한국인 식이섬유 섭취 상태의 연차적 추이 $(1991{\sim}2001)$ (Annual Changes in the Estimated Dietary Fiber Intake of Korean during $1991{\sim}2001$)

  • The present study determined the estimated dietary fiber (DF) intake per capita of Korean from 1991 to 2001 and analyzed the major food sources of dietary fiber for Korean using the data on per capita consumption of each food reported in the Korean National (Health and) Nutrition Survey Reports and the recently established DF database by the Korean Nutrition Society. The estimated mean daily DF intakes of Korean during the last 11 years $(1991{\sim}2001)$ were in the range of $19.25{\sim}21.22\;g\;or\;9.97{\sim}11.99\;g/1,000\;kcal$ with a small range of fluctuations. As of 2001, average DF intake per capita of Korean was estimated as 20.92 g or 10.59 g/1,000 kcal. The average DF intake level was under the Adequate Intake for DF (12 g per 1,000 kcal) of the Dietary Reference Intakes (DRIs) for Koreans established in 2005. The level of DF intake was relatively lower in small city residents compared to the large city or rural area. The top two major food group sources of DF for Korean were vegetables and cereals, in addition, legumes, fruits, seaweeds and seasonings were included among top five food groups. The ratio of DF intake from fruits was gradually increased but the ratio from legumes was steadily decreased during the last 11 years. The mostly contributed single foods for DF intakes of Korean were Kimchi and rice regardless of year and area. Percentage of DF intake from top ten single foods was continuously decreased from 65% in 1991 to 51 % in 2001. The results of this study revealed that DF intakes of Korean as of 2001 is insufficient compared to the Adequate Intake for DF for Korean and the source of fiber in Korean diet has been more various. Therefore the beneficial health effects of DF and the increased consumption of DF from a variety of food sources should be continuously emphasized through the nutritional education.

한국 청소년의 일상 섭취량 추정 및 영양소 섭취량 평가: 국민건강영양조사 제5기(2010-2012) 분석 (Estimation of Usual Intake and Assessment of Nutrient Intake for Korean Adolescents: Analysis of the 2010-2012 Korea National Health and Nutrition Examination Survey)

  • This study assessed the nutrient intake for Korean adolescents using the estimated usual intake. The usual intake of 1,763 adolescents aged 12-17 years old was estimated from one-day 24-hour recall data in the 2010-2012 Korean National Health and Nutrition Survey. The nutrient intakes of four groups according to sex (male and female) and age (12-14 and 15-17 years old) were then assessed in reference to the 2015 Dietary Reference Intakes for Koreans. For macronutrients, the proportions of subjects below and above the Acceptable Macronutrient Distribution Ranges (AMDR) were calculated. The Estimated Average Requirement (EAR) cut-point method was used to assess insufficient intakes of protein, vitamin C, thiamin, riboflavin, niacin, calcium, phosphorus, and iron; the full probability approach was used to assess the insufficient intake for iron among females. The proportions of subjects over the Tolerable Upper Intake Levels for vitamin C, niacin, calcium, phosphorus, and iron and the proportions over the Intake Goal for sodium were calculated to assess excessive intake. The proportions of subjects above AMDR for carbohydrates ranged from 22.5% to 38.0% by group. The proportions of subjects by group with insufficient intake for vitamin C, riboflavin, calcium, phosphorus, and iron ranged 29.1-39.7%, 22.5-34.3%, 73.1-89.3%, 14.3-43.6%, and 23.2-55.5%, respectively. The proportions of subjects by group with excessive intake for sodium ranged from 91.5% to 99.3%. The results of this study represent important basic information to establish nutritional standards for school lunches as well as to decide on relevant nutrition policies for adolescents.

  • 제3기 (2005년) 국민건강영양조사 자료를 이용하여 분석한 결과 연구 대상자의 과반수에서 식품확보가 불안정하였으며, 식품안정성은 식사섭취와 관련이 있어 식품확보가 불안정한 군에서 영양소 섭취량이 낮고 섭취 식품의 다양성이 적었다. 본 연구는 우리 국민의 식품안정성과 식사섭취의 관련성에 대한 거의 초기 연구로 식품확보에 대한 안정성의 문제 크기를 파악하고 식품확보 여부에 따른 식사섭취의 양적${\cdot}$질적 측면을 다루었다. 이와 함께 식생활은 건강과 밀접한 관련이 있으므로 식품안정성과 건강 및 질병에 관한 후속 연구가 필요하며, 이러한 연구 결과에 근거하여 향후에는 저소득층의 안정적인 식품확보를 지원하는 정책을 마련하고 시행중인 정책의 효과 평가와 개선 방안을 모색할 필요가 있겠다.

