Four cases of vitiligo patients treated by Oriental medical treatment who have experienced Excimer Laser treatment (엑시머레이저 치료를 경험했던 백반증 환자가 한방복합치료와 엑시머레이저 병행치료로 호전된 치험 4례)

  • Purpose: This study aims to prove effects of an oriental medical complex therapy on vitiligo patients who have experienced the Eximer laser therapy. Method: The study conducted its investigation targeting these four patients who have not been observed with any satisfactory reaction to the Eximer laser therapy. The study analyzed the findings through the comparison on the pictures before and after the therapy. Result: After comparing the pictures, the study confirmed positive changes in the conditions of the patients. The oriental medical complex therapy turned out to be effective enough to treat the patients who have gone through the Eximer laser therapy before. Conclusion: Such result leads the study to a conclusion that the oriental medical complex therapy can be an effective way to treat these patients who do not have a satisfactory reaction to the Eximer laser therapy. In addition, the author of the study suggests that more clinical trials should be conducted.

  • Objective : This study was case report to show the good effects of herbal medicine (Daekumeum-ja) and acupuncture complex therapy for central pontine myelinolysis (CPM). Method : A 44-year-old man suffering from CPM was chronic alcoholic without electrolyte disturbance. He showed symptoms of dysarthria, dysphagia, dizziness, weakness of lower extremities and gait disturbance. CPM was diagnosed at his former hospital by MR imaging. He had no positive response to western treatments. We treated the patient with herbal extract (Daekumeum-ja) and acupuncture. Results : After our treatments, there was no change of Brain CT. But overall symptoms of CPM were improved. Conclusion : This study suggests that herbal medicine (Daekumeum-ja) and acupuncture complex therapy is probably effective in the treatment of CPM.

  • This is a case report about left leg radiating pain treated with complex traditional Korean medical intervention, while the case did not responded to prolotherapy and extracorporeal shock wave treatment for lumbar herniated intervertebral disc (HIVD). The patient was diagnosed with a piriformis problem after a physical examination and was treated with lumbar and piriformis muscle treatment. After 9 sessions of treatment for 33 days, Numeric Rating Scale (NRS) for pain has been decreased from 8 to 3. Further studies with more cases and longer observation period should be conducted to provide evidence for optimum acupuncture procedure considering its effectiveness, safety and patients' compliance altogether.

  • Objectives : To report the R/O CRPS patient after Korean medicine treatment, suffering severe buring pain and insomnia. Methods : The patient was a 56-year-old female patient and treated with herbal medicines and acupuncture in 32 days. The treatment effect was evaluated by measuring VNRS of pain, sleeping time and change in mood status. Results : The Korean Medical therapy had good effect on the patient. Especially, relaxing mental treatment is effective in improving and maintaining symptoms. Conclusions : The Korean medical treatments appeared to be effective in reducing R/O CRPS symptoms. Further clinical research of patients with CRPS is needed.

The Effect of Needle-Embedding Therapy on Peripheral Facial Paralysis (말초성 안면신경마비에 대한 매선요법 복합치료 효과)

  • Objective : This study was performed to investigate the effect of Needle-Embedding Therapy on peripheral facial paralysis. Method : We investigated 60 cases of patients with peripheral facial paralysis, and devided patients into two groups : We treated one group by complex korean medical treatment with Needle-Embedding therapy, and did the other group by complex korean medical treatment without Needle-Embedding therapy. Yanagihara grading system at baseline and final were used for evaluating the effect of the treatment. Results : 1. In Needle-Embedding therapy group and non Needle-Embedding therapy group, compared with baseline, at final, Y score was significantly increased.2. At final, there was significant difference in improvement between Needle-Embedding therapy group and non Needle-Embedding therapy group. Conclusions : Needle-Embedding therapy seem to be effective to improve symptoms of peripheral facial paralysis. Further studies will be needed to identify the beneficial of Needle-Embedding therapy on peripheral facial paralysis.

