The Effects of a Epilepsy Education Program on Self Efficacy and Self Management in Patients with Epilepsy (간질교육 프로그램이 성인 간질환자의 자기효능과 자기관리에 미치는 효과)

  • The purpose of this study was to test the effects of a epilepsy education program as a nursing intervention for patients with epilepsy. A quasi treatment research (non equivalent control group pretest-posttest design) was used in this study. The subjects were 40 epilepsy patients visiting an outpatient department of a general hospital in Daegu city.(treatment group : 20 patients, control group : 20 patients). The study was carried out from June, 1998 to September, 1998. Data was collected before the education program (pretest), immedately after(posttest 1) and 4 weeks later(posttest 2) and were analyzed with repeated measures ANOVA, t-test, Chi-sqare test and Pearson correlation coefficient. The results are as follows : There was a significant difference in epilepsy self efficacy between two groups(F=26.27, p=.000). There was a significant difference according to pretest, posttest 1 and posttest 2(F=111.20, p=.000), and interaction effect between treatment and time(F=109.42, p=.000). There was a significant difference in epilepsy self management between two groups(F=78.02, p=.000). There was a significant difference according to pretest, posttest 1 and posttest 2 test(F=94.02, p=.000), and interaction effect between treatment and time(F=88.14, p=.000). There was a significant correlation(r=.76, p=.000) between epilepsy self efficacy and epilepsy self management. These results suggest that a epilepsy education program is effective ill promoting self efficacy and self management of the patient with epilepsy. Thus this program can be recommended as an effective nursing intervention for the epilepsy patients.

The Effects of a Cardiac Rehabilitation Educational Program on Anxiety, Health Compliance and Quality of Life of the Patient with Cardiac Valve Replacement (심장재활교육이 심장판막대치술 환자의 불안, 건강행위 이행 및 삶의 질에 미치는 영향)

  • The purpose of this study was to determine the effects of an rehabilitation educational program on anxiety, health compliance and quality of life of the patient with cardiac valve replacement. The 80 Subjects for the study were surveyed at the outpatient clinic in S medical center in Buchon City in Korea. Among them, 40 patients had been taught about the cardiac rehabilitation education program and 40 patients had not been taught while they were in the hospital. Those 2 groups of patients were surveyed with the instruments of Spilberger's anxiety scale, and the questionnaire of health compliance and quality of life. The data were analyzed by using $X^2$-test, t-test, Pearson's correlation. The results of this study were as follows: 1. Anxiety in educated group was significantly decreased (t=-2.60, p= .011) comparing to u n edu cat ed group. 2. The health compliance in educated group was significantly in creased (t=2.09, p= .040) comparing to uneducated group. 3. The quality of life of the two groups of pa tients was not different significantly (t=1.83, p= .072). 4. The correlation between anxiety and health compliance was significantly reversed (r=- .461, p= .000). The correlation between anxiety and quality of life was significantly reversed (r=- .654, p= .000). The relationship between health compliance and quality of life revealed significant (r=.438, p= .000). Considering above results, the cardiac rehabilitation educational program for the patients with the cardiac valve replacement was strongly recommended to be used.

Perceptions of Anticoagulation Therapy and Medication Adherence among Patients taking Warfarin (와파린 복용 환자의 항응고요법에 대한 인식과 약물복용 이행)

  • Purpose: This study was conducted to measure medication adherence and perceptions of anticoagulation therapy and its relationship to patients taking warfarin. Methods: in a cross-sectional survey, 150 patients taking warfarin who had visited an outpatient clinic of a cardiovascular center in Seoul were included as research subjects. Medication adherence was measured using a visual analogue rating scale (VAS). Barriers and attitudes toward adherence, and oral anticoagulation knowledge, were measured using a structured questionnaire. Participants' medical records also were reviewed to identify clinical characteristics. Results: About one third of the participants (30.7%) reported medication adherence as 100%. The major barriers to adherence were identified as "Forgetting the time of medication" and "Not carrying their medication". Overall attitudes toward medication adherence were high, but oral anticoagulation therapy knowledge was low. To determine the relationship of medication adherence and perceptions of anticoagulation therapy, participants were stratified into three groups, based on their medication adherence levels (high adherence, moderate adherence, low adherence). Participants in the high adherent group more likely to be older (OR: 1.04, 95% CI: 1.005-1.071) and to have positive attitudes toward medication adherence (OR: 1.12, 95% CI:1.013-1.229) compared to the other two groups. Conclusion: The results show that age and attitude exerts significant influence on medication adherence in patients taking warfarin.

