뇌졸중 환자의 상지 체감각 자극을 통한 감각, 손 기능, 자세조절 및 일상생활수행력의 변화 (Effect of Somatosensory Stimulation on Upper Limb in Sensory, Hand Function, Postural Control and ADLs within Sensorimotor Deficits after Stroke)

  • Purpose: This study examined the improved sensory, hand function, postural balance and activities of daily living (ADL) through somatosensory stimulation, such as the facilitation of functional reaching and tactile, proprioceptive stimulus of the upper limb (UL) and hand. Methods: Seventeen stroke patients having problems with motor and somatosensory deficits were selected in Bobath Memorial Hospital adult rehabilitation center. The patients were divided into two groups; the sensorimotor deficit group (SMDG) and motor deficit group (MDG). Somatosensory stimulation on the UL, physical therapy and occupational therapy were carried out three times a week over a six week these treatments were performed in both group period. To compare each group, the following assessment tools were used: such as tactile detection thresholds (TDT), two point discrimination on the affected side (TPDas), unaffected side (TPDus) stereognosis (ST) manual function test, hand function on the affected side (HFas) and unaffected side (HFus), Postural Assessment Scale for Stroke (PASS) and Korean version Modified Barthel Index (K-MBI). Results: In the SMDG, somatosensory stimulation on the UL was statistically important for TDT, TPDas, TPDus (except for the thener), ST, hand function on HFas, on HFus, PASS length of displacement with foam (LDFSEO), and K-MBI. In the MDG, somatosensory stimulation on the UL was important for TDT, TPDas, TPDus (except index finger) length of displacement with the eyes open, LDFSEO, HFas, HFus, PASS and K-MBI. In addition, there was a significant difference in the PASS between SMDG and MDG. Conclusion: Somatosensory stimulation on the UL affects the sensory, hand function, postural control and ADLs performance.

뇌졸중 환자의 재활참여가 기능회복에 미치는 영향 (Impact of Patients' Level of Participation in Rehabilitation on Functional Outcome in Patients With Stroke)

  • Background: Stroke recovery is a long and complex process. Successful stroke recovery seems to be strongly associated with patients' high motivation and committed participation. Patients' motivation is a key determinant of successful rehabilitation outcomes, but it is difficult in defining and measuring. Patients' participation is defined as the degree or extent to which subjects take part in rehabilitation activities and can be measured by observable behavior. Objects: The purpose of this study was to investigate the impact of patients' level of participation in rehabilitation on functional outcomes in patients with stroke. Methods: Forty post-stroke inpatients participated in this study. The level of rehabilitation participation was measured by the Hopkins Rehabilitation Engagement Rating Scale (HRERS). Other measures used for the evaluation were the Rivermead Mobility Index (RMI) and Korean version of the Modified Barthel Index (K-MBI). Overall measurements were made at early intervention and late intervention. Spearman correlation and multiple regression were used to measure the relationships between HRERS, RMI, and K-MBI. Results: The correlation found between HRERS total scores at early intervention and RMI total scores of late intervention was above moderate (r = 0.607, p < 0.01). RMI total scores at early intervention (p < 0.000), HRERS total scores at early intervention (p < 0.001), and disease duration (p < 0.003) were significant predictors of RMI total scores at late intervention. Conclusion: The level of participation at early intervention was associated with improvement in mobility. The level of mobility at early intervention, disease duration, and patients' participation at early intervention were important determinants of functional outcome. These findings suggest that patients' participation should be encouraged in order to achieve successful stroke recovery.

치매노인의 일반적 및 병리학적 특성에 따른 인지기능, 일상생활동작, 삶의 질의 비교연구 (A Comparative Study of the Cognition, ADL and Quality of Life According to General and Pathological Characteristics of the Elderly with Dementia)

  • 본 연구는 치매노인의 일반적 특성 및 병리학적 특성에 따라 인지능력, 일상생활동작 수행능력, 삶의 질에 미치는 영향을 비교하고자 실시되었다. 2016년 7월 15일부터 동년 8월 30일까지 주간보호시설을 이용하는 치매노인 136명을 대상으로 한국형 간이 정신상태 검사(MMSE-KC), 한글판 수정바델지수(K-MBI), 세계보건기구의 삶의 질 간편형 척도을 번역된 한국어판(Korean version WHOQOL-BREF)을 사용하여 정보를 수집하고 값을 분석하였다. 연구결과는 치매노인의 인지기능 비교에서, 연령대, 교육수준, 치매정도에 따라 인지능력에 차이가 있었고(p < .05), 치매노인의 일상생활동작 수행능력 비교에서, 연령대, 교육수준, 치매경과기간, 치매정도, 과거직업에 따라 차이가 있었으며(p < .05), 치매노인의 삶의 질 비교에서, 연령대, 치매경과기간, 치매정도, 과거직업에 따라 유의한 차이가 있었다(p < .05). 인지능력과 일상생활동작 수행능력, 삶의 질과 인지능력, 삶의 질과 일상생활동작 수행능력 간의 유의한 상관성을 보였으며(p < .001), 치매노인의 삶의 질은 인지능력과 일상생활동작 수행능력에 크게 영향을 받는 것을 알 수 있었다(p < .01). 따라서 치매노인의 사회적 물리적 측면에서 더 활발한 치료적 중재 및 연구가 이루어져야 할 것으로 생각된다.

