소아환자의 수술후 P.R.N. 근주와 PCA의 제통효과의 비교 (A Comparison of Patient Controlled Analgesia and P.R.N. Intramuscular Injection for Postoperative Pain Control in Children and Adolescents)

  • Background: The postoperative pain in children and adolescents is most commonly managed by intramuscular injections of NSAIDs or opioids. This approach may result in fluctuating plasma drug levels and cycles of pain, comfort, and sedation. Patient-controlled analgesia(PCA) is a method of analgesia administration that consists of a computer-driven pump with a button that the patient may press to administer a small dose of analgesic drug. Materials & Methods: Forty ASA physical status 1 or 2 children and adolescents were divided into two groups. In the PRN group, patients received intramuscularly diclofenac(Valentac$^{(R)}$) on a p.r.n. basis. The PCA group patients received a mixture of nalbuphine and ketorolac by WalkMed$^{(R)}$PCA infusor. Analgesic efficacy was evaluated with NRS(numerical rating scale) and Faces Pain Rating Scale. The side effects were evaluated. Results: The patients of PCA group had less pain than those of PRN group. Complications were similar in both group. Conclusions: PCA with nalbuphine and ketorolac is a safe and effective methods of pain relief in children and adolescents after surgery, and is better accepted than intramuscular injections.

The results of cardiopulmonary exercise test in healthy Korean children and adolescents: single center study

  • Purpose: The cardiopulmonary exercise test (CPET) is an important clinical tool for evaluating exercise capacity and is frequently used to evaluate chronic conditions including congenital heart disease. However, data on the normal CPET values for Korean children and adolescents are lacking. The aim of this study was to provide reference data for CPET variables in children and adolescents. Methods: From August 2006 to April 2009, 76 healthy children and adolescents underwent the CPET performed using the modified Bruce protocol. Here, we performed a medical record review to obtain data regarding patient' demographics, medical history, and clinical status. Results: The peak oxygen uptake ($VO_{2Peak}$) and metabolic equivalent ($MET_{Max}$) were higher in boys than girls. The respiratory minute volume $(V_E)/CO_2$ production ($VCO_2$) slope did not significantly differ between boys and girls. The cardiopulmonary exercise test data did not significantly differ between the boys and girls in younger age group (age, 10 to 14 years). However, in older age group (age, 15 to 19 years), the boys had higher $VO_{2Peak}$ and $MET_{Max}$ values and lower $V_E/VCO_2$ values than the girls. Conclusion: This study provides reference data for CPET variables in case of children and adolescents and will make it easier to use the CPET for clinical decision-making.

청소년이 지각한 부모의 양성평등의식과 자녀교육지원 (Androgynous Equalitarianism of Parents Recognized by Adolescents and Educational Support for Children)

  • The purpose of the research lies in investigating how the androgynous equalitarianism of parents recognized by adolescents and educational support for children differ in accordance with social and population factors. The subjects were 397 adolescents of secondary schools in Chonnam. The analysis of the results was carried out using SPSS/WIN 10.0 program. The results were as follows: (1) Adolescents were found to recognize the androgynous equalitarianism in their parents differently in accordance with gender, birth ranking, academic record, and father education level. (2) Educational support of parents recognized by adolescents, showed some notable differences in accordance with type of school, birth ranking, academic record, place of residence, mother's age, father education level, form of marriage, and economic class. (3) The relationship between androgynous equalitarianism and Educational support of parents recognized by adolescents had a positive correlation. In conclusion, it is essential to provide an atmosphere wherein the budding children can display their utmost potentiality regardless of gender to grow independent way of thinking and behaving and ability. Also consistent willingness to practice education in androgynous equalitarianism for the realization of equal society for both men and women is believed prerequisite.

