  • Objective : The diagnosis of insufficiency fractures of the sacrum in an elder population increases annually. Fractures show very different morphology. We aimed to classify sacral insufficiency fractures according to the position of cortical break and possible need for intervention. Methods : Between January 1, 2008 and December 31, 2014, all patients with a proven fracture of the sacrum following a low-energy or an even unnoticed trauma were prospectively registered : 117 females and 13 males. All patients had a computer tomography of the pelvic ring, two patients had a magnetic resonance imaging additionally : localization and involvement of the fracture lines into the sacroiliac joint, neural foramina or the spinal canal were identified. Results : Patients were aged between 46 and 98 years (mean, 79.8 years). Seventy-seven patients had an unilateral fracture of the sacral ala, 41 bilateral ala fractures and 12 patients showed a fracture of the sacral corpus : a total of 171 fractures were analyzed. The first group A included fractures of the sacral ala which were assessed to have no or less mechanical importance (n=53) : fractures with no cortical disruption ("bone bruise") (A1; n=2), cortical deformation of the anterior cortical bone (A2; n=4), and fracture of the anterolateral rim of ala (A3; n=47). Complete fractures of the sacral ala (B; n=106) : parallel to the sacroiliac joint (B1; n=63), into the sacroiliac joint (B2; n=19), and involvement of the sacral foramina respectively the spinal canal (B3; n=24). Central fractures involving the sacral corpus (C; n=12) : fracture limited to the corpus or finishing into one ala (C1; n=3), unidirectional including the neural foramina or the spinal canal or both (C2; n=2), and horizontal fractures of the corpus with bilateral sagittal completion (C3; n=8). Sixty-eight fractures proceeded into the sacroiliac joint, 34 fractures showed an injury of foramina or canal. Conclusion : The new classification allowes the differentiation of fractures of less mechanical importance and a risk assessment for possible polymethyl methacrylate leaks during sacroplasty in the direction of the neurological structures. In addition, identification of instable fractures in need for laminectomy and surgical stabilization is possible.

  • Under the limited arable land, the enhancement of agricultural productivity is indispensable to provide the food demand which is concomitant with the rapid increase in population. From this viewpoint, the upbringing and dissemination of high-yielding varieties has been promoted continuously and several modifications in cultural practices, including heavy fertilization, dense planting, and early transplanting, also have been gradually developed. However these changes in cultivation have led to the increased outbreak of insect pests and diseases. And this unexpected results have accelerated the number and complexity of pesticides employed as well as their consumption. Even though pesticides are essential materials contributing to the steady production of agricultural crops, large scale consumption of them has given rise to several adverse impacts, such as mammalian hazard and/or environmental contamination. In this respect, recent development of new pesticides has been concentrated on 'safe pesticide', as it were, that has the highly selective properties without unfavorable side influences on other ecosystem. According to literature cited up to now, feasibilities of safe pesticide development would be summarized as two categories. One of them is the development of chemical pesticides, which include the molecular structure modification of established pesticides for increased safety and synthesis of new safe chemicals which can attack the vulnerable point of physio-ecological characteristics in insect pests and diseases. The other is the biological pesticides which comprise natural enemies and microorganisms to act selectively on confined insect pests and diseases, In addition, improvement of physico-chemical properties of available pesticide formulations would be one of the desirable means for safe pesticide development in view of efficacy enhancement and minimization of hazardous properties or safe pesticide development, various approaches are feasible and needed to study, however, long period and much financial outlay are necessary to develop a new item. And under the present situation in Korea, there are many difficulties for performing research on all the possible routes. Therefore, combined pesticides by the reasonable combination of already registered resticides evaluated as the fairly safe pesticides and safe formulation based on their physico-chemical properties would be developed primarily. And many efforts would be given gradually for the development of new chemical and biological pesticides.