How Long the Effect of Social Support Would Be Continued for the Patients with Hypertension? (사회적지지 요법이 고혈압 환자 역할 행위 이행에 미친 효과의 지속에 관한 연구II)

  • The purpose of this study was to identify the effect of social support revealed in the time duration of sick role behavior compliance on the patients with hypertension using Quasiexperimental research design. Data collection was made through the interview survey technique from the hypertensive patients who received social support intervention (experimental group, n=41) and from those who were not exposed to the intervention(control group, n= 34). The subjects were registered in the cardiovascular outpatient clinic at the Chonnam National University Hospital from June 3, 1996 to November 30, 1997. $\chi^2$-test or t- test, Repeated measures ANOVA were utilized in the data analysis. The results were as follows: 1. The effect of social support intervention on sick role behavior compliance was significant in 1 month(F=69.17, p=.000), 6 months (F=11.51, p=.001), and 12 months(F=.07, p=.789) and between two groups(1 month; F=153.70, p=.000, 6 months; F=13.94, p=.000, 12 months; F=6.72, p= .011). 2. The effect of social support intervention on blood pressure was not significant through all the periods of time (F=1.21, p=.274) between the two groups(F=.12, p=.732). In conclusion, it was showed that social support had an effect on sick role behavior compliance and the effect of social support continued for twelve months(F= 10.03, p=.002) However, the score of compliance tends to decrease after 6 months of intervention. Therefore, this study indicated that social support re-intervention would be needed between six and twelve months.

Psychological Adaptation in Pregnancy and Perception of Birth Experience (임부의 사회심리적 적응과 분만경험 지각에 대한 연구)

  • The purposes of study were to investigate the prenatal psychological adaptation and the perception of birth experience, and to identify the relationship between them. The subjects consisted of 162 women who visited the obstetrical outpatient clinic for prenatal examinations and who delivered the in babies at SNUH during the period from June 20 to August 10, 1990. The tools used for measurement were Lederman's Prenatal Self Evaluation Questionnaire and Marut & Mercer's scale of the Perception of Birth. The results are summarized as follows : 1. The orders and item means of psychosocial adaptation in pregnancy were the Acceptance of pregnancy(1.58). Identification of motherhood role(1.63). Relationship with husband(1.65) and Relationship with mother(1.67). The preparation for labor, concern for wellbeing of self and baby, and fear of pain, helplessness and loss of control were found to be less adaptive. 2. The level of the perception of the birth experience was mid-range(item mean : 3.22). The score of the perception of birth experience for primiparas was higher than for multiparas. However there was not a significant difference the groups. There were significant differences in the perception of the birth experience between certain general characteristics, namely, sex of the baby(p<0.05), type of delivery(p<0.005), and type of anesthesia(p<0.005). 3. There were significant differences in the perception of the birth experience between the groups below the mean and above the mean of concerti for wellbeing of self and baby, Fear of pain, Helplessness and loss of control, Relationship with husband and Identification of motherhood role (p<0.05). The perception of the birth experience was predicted by Fear of pain, Helplessness and loss of control (11%), Type of Delivery(6%), Concern for wellbeing of self and baby(3%), Preparation for labor(1%), sex of baby(1%), Relationship with mother(1%), Parity(1%) and Identification of motherhood role(1%). The Childbirth education should be revised to improve the psychosocial adaptation in pregnancy.

