임신·수유부의 안전한 약물 사용에 대한 지식, 태도, 행동 및 교육 요구 분석: 설문지 기반 연구 (Analysis of Knowledge, Attitudes, Practices, and Educational Needs for Safe Medication Use in Pregnant or Breastfeeding Women: A Questionnaire-Based Study)

  • Objective: There is a growing interest in the safe use of medications in pregnant and breastfeeding women. Therefore, the purpose of this study is to find ways to improve education about safe medication use by investigating the status of medication use of pregnant and breastfeeding women, and by evaluating their knowledge, attitudes, and practices toward safe medication use. Methods: In this study, a self-report questionnaire was conducted on pregnant or breastfeeding women in Seoul and Gyeonggi Province. The questionnaire consisted of the following four sections; 1) sociodemographic characteristics of participants, 2) experience of medication use, 3) experience of being educated on safe medication use, and 4) knowledge, attitudes, and practices about medication use during pregnancy/breastfeeding. Results: A total of 203 participants were included in the analysis. Of these, 38.4% reported to take prescription medications during pregnancy and breastfeeding. Regarding education on safe medication use, nearly 90% of the participants answered that they were not educated or were unsure whether they had it. In the knowledge-attitude-practice evaluation on safe medication use, the knowledge level was the highest (mean, 4.45), followed by the attitude level (mean, 3.58) and the practice level (mean, 3.33). The preferred education method of the participants was online education using a computer or mobile phone application, etc. Conclusions: This study suggests that there is a need for systematic and effective education that can link knowledge of safe medication use with attitudes and practices in pregnant and breastfeeding women.

임부의 임신스트레스에 영향을 미치는 요인에 관한 단면연구 (Factors influencing pregnancy stress in pregnant women in Korea: a cross-sectional study)

  • Purpose: The purpose of this study was to investigate the association between maternal knowledge and social support on pregnancy stress among pregnant women in Korea. Methods: The participants in this study were 148 pregnant women in Korea, recruited from online communities on pregnancy and/or childbirth, from June 2019 to April 2020. The collected data were analyzed using the independent t-test, one-way analysis of variance, Pearson correlation coefficient, and multiple regression. Results: Participants were at average 18.25±8.28 weeks gestation, 56% were in the second trimester, 31% had one or more health issues in the current pregnancy (e.g., hyperemesis gravidarum), and 76% were first-time mothers. Participants had moderate levels of pregnancy stress (mean, 23.09±7.11 points out of 48) and maternal knowledge (mean, 14.42±4.67 points out of 21), whereas social support was somewhat high (mean 45.88±7.81 points out of 60). Pregnancy stress was weakly negatively correlated with social support (r=-.37, p<.001). Main source of pregnancy information (β=-.21, p=.011), marital satisfaction (β=-.18, p=.036), and social support (β=-.19, p=.038) were identified as significant factors affecting pregnancy stress, and these variables had an explanatory power of 22.7% for pregnancy stress. Conclusion: Based on these findings, nurses should assess pregnancy-related stress during pregnancy and consider main source of pregnancy information and marital satisfaction when providing education or counseling. Moreover, strategies to reduce pregnancy stress through social support are needed to improve the quality of life for pregnant women.

An explanatory model of quality of life in high-risk pregnant women in Korea: a structural equation model

  • Purpose: This study aimed to develop and validate a structural model for the quality of life (QoL) among high-risk pregnant women, based on Roy's adaptation model. Methods: This cross-sectional study collected data from 333 first-time mothers diagnosed with a high-risk pregnancy in two obstetrics and gynecology clinics in Cheonan, Korea, or participating in an online community, between October 20, 2021 and February 20, 2022. Structured questionnaires measured QoL, contextual stimuli (uncertainty), coping (adaptive or maladaptive), and adaptation mode (fatigue, state anxiety, antenatal depression, maternal identity, and marital adjustment). Results: The mean age of the respondents was 35.29±3.72 years, ranging from 26 to 45 years. The most common high-risk pregnancy diagnosis was gestational diabetes (26.1%). followed by preterm labor (21.6%). QoL was higher than average (18.63±3.80). Above-moderate mean scores were obtained for all domains (psychological/baby, 19.03; socioeconomic, 19.00; relational/spouse-partner, 20.99; relational/family-friends, 19.18; and health and functioning, 16.18). The final model explained 51% of variance in QoL in high-risk pregnant women, with acceptable overall model fit. Adaptation mode (β=-.81, p=.034) and maladaptive coping (β=.46 p=.043) directly affected QoL, and uncertainty (β=-. 21, p=.004), adaptive coping (β=.36 p=.026), and maladaptive coping (β=-.56 p=.023) indirectly affected QoL. Conclusion: It is essential to develop nursing interventions aimed at enhancing appropriate coping strategies to improve QoL in high-risk pregnant women. By reinforcing adaptive coping strategies and mitigating maladaptive coping, these interventions can contribute to better maternal and fetal outcomes and improve the overall well-being of high-risk pregnant women.

