The clinical study on the cased of Herpes Zoster Treated with Korean Oriental Medicine (대상포진 치험 증례 임상보고)

  • Objective : These case study were made to evaluate and observe the treatment for the Herpes-Zoster through the korean oriental medicine. Method : Clinical observation and analysis about 4 cases of Herpes-zoster including the Ramsay's-hunt syndrome had been done the patients of the Sang-Ji Oriental Medicine Hospital. These cases were mainly treated with oriental medicine using the Herb medication, Korean Bee-Venom therapy, Acupuncture and Electro-acupuncture therapy. Acupuncture technic was mainly used Sa-Am acupuncture(Four needle technique). Result : After treatment, all of cases were completely cured without any complication. Conclusion : Based on the clinical results, Korean Oriental Medicine is believed to be effective for treating Herpes-zoster, and further studies should be conducted to provide more valuable information.

A Cytologic Study of Fine Needle Aspiration Biopsy of Salivary Gland Diseases (타액선 질환의 세침흡인생검에 관한 세포학적 연구)

  • Fine needle aspiration biopsy cytology is a widely recognized and useful technique which can provide diagnosis in lesions of the head and neck, enabling appropriate management plans for individual patient to be made. Fifty one fine needle aspirates from salivary gland masses were examined. Four aspirates (8%) were inadequate for examination. Of the remaning 47 samples, 42 cases (82%) were benign lesions which consist of 30 pleomorphic adenoma(58%), 7 inflammatory lesion (14%), 4 Warthin's tumor(8%) and 1 benign lesion(2%). Two cases(4%) were atypical lesions. Three cases(6%) were malignant lesions consisting of 2 adenoid cystic carcinomas(4%) and 1 mucoepidermoid carcinoma (2%). The cytologic diagnoses were compared with the subsequent histologic diagnosis of surgical resected specimen in 24 cases. 19 cases of 21 aspirates from benign tumors were correctly diagnosed by fine needle aspiration cytology, with a specificity of 90%. All 3 aspirates from the 3 patients with malignant tumor were correctly diagnosed by fine needle aspiration cytology, with a sensitivity at 100%. Overall acurracy was 88%. Diagnostic error was encountered in adenoid cystic carcinoma, mucoepidermoid carcinoma and Warthin's tumor Correct histologic diagnosis was made in 86% of benign tumors(84% for pleomorphic adenoma and 100% for Warthin's tumor) and in 100% of malignant tumors.

Review on the Quantitative Standards for Twirling Supplementation and Draining (염전보사의 정량적 기준에 대한 고찰)

  • Objectives and Methods : The aim of this study was to investigate twirling and supplementation and draining by analyzing ancient acupuncture literature and modern research papers, and to discuss the quantitative standards for the procedure. Results : Twirling supplementation and draining techniques were first described in Huang do neijing, after which they were recorded in various other ancient acupuncture texts. Not only were these techniques used in the active treatment of patient historically, but they also form the foundations of modern-day acupuncture treatment. Twirling supplementation and draining techniques described in ancient literatures can be divided into Yin and Yang. Examples of Yang characteristics include Up, Male, Morning, Hands, the governor vessel, Outside, and Left. The left rotation technique is a reinforcing method performed by moving the thumb forward while rotating the needle. Examples of Yin characteristics include Down, Female, Afternoon, Foot, the conception vessel(CV), Inside, and Right. The right rotation technique is a reducing method performed by moving the thumb backward while rotating the needle. Twirling supplementation and draining has been divided for four ways by classical literature and research that worked before. the direction of acupuncture, the stimulus-quantity of acupuncture, the performance of doctor(the uses of both hands, left and right hand) and the feeling of patient. Conclusions : Therefore, continuous research is required to implement Twirling supplementation and draining in a consistent manner. In addition, more active clinical trials using the techniques are necessary as well.

A Total Spinal Anesthesia Developed during an Induction of an Epidural Block -A case report- (경막외차단 유도중 발생한 전척추마취 -증례보고-)

  • Total spinal anesthesia is a well documented serious life threatening complication which results from an attempted spinal or epidural analgesia. We had an accidental total spinal anesthesia associated with a cranial nerve paralysis and an eventual unconsciousness during epidural analgesia. A 45-year-old female with an uterine myoma was scheduled for a total abdominal hysterectomy under the epidural analgesia. A lumbar tapping for the epidural analgesia was performed in a sitting position at a level between $L_{3-4}$, using a 18 gauge Tuohy needle. Using the "Loss of Resistance" technique to identify the epidural space, the first attempt failed; however, the second attempt with the same level and the technique was successful. The epidural space was identified erroneously. However, fluid was dripping very slowly through the needle, which we thought was the fluid from the normal saline which was injected from the outside to identify the space. Then 20 ml of 2% lidocaine was administered into the epidural space. Shortly after the spinal injection of lidocaine, many signs of total spinal anesthesia could be clearly observed, accompanied by the following progressing signs of intracrainal nerve paralysis: phrenic nerve, vagus nerve, glossopharyngeal nerve and trigeminal nerve in that order. Then female was intubated and her respiration was controlled without delay. The scheduled operation was carried out uneventfully for 2 hours and 20 minutes. The patient recovered gradually in th4e reverse order four hours from that time.