  • Objectives : The purpose of this study was to assess the effectiveness of integrative Korean Medical treatments for patients with L-spine disk herniation that showed no response to Epidural Steroid Injections (ESI). Methods : In this study, we reviewed the medical records of ten patients who showed no improvement or relapsed after ESI. The patients each received more than three weeks of integrative treatments at Mokhuri Neck&Back Hospital. The Visual Analog Scale (VAS) and Oswestry Disability Index (ODI) was measured before and after treatment. A statistical analysis to compare before and after treatments was assessed using paired t-test the SPSS 12.0 Windows program. Results : After three weeks of treatment, patients showed a significant decrease in VAS and ODI scores compared to before receiving treatment. VAS scores decreased from $5.3{\pm}1.25$ to $1.9{\pm}0.99$ (p<0.001), and ODI scores decreased from $59.8{\pm}22.2$ to $32.7{\pm}14.11$ (p<0.001). Conclusions : Complex Korean Medical treatment showed effectiveness in the treatment of L-spine HIVD patients that showed no response to ESI, and more objective research is needed.

Anti-neoplastic and Anti-inflammatory Effects of Single and Mixed Extracts of Ulmus Davidiana and Oldenlandia Diffusa on Azoxymethane/dextran Sodium Sulfate-induced Colonic Neoplasms (AOM/DSS 유발 대장암에 대한 유근피(楡根皮)와 백화사설초(白花蛇舌草) 단일 및 배합 추출물의 항암 및 항염 효과)

  • Objectives: The aim of this experimental study was to evaluate the anti-neoplastic and anti-inflammatory effects of single and mixed extracts of Ulmus davidiana (UD) and Oldenlandia diffusa (OD) on azoxymethane/dextran sodium sulfate (AOM/DSS)-induced colonic neoplasms in mice. Methods: AOM/DSS induces colitis-associated colonic neoplasms in mice. Mice were divided into seven groups: normal-no inducement and no treatment; control-colonic neoplasms with no treatment; UD-colonic neoplasms and treatment with UD; OD-colonic neoplasms and treatment with OD; UD1+OD1-colonic neoplasms and treatment with UD1 and OD1. UD1+OD2-colonic neoplasms and treatment with UD1 and OD2; UD2+OD1-colonic neoplasms and treatment with UD2 and OD1. Single and mixed preparations of UD and OD were applied to mice for six weeks. The colon length and weight and histopathologic changes of colon tissue were observed. Serum pro-inflammatory cytokines, including tumor necrosis factor-alpha ($TNF-{\alpha}$) and interleukin-6 (IL-6), were determined by enzyme-linked immunosorbent assay. The mRNA expression levels of Bax, Bcl-2, and interferon-gamma ($INF-{\gamma}$) were measured by RT-PCR. Results: The colon length was significantly increased in OD, UD1+OD2, and UD2+OD1 mice, and the colon weight was significantly decreased in OD and UD1+OD2 mice. The morphological change of colon epithelial cells was more suppressed in complex-treatment groups than in single-treatment groups. The inhibitory effect on inflammatory cell invasion was especially shown in UD1+OD2 mice. The serum level of the pro-inflammatory $TNF-{\alpha}$ was decreased in all complex-treatment groups, and the IL-6 level was decreased in UD1+OD1 mice. Single-treatment groups had an increase in the mRNA expression of the pro-apoptosis regulator Bax, and UD2+OD1 decreased the mRNA expression of the anti-apoptosis regulator Bcl-2. The mRNA expression of $INF-{\gamma}$ associated with inflammation was decreased in OD and UD1+OD2 mice. Conclusions: This study suggests that single and mixed extracts of Ulmus davidiana and Oldenlandia diffusa have anti-neoplastic and anti-inflammatory effects on AOM/DSS-induced colonic neoplasms in mice. Therefore, we conclude that UD, OD, and a mixture of UD and OD are potential therapeutic agents for colitis-associated colonic neoplasms.