경북 농촌지역 60세 이상 성인 및 노인의 열량영양소 및 무기질, 비타민 섭취조사 (Macronutrient, Mineral and Vitamin Intakes in Elderly People in Rural Area of North Kyungpook Province in South Korea)

  • Poor dietary habits and inadequate nutrient intakes are of concern in the elderly, even it is worse in rural areas. In the present study, we conducted the anthropometric measurement and the dietary intakes including macronutrients, minerals and vitamins to assess the nutrient intakes and nutritional risk in elderly people in rural kyungpook province in South Korea. Subjects (n = 168, mean age, 67.3 yrs) were interviewed using d general questionnaire and 3 days of 24-hours recall for dietary intake. Nutrient intakes were analyzed using CAN-pro soft program and compared to Korean RDA and nutrition reference values (NRV). The anthropometric measurement showed that the weight and the height of the subjects in the rural area were below the average of the same age of Korean elderly people. The energy and protein intakes were 85% and 90% of Korean RDA, respectively. The intakes of lipid, cholesterol and dietary fiber were 62%, 40% and 22% of NRV for Korean adults. Main sources for protein and lipid intakes came from the vegetable sources and this pattern was more prominent in female elderly people. Ca intake was half of Korean RDA (56%), while P intake was 132% of Korean RDA. For the antioxidant trace mineral (Fe, Cu, Mn, Zn, and Se) intakes, Fe and Zn intakes were 78% and > 103% of Korean RDA. Cu, Mn and Se intakes were > 113%, > 275%, and > 185% of Korean NRV. Thiamin, niacin and vitamin C intakes were above Korean RDA, but the intakes of vitamin A and riboflavin were 88% and 63% of Korean RDA, respectively. On summarizing the results of the present study, the elderly people in rural area consume less lipid, cholesterol, Ca, and dietary fiber. Ca intake is lower, while P intake is higher, and this would be the potential risk for bone health. Also, Na intake was high, which can be the potential risk for the cardiovascular disease prevailance. Vitamin intakes were fairly good status, excepting riboflavin. Antioxidant mineral intakes were much higher than Korean NRV, unexpectedly. The results suggest that the elderly people in rural area have inadequate intakes of protein, lipid, dietary fiber and Ca, which mainly should be supplied from animal products. Recommendations to increase diet variety would be emphasized for this nutritionally poor-conditioned subjects, specially including animal food products and high dietary fiber food.

한국인 총당류 섭취기준 설정 (Establishment of Total Sugar Reference Value for Koreans)

  • Sugars are a ubiquitous component of our food supply and are consumed as a naturally occurring component of many foods and as additions to foods during processing, preparation, or at the table. Most fruits and dairy products are high in sugars and thus naturally occurring sugars are consumed as part of a healthy diet. Some countries developed recommended daily intake figures(daily values : DVs or guideline daily amounts: GDA) for nutrients, and some countries, but not most have developed DV/GDA for total sugars. Dietary Reference Intakes for Koreans established by the Korean Nutrition Society in 2005, did not include the reference values for total sugar or added sugar. The committee on Dietary Reference Intakes for sugar was constituted in 2006 and discussed whether to special added sugars or total sugar. Although added sugars are not chemically or physiologically different from naturally occurring sugars, many foods and beverages that are major sources of added sugars have lower micronutrient densities compared with foods and beverages that are major sources of naturally occuring sugars. But it was so hard to calculate a dietary intake of added sugar for Korean people, because there was insufficient information about contents of added sugar during processing or preparation of Korean food. Currently Korean or US food labels contain information on total sugars per serving but do not distinguish between sugars naturally present in food and added sugars. Therefore the committee decided to set the reference value for total sugar for Koreans. According to the recommended diet pattern for Koreans suggested by the Korean Nutrition Society, estimated sugar intake from the sugar containing food based on 2,000 kilocalories is 67 g or 13% of total energy. Based on the data available on risk of obesity, hypertension, hyperlipidemia, insulin resistance, and metabolic syndrome from the analysis of Korean NHANES, it was insufficient evidence to set a UL for total sugar, but tended to increase serum LDL cholesterol and obesity at over 20-25% of energy from total sugar when consumed with high carbohydrates. Therefore the committee on Dietary Reference Intakes for sugar set the Acceptable Macronutrient Distribution Range for total sugar as 10-20% of total energy intake.