Safety and Availability of Monitored-Anesthesia Care using Propofol during Implant Surgery of the One-day Admission Patients (당일 입원 환자의 치과 수술 시 Propofol을 이용한 Monitored-Aesthesia Care (MAC)의 안정성 및 유용성에 관한 연구)

  • Background: Propofol has been used extensively for short-acting intravenous sedative agent during monitored anesthesia care (MAC). This study was designed to evaluate the safety and availability of MAC using propofol in implant surgery of the one-day admission patients. Methods: In this study, subjects were divided into two groups according to ASA physical status. The heart rate, blood pressure, peripheral oxygen saturation and ECG of a patient were estimated under MAC by an anesthesiologist and the vital signs were recorded in recovery room periodically afterwards. The subjective satisfaction with regard to outpatient ambulatory surgery under MAC procedure was evaluated the next day. Results: Systolic and diastolic blood pressure were significantly decreased during MAC in ASA I group, but other remarkable changes in vital sign were not observed. There was no significant difference in pain and anxiety level between ASA I and ASA II, III group. Satisfaction rate was high in both groups. Conclusion: Monitored-Anesthesia Care using propofol during implant surgery of the one-day admission patients might be safe and available procedure because heart rate, blood pressure and oxygen saturation are stable before and during surgery, and adequate control of pain and anxiety is supported.

Feasibility of Early and Repeated Low-dose Interscalene Brachial Plexus Block for Residual Pain in Acute Cervical Radiculopathy Treated with NSAIDS

  • Background: To improve residual pain management in acute cervical radiculopathy treated with NSAIDs, the feasibility of early and repeated low-dose interscalene brachial plexus block (IS-BPB) needs to be assessed. Methods: This was a prospective study on patients receiving NSAIDs (loxoprofen) for cervical radiculopathy of ${\leq}2$-week onset. Pain was assessed using the visual analogue scale (VAS). A low-dose ultrasonography (USG)-guided IS-BPB (dexamethasone [1.65 mg; 0.5 ml] and mepivacaine [1%; 3.0 ml]) was performed at baseline and weekly thereafter for 4 weeks in an outpatient setting for the intervention group. All patients were evaluated using a visual satisfaction score (VSS) at week 4. Patients with baseline VAS scores < 70 (mild to moderate pain; MM group) and ${\geq}70$ (severe pain; SE group) were compared to the controls receiving NSAIDs. Results: A total of 316 IS-BPBs were performed in the intervention group. There was a significant difference in the decline in the VAS from week 0 to week 3 in the MM and SE groups (P < 0.05); however, from week 3 to week 4, the therapeutic effect exhibited no significant difference. Thirteen patients at week 2 (15.5%; MM: 27.7%; SE: 0%), 43 at week 3 (51.2%; MM: 83.0%; SE: 10.8%), and 47 at week 4 (56.0%; MM: 85.1%; SE: 18.9%) achieved a VAS score of ${\leq}20$. Patient satisfaction was high, and the decrease in VAS scores in both groups was significant (P < 0.05) compared to the controls. Conclusions: Weekly, low-dose, USG-guided IS-BPB can be implemented for early pain relief in acute cervical radiculopathy, with high patient satisfaction.

Development and Psychometric Evaluation of the Ischemic Stroke Distress Scale (ISDS) (허혈성 뇌졸중 환자의 디스트레스 측정도구 개발 및 평가)

  • Purpose: This study aimed to develop a scale to measure distress in patients with ischemic stroke and verify its validity and reliability. Methods: Preliminary items were developed from literature review and in-depth interviews. The final preliminary scale was confirmed through a content validity test of eight experts and a preliminary survey of 10 stroke patients. The participants for psychometric testing were 305 stroke patients in the outpatient clinic. Validity and reliability analyses included item analysis, exploratory and confirmatory factor analysis, convergent validity, known-group validity, and internal consistency of the scale. Results: The final scale consisted of 17 items and 3 factors. The three distinct factors were 'self-deprecation, worry about future health, and withdrawal from society' and this structure was validated using a confirmatory factor analysis. Convergent validity was supported by comparison with the Center for Epidemiologic Studies Depression Scale (r = .54, p < .001) and Brief Illness Perception Questionnaire (r = .67, p < .001). Known-groups validity was verified by dividing groups according to 'duration since diagnosis' (t = 2.65, p = .009), 'presence of sequela' (t = 10.16, p < .001), and 'awareness of distress' (t = 12.09, p < .001). The internal consistency of the scale using Cronbach's α for the total items was .93. Conclusion: The Ischemic Stroke Distress Scale is a valid and reliable tool that reflects stroke distress effectively. It is expected to be used as a basic tool to develop various intervention strategies to reduce distress in ischemic stroke patients.

