The Study of the Sham Acupuncture for Double Blind (이중맹검을 위한 Sham Acupuncture에 대한 임상연구)

  • Background and Purpose : Though there were many clinical studies of acupuncture effects they didn't have appropriate control group. So we didn't say it was true acupuncture effect, though subjects in clinical study improved. To investigate the possibility of sham acupuncture application, we examined how well subjects distinguished real acupuncture from sham acupuncture. Methods : We investigate the possibility of sham acupuncture application as the following 3 kinds of methods. 1. The public (n=60) and Oriental Medicine Doctors (n=20) looked at the appearance of acupuncture and then were treated with randomly allocated acupuncture and judged it as real or sham. 2. The public (n=60) and Oriental Medicine doctors (n=20) were treated with randomly allocated acupuncture, then they looked at the appearance of acupuncture and judged it as real or sham. 3. The public (n=60) were treated with randomly allocated acupuncture (right-real-left-sham), (right-sham-left-real), (left-real-right-sham), (left-sham-right-real), then they judged it as real or sham. Results : 1. When the public (n=60) looked at the acupuncture appearance, the correct judgement ratio was 63.3%. After they were treated with acupuncture, the correct judgement ratio was 50.0%. 2. When Oriental Medicine doctors (n=20) looked at the acupuncture appearance, the correct judgement ratio was 25.0%. After they were treated with acupuncture, the correct judgement ratio was 45.0%. 3. When the public (n=60) were treated with acupuncture, the correct judgement ratio was 50.0%. After they looked at the acupuncture appearance, the correct judgement ratio was 63.3%. 4. When Oriental Medicine doctors (n=20) were treated with acupuncture, the correct judgement ratio was 65.5%. After they looked at the acupuncture appearance, the correct judgement ratio was 65.0%. 5. After the public (n=60) were treated with randomly allocated sham or real acupuncture as (right-real-left-sham), (right-sham-left-real), (left-real-right-sham), or (left-sham-right-real). the correct judgement ratio was 66.7% in looking at the acupuncture appearance group, and in not looking at 60.0%. Conclusion : These results indicated that Kim Sham Acupuncture could apply to the double blind clinical study of acupuncture effect. And more continuous studies are needed on Sham Acupuncture apparatus.

Influence of Bioelectric Current on the Acupuncture: A Pilot Study - Focusing on the Effect of PC6 Against Heart Rate - (생체전류가 침 치료에 미치는 영향: 예비연구 - 심박수에 대한 내관(內關) 자침 효과를 중심으로 -)

  • Objectives : Many studies have investigated the electric specificity of meridian and acupoint. However, the definition about the real substance of 'Ki(Qi)' that flows in the meridian has not been established yet. The authors hypothesized that the 'Ki(Qi)' may be the very 'bioelectricity' of western medicine, from two common features that they are not visible with naked eyes and that they function in the living body only. Methods : 20 healthy adults participated in this study. Heart rate was measured before and after exercise. Acupuncture was performed at PC6 immediately after exercise with counter balance in the first experiment. In the second experiment, acupuncture group was further divided to the three groups, i.e. glove acupuncture group, manual acupuncture group, and NaCl acupuncture group. In the glove group, acupuncturist put on the two folds of latex gloves to block bioelectric currents between the acupuncturist and subject. In the NaCl group, acupuncturist had his fingertips wet with NaCl solution. Results : Exercise increased Heart Rate and acupuncture at PC6 inhibited this increase. In the second experiment, the significant difference compared to the comtrol group was the most in NaCl acupuncture and the least in Glove acupuncture. However, there was no significant difference between three groups. Conclusions : The results of this study seem to be deficient as a conclusive evidence for the hypothesis that the 'Ki(Qi)' of Korean Medicine is the 'bioelectricity' of western medicine.