A Clinical Study on the case of Fascioscapulohumeral (FSH) Muscular Dystrophy Treated with Traditional Korean Medicine (안면견갑상완형(Fascioscapulohumeral:FSH) 근이영양증 1례에 대한 증례보고)

  • Objective : There was no report on the treatment of Fascioscapulohumeral (FSH) Muscular Dystrophy by Oriental medicine. But the treatment conducted on the patient admitted to the Sangji Oriental Medicine Hospital from January 9, 2001 to February 23, 2001, a significant treatment result was yielded and would like to suggest treatment plan for the future treatments. Methods : Under the assumption that Korean Bee-Venom Therapy may be affective for treating FSH Muscular Dystrophy, the following points were administered : SI10(노유), SI11(天宗), BL23(腎兪), BL26(關元兪), ST36(足三里), LI4(合谷), Liv3(太衝), SI9(肩貞). CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;鹿茸) herbal extract was treated on the other acupuncture points. Sa-Am(Four needle technique) Acupuncture (tonifying SI5 and ST42, sedating GB41 and ST43) was done every day. For herbal medicine, TaeEumIn ChoWiSeungChung-Tang was given based on the constitutional diagnosis. Results : After 7 weeks of treatment, a remarkable improvement was made for facial muscular movement and muscular strength of the scapular and another regions. Conclusions : 1. Significant improvement in the muscular strength for the case of FSH Muscular Dystrophy was obtained with through Korean Bee-Venom Therapy, Four needle technique, and herbal medication. 2. For progressive muscular dystrophy, it is necessary to practice muscular strength recovery exercise in conjunction with Korean Bee-Venom Therapy. 3. Although this case yielded favorable result, further observation and study must be made to concretely prove the effectiveness of Korean Bee-Venom Therapy for treating muscular dystrophies.

Volatile Components of Pine Needle(Pinus densiflora S.) by Purge and Trap Headspace (Purge and Trap Headspace 법에 의한 솔잎(Pinus densiflora S.)의 휘발성 성분)

  • Volatile components of pine needle(Pinus densiflora S.) were isolated by purge & trap headspace technique and analyzed by gas chromatography-mass spectrometry(GC-MS). And then volatile components were extracted for 2 hr and 20 hr at the two different temperature settings: room temperature and 60$^{\circ}C$. A total of 61 volatile components were identified by the four different conditions. These compounds are classified into six categories in terms of chemical functionality: 35 hydrocarbons, 16 alcohols, 4 carbonyls, 2 esters, 1 acid and 3 ethers. The major components were ${\alpha}$-pinene(1.5~15.7%), ${\beta}$-myrcene(13.2~15.6%), ${\beta}$-phellandrene(l2.0~16.0%) and cis-3-hexenol(4.0~18.3%). In the comparison of the four extraction conditions, longer extraction can be effective to extract components that have a high boiling point, but proved useless in obtaining low boiling point components. As a result of these experiments under the four different conditions, the 20 hr extraction at room temperature appeared to be the most optimized condition for the analysis of volatile compounds by using the purge & trap headspace technique.

Comparison of Fine Needle Aspiration Cytologic Diagnoses and Histologic Diagnoses in 256 Breast Lesions (유방 병변 256례의 세침흡인 세포학적 진단 및 조직학적 진단과의 비교연구)

  • Fine needle aspiration cytology of breast lesion is well known as a simple, economic and effective diagnostic modality. For the evaluation of cytohistologic correlation, 256 cases of cytologic smears and subsequent histologic sections during 2-year period from Jan. 1995 to Dec. 1996 were reviewed. 1. Fifteen cases(5.9%) were proven as insufficient for evaluation, and 13 of them were fibrocystic change histologically. One case of carcinoma exhibiting sufficient amount of aspirates with no malignant cells on smear was regarded as inadequate. 2. Cytohistologic correlation of 240 cases revealed sensitivity 87.0%, specificity 100.0%, positive predictive value 100.0%, negative predictive value 97.0%, false positive rate 0.0% and false negative rate 13.0%. Total diagnostic accuracy is 95.7%. 3. Total 6 cases of negative were due to small amount of aspirates containing scantiness of malignant cells in two and underestimation in four. 4. Diagnostic concordance rates of fibrocystic change and fibroadenoma were 95.5% and 80.0%, respectively. Diagnostic discrepancies were noted in 7 cases of fibrocystic change and 6 cases of fibroadenoma, however, cytologic discrimination of two entities was not easy in seven of them. 5. In a case of phyllodes tumor and a case of duct ectasia, the discrepancy was due to targeting error. Other three cases(lymphoma, adenomyoepithelioma and granulomatous mastitis) were misinterpreted because of poor acquaintance with those entities. Diagnostic accuracy of fine needle aspiration cytology of breast lesions are relatively high. However, good technique on aspiration and adequate interpretation are necessary to reduce the false negative rate and the discrepancy between cytologic and histologic diagnoses.