  • In recent year, problems caused by the abuse of drugs hove been analyzed in many cases, especially women's pregnancy. The purchasing of drugs without prescription, the misunderstanding of symptoms of pregnancy(such os vomiting, headache) os other ilnnesses, taking medicine during the pregnancy because of a chronic disease has caused many unfortunate cases Apart from these cases, pregnant women may take several medicines such as anodyne, tranquilizers, hypnotics, and diuretics which also cause critical situations. According to Piper and his colleagues 1987 study, in overage, pregnant women in the United Slates intake 3.1 kinds of additional dregs other than prenatal vitamins and mineral supplements. In those cases, both pregnant women and physicians anticipate inborn deformity. Most drugs which have whole body effects get to the unborn child via the placenta, however, many of these drugs do not have adverse effects(Shepard 1986, 1989). In general, drugs present a stronger effect to unborn children than they do to pregnant women due to the baby's excretion and to the drug metabolism that occurs in the mother's body through the placenta. The effects of dregs on unborn children show different results. depending upon the type of drug, dosage, characteristics. gestational weeks, genetic characteristics of the mother an baby, and many other environmental factors.

보건소에서의 임신부 영양교육 프로그램 운영 실태와 요구도 조사 (A Study on the Actual Conditions and Needs for Nutrition Education for Pregnant Women in Health Centers)

  • This study was conducted to investigate actual conditions and needs of nutrition education in order to develop a nutrition education program for pregnant women in health centers. The questionnaires were mailed to 245 health centers and 146 questionnaires were returned. Most health centers(76%) had nutrition education program for pregnant women. About 63% of supervisors were the nurses and 43% of educators were dieticians. The teaching method which was used most frequently was lecturing(34%). Teaching material which was used most frequently was material brought by invited speakers(31%). The subjects of education were the relationship between nutrition for pregnant women and the baby's health(19%), dietary guide and directions for pregnancy(19%), nutrient supplement for pregnant woman(17%), weight gain during pregnancy(16%), abnormal symptoms of pregnancy and health(15%), pregnancy complications and health(13.0%), and others. These subjects were the same ones which educators thought were needed in education. Important success factors in education were giving accurate information and guide and practice, while failure factors were lack of proper space, lack of practice, and others. Lack of a standardized nutrition education program was the biggest barrier to running a program. The subjects which were taught and the needs in nutrition education were significantly different according to respondents' age, educational level, job position, and residence of health center. Therefore, a standardized program, proper space for practice, and professional educators are needed to promote the effectiveness of nutrition education.

서울시 일부 임산부 모발중의 수은농도에 관한 조사 연구 (Mercury Concentrations in Pregnant Women발s Hair in the Seoul Area)

  • Fifty samples of pregnant women's hair during December 1990~April 1991 in the Seoul area were analyzed for mercury. Mean mercury concentrations for pregnant women were 0.65$\mu$g/g and varied from 0.21$\mu$g/g to 2.44$\mu$g/g. The differences were believed to be due to diet, living area, and personal activity. Mean mercury concentrations were significantly different for the group that at twice a week(0.67$\mu$g/g) and once or less a week(0.55$\mu$g/g). Mean mercury concentrations were compared with the THI (Todai Health Index) Scores. The THI results suggested that the higher scores in MENT(Mental irritability) item had the higher mean mercury concentrations. The factors affecting mercury concentrations of pregnant women's hair must be considered further study. However, hair analysis seemed potentially useful in environmental medicine but its use in clinical investigation for basic diagnosis of mercury poisoning.

  • Purpose: To provide accurate information on induced labor and find strategies to enhance women's childbirth satisfaction. Methods: Participants were pregnant women expected to have normal vaginal delivery. A total of 113 women with induced labor and 61 women with spontaneous labor were surveyed. Data were collected using a questionnaire and electronic medical records. Results: The following variables related to labor progress showed significant differences between the induced labor group and the spontaneous labor group: length of the first stage of labor in primigravidas, use of analgesic, incidence of uterine hyperstimulation, incidence of fetal distress, and medical treatment for the expectant mother. Delivery type and the incidence of postpartum complications showed significant difference between the two groups. Induced labor women's childbirth satisfaction was mainly affected by the process of labor whereas spontaneous labor women's childbirth satisfaction was affected by the outcome of childbirth. Conclusion: Medical staff should have accurate information on the risk of induced labor and the benefits of a natural delivery. Moreover, medical staff should provide necessary information and environment for women to participate in the decision-making process.

