was lowest for the highest supplemented group (HCH), with no significantly difference from the normal group. In conclusion, the reduction in serum and hepatic lipid composition by mulberry leaf extract may be due to its modulation of HMG-CoA reductase and ACAT activities.

  • Objectives : We investigated the effects of mountain cultivated ginseng water boiled extract(MCG) on blood glucose and insulin levels, and examined whether lipid metabolism are improved by it in male db/db mice(a murine model of type 2 diabetes mellitus). Methods : 9 weeks old, male db/db mice were divided into 5 groups : C57BL/6J normal, control, MCG-250mg/kg (MCG-1), MCG-500mg/kg(MCG-2) and MCG-1000mg/kg(MCG-3). After mice were treated with MCG for 8 weeks, we measured body weight, food intake, fat weight, visceral organ weight and blood glucose, insulin and lipid levels. Results : 1. We found no difference in body weight, food intake, fat weight and visceral organ weight among the animal groups. 2. Compared with controls, MCG-treated mice had lower blood glucose level and higher blood insulin levels, the magnitude of which was prominent in MCG-2. 3. Compared with controls, MCG-treated mice had lower LDL-cholesterol and higher HDL-cholesterol levels. 4. Compared with controls, MCG-treated mice had blood triglyceride and free fatty acid levels, the magnitude of which was prominent in MCG-2. 5. Blood AST and ALT concentrations were not changed by MCG, indicating MCG do not show any toxic effects. Conclusions : These results demonstrate that MCG effectively increases blood insulin level and decreases blood glucose level, blood lipid levels, and prevents and improves diabetic dyslipidemia and cardiovascular disease.