Pain Relief Efficacy of Ibuprofen Releasing Foam Dressing (Biatain Ibu®) on Outpatient Patient with Partial Thickness Burn Wound (부분층화상을 입은 외래 환자에서 이부프로펜 방출성 드레싱 제재(Biatain Ibu®)의 창상부위 통증의 경감 효과에 대한 연구)

  • Purpose: Pain management in burn treatment is important in improving wound healing and quality of life. Ibuprofen is a proven pain relieving agent in patients with partial thickness burn by intraveous injection. The purpose of this study is to evaluate the efficacy of Biatain Ibu® (polyurethane foam containing ibuprofen) in pain control for outpatients with partial thickness burns. Methods: A prospective randomized clinical trial was performed in outpatients with partial thickness burn from August 1, 2017 to July 31, 2018. Acute pain, chronic pain, complications, days for re-epithelialization and patient's satisfaction were compared between Biatain Ibu® and Biatain® groups. Results: A total of 20 patients (Biatain Ibu®, n=10; Biatain®, n=10) were assessed in the trial. On Burn days 3, 5, 7, 11, 13, and 15, the acute pain levels were significantly lower in the Biatain Ibu® group than in the Biatain® group. Complications, chronic pain levels and days for re-epithelialization were not significantly different between the two groups. Patient's satisfaction was not statistically significant but was higher in the Biatain Ibu® group. Conclusion: Biatain Ibu® is effective in relieving pain in outpatients with partial thickness burn without decreasing patient satisfaction, wound healing ability or developing any complications.

The Statistical Analysis on Outpatients Who Had Visited Chuncheon Oriental Medical Hospital with Otolaryngology and Respiratory Diseases (춘천한방병원에 내원한 이비인후과 및 호흡기계 질환 외래 환자의 통계적 고찰)

  • Objective : This study was performed to investigate the prevalence of otolaryngology and respiratory diseases and characteristics of the outpatients who had visited Chuncheon oriental medical hospital which located in Gangwon province with the diseases. Methods : We analyzed data from 627 patients, who had visited to the Department of ophthalmology & otolaryngology & dermatology of Chuncheon oriental medical hospital from November 1st, 2007 to May 31th, 2010. The data were analyzed and demonstrated by age, gender, season, period between the onset of disease and the first examination, times of treatment, and treatment methods. Results : The results of this study are summarized as follows. 1. Among the 627 outpatients, male patients outnumbered female patients, and the most frequent age group was 0-10 year-old group(24.9%) and it is followed by 31-40 and 41-50 year-old groups. The majority of outpatients, 79.7% of them were from Chuncheon, and Hongcheon and Gapyeong residents were respectively 4.7% and 3.8% of the patients. 2. The shares of outpatients by department were 47.4% in Rhinology disease, 29.8% in respiratory disease, 14.5% in Otology disease, and 8.3% in Laryngology disease. The seasons with the most frequent prevalence by department were spring and winter for otology group, autumn and spring for rhinology group, summer and spring for laryngology group, and winter and spring for respiratory disease group. Overall, most outpatients had visited the hospital for 1-5 times or 6-10 times. Acupuncture and herbal medicine were the most frequent treatment methods that these patients received. 3. The most prevalent diseases were rhinitis, common cold, tinnitus, and sinusitis. The most frequently visited age groups for tinnitus and hearing loss were 41-50 and 51-60 year-old groups, and for rhinitis and sinusitis 0-10 and 11-20 year-old groups. Irritation in larynx was almost evenly prevalent among 31-40, 41-50 and 51-60 age groups. Regarding the period between the onset of disease and the first medical examination, outpatients with tinnitus, rhinitis, sinusitis, and irritation in larynx came to the hospital after one year since the onset and outpatients with common cold first visited the hospital within one week since the onset of disease. Conclusions : This study shows that the most frequent diseases were rhinitis, common cold, sinusitis, tinnitus, and cough. In order to do better treatment for these diseases in the future, there is a need to investigate long-term, multifaceted studies, connected studies with other hospital in Gangwon province.
