An Exploratory Study of the Effect of Ongyeong-tang Administration on Insomnia Disorder in Patients with Neurological Diseases: A Retrospective Chart Review Study

  • Objectives: The aim of this study was to retrospectively evaluate the efficacy and safety of Ongyeong-tang (OGT) in the treatment of insomnia in patients afflicted with neurological diseases, according to the medical records of one medical institution. OGT is a herbal prescription that originates from the "Synopsis of Prescriptions of the Golden Chamber" and has been used for menopausal symptoms, uterine diseases, skin diseases, insomnia, and other neuropsychiatric symptoms. Methods: Medical records of patients who visited Kyung Hee University Korean Medicine Hospital from January 1, 2021 to June 30, 2022, received outpatient or inpatient treatment, and met the inclusion criteria were used in this study. Results: Twenty-four patients satisfied the inclusion criteria and were included. The Pittsburgh Sleep Quality Index-Korean version (PSQI-K) score was used to evaluate the outcomes before and after treatment. After an average of 14.42±9.2 days of OGT extract intake, 20 patients (83.3%) experienced improvement. The remaining four (16.7%) had no change in their symptoms, but none demonstrated worsening of symptoms. The total PSQI-K score showed a statistically significant decrease from 13.08±4.54 to 10.42±4.58 (p<0.001). Sleep quality, sleep duration, sleep disturbance, use of sleeping medications, and daytime dysfunction also showed statistically significant improvements. Conclusions: Administration of OGT extract may be a relatively safe and effective therapeutic option for insomnia in patients with neurological diseases.

경추성 현훈 환자의 복합 한의진료 경과 및 삶의 질 변화 : CARE 가이드라인에 따른 증례보고 (Herbal Medicine and Local Acupuncture Point Stimulation Administration and Their Clinical Effect on Quality of Life for Cervicogenic Dizziness (CGD) Patient: A CARE Guideline Compliant Case Report)

  • Background: This study examines the clinical effect on quality of life of herbal medicine and local acupuncture point stimulation administration for a cervicogenic dizziness (CGD) patient. This case report was described in accordance with CARE guidelines. Case Report: A 25 year-old female patient suspected to be suffering from CGD was examined. The patient was treated with herbal medicine and local acupuncture point stimulation. We used the Headache Impact Test-6 (HIT-6), Dizziness Handicap Inventory (DHI), Pittsburgh Sleep Quality Index (PSQI), 36-Item Short Form Health Survey Version 2 (SF-36v2), and verbal numerical rating scale (VNRS) to assess the patient's symptoms. As new herbal medicine and local acupuncture point stimulation were applied, dizziness, headache, and neck pain symptoms improved. Quality of life affected by the symptoms significantly improved. Adverse effects were not observed. Conclusions: This study may suggest that herbal medicine and local acupuncture point stimulation may be effective therapy for the treatment of typical symptoms in patients with CGD.

뇌교육 기반 운동 및 KPEM도수치료 통합 프로그램이 암환자의 수면과 삶의 질에 미치는 영향 (The effect of brain education-based exercise and KPEM manual therapy integrated program on sleep and quality of life in cancer patients)

  • Background: The purpose of this study was to investigate the effect of brain education-based exercise and KPEM manual therapy integrated program on the sleep and quality of life of cancer patients. Design: Seventy subjects who were diagnosed with cancer and were undergoing treatment volunteered to participate in this study. All subjects used a nonequivalent control group pretest-posttest design for either the experimental group or the control group. In the final analysis, there were 25 subjects in the experimental group and 18 subjects in the control group. Methods: For 12 weeks, the experimental group performed brain education-based exercise (20 minutes) and KPEM manual therapy (50 minutes), and the control group performed basic physical therapy and autonomous exercise. For evaluation, the Korean version of the Pittsburgh Sleep Quality Index (PSQI-K) and the quality of life index were measured after intervention using the European Organization for Research and Treatment of Cancer (EORTC-3.0Ver). Effect between groups, time effect over time, and group*time interaction were analyzed through a pre-test before and after the 12-week intervention period, and repeated measure ANOVA after 12 weeks of the integrated program intervention. All statistical significance levels were set at α=.05. Results: The PSQI in the time effect (p=.001) and the group*time interaction (p<.001) were statistically significant. In terms of EORTC, QL2 and PF2 were significant in time effect (p=.024; p=.021) and group*time interaction (p=.007; p=.021), whereas in RF2, significance was only found in group*time interaction (p=.028). In symptom indicators, time effect was the only significant factor in FA, SL, AP, and CO, respectively (p=.002; p=.028; p=.041; p =.005) and in DY, there were significant differences in the time effect (p=.016) and group*time interaction (p=.002). Conclusion: The brain education-based exercise and KPEM manual therapy integrated program effectively improves the sleep and quality of life of cancer patients. It is considered that this exercise and therapy can be actively used as a psychological, emotional, and physically complementary physical therapy intervention to improve the quality of life of cancer patients.