Knit Design Represented with the Technique of Hand-knit (핸드니트 기법을 응용한 니트디자인 연구)

  • This study analyzed the knit wear trend, which has gained attention as a sensuous and luxury item, through collection magazines, and used the technique of hand-knit to produce and present the knit wear that was designed based on the study results. Accordingly, the purpose of this study is to raise awareness of the expansion of hand knit's new expression techniques and expression areas and to verify its possibility as a higher value-added product. The trends of hand-knit wear resulted from the analysis of collections divided into the following four groups, and the knit designs were also grouped accordingly: First was "The design using object." The trend survey showed the results that the form of drops or flowers was often used as a factor of object. Accordingly, as for the study design, the drop-knitting technique, which is one of the pattern knitting techniques, was used to present a design that employed drops as the object. Second was "The design matched with woven material." It is a way to emphasize aesthetic formativeness and at the same time to activate the area of knit design by matching woven material, which is different from knit in texture, with knit. This study presented a design that tried to combine knit texture and woven material by connecting denim material with various textures and colors. Third was "The design to express see-through." A sense of showing through knit wear was expressed through changes in the type of thread or knitting technique. In this study, the sense of see-through was expressed using piano wires and wool threads and through changes in the thickness of needle. Fourth was "The design to emphasize coloring." It was analyzed that regarding the production of hand knit, the method of coloring expression was to express points, lines and sides or to create harmony by changing the material's color or texture to show particular objects or shapes. In this study, a jacket was designed using the method to express points, lines and sides by changing the material's color.

A Clinical Study on the case of Diffuse Idiopathic Skeletal Hyperostosis(DISH) Treated with Traditional Korean Medicine, Especially Korean Bee-Venom Therapy (봉약침요법을 시행한 미만성 특발성 골격 과골증(Diffuse Idiopathic Skeletal Hyperostosis:DISH) 1례)

  • Objective : There are little report on treatment of Diffuse Idiopathic Skeletal Hyperostosis(DISH) in Traditional Korean Medicine. We suggest to treat DISH with a Korean Bee-Vonom Therapy and would like to suggest treatment plan for the further treatments. Methods : Under the assumption that Korean Bee-Venom Therapy may be affective for treatment DISH, the following points were administreted :Hua-Tuo-Jia-Ji-Xue Results : After 14 weeks of treatment-especailly Korean Bee-Venom Therapy, a remarkable improvement was made for DISH. Conclusion : Significant improvement in DISH was obtained with through Korean Bee-venom Therapy, Four needle technique, Common Acupucture Treatment and Herbal medication. Although this case yielded favorable result, more study should be made to prove the effectiveness of Korean Bee-Venom Therapy for treating DISH.

Sa-Am Acupuncture with Spleen Seung-gyuck induces an acute increase of radial pulse parameters related to arterial stiffness in healthy subjects: A comparative study of Jung-gyuck and Seung-gyuck

  • Objectives: To investigate the different effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck on the radial pulse in healthy subjects. Methods: Sixty healthy volunteers (30 males and 30 females) participated in this study. The participants were randomly divided into three groups: control (C), Sa-Am acupuncture with Spleen Jung-gyuck (SP+) and Sa-Am acupuncture with Spleen Seung-gyuck (SP-). The radial pulse was measured using a multi-step tonometry system before, and 0, 30 and 60 minutes after acupuncture. Results: The heart rate adjusted radial augmentation index (RAI/HR) and high-tensioned pulse area (W area) significantly increased in the SP- group compared to the C and SP+ groups in males. The systolic pulse period (T4) increased significantly in the SP- group compared to the C and SP+ groups in females. The RAI/HR had positive correlations with W area and T4. Conclusion: The effects of Sa-Am acupuncture with Spleen Jung-gyuck and Spleen Seung-gyuck are different. Sa-Am acupuncture with Spleen Seung-gyuck induces acute increases of radial pulse parameters related to arterial stiffness in healthy subjects.
