Korean Critical Care Advanced Practice Nurses' Work Experience: A Focus Group Study (중환자전문간호사들의 실무경험: 포커스 그룹 연구)

  • Purpose: This study was aimed to understand and describe the Korean critical care advanced practice nurses' work experience. Methods: Data was collected through 2 focus group interviews that were held in two different university hospitals in Seoul, Korea. All interviews were recorded and transcribed, and data were analysed using modified qualitative content analysis. Results: Ten themes were drawn from data analysis. They are "Various and complex tasks", "Excellency of advanced nursing practice", "Pressure and responsibility accompanied with pride", "Role identity confusion", "Role conflict", "Leaping and evolving role", "Gap between role and outcome evaluation", "Underestimated outcomes", "Where are my outcomes?" "Searching for outcomes: publicize the role". Conclusion: The results of this study help to understand the role of Korean critical care advanced practice nurses and to guide to outcome evaluation of their role effectiveness. Developing qualitative outcome indicators, cost-effectiveness ananlysis of Korean critical care advanced practice nurses' role, and accumulation of evidences through researches on outcome evaluation will be needed to successfully settle down advanced practice nursing in Korea.

Effect of Taking Meal on Pulse Diagnosis in Healthy Subjects (식사가 정상인의 맥에 미치는 영향 분석)

  • The pulse diagnosis studies reported to date has mainly been performed to clinically reveal the pulse wave characteristics according to the specific diseases, whereas no attempts have been made to study the effects on the pulse wave characteristics of the daily activities such as taking meals, exercise, and sleep, etc. This work reports the effect of feeding stimulus on the healthy subjects on the pulse wave pattern which has quantitatively been analyzed using the objective model for the pulse diagnosis in oriental medicine. The pulse waves right before/after the meal and 30 minutes after the meal were measured using the pulse analyzing equipment (3D-Mac, Daeyo Medi, Korea) and at the same time oriental medicine doctors' diagnoses were given. The pulse parameters obtained from the equipment and clinical records on the subjects were statistically processed and the variables showing statistically significant differences were analyzed. The results indicate that the pulse pressure, the pulse rate, and the respiratory rate increase while the blood pressure decreases after the meal. For the floating/sinking and the deficient/excess coefficients characterizing the pulse states described in the oriental medicine, the floating/sinking coefficients were observed to decrease whereas the deficiency/excess coefficients increase after the meal. The results indicates that besides the standard bio-indicators like blood pressure and respiratory rate, etc., the pulse wave characterization in terms of the pulse classifications in the oriental medicine using the floating/sinking, deficient/excess pulse states provide an important piece of biomedical information.

Chloride and lactate as prognostic indicators of calf diarrhea from eighty-nine cases

  • Importance: Deaths due to neonatal calf diarrhea are still one of the most critical problems of cattle breeding worldwide. Determining the parameters that can predict diarrhea-related deaths in calves is especially important in terms of prognosis and treatment strategies for the disease. Objective: The primary purpose of this study was to determine mortality rates and durations, survival status, and predictive prognosis parameters based on vital signs, hematology, and blood gas analyses in neonatal diarrheic calves. Methods: The hospital automation system retrospectively obtained data from 89 neonatal diarrheic calves. Results: It was found that 42.7% (38/89) of the calves brought with the complaint of diarrhea died during hospitalization or after discharge. Short-term and long-term fatalities were a median of 9.25 hours and a median of 51.50 hours, respectively. When the data obtained from this study is evaluated, body temperature (℃), pH, base excess (mmol/L), and sodium bicarbonate (mmol/L) parameters were found to be lower, and hemoglobin (g/dL), hematocrit (%), lactate (mmol/L), chloride (mmol/L), sodium (mmol/L) and anion gap (mmol/L) parameters were found to be higher in dead calves compared to survivors. Accordingly, hypothermia, metabolic acidosis, and dehydration findings were seen as clinical conditions that should be considered. Logistic regression analysis showed that lactate (odds ratio, 1.429) and CI- (odds ratio, 1.232) concentration were significant risk factors associated with death in calves with diarrhea. Conclusions and Relevance: According to the findings obtained from this study, the determination of lactate and Cl- levels can be used as an adjunctive supplementary test in distinguishing calves with diarrhea with a good prognosis.