노인 하지불안증후군에서의 인지기능 저하 (Mild Impairments in Cognitive Function in the Elderly with Restless Legs Syndrome)

  • 목 적: 하지불안증후군에서의 인지기능 저하는 흔히 동반되는 수면박탈, 불안, 우울 등에 의해 영향을 받는다. 본 연구의 목적은 투약 받지 않고 있는 한국 노인 인구에서 하지불안증후군과 인지기능 저하와의 관련성을 동반 증상의 영향을 배제하고 살펴보는 것이다. 방 법: 연구대상은 25명의 투약 받고 있지 않은 한국 노인 하지불안증후군 환자군과 나이, 성별, 교육연한에 따라 짝지은 50명의 대조군이다. 모든 연구대상에 대해 CERAD-K 신경심리평가, 중증인지감퇴평가 척도, 전두엽 기능 평가, 그리고 시계 그리기 검사(CLOX)를 포함한 광범위한 인지기능 평가를 시행하였다. 또한 수면의 질 평가를 위해 피츠버그 수면의 질 지수(Pittsburgh Sleep Quality Index, PSQI), 우울증의 평가를 위해 노인우울척도(Geriatric Depression Scale, GDS)를 사용하였다. 결 과: 환자군과 대조군 사이의 PSQI와 GDS 점수의 차이는 없었다. 구성 재인 검사에서 환자군이 대조군에 비해 더 낮은 점수를 보였으며(t=-2.384, p=0.02), 이를 제외한 모든 영역의 인지기능 평가에서 환자군과 대조군의 유의미한 차이는 관찰되지 않았다. 우울감이 있는 대상($GDS{\geq}10$)은 그렇지 않은 군에 비해 언어 유창성, 간이 정신상태 검사, 단어목록기억, 길 만들기 검사, 전두엽 기능 평가의 영역에서 유의한 인지기능 저하를 보였다. 수면의 질이 낮은 대상(PSQI>5)은 그렇지 않은 군에 비해 인지기능 평가의 전 영역에서 차이를 보이지 않았다. 결 론: 본 연구에서 하지불안증후군 환자군을 대조군과 비교한 결과 환자군은 불면과 우울의 영향을 배제하면 비교적 인지기능이 보존된 것으로 관찰되었다. 환자군에서 관찰되었던 시각 인지 영역의 저하는 하지불안증후군에서의 도파민 신경전달계의 장애와 연관하여 해석해 볼 수 있다.

Relationship Between Psychological Factors and Pain Intensity in Temporomandibular Disorders with or without Central Sensitization: A Cross-Sectional Observational Study Using Multiple Regression Analysis