Acupuncture Treatment for Shoulder Impingement Syndrome: A Review of Randomized Controlled Trials

  • To evaluate the effectiveness of acupuncture treatment for shoulder impingement syndrome (SIS) a literature review was conducted of randomized controlled trials (RCTs) where acupuncture was used as an intervention for patients diagnosed with SIS. Relevant clinical studies (N = 181) were retrieved from several databases based on the inclusion/exclusion criteria, and the interventions and results were analyzed. Six RCTs were selected to review based on the inclusion and exclusion criteria. In all 6 studies, the acupuncture treatment group showed significant positive changes in indicators evaluating pain, disability, and quality of life. A significant decrease in the evaluation indicators (Shoulder Pain and Disability Index, numeric rating scale, Visual Analogue Scale, Constant Murley Shoulder Assessment Score, patient's global assessment, and doctor's global assessment) and a significant increase in the questionnaire scores (UCLA, AL-score, EuroQol 5 Dimension Self-Report Questionnaire, and disabilities of the arm, shoulder, and hand) were observed. In addition, 1 study showed similar improvements in pain and quality of life measures in the acupuncture group and corticosteroid injection group. No major side effects were reported. Acupuncture may be an effective and safe treatment for SIS however, further RCTs are required.

  • Objectives: Examine the difference of Chosun acupuncture from the acupuncture in Myung, Chung Dynasty Method: This paper examines the acupuncture mentioned in the classic of Introduction to Medicine For this, the special traits of Introduction to Medicine acupuncture was analyzed. Secondly, the research on the contents of Introduction to Medicine acupuncture quoted in the classic of Essential Rhymes of Acupuncture and Moxibustion, Treasured Mirror of Eastern Medicine, and Experimental Prescriptions of Acupuncture and Moxibustion was carried out. Result: Introduction to Medicine put much importance on "method of acupuncture following the five circuits and six qi" (子午流注針法) and Song of Acupuncture and Moxibustion through comparing "eight methods of acupuncture following the five circuits and six qi" (子午八法) and "scattered needling method". The doctors in Chosun Dynasty, howevery, did not adopt this theory with their own standards on acupuncture. On the contrary, they put more importance on the inquiry of historical evidence on river point and extraordinary points. Conclusion: The acupuncture studies of Chosun Dynasty had already formed its own standards on the medical theories, and its direction to which it was headed was different from that of the Myung, and Chung Dynasty in China.

  • On this volume it is said that the general nine-acupuncture is correctly used for it. Since the structure and effect of the nine-acupuncture are difference with each other, it insist on being well-matched disease of it own on first and second chapter There is described the method of nine-acupuncture on third chapter, on fourth chapter, there is described the 12chol-acupuncture according to twelve cases, on fifth chapter, there is described the three-acupuncture according to invasion depth of pathogenic cold and on the sixth chapter, there is described the five-acupuncture used in the diseases of five zang. The these acupuncture reflect the affluence of the ways of ancient acupuncture and these contents may affect on wield influence on the development of acupuncture. Hence, to study source of acupuncture it is necessary to investigate this volume thoroughly and we try to find improved method used in clinical treatment. On this study, we try to contribute understanding the original text with translation in korean.

  • Objective : This study was performed to evaluate the treatment of acupuncture therapy including. Bee Venom and Hominis Placenta herbal acupuncture on the patient with Transverse myelitis. Methods : We treated the patient with Transverse myelitis by Bee Venom herbal acupuncture at beginning, since then we treated him adding to Hominis Placenta herbal acupuncture. Conclusions: The patient was effectively reduced symptoms with Bee Venom herbal acupuncture, since then he get more effective improvement of symptoms by adding Hominis Placenta herbal acupuncture. Therefore we are able to expect Bee Venom and Hominis Placenta herbal acupuncture will be more effective than simply acupuncture on the patient with Transverse myelitis.