Study on the Relationship between Cold type and Sleep Quality in Koreans (한국 일반인의 한증과 수면의 질과의 상관성 연구)

  • The purpose of this study was to investigate the relationship between sleep quality and sleep duration according to individual characteristics, and the relationship between sleep quality and cold type. A total of 1998 participants aged 30-55 who participated in a community cohort study at KIOM's Oriental Medicine Data Center (KDC) was analyzed. Cold type was diagnosed with a common the Questionnaire of Cold Pattern Identification (8 items). Sleep quality was assessed by the Pittsburgh Sleep Quality Index. The participants in this study were classified into 1193 with cold type and 805 with non-cold type. The poor sleep quality was found in more women than men, and the Body Mass Index (BMI) was higher in the non-cold type. In the cold type group, average sleep quality was poor and sleep duration was short. There was a statistically significant correlation between cold score, sleep quality, and sleep duration. Cold type was related to the sleep quality. The risk of poor sleep quality increased 1.46 with increased cold type (95% CI = 1.16-1.84, p <.001). In particular, it was found that various programs to improve the quality of sleep, focusing on the cold type among personal characteristics, are needed to maintain adequate sleep to improve the sleep quality. In the future, when analyzing the relationship between cold type and sleep quality, it is thought that an attempt to derive other objective quantitative indicators is needed.

Oral spindle cell/sclerosing rhabdomyosarcoma on mandible with anaplastic lymphoma kinase expression mimicking inflammatory myofibroblastic tumor

  • Oral spindle cell/sclerosing rhabdomyosarcoma (SCRMS) with anaplastic lymphoma kinase (ALK) expression is extremely rare, and its diagnosis is very challenging in the absence of clinical or pathological indicators. This case presented with gingival swelling and alveolar bone resorption and was suspected clinically to be periodontitis. A biopsy was performed and, due to immunoreactivity with ALK, the patient was misdiagnosed with inflammatory myofibroblastic tumor. However, based on the combined histological and immunohistochemical features, a revised diagnosis of SCRMS with ALK expression was finally concluded. We believe that this report makes a significant contribution to the precise diagnosis of this rare disease for proper treatment.

The Effect of Bobath Therapy on Balance and Walking in Patients with Stroke (보바스 치료가 뇌졸중 환자에 균형과 보행에 미치는 효과)

  • Purpose : The purpose of this study was to examine the effect of bobath treatment on the balance and gait in adult hemiplegia. Methods : The study was performed with patients of hemiparesis caused by cerebral stroke. The participants were based on random sampling method. The hemiplegia patient received Bobath therapy. Each rehabilitation program lasted 40 minutes a day, 5 days a week, for 6 weeks. Pertinent indicators Berg's balance scale (BBS), gait velocity, and static balance analysis were recorded before and after the programs, as well as every 2 weeks during the rehabilitation programs. Results : There was showed a significantly increase of BBS score. Static open and close showed statistically significant in interaction by time and groups. There was significant difference of gait velocity. Conclusion : These findings in this study that the Bobath therapy was effective therapy in improving dynamic balance and gait velocity.

  • The information relating to the health of person has been increasing. The information is such as medical information and personal health record and the information collected by utilization and dissemination of mobile devices. Therefore, the interest and demand for systems that can integrate and manage the Personal Health Record(PHR) is increasing. Quantity and quality of information that is collected from the patient can have a major impact on the diagnosis and treatment of Korean Medicine(KM) in clinical practice. Because closely observe the usual clinical symptoms of patients to utilize the treatment. But if the interview when memories are not sure of the correct answer does not get much easier to find exactly the symptoms. So when recording original symptom(素證) and daily subjective symptom can be helpful for care. Therefore, the personal health care services that can record and manage and own is necessary based on KM. In this paper, we propose Korean Medicine Personal Health Record Platform(KM PHR Platform). We have selected the significant symptoms that mean to the personal records from symptom information required for diagnosis in KM. And classifying and scoring as the symptoms were used as personal health care indicators. And significant symptoms were easily configure a screen that can be recorded. simple operation is recorded as a symptom. It was designed to reflect these functions. So KM PHR Platform helps to Personal health care. Doctor may be able to help in the diagnosis and prognosis observation by reference to shared symptom. We look forward to a variety of health services based on KM using a symptom, a medical record, personal health device information.