  • Background : There has been a gradual increase in the number of newly diagnosed cases of Mycobacterium avium complex (MAC) pulmonary disease. However, the optimal therapeutic regimen for the disease has not yet established and there is no report about the treatment outcome of MAC pulmonary disease in Korea. This study examined the effect of clarithromycin-based regimen in patients with pulmonary MAC disease without a HIV infection. Materials and Methods : Fifty-six patients with pulmonary MAC disease were diagnosed according to the American Thoracic Society criteria from January 2000 to December 2003 at this hospital. Of these patients, 15 were treated with clarithromycin, rifampin, and ethambutol for more than 6 months, together with streptomycin initially (first 6 months) in 8 patients. Results : Six months after the treatment, the sputum cultures converted from positive to negative in 8 patients (53%) and the radiological findings improved in 10 (67%). At 12 months 4 patients (44%) achieved sputum negative conversion and 6 patients out of 9 patients (67%) who were treated for more than 12 months showed radiological improvement. Overall, the sputum findings converted to negative in nine patients (60%) who underwent medical treatment. A pulmonary resection was successfully performed in one patient. Only one patient discontinued the treatment due to side effects such as gastrointestinal intolerance and optic neuritis. Conclusion : A combined regimen containing clarithromycin is relatively safe and tolerable even in the elderly outpatients. However, the results of this combined chemotherapy were unsatisfactory and new companion drugs for MAC pulmonary disease are needed. A resection may be considered for localized disease.

A Survey on Korean Medicine Doctors' Recognition for Developing Korean Medicine Clinical Practice Guideline for Growth Disorders (성장장애 한의표준임상진료지침 개발을 위한 한의사 인식조사)

  • Objectives The purpose of this study is to provide data for development of the korean medicine (KM) clinical practice guidelines (CPG) for growth disorder (GD) by identifying the awareness and knowledge needs of KM doctors (KMD) through online survey. Methods Survey questionnaire was produced by referring to the previous recognition survey studies for clinical KMDs. The survey questionnaire was composed 18 questions regarding the current status of clinical care for GD, clinician's knowledge level about GD, and other details that clinicians use during practice. The survey was conducted from January 2021 to March 2021. An online survey was conducted on 101 KMDs from the association of pediatrics of KM. Results According to the survey, 96 respondents (40.3%) said the causation of GD without growth hormone deficiency needs to be included in CPG. 96 (23.5%) of the respondents wishes to utilize percentile in diagnosis and evaluation of the growth assessment. 24.7% of the clinicians were using the herbal medicine treatment. Currently, when treating with KM for GD, herbal medicine (100%) is the most widely used, followed by acupuncture (77.6%), and moxibustion (36.7%). In terms of a complex treatment, growth therapy efficacy (26.8%) is shown to be the most important factor to consider and needed to be included in CPG for the complex treatment, and diet (22.3%) is also considered to be important in GD. Conclusions In this study, we were able to understand the clinical KMDs' perception of GD, knowledge level, and the requirements in the CPG. The results of this study will provide the basic data for development of CPG for GD.

A Review Studies on the Treatment of Anorexia Nervosa in Traditional Chinese Medicine (신경성 식욕부진증의 치료에 관한 임상연구 동향 -중의학 논문을 중심으로-)

  • Objectives: This study was performed to review the research trends in the treatment of anorexia nervosa (AN) in Traditional Chinese Medicine (TCM). Methods: Studies were searched through The China National Knowledge Infrastructure (CNKI) database. The search term for the subject were 'Eating Disorder', and 'Anorexia Nervosa'. Clinical research that conducted TCM treatment for AN patients were included. Studies were analyze according to study design, therapy method, diagnostic criteria, population and intervention. Results: 23 articles were selected. According to study design, studies were classified into two randomized controlled trial (RCT), three single group before-and-after studies, and 18 case reports. According to therapy method, they included 19 herbal medicine studies, two acupuncture treatment studies, and two complex treatment studies. The most common pattern identification was 'liver depression with spleen deficiency' (肝鬱脾虛), and the most used herb was 'Root of Bupleurum falcatum' (柴胡). Most of the studies showed effective results in increasing appetite and weight. Conclusions: The results of our study demonstrate that TCM treatments for AN has been effective in alleviating chief symptoms. However, in terms of evidence-based medicine, lack of RCT on TCM treatments for AN and absence of objective diagnostic criteria in case reports are limitations. Therefore well-designed further clinical studies should be performed based on the results of this study are recommended.