  • Objectives : The aim of this study was to show the rationale of point-selection on the methods of acupuncture and moxibustion in the Shin section(in the Naegyeong chapter) of the Donguibogam. Methods : First, We summarized the cause of each disease in the Shin section(in the Naegyeong chapter) of the Donguibogam. Then, We explained the rationale of acupuncture point-selection referring to the cause of disease, physiology of the Oriental medicine, exposition of acupuncture point name, character of each acupuncture points, flow of meridian pathways and specific acupuncture points etc. Results and Conclusions : Total 44 acupuncture points were used in the Shin section(in the Naegyeong chapter) of the Donguibogam. Most of acupuncture points were specific acupuncture points. but, some rationale of acupuncture point-selection were explained by the cause of disease, physiology of the Oriental medicine, exposition of acupuncture point name, flow of meridian pathways etc.

  • Effects of cheungho aqua-acupuncture at gansoo$(BL_{18})$ and illweul$(GB_{14})$ on plasma lipid composition were investigated in rat fed high fat diet. Body weight gain showed a tendency to decrease in aqua-acupuncture groups and in aqua-acupuncture groups, gansoo and illweul join aqua-acupuncture group showed a lower values than other aqua-acupuncture groups. Concentrations of plasma triglyceride, free fatty acids, lipoprotein and glucose showed a tendency to decreasea in the aqua-acupuncture groups. However in aqua-acupuncture groups, these values showed no significantly different. In plasma lipid composition, total cholesterol and LDL- cholesterol showed a tendency to decrease in aqua-acupuncture groups, however the values of LDL-cholesterol showed no significantly different in the aqua-acupuncture groups. HDL-cholesterol concentration showed a tendency to increase in aqua-acupuncture groups and in aqua-acupuncture groups, gansoo aqua-acupuncture group showed a high value.

Individualized Traditional Korean Acupuncture for Knee Osteoarthritis : a Protocol for a Randomized Controlled Trial

  • Objective : To test the hypotheses that individualized traditional Korean acupuncture improves pain and disability in patients with osteoarthritis of the knee and that benefits remain after stopping treatment more so than is the case for standardized minimal acupuncture. Design : Randomized single blind controlled trial with two intervention arms (individualized traditional Korean acupuncture, standardized minimal acupuncture) of six weeks' duration and three months follow-up. Setting : Acupuncture interventions were applied by two training doctors in the Department of Acupuncture and Moxibustion in a 1000-bed hospital. Assessment of the result was performed in a university-based laboratory. Participants : 50 patients with symptoms of knee osteoarthritis as diagnosed by an orthopedist. Intervention : Individualized traditional Korean acupuncture or standardized minimal acupuncture for six weeks. Main outcome measures: Primary outcome measure was pain as measured by the visual analogue scale. Secondary measures of pain and disability included the Western Ontario and McMaster Universities (WOMAC) index, Short Form-36 (SF-36), Lequesne Functional Index (LFI) score and Korean version of Health Assessment Questionnaire (KHAQ). Discussion : This paper presents detail on the rationale, design, methods and operational aspects of the trial.

The Effect of Sa-Am Stomach Tonifying Acupuncture on Radial Pulse in Healthy Human Subjects (위정격(胃正格) 자침이 정상 성인의 맥파(脈波)에 미치는 영향)

  • Objectives : The purpose of this study is to find the effects of Sa-Am stomach tonifying acupuncture on radial pulse through the parameters difference. Methods : Forty healthy subjects participated in this study, acupuncture group and control group were divided. Radial pulse were measured using 3 dimensional pulse imaging system(DMP-3000). 19 parameters that significantly changed were selected after comparing between acupuncture group and control group. Then, 19 parameters were analyzed before, immediately after, 30 minutes after and 60 minutes after acupuncture in acupucture group. Results : 1. Heart rate, pulse period, T/T1, T2/T, T4/T, T5/T, T-T4, (T-T4)/T and T4/(T-T4) significantly changed after acupuncture. 2. H1 Amplitude, H2 Amplitude, H4 Amplitude and H5 Amplitude significantly changed after acupuncture. 3. Area of Pulse Wave, Systolic Pulse Area, Diastolic Pulse Area and Area of W significantly changed after acupuncture. 4. AIx/HR significantly changed after acupuncture. Conclusion : Stomach Tonifying Acupuncture exerts an influence on radial pulse parameters. Further radial pulse change study on various acupuncture treatment is required.
