A Literatural study on the hemorrhoids and hemorrhoids complicated by anal fistula (痔瘡과 痔瘻에 對한 文獻的 考察)

  • A Literatural study on the etiological factors, classification, prescription of hemorrhoids and hemorrhoids complicated by anal fistula following results were obtained. 1. The cause of hemorrhoids are long time sit, long time gate, overfatigue, overeating, imbalance of stool( constipation or diarrhea), pregnant fertility(overfatigue after childbirth, insufficiency of middle warmer energy), uncontrol sexual excess, pathgenic factors of wetness, heat, wind, dry, genetic cause, excess of anxiety, pile up of heat poison, weakness of entrails and viscera. The cause of hemorrhoid complicated by anal fistula are attack of external wind, heatness, dry, fire, wetness(pathgenic factors), inapporiate treatment and chronic disease, greasy diet, excess of anxiety, constipation, uncontrol sexual excess, obstacle of circulation of vital energy and blood on anal site. 2. Classification of hemorrhoids are female hemorrhoids, male hemorrhoids, pulse hemorrhoids, intestines hemorrhoids, vital energy hemorrhoids, wine hemorrhoids, blood hemonhoids, flowing hemorrhoids. Classification with other method are external hemorrhoids, internal hemorrhoids, mixed hemorrhoids, excrescence hemorrhoids, nipple homorrhoids. External hemorrhoids is classified of varicosis of hemorrhoidal vein, connective tissue form, thrombus form. Classification of hemorrhoid complicated by anal fistula are simple lower hemorrhoid, lower mixed hemorrhoid, deep hemorrhoid, outer of one hole hemorrhoid, a horseshoe hemorrhoids. Once more classificated of four are space of sphincter muscle form, penetration sphincter muscle form, upper of sphincter muscle form, outer of sphincter muscle form. 3. Therapy method of hermorrhoid and hemorrhoid complicated by anal fistula are internal method, fumigation method method, ointment, method of close with medicine, necrotizing method, hot medicated compress( gxternal method), injection, insertion, bind, (operation) and acupuncture therapy (the others method) 4. Herb medicine for many used of internal method are Scutellaria baikalensis George(黃芩), Coptis japonia Makino(黃連), Rehmania giutinosa Liboschitz ex Fischer & Meyer(生地黃), Poncirus trifoliata Refinesque(枳殼), Sanguisorba officinalis Linne(地楡), Sophora japonica L.(槐花), Cnidium officinale Makino (川芎), Astragalus membranaceus Bunge(황기), Angelica gigas Nakai (當歸). 5. Herb medicine for many used of fumigation are Schlechtendalia Chinesis J. Bell (五倍子), Artemisia Vulgaris L. var indica Maxim(艾葉), Poncirus trifoliata Refinesque (枳殼), Nepeta japonica Maximowicy(荊芥), And herb medicine for many used of ointment are Calomelas(輕粉), Alum(白礬), Boswellia carterii Birdwood(乳香), Os Draconis Fossilia Ossis Mastodi(龍骨).

The Anti-depressive Effect of Samul-tanggahyangbuja on Chronic Mild Stress in Ovariectomized Rats (만성 스트레스 모델에서 사물탕가향부자의 항우울 효과)

  • Objectives: The purpose of the present study is to investigate anti-depressive effects of Samul-tanggahyangbuja (SGH) on ovariectomized and chronic mild stress (CMS) induced rats. Methods: Ovariectomized rats were exposed to CMS for 4 weeks. Changes of depression behavior were tested by using sucrose intake test (SIT), elevated plus maze (EPM), forced swimming test (FST) and Morris water maze test (MWMT) in rats until being orally medicated with SGH (100 or 400 mg/kg/day). In addition, the serum levels of corticosterone (CORT), IL-4, IL-$1{\beta}$ and changes of 5-HT in the brain were measured. Results: 1. SGH 400 mg/kg treated group (SGH 400) significantly increased amount of sucrose intake compared with the control group (p<0.05). 2. SGH 100 mg/kg treated group (SGH 100) and SGH 400 significantly increased the time spent in the open arms of the EPM compared with the control group (p<0.01). SGH 400 also significantly increased the number of crossing of the open and closed arms compared with the control group (p<0.05). 3. SGH significantly shortened the immobility time in FST compared with the control group (SGH 100 p<0.05, SGH 400 p<0.01). 4. SGH significantly increased performance of acquisition trials compared with the control group (p<0.05, on day 4, 5 of SGH 100 and 400). SGH 400 also significantly increased performance of retention trials compared with the control group (p<0.05). 5. The serum levels of corticosterone and IL-4 were not significantly different among the groups. There were no changes on the serum levels of corticosterone, IL-$1{\beta}$ and IL-4 after administration with SGH. 6. SGH 400 significantly increased the level of 5-HT in the hippocampus compared with the control group (p<0.05). SGH significantly increased the levels of 5-HT in the hypothalamus compared with the control group (SGH 100 p<0.05, SGH 400 p<0.01). Conclusions: These results suggest that SGH has the anti-depressive effect on ovariectomized rat and affect 5-HT system rather than hypothalamic-pituitary-adrenal (HPA) axis and immune system.