  • Hydrogen emerged as a sustainable transport energy source. To increase hydrogen utilization, hydrogen refueling stations must be available in many places. However, this requires large-scale financial investment. This paper proposed a methodology for selecting the optimal location to maximize the use of hydrogen charging stations. The location of gas stations and natural gas charging stations, which are competing energy sources, was first considered, and the expected charging demand of hydrogen cars was calculated by further reflecting data such as population, number of registered vehicles, etc. Using k-medoids clustering, one of the machine learning techniques, the optimal location of hydrogen charging stations to meet demand was calculated. The applicability of the proposed method was illustrated in a numerical case of Seoul. Data-based methods, such as this methodology, could contribute to constructing efficient hydrogen economic systems by increasing the speed of hydrogen distribution in the future.

  • Background: Singapore has the stable healthcare system with utilizing pharmacist manpower in proper positions by demand of populations' health among Asian countries. Objective: This study aims to systematically review (1) the pharmacists' role and (2) the pharmacy education system of Singapore in comparison with Korea. Method: We searched for information about academic, medical and governmental institutions related to professional pharmacists' practice in Singapore by primarily using database such as DBpia, KISS, Google Scholar and ProQuest and the official website of the Singapore Ministry of Health. We contacted and arranged the visit schedules with National University of Singapore, National Health Group's polyclinics, Agency for Integrated Care, National University Hospital, and community chain pharmacies. During onsite visits, we interviewed pharmacists working in each institution and obtained additional documents and materials relevant to this manuscript work. Results: To become a registered pharmacist in Singapore, the pharmacy curriculum requires four full-time academic years and six additional months allotted for pre-registration training. Pharm.D. course is offered for pharmacy graduate students with additional two full-time years of study. Team teaching and inter professional education program seem the most significant method in pharmacy education. Pharmacists working at hospitals, polyclinics, and community pharmacies in Singapore take broader roles and offer more cognitive services such as smoking cessation program and medication reconciliation. Especially, pharmacists in Agency for Integrated Care fill the role of primary care providers for the continuing care of the community through the governmental support toward the patients-centered integrated care. Conclusion: Singaporean pharmacists take significant and active roles in collaboration with other healthcare providers. Efforts such as interprofessional pharmacy education and governmental endorsement of the systematic and interactive care between pharmacists and other medical providers in Singapore are needed to be urgently applied to Korea healthcare system for the promotion of population health.

  • Objective : The purpose of this study is to evaluate the associations between 30-day mortality and various radiological and clinical factors in patients with traumatic acute subdural hematoma (SDH). During the 11-year study period, young patients who underwent surgery for SDH were followed for 30 days. Patients who died due to other medical comorbidities or other organ problems were not included in the study population. Methods : From January 1, 2004 to December 31, 2014, 318 consecutive surgically-treated traumatic acute SDH patients were registered for the study. The Kaplan-Meier method was used to analyze 30-day survival rates. We also estimated the hazard ratios of various variables in order to identify the independent predictors of 30-day mortality. Results : We observed a negative correlation between 30-day mortality and Glasgow coma scale score (per 1-point score increase) (hazard ratio [HR], 0.60; 95% confidence interval [CI], 0.52-0.70; p<0.001). In addition, use of antithrombotics (HR, 2.34; 95% CI, 1.27-4.33; p=0.008), history of diabetes mellitus (HR, 2.28; 95% CI, 1.20-4.32; p=0.015), and accompanying traumatic subarachnoid hemorrhage (hazard ratio, 2.13; 95% CI, 1.27-3.58; p=0.005) were positively associated with 30-day mortality. Conclusion : We found significant associations between short-term mortality after surgery for traumatic acute SDH and lower Glasgow Coma Scale scores, use of antithrombotics, history of diabetes mellitus, and accompanying traumatic subarachnoid hemorrhage at admission. We expect these findings to be helpful for selecting patients for surgical treatment of traumatic acute SDH, and for making accurate prognoses.