Characteristics and Treatment of Temporomandibular Disorder in Children and Adolescents: An Analytic Review

  • Purpose: The purpose of this study is to investigate the prevalence of temporomandibular disorders (TMDs) in children and adolescents, their characteristic contributing factors, the characteristic features of symptoms and symptoms, and the response to treatment. Methods: We studied the researches, that were the results of the searches for words such as temporomandibular disorder, TMD, children, adolescents, and juvenile through PubMed and DBpia. Results: According to a study conducted in Busan, the ratio of adolescents increased from 18.3% to 21% in 2008 compared to 2000, and the proportion of boys increased from 38.58% to 45.38%. One of the characteristic contributing factors for adolescents is the macrotrauma such as jaw trauma, vehicle accidents, sports, physical abuse, forceful intubation, and third molar extraction. The second is a microtrauma from parafunctional habit such as bruxism, clenching, hyperextension, wind instrument, and fingernail biting that can cause joint overload, cartilage breakdown, synovial fluid alterations, and other changes within the joint. The diagnosis of TMDs in juvenile adolescents is not significantly different from that of adults. Medical history, clinical examination and radiological examinations are required. Conclusions: In the temporomandibular joint history and assessment, all comprehensive dental history examination is required, including head and neck pain, mandibular dysfunction, previous orofacial trauma, history of present illness with an account of current symptoms. For the treatment and management of temporomandibular arthritis in juvenile adolescents, understanding the characteristics of TMDs in juvenile adolescents and thoroughly analyzing appropriate diagnosis and possible contributing factors through comprehensive history taking & examination, conservative treatment, including fast and active cautions education, will be essential.

소아 및 청소년 결핵의 특징 (Characteristics of tuberculosis in children and adolescents)

  • In childhood tuberculosis, it is possible to clearly distinguish among three basic stages: exposure, infection, and disease. The incidence of tuberculosis in children is low compared with that in adults, but latent infection is a major concern because children, who are exposed to Mycobacterium tuberculosis especially early in childhood, are at increased risk of developing the disease. Younger children particularly infants have a high relative risk of miliary or meningeal disease. The clinical manifestations of childhood tuberculosis differ noticeably from those of the disease seen in adults. Adolescents with tuberculosis have different demographic and clinical features from those of adults or children. Tuberculosis in adolescents has become relatively more important as the incidence of infection in childhood has decreased.

The Current State and Legal Issues of Online Crimes Related to Children and Adolescents

  • There are two categories of online crimes related to children and adolescents: those committed by adolescents and those committed against children and adolescents. While recent trends in criminal law show consensus on strengthening punishment in cases of crimes against children and adolescents, there are mixed stances in cases of juvenile delinquency. One perspective emphasizes strict punishment, whereas the other emphasizes dispositions aligned with human rights. While various forms of online crime share the commonality in that the main part of the criminal act occurs online, they can be categorized into three types: those seeking financial gain, those driven by sexual motives, and those engaged in bullying. Among these, crimes driven by sexual motives are the most serious. Second-hand trading fraud and conditional (sexual) meeting fraud fall under the category of seeking financial gain and occur frequently. Crimes driven by sexual motives include obscenity via telecommunication, filming with discrete cameras, child and adolescent sexual exploitation material, fake video distribution, and blackmail/coercion using intimate images/videos ("sextortion"). These crimes lead to various legal issues such as whether to view vulgar acronyms or body cams that teenagers frequently use as simple subcultures or crimes, what criteria should be applied to judge whether a recorded material induces sexual desire or shame, and at what stage sexual grooming becomes punishable. For example, sniping posts, KakaoTalk prisons, and chat room explosions are tricky issues, as they may or may not be punished depending on the case. Particular caution should be exercised against the indiscriminate application of a strict punishment-oriented approach to the juvenile justice system, which is being discussed in relation to online sexual offenses. In the punishment case of online crime, juvenile offenders with a high potential for future improvement and reform must be treated with special consideration.

Osteosarcoma in Korean children and adolescents

  • Osteosarcoma is the most frequent primary bone tumor. Advances in combination chemotherapy and surgical technique have greatly improved the survival of patients with osteosarcoma. In Korea, improvements in osteosarcoma treatment have been made over the past two decades. The 5-year event-free survival rate of Korean children and adolescents with localized disease is 64.6%, comparable to that of American or European patients. This article provides an overview of current therapies for osteosarcoma in Korea.