Surgical Results of en Bloc Open-door Laminoplasty

  • Objective : The purposes of this study are to evaluate the efficacy of en bloc open-door laminoplasty and to investigate the validity of various factors as prognotic indicators in patients with multisegmental spondylotic myelopathy and ossification of posterior longitudinal ligament[OPLL]. Methods : The authors reviewed 43 cases in whom laminoplasty were performed for cervical myelopathy between January 2000 and December 2002. Clinical symptoms and results were evaluated using the Japanese Orthopaedic Association[JOA] scale. The recovery rate was calculated and then assessed for prognostic factors such as preoperative JOA scores, ages, history of previous trauma, duration of symptoms and signal change in cord on T2-weighted magnetic resonance Image. Results : In cervical stenosis, canal widening of antero-posterior diameter and dimension after laminoplasty is 4.16mm, $87.43mm^2$ and in OPLL is 6.20mm, $117.61mm^2$. In all cases there wasn't neurologic deterioration, mild postoperative complications developed in seven cases. Four patient had a limitation of range of neck motion and the other one showed kyphotic change and another two showed C5 radiculopathy. The recovery rate of JOA score in cervical stenosis and OPLL was 62% and 68% respectively. Duration of symptoms, the severity[preoperative JOA score], and signal change in cord on T2-weighted magnetic resonance image had close relationship to the clinical outcomes. Conclusion : Unilateral en bloc laminoplasty is simultaneous expansile and decompressive method. And preoperative JOA score, symptom duration and high signal intensity on T2-weighted magnetic resonance image can be used to predict prognosis.

  • The management of rectal cancer is a major undertaking. There are currently multiple treatment modalities with variable degrees of complications. Radiotherapy (RT) is one of the more frequently used modalities either on its own or more frequently with chemotherapy mostly before the definitive surgery. The outcome of RT is unpredictable. RT has its serious side effects and there are no guarantees of its usefulness in all patients. This article outlines the effect of RT on the tumor, reviews the various staging systems of responses to RT and present recent evidence of which case is less responsive to such treatments to avoid unnecessary complications.

Prognostic Factors of Patients Requiring Prolonged Mechanical Ventilation in a Medical Intensive Care Unit of Korea

  • Background: We evaluated the clinical outcomes and prognostic factors of patients requiring prolonged mechanical ventilation (PMV), defined as ventilator care for ${\geq}21$ days, who were admitted to the medical intensive care unit (ICU) of a university hospital in Korea. Methods: During the study period, a total of 2,644 patients were admitted to the medical ICU, and 136 patients (5.1%) were enrolled between 2005 and 2010. Results: The mean age of the patients was $61.3{\pm}14.5$ years, and 94 (69.1%) were male. The ICU and six-month cumulative mortality rates were 45.6 and 58.8%, respectively. There were 96 patients with tracheostomy placement after admission and their mean period from admission to the day of tracheostomy was $21.3{\pm}8.4$ days. Sixty-three patients (46.3%) were successfully weaned from ventilator care. Of the ICU survivors (n=74), 34 patients (45.9%) were transferred to other hospitals (not university hospitals). Two variables (thrombocytopenia [hazard ratio (HR), 1.964; 95% confidence interval (CI), 1.225~3.148; p=0.005] and the requirement for vasopressors [HR, 1.822; 95% CI, 1.111~2.986; p=0.017] on day 21) were found to be independent factors of survival on based on the Cox proportional hazard model. Conclusion: We found that patients requiring PMV had high six-month cumulative mortality rates, and that two clinical variables (measured on day 21), thrombocytopenia and requirement for vasopressors, may be associated with prognostic indicators.