원피스드레스형 임부복의 형태구성요인의 조합에 따른 시각효과 (The Visual Effect in Combination of Details on the Maternity Clothes of One-piece type)

  • The purpose of this study is to evaluate the combination of details on the maternity clothes of one-piece type through visual evaluation which helps compensating pregnant women's body defects for their more attractive fashion styles. The data evaluated by a multiple ranking test were analyzed by mean, paired t-test, general linear models procedure and Duncan's multiple ranged test. The result are as follows : 1) The pregnant woman wearing the one-piece dress with notched collar, pleats and whole button looks longer in lower part of bodies, smaller in upper body, slimmer, have less appeared bust and abdomen, more balanced as a Whole than when wearing others. And also, it makes a pregnant woman be seen more refined and simple. 2) In case of a pregnant woman, a one-piece dress with notched collar, tuck and whole button makes her look longer in neck, narrower in shoulder, and more active than when wearing others. 3) With roll collar, pleats and whole button, it looks taller and more graceful than when wearing others.

임신 말 모체 및 제대혈의 비타민 $B_6$ 농도와 임신결과와의 상관성 (Relationships between Vitamin $B_6$ Status of Maternal-Umbilical Cord)

  • The purpose of this study was to evaluate the concentration of vitamin B6 in 16 pregnant-infant pairs and 15 nonpregnant women and to investigate the relationships between vitamin B6 status of maternal-umbilical cord plasma and pregnancy outcomes. dietary intake was obtained from semiquantitative frequency questionnaire. The daily mean energy and protein intakes were higher than the recommended dietary allowance, while daily vitamin B6 was only 74% of RDA in pregnant and 73% of RDA in nonpregnant women. The main sources of vitamin B6 were vegetables and fruits in pregnant women, while cereal and starch in nonpregnant women. The plasma PLP and PL levels of pregnant women were 14.85nmol/l and 20.56nmol/l, significantly lower than those of nonpregnant women. the PLP/PL ratios of pregnant and nonpregnant women were 1.65 and 0.33, indicating that the levels of vitamin B6 was altered during pregnancy. The PLP and PL levels of umbilical cord plasma were 63.55nmol/l and 32.25nmol/l, respectively. The vitamin B6 levels of umbilical cord plasma were significantly higher than that of maternal plasm. This finding indicates that the uptake of vitamin B6 in the fetus may be due to an active placental transport mechanism. The PLP level of maternal plasma correlated positively with that of umbilical cord plasma, showing the PLP concentration of umbilical cord plasma is affected by maternal vitamin B6 status. The maternal plasma PL level showed a positive correlation to infant birth weight. The positive association has bee also found between plasma PL level of umbilical cord and Apgar 1 min score.

Lifestyle, nutrient intake, iron status, and pregnancy outcome in pregnant women of advanced maternal age

  • The purpose of this study was to investigate how advanced maternal age influences lifestyle, nutrient intake, iron status, and pregnancy outcomes in pregnant women. The subjects of this study were 112 pregnant women who were receiving prenatal care at gynecologists located in Seoul. The subjects were divided into two groups according to their ages: those over age 35 were the advanced age group of pregnant women (AP) and those under age 35 were the young age group of pregnant women (YP). General factors, nutrient intakes, iron status, and pregnancy outcomes of the two groups were then compared. It was found that 72.5% of the YP group and 51.2% of the AP group had pre-pregnancy alcohol drinking experience; indicating that the YP group had more pre-pregnancy alcohol consumption than the AP group (P<0.05). The only difference found in nutrient intake between the two groups was their niacin intakes which were $16.83{\pm}8.20\;mg$/day and $13.76{\pm}5.28\;mg$/day, respectively. When gestational age was shorter than 38.7 weeks, the average infant birth weight was $2.95{\pm}0.08\;kg$, and when gestational age was longer than 40 weeks, it averaged at about $3.42{\pm}0.08\;kg$. In other words, as gestational age increased, infant birth weight increased (P<0.0001), and when maternal weight increased more than 15 kg, the infant birth weight increased significantly (P<0.05). In conclusion, in order to secure healthy human resources, with respect to advanced aged women, it is necessary to intervene by promoting daily habits that consist of strategic increases in folate and calcium intake along with appropriate amounts of exercise.