Dietary patterns of children and adolescents analyzed from 2001 Korea National Health and Nutrition Survey

  • The purpose of this study was to identify dietary patterns among children and adolescents in Korea and to examine their associations with obesity and some blood profiles. One day food consumption data measured by 24-hour recalls on 2704 subjects aged 1 to 19 were used from 2001 Korea National Health and Nutrition Survey. The data of blood profiles available in the ages of 10 or older was also used. After categorizing each food consumed into 29 food or food groups, five dietary patterns were derived through a factor analysis and subjects were classified into three major dietary patterns via a cluster analysis using the factor scores. Three dietary patterns were identified as 'traditional diet' (25.6%), 'westernized-fast food' (6.2%), and 'mixed diet' (68.2%). The 'traditional diet' pattern had a higher percentage in boys. Both the 'traditional diet' and the 'westernized-fast food' had higher proportions of adolescents (12-19 y) than younger children, while the 'mixed diet' had a higher percentage of preschool children (1-5 y). Obesity rate analyzed within each age group showed no differences among 3 dietary pattern clusters. Blood pressure and all plasma profiles were not different among dietary patterns when adjusted with age and gender. Conclusively, children and adolescents in Korea had three distinct dietary patterns, which were associated with gender and age. These patterns could be useful to plan nutrition interventions for teenager health promotion.

건강·복지시설에 적용된 바이오필릭 디자인 문헌연구: 노인과 아동청소년 시설 비교 (A Literature Review of Biophilic Design Applied to Healthcare and Welfare Facilities: Comparison of Facilities for the Elderly and Children/Adolescents)

  • 본 연구는 노인과 아동청소년을 위한 건강 및 복지시설의 바이오필릭 디자인 특성을 비교 분석하기 위해 시행된 문 헌연구이다. 한국교육학술정보원 데이터베이스에서 노인과 어린이, 청소년 건강 및 복지시설의 바이오필릭 디자인 관련 문헌을 선정하여 바이오필릭 디자인 요소별 빈도를 분석한 후 비교하였다. 노인시설 관련 문헌 네 개와 어린이 및 청소년 시설 관련 문헌 네 개를 분석한 결과, 노인시설의 '자연의 직접 체험'에 해당하는 바이오필릭 패턴이 아동청소년 시설보다 더 많았고, '공간과 장소 체험' 패턴은 아동청소년 시설이 더 많았다. '자연의 간접체험' 패턴은 노인시설과 아동청소년 시설이 유사하였다. 건강 및 복지시설을 이용하는 취약아동의 스트레스 감소를 위해 '자연의 직접체험' 패턴을 더 많이 활용할 것과 노인의 스트레스 감소 및 인지강화를 위하여 '공간과 장소 체험' 패턴을 더 많이 활용할 것을 제안한다.

유아-청소년 자녀를 둔 어머니의 행복감 영향 변인 : 자녀의 연령, 자녀수 및 어머니 행복 조건에 대한 인식 (The Variables Related to Maternal Happiness for Mothers of Young Children, School-Aged Children, and Adolescents : A Child's Age, the Numbers of Children, and Maternal Perceptions of the Conditions of Happiness)

  • The purpose of this study was to examine the effects of a child's demographic variables and maternal perceptions of the conditions of happiness on their happiness for mothers. The participants of this study comprised 916 mothers of young children, school-aged children, and adolescents. The results revealed that a child's age and the numbers of children influenced maternal happiness when mothers' demographic variables were controlled. In addition, mothers who exhibited high degree of need in terms of the conditions of happiness regarding existence, relatedness, and growth were more likely to be happy than mothers who exhibited low degree of needs. Finally, the expectations regarding growth, which refers to the pursuit of self-accomplishment and meaning, was the most important predictor of maternal happiness. These findings have implications for parent education programs for mothers of young children, school-aged children, and adolescents.