한국 성인의 단백질 섭취량 추이 및 적절성 평가: 2010 ~ 2019년 국민건강영양조사 자료를 활용하여 (Trends in Dietary Protein Intake and Its Adequacy among Korean Adults: Data from the 2010 ~ 2019 Korea National Health and Nutrition Examination Survey (KNHANES))

  • Objectives: This study aimed to evaluate dietary protein intake and its adequacy among Korean adults during recent 10 years. Methods: Based on the 2010 ~ 2019 Korea National Health and Nutrition Examination Survey (KNHANES) data, a total of 51,296 adults aged 19 years old or more who participated in a one-day 24-hr dietary recall were included. Dietary protein intake was estimated as percentages of total energy (% of energy) and grams per body weight (g/kg/day) and compared with the 2020 Dietary Reference Intakes for Koreans to evaluate the adequacy of protein intake. In addition, proportions of people whose protein intakes were less than the estimated average requirement (EAR) and above the upper limit of the acceptable macronutrient distribution range (AMDR) (> 20% of energy) were calculated according to sociodemographic characteristics. Results: Protein intake was increased from 14.7% of energy in 2010 to 15.6% of energy in 2019 among Korean adults. However, there was no increase in protein intake relative to the recommended nutrient intake (% RNI) during the recent 10 years. Protein intake relative to the RNI was decreased from 130.2% in 2010 to 121.1% in 2019 (P for trend < 0.0001) among total participants, and a significant decreasing trend was observed in all age groups except for over 65 years old. However, protein intake relative to the RNI was lowest in the elderly (98.6%). Proportions of low protein intake (< EAR) and high protein intake (> AMDR) increased in the past 10 years (P for trend < 0.0001 for all), and these were associated with socioeconomic statuses, such as education and household income levels. Conclusions: These findings suggest that protein adequacy in Korean adults has not been improved over the past decade compared with recommended levels. Nutritional education and intervention programs should consider different intake levels according to sociodemographic characteristics.

도시에 거주하는 여자노인의 영양상태와 건강상태에 관한 조사연구 (A Study on the Nutritional Status and Health Condition of Elderly Women Living in Urban Community)

  • This survey was carried out to obtain the information concerning nutritional status, including factors of food intake and health condition of elderly women living in Ulsan city. Main results were as follows. 1) Dietary intake data showed the average daily intake of energy, protein, vitamin A, riboflavin and ascorbic acid were lower than Korean recommended dietary allowances (RDA). According to the percentage of RDA, Vitamin A was the nutrient found to be least sufficient. 2) Among the various influencial factors, pocket money, meal-mate, skip meal and health concern influenced on nutrient intake considerably. A meal condition score based on influencial factors, showed significant correlations with all nutrient intake(P<0.001). A health score based n the subjects complains about health condition, showed significant correlations with intake of energy, carbohydrate, thiamin, and niacin(P<0.05). 3) Average hight was below Korea reference, while average body weight of subjects was close to Korean reference. Average body mass index was 23.0. 4) The average systolic and diastolic blood pressure was 140mmHg/88mmHg, the proportion of hypertension was 32.4 percent. The average hemglobin concentration was 12.7g/dl, only 6.7 percent of subjects were anemic. Serum cholesterol concentration was normal range(140~220mg/dl) in all subjects.

셀레늄의 2020 한국인 영양소 섭취기준 개정 및 권장식단의 셀레늄 함량 평가를 통한 식품의 셀레늄 데이터베이스 검토 (2020 Korean Dietary Reference Intakes of selenium and a review of selenium database of foods by evaluating of selenium contents of the recommended menus)