Effects of Hamstring Flexibility and Dynamic Stability of Lower Lumbar according to Stretching and Massage Techniques (스트레칭과 마사지 기법이 넙다리뒤근의 유연성 및 아래 허리뼈의 동적 안정성에 미치는 영향)

  • PURPOSE: This study aims to provide effects of therapeutic techniques as well as basic materials of safety by comparing and analyzing the effects of hamstring flexibility and dynamic stability of lower lumbar according to Stretching and Massage Techniques to adults with reduced the flexibility of hamstring. METHODS: This study conducted differential diagnosis through sit and reach test(SRT) and Schober test to select subjects who have shortened hamstring without any spinal problem. Selected subjects were divided into two groups randomly; HSG(Hamstring Stretching Group, n=8) and HMG(Hamstring Massage Group, n=8) and they received treatment for 2 weeks. To take statistics, SRT and dynamic view using x-ray were used. RESULTS: On SRT, HSG and HMG showed significant difference between pre and post test. A comparison of the difference value between HSG and HMG, HSG($9.73{\pm}1.78$) has more remarkable outcome than HMG($2.78{\pm}0.56$). Lower lumbar intervertebral disc length test for Intervertebral disc length(IDL)L45 and IDLL5S1 did not show significant differences between two groups and difference value. CONCLUSION: This study showed that stretching is more effective to improve hamstring flexibility than massage technique. Especially, flexibility increase of the hamstring in vertebral stabilization cannot affect improvement possibility will make a flexibility in order and the intervention and stabilization exercise of the spine.

Bone Cement-Augmented Percutaneous Screw Fixation for Malignant Spinal Metastases : Is It Feasible?

  • Objective : We evaluated the validity of bone cement-augmented percutaneous screw fixation for treating malignant spinal metastases. Methods : Between 2011 and 2015, 14 patients (eight men and six women) who underwent bone cement-augmented percutaneous screw fixation for malignant spinal metastases were enrolled in this study. Their life expectancy was considered to be more than one month and less than one year, based on the revised Tokuhashi scoring system. Clinical findings including the back pain scale score, functional outcome, procedure related complications, and survival were assessed preoperatively, postoperatively, and then six months after the procedure. Results : Twelve of the patients (86%) survived up to six months after the procedure. Three required mini-open decompressive laminectomy for severe epidural compression. Bone cement-augmented percutaneous screw fixation was performed one level above, one level below, and at the pathologic level itself. The mean operation time was 60 minutes (45-180) and blood loss was less than 100 mL. Prior to surgery, the mean pain score on the visual analogue scale was 8.8, while one month after the procedure, it had reduced to 3.0; this improvement was maintained until the six-month assessment in the surviving patients. All patients were able to sit within the first two days after surgery, and no patient experienced neurological deterioration at the one-month follow up after the surgery. No patient experienced screw loosening during the six months of follow-up. Asymptomatic cement leakage into the epidural space was observed in two patients, but no major complications were observed. Conclusion : For selected patients with malignant spinal metastases, bone cement-augmented percutaneous screw fixation can provide significant pain relief and improve quality of life.