  • Purpose: To quantify the relationship between perceived pain intensity and psychological variables in a sample of participants with temporomandibular disorder, with or without central sensitization (CS). Methods: A cross-sectional study with nonprobability convenience sampling was conducted from January 1, 2022, to June 30, 2023. Pain intensity (Numeric Pain Rating Scale), anxiety (State-Trait Anxiety Questionnaire, STAI), catastrophizing (Pain Catastrophizing Scale, PCS), perceived stress (Perceived Stress Scale, PSS), and sleep quality (Pittsburgh Sleep Quality Index, PSQI) were assessed. Statistical analysis was performed using IBM SPSS Statistics for Windows, Version 20.0 (IBM Co.), which included descriptive and normality analyses and the calculation of strength of multiple correlational regression. Results: A total of 52 (n=34 female 65.4%; n=18 male 34.6%) subjects with diagnosis of temporomandibular disorders (TMD) were finally included. A total of 26 participants (n=26, 50.0%) were cases suffered from CS (TMD-CS mean=46.62±11.24) while the remaining participants (n=26, 50.0%) were the controls (TMD-nCS mean=26.77, standard deviation [SD]=8.42). The pain intensity was moderate in both groups TMDCS (mean=7.62, SD=0.83) and TMD-nCS (mean=7.05, SD=0.86), anxiety (TMD-CS STAI mean=53.27, SD=11.54; TMD-nCS STAI mean=49, SD=11.55), catastrophizing (TMD-CS PCS mean=46.27, SD=9.75; TMD-nCS PCS mean=26.69, SD=4.97), perceived stress (TMDCS PSS mean=30.35, SD=4.91; TMD-nCS PSS mean=26.12, SD=6.60) and sleep quality (TMD-CS PSQI mean=15.81, SD=3.65; TMD-nCS PSQI mean=12.77, SD=2.76) levels were measured in both groups. In TMD-CS and TMD-nCS, higher anxiety levels were moderately and significantly associated with greater pain intensity β=0.4467 (t=2.477, p=0.021) and β=0.5087 (t=2.672, p=0.014). Nevertheless, catastrophizing, perceived stress and sleep quality were not associated to pain intensity in neither of group. Conclusions: In both TMD-CS and TMD-nCS patients, elevated anxiety levels were moderately and significantly associated with increased pain intensity. However, heightened levels of pain catastrophizing, perceived stress, and poor sleep quality were not significantly associated with increased pain intensity in either of the two analyzed groups.

양극성장애 환자의 하지불안증후군 유병율과 관련 특성 (Prevalence and Its Correlates of Restless Legs Syndrome in Outpatients with Bipolar Disorders)

  • 연구목적 본 연구는 외래 치료 중인 양극성 장애 환자에서 하지불안증후군(restless legs syndrome, RLS)의 유병률과 관련 특성을 알아보기 위해서 시도 되었다. 방 법 안정적으로 외래 치료 중인 양극성 장애 환자 100명이 본 연구에 포함되었다. RLS의 진단과 증상의 심한 정도는 International Restless Legs Syndrome Study Group에서 개발한 진단 기준을 이용하였다. Beck 우울증 척도(Beck's Depression Inventory), 상태 불안 척도(Spielberg's State Anxiety Inventory), 피츠버그 수면의 질 설문지(Pittsburgh Sleep Quality Index), 한국판 약물태도 척도(Korean version Drug Attitude Inventory), 한국어판 항정신병약물치료 중 주관적 안녕감 척도-단축형(Subjective Well-Beings under Neuroleptic Treatment Scale-Short Form)과 반즈 좌불안석증 평가 척도(Barnes Akathisia Rating Scale) 등이 각각 우울 증상, 불안의 수준, 주관적 수면의 질, 주관적 안녕감, 약물에 대한 주관적 태도, 좌불안석증의 유무를 평가하는데 사용되었다. 결 과 연구대상자 100명 중 7명(7%)이 RLS로 진단되었으며, RLS의 필수 진단기준을 1개 이상 가지는 환자는 36명(36%)이었다. 본 연구의 표본의 크기가 작은 이유로 세 그룹 간 비모수적 검정을 시행하였다(RLS의 필수진단 기준을 모두 만족하는 그룹, RLS로 진단할 수는 없지만 RLS 필수진단 기준을 1개 이상 가지고 있는 그룹, RLS 필수진단 기준에 해당되지 않는 그룹). 세 그룹 간에 성별, 나이, 그 밖의 인구사회학적 특성에 유의한 차이는 없었다. RLS환자군 및 RLS증상군은 대조군에 비해 우울 및 불안증상이 더 많았고, 수면의 질이 더 낮은 경향을 보였다. 결 론 본 연구는 양극성장애 환자에서 RLS 유병률과 이와 관련된 특성을 살펴본 첫 예비 연구라 할 수 있다. 연구의 결과는 조현병 환자와 비교할 때 양극성 장애와 RLS의 연관성이 비교적 적다는 것을 시사한다. RLS가 있는 양극성 장애 환자에서 우울 및 불안 증상이 더 많고 수면의 질이 더 낮은 경향은 조현병 환자에서도 관찰이 된다. 향후 대규모 연구에서 보다 과학적인 연구방법을 통해 양극성장애와 RLS와의 연관성에 대해 조사할 필요가 있을 것이다.