  • 셀레늄은 산화스트레스 방어 기능으로 인체의 면역기능 유지에 필수적 역할을 하고, 세포의 항산화기능, 면역기능, 갑상선호르몬 조절, 약물이나 중금속 위해에 대한 방어, 만성질환의 위험 감소에 필수 역할을 하는 미량무기질이다. 셀레늄 섭취의 부족은 암, 심혈관질환, 당뇨병, 신경질환, 골관절 및 근육괴사와 약화, 갑상선질환, 염증성 질환 등 다양한 만성적 질환의 위험을 높인다. 본 논문은 2020 한국인 영양소 섭취기준에서 셀레늄의 제정과 개정 근거 기준에 대해 설명하고, 셀레늄 데이터베이스의 현황과 향후 2025 한국인 영양소 섭취기준 설정을 위한 필요 연구에 대해 논의하였다. 셀레늄의 2020 한국인 영양소 섭취기준은 2015년에 이어 혈장 셀레노프로테인 P 수준이 최대가 되는 셀레늄의 평균필요량을 지표로 사용하였고, 중국인 대상의 보고치에 한국인의 기준체중과 변이계수를 적용하는 방법이 적용되었으며, 2015년에 비해 참고체중치의 변경에 따라 성별, 연령별 셀레늄 섭취기준에서 약간의 개정이 있었다. 향후 셀레늄 섭취기준 설정의 발전을 위해서는 1) 한국인 대상의 혈장 셀레노프로테인 P의 최대수준을 나타내는 셀레늄 섭취량을 파악하기 위한 셀레늄 중재연구가 필요하고, 2) 셀레늄 섭취량 판정의 정확성을 높이기 위한 영양평가 프로그램내 식품 셀레늄 데이터베이스의 검토와 식품의 셀레늄 함량 분석을 확대해야 하며, 3) 국민의 셀레늄 섭취와 체내 생물학적 지표와의 관계를 건강한 일반인뿐만 아니라 질환자 및 운동선수와 같은 특수 환경의 대상자들로 확대하는 연구가 요구된다.

우리나라 고등학생의 식품 및 영양소 섭취 변화 추이 - 2007~2015 국민건강영양조사를 이용하여 - (Trends in Food and Nutrient Intake of High School Students based on the Korea National Health and Nutrition Examination Survey 2007~2015)

  • This study assessed yearly trends of food and nutrient intake among high school students aged 16 to 18 years (n=2,377) using the 2007~2015 Korea National Health and Nutrition Examination Survey (KNHANES). Yearly trends of food or nutrient intake were analyzed via logistic regression analysis. The results showed that consumption of sugars & sweets, and beverages & alcohols was increased rapidly during this period (p<0.0001). Intake of meat & meat products, and fish & shellfish also was increased (p=0.0008). Intake of grains and grain products was increased until year 2011 but declined after 2012 (p=0.0025). Consumption of vegetables, and milk & milk products was decreased (p=0.0395). Intake of protein, fat, thiamin, riboflavin, niacin, and iron was increased (p=0.0445). Carbohydrate energy ratio was decreased, whereas fat energy ratio was increased (p=0.0235). Most nutrient intakes satisfied the dietary reference intakes for Koreans except dietary fiber (19.6~26.2%), calcium (46.9~55.2%) and sodium (more than 221.4%) during this period. There was a significant positive correlation between most food group intakes and most nutrient intakes (p=0.0468). Therefore, it is crucial to increase dietary fiber and calcium intake and decrease consumption of sugars, fats and sodium through diverse eating of food groups to ensure balanced nutrition of subjects.

Correlation between attention deficit hyperactivity disorder and sugar consumption, quality of diet, and dietary behavior in school children

  • This study investigated the correlation between consumption of sugar intake by fifth grade students in primary schools and development of Attention Deficit Hyperactivity Disorder (ADHD). A total of 107 students participated, and eight boys and one girl (8.4% of the total) categorized as high risk for ADHD according to diagnostic criteria. There were significant differences in the occupations and drinking habits of the respondents' fathers between the normal group and risk group. In a comparison of students' nutrition intake status with daily nutrition intake standards for Koreans, students consumed twice as much protein as the recommended level, whereas their calcium intake was only 60% of the recommended DRI (dietary reference intake). Regarding intake volume of vitamin C, the normal group posted 143.9% of the recommended DRI, whereas the risk group showed only 65.5% of the recommended DRI. In terms of simple sugar intake from snacks, students in the normal group consumed 58.4 g while the risk group consumed 50.2 g. These levels constituted 12.5% of their total daily volume of sugar intake from snacks, which is higher than the 10% standard recommended by the WHO. In conclusion, children who consumed less sugar from fruit snacks or whose vitamin C intake was less than RI was at increased risks for ADHD (P < 0.05). However, no significant association was observed between total volume of simple sugar intake from snacks and ADHD development.