  • Since out of a lot of traffic problems traffic accidents cause damage to life and properties of people, it stands out as one of traffic problems which needs improvement, and the loss due to traffic accident negatively affects not only the parties to the accident but also the national economy. Thus, continual concern of the government toward traffic safety is getting bigger and lately each local government is preparing a basic plan for traffic safety and vitalizing traffic safety policies. As expanding the responsibility and role of local governments for traffic safety, traffic safety measures which are based on the characteristics of each local government should be studied. Most of analytical methods in the existing traffic accidents prediction models with macroscopic vision focus on socioeconomic variables such as local population and the number of registered vehicles, and present a great deal of prediction error when they are applied in practice. In this context, this study proposed a traffic accident prediction model in respect of macroscopic level for autonomous districts (administrative districts) of Seoul City. The model development was not based on the entire city but on the type of local land usage (development density) whose relationship with traffic accident frequency was analyzed.

  • The present study analyzes the role of Lactic Acid Bacteria Mixture (LBM) on improving rye silage quality. Rye of four different stages (Booting, Heading, Flowering, and Late flowering) was collected and silage was prepared. The nutrient profile analysis of experimental silage groups showed no significant changes between control and LBM inoculation. Interestingly, the pH of rye silage in LBM treatments showed significant reduction than control (p<0.05) in all stages of rye silage. However, lowest pH (3.69) resulted on booting stage among other stages of rye. Subsequently significant lactic acid production was noted in all stages of LBM inoculation than control. Conversely maximum lactic acid production of (5.33%DM) was noted at booting stage followed by (4.86%DM) in heading stage. Further the lactic acid bacterial (LAB) count in LBM inoculated group showed significant increase than control. Similarly, the silage of booting stage group registered maximum LAB population ($63.7{\times}10^6CFU/g$) after that heading stage ($32.3{\times}10^6CFU/g$). Further significant reduction in yeast growth and no fungal growth was noted in all LPM treatment groups. Hence, LBM inoculants could be a better additive for improving rye silage quality.

  • The purpose of this study is to analyze characteristics of welfare level for the persons with disabilities in Korean local governments. The study adopted multidimensional scaling and cluster analysis. Based on prior studies and comparative importance research, three indicators were extracted from the welfare circumstance index for the disabled. They are the ratio of the registered disabled population, the ratio of the disabled household in poor households, the ratio of dependence on public finance. Also, nine dimensions and twenty seven indicators were extracted from the welfare level index. The dimensions are as follows : Education, income & economic activities support, welfare services infra-structure, health & welfare service support, movement convenience & safety, rights protection, culture, leisure & information accessibility, welfare administration & budget for the disabled. The findings from the welfare circumstance index show that local governments are divided into three groups and all indicators are statistically significant. On the other hand, the findings from the welfare level index, show that local governments are divided into four groups and five indicators are statistically significant. Based on these findings, the study proposes improvement in the disabled welfare level, and balanced development of local governments.

  • Purpose : Recently, Korea has entered an aging society due to the steadily increasing proportion of the elderly population. Additionally, elderly suicidal rate in Korea is the highest among OECD member countries. Because old age is a time when all functions are weakened physically, mentally, and socially, negative experiences, including depression and poor quality of life in the elderly, may lead to suicidal intentions. This study attempts to understand the relationship between depression and quality of life experienced by the elderly and their suicidal intentions. Methods : The survey was conducted from January 2 to March 31, 2019 by administering a questionnaire targeting people aged ≥ 65 years, who were registered at public health centers located in U-city, Gyeonggi-do. In total, 1,015 copies of data were used for the final analysis. The collected data were analyzed using the SPSS/WIN 23.0 program. The χ2 test was performed to investigate the relationship between the demographic characteristics and the suicidal intentions of the elderly subjects. The t-test was performed to determine the relationship between suicidal intentions, depression, and quality of life in elderly subjects. Results : The study results revealed that 168 people (16.6 %) had thought of suicide and 3.2 % had actually tried to commit suicide. The reasons for suicidal thoughts were family problems in 60 (5.6 %), economic problems in 49 (4.6 %), and physical illness in 44 (4.1 %). Those with suicidal intentions had higher levels of depression (t=11.712, p<.001) and higher quality of life scores (t=-9.565, p<.001). Conclusion : Based on these results, it is proposed that a suicide prevention policy and program that can improve the elderly's depression and quality of life is necessary.