시상출혈 환자의 불면에 대한 한방 치험 1례 (A case of thalamic hemorrhage patient who revealed insomnia treated with Traditional Korean Medicine)

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with insomnia occurred after onset of thalamic hemorrhage. ■ Methods A patient with insomnia diagnosed as thalamic hemorrhage was treated with herbal medication, acupuncture, electro-acupuncture, cupping and moxibustion. Then we evaluated the improvement with the number of times the patient woke up, PSQI-K(Pittsburgh Sleep Quality Index-Korean), ISI-K(Insomnia Severity Index-Korean) and KESS(Korean version of Epworth sleepiness scale). ■ Results Decrease in scores of PSQI-K, ISI-K, KESS and the number of times the patients woke up were observed after the TKM treatment. ■ Conclusion This case showed the effect of TKM treatment on poststroke insomnia.

불면증 변증도구 신뢰도와 타당도 평가 및 심리검사와의 상관성에 대한 초기연구 (A Pilot Study of Evaluating the Reliability and Validity of Pattern Identification Tool for Insomnia and Analyzing Correlation with Psychological Tests)

  • Objectives: The purpose of this study was to evaluate the reliability and validity of the instrument on pattern identification for insomnia (PIT-Insomnia) and verify the correlation between PIT-Insomnia and psychological tests. Methods: Two evaluators examined the pattern identification of the participants who met insomnia disorder diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorder, Fifth Edition (DSM-5) and took the Insomnia Severity Index (ISI) score over 15 once manually and twice using the PIT-Insomnia to measure the inter-rater and test-retest reliability. We also conducted the following surveys: the Pittsburgh Sleep Quality Index (PSQI), the Korean version of Beck's depression inventory (K-BDI), the Korean version of the State-Trait Anxiety Inventory (STAI-K), the Korean Symptom checklist-95 (KSCL-95), and the EuroQol-5 dimension (EQ-5D), to measure concurrent validity and correlation between the PTI-Insomnia and psychological tests. Results: 1. The test-retest reliability analysis of the pattern identification results showed moderate agreement, and test-retest reliability analysis of each pattern identification score showed agreements from poor to moderate. 2. The inter-rater reliability analysis of the pattern identification results via manual showed slight agreement, when analysis was performed with calibration, the inter-rater reliability analysis of the pattern identification results via manual showed fair agreement. 3. The concordance analysis between results via manual and the PIT-Insomnia showed poor agreement, when the analysis was performed with calibration, concordance analysis showed fair agreement. 4. The concordance analysis between the PIT-Insomnia and the PSQI showed positive linear correlation. 5. The concordance analysis between the PIT-Insomnia and the PSQI, K-BDI, STAI-K, KSCL-95, and EQ-5D showed that non-interaction between the heart and kidney have positive linear correlation with the K-BDI, anxiety item of KSCL-95, dual deficiency of the heart-spleen have positive linear correlation with somatization item of KSCL-95, paranoia item of KSCL-95, heart deficiency with timidity have positive linear correlation with stress vulnerability item of KSCL-95, parania item of KSCL-95, phlegm-fire harassing the heart have positive linear correlation with K-BDI, paranoia item of KSCL-95, depressed liver qi transforming into fire have positive linear correlation with the anxiety item of KSCL-95, parania item of KSCL-95, all pattern identification have negative linear correlation with EQ-5D. Conclusions: The PIT-Insomnia has moderate agreement of reliability and reflects the severity of insomnia since it has some concurrent validity with the PSQI. There are some correlations between the PTI-Insomnia with specific psychological tests, so we could suggest it can be used appropriately in the clinical situation.