Climate Change Impacts in Natural Resources and Livestock in Mongolia Climate

  • This paper discuss some results of observed changes of meteorological elements as temperature, precipitation and some extreme indexes in Mongolia. Mongolia is one of the largest landlocked countries in the world. The climate is characterized by a long lasting cold winter, dry and hot summer, low precipitation, high temperature fluctuation and relatively high number of sunny days per year. During last 60 years the annual mean air temperature has risen $1.66^{\circ}C$. Intensive warming of > $2^{\circ}C$ was observed at higher altitudes of high mountains when warming of < $1^{\circ}C$ was observed the Domod steppe and the Gobi Desert. Heat Wave Duration have statistically significant risen trend with increaded number of days by 8-18 at significance level of 95-99.9% depending on geography and Cold Wave Duration have shortened by 13.3 days significance level of 95-99%. In general, by the amount of precipitation, Mongolia falls in semi-arid and arid region. It is 300-350 mm in the high mountain regions while it is only 50-150 mm in Gobi Desert regions. The changes of annual precipitation have very localized character i.e.decreasing at one site and increasing at a sit nearby. Annual precipitation decreased by 30-90 mm in the northern-central region and increased by 2-60 mm in the western and eastern region. The magnitude of alteration changes in precipitation regardless increasing or decreasing is 5-25%. A trends, significant at the level of 90%, found where changes are more than 40 mm or more than 15% of annual mean value. Moreover, the soil moisture resources was decreased in the last 40 years. Specially, moisture contents of the top soil have decreased 2 times(N. Natsagsuren, 2002). Months of June and July in Mongolia is the year that moisture is not inhibiting vegetation growth. Unfortunately, its also found that moisture in this time tends to decrease. Increased temperature, decreased precipitation and soil moisture are most likely resulted in occurences of more intense drought spells that have taken place during the recent years. Intimately, these changes have considerable impact on livestock in Mongolia.

Development of Slacks Pattern for Old Aged Abdominal Obese Women (복부 비만 노년 여성을 위한 슬랙스 패턴 개발)

  • This study was conducted to evaluate the differences in external appearances and functional qualities between the slacks which were patterned on existing designs and on a new design in order to improve on their level of comfort of ready-made clothes for abdominally fat women. Five abdominally fat women in their 60's were chosen as experimental participants. Twenty-four external evaluation items, and seven different actions involving six moving parts of the body were investigated with a 5-level scale in the repeated wearing test. The results were analyzed by statistical methods. The newly designed pattern in this study was definitely superior to the other two patterns(L-type and K-type) on the external appearance evaluation. The newly pattern removed a waist belt and a dart from the front part of the garment and gave surplus space around the waist. It prevented superfluous wrinkles at the abdominal region and thighs. The superfluous wrinkles at the side regions also disappeared due to the length (inseam) reduction from waist to crotch. The newly pattern made the center-line of the back side slightly sloped to the main pattern in order to remove the drooped line. The newly pattern showed the highest score in the functional test of several actions. It was due to the reduction of the length from waist to crotch as well as the belt width. It made waist-line movement better, and made it easier and more comfortable to sit down. The newly pattern also showed the highest score in the functional test classified by specific movements of parts of the body for the same purpose. The slacks which were made of elastic materials showed better results than the non-elastic ones on external appearances and functional test items. It showed that the elastic materials played a better role in the increase of functional action of abdominally fat, aged women. This study suggests that the newly designed slacks pattern could give abdominally fat, aged women better external features and a more comfortable fitting sensation. As well, the results were significant as basic data to produce ready-made and/or tailer-made garments.

Comparison of behavior of high-rise residential buildings with and without post-tensioned transfer plate system

  • Shear wall is commonly used as a lateral force resisting system of concrete mid-rise and high-rise buildings, but it brings challenges in providing relatively large space throughout the building height. For this reason, the structure system where the upper structure with bearing, non-bearing and/or shear walls that sits on top of a transfer plate system supported by widely spaced columns at the lower stories is preferred in some regions, particularly in low to moderate seismic regions in Asia. A thick reinforced concrete (RC) plate has often been used as a transfer system, along with RC transfer girders; however, the RC plate becomes very thick for tall buildings. Applying the post-tensioning (PT) technique to RC plates can effectively reduce the thickness and reinforcement as an economical design method. Currently, a simplified model is used for numerical modeling of PT transfer plate, which does not consider the interaction of the plate and the upper structure. To observe the actual behavior of PT transfer plate under seismic loads, it is necessary to model whole parts of the structure and tendons to precisely include the interaction and the secondary effect of PT tendons in the results. This research evaluated the seismic behavior of shear wall-type residential buildings with PT transfer plates for the condition that PT tendons are included or excluded in the modeling. Three-dimensional finite element models were developed, which includes prestressing tendon elements, and response spectrum analyses were carried out to evaluate seismic forces. Two buildings with flat-shape and L-shape plans were considered, and design forces of shear walls and transfer columns for a system with and without PT tendons were compared. The results showed that, in some cases, excluding PT tendons from the model leads to an unrealistic estimation of the demands for shear walls sit on transfer plate and transfer columns due to excluding the secondary effect of PT tendons. Based on the results, generally, the secondary effect reduces shear force demand and axial-flexural demands of transfer columns but increases the shear force demand of shear walls. The results of this study suggested that, in addition to the effect of PT on the resistance of transfer plate, it is necessary to include PT tendons in the modeling to consider its effect on force demand.