노인의 치매행동 관련 홀니스 프로그램 개발과 완충효과 검증 (The Development of Wholeness Program for Effects Dementia-Buffering Testing of the Demented Elderly)

  • 이 연구는 치매행동 관련 홀니스 프로그램 적용 및 완충효과 검증 단계로 서울시에 위치한 M1과 M2 노인복지관에 개설되어 있는 주간보호시설 참여 노인 총 40명을 대상자로 선정하였다. 선정된 대상자들의 사전조사를 실시한 후 무작위로 M1노인복지관 노인 20명을 실험군으로, M2노인복지관 노인 20명을 통제군으로 배치하였다. 치매행동 관련 홀니스 프로그램 활동은 2016년 3월 14일부터 2016년 5월 30일까지 12주간, 주 2회, 60분간 수행하였다. 측정도구는 MMSE-K와 K-MBI, GDS-K를 사용하였다. 치매행동 관련 홀니스 프로그램에 참여하는 동안 치매행동 참여 전과 참여 후 총 2회를 측정하였으며 SPSS-PC(version 21.0) 통계프로그램을 이용하여 첫째, 실험군과 통제군의 동질성여부를 파악하기 위하여 독립표본 t-검증, 둘째, MMSE-K와 K-MBI, GDS-K에 대한 효과를 알아보기 위하여 사전검사와 사후검사 간 대응표본 t-검증, 셋째, 사전·사후 간 변화량을 확인하기 위하여 ANOVA를 실시하였다. 연구결과 노인의 치매행동 관련 홀니스 프로그램은 인지능력의 변화(MMSE-K), 신체능력의 변화(K-MBI), 우울수준의 변화(GDS-K)에 모두 효과가 있는 것으로 나타났다.

사지위약감과 안면마비를 주소로 하는 길랑바레 증후군 환자의 한방치료 증례보고 1례 (A Case Report of a Patient with Guillain-Barre Syndrome Complaining of Limb Weakness and Facial Paralysis That Improved After Korean Medicine Treatment)

  • Objectives: This study reports a case of Guillain-Barre Syndrome (GBS) in which the patient experienced improved limb weakness, facial paralysis, paresthesia, and systemic pain after Korean medicine treatment. Methods: A 25-year-old female patient diagnosed with GBS received the herbal medicine Banhasasim-tang Soft Ext., acupuncture, electroacupuncture, moxibustion, cupping, and rehabilitation treatment. To confirm the change in symptoms, the manual muscle test (MMT), Korean Version of Modified Barthel Index (K-MBI), Yanagihara grading system (Y-score) and Numeric Rating Scale (NRS) were performed. Results: After Korean medicine treatment, there was a significant improvement in GBS-related clinical symptoms. Conclusions: Korean medicine treatment could be effective in improving symptoms of limb weakness, facial paralysis, paresthesia, and systemic pain related to GBS. However, this study has limitations as a case report, and more studies are needed.

  • Objectives: The aim of this study is to describe the effects of treatment with traditional Korean medicine on a patient with voiding dysfunction caused by cerebral infarction. Methods: The patient was treated with herbal medicine, moxibustion, acupuncture, and electroacupuncture. The effects of these treatments were evaluated using the Korean version of the Modified Barthel Index and a post-void residual volume. Results: In this study, the Korean medicine Dangguishaoyao-san-hap-Jeoryeong-tang resulted in improvements in this patient with post-stroke urinary retention and successful removal of the indwelling catheter. Conclusion: The results suggest that traditional Korean medicine may be effective in ameliorating dysfunction caused by cerebral infarction.

기저핵 뇌출혈로 인한 반신부전마비, 감각장애에 대한 한방치험 1례 (A Case Report of Hemiparesis and Hypesthesia in a Patient with an Intracerebral Hemorrhage at the Basal Ganglia Treated with Traditional Korean Medicine)

  • Objectives: The aim of this study is to describe the effects of traditional Korean medicine treatment on a patient with an intracerebral hemorrhage (ICH) at the left basal ganglia. Methods: The patient was treated with herbal medicine (Gamicheongsim-tang, Hwangryunhaedok-tang-tablet), acupuncture, moxibustion, and rehabilitative therapy. The effects of these treatments were evaluated using the manual muscle test (MMT), sensory scale, Korean version of the modified Barthel index (K-MBI), and the National Institutes of Health Stroke Scale (NIHSS). Results: After treatment, the MMT grade improved from 3+/3+ to 4+/4+, and the sensory scale increased from 40%/40%/40% to 80%/80%/80%. In addition, the K-MBI score improved from 56 to 100, and the NIHSS score decreased from 7 to 1. Conclusion: The results suggest that traditional Korean medicine can be effective in the treatment of patients with ICH.

  • Objective: This study examined the effectiveness of Pyung-Hyung acupuncture and herbal medicine for a hemiplegic patient diagnosed with intracerebral hemorrhage. Methods: The patient was treated with Pyung-Hyung acupuncture and herbal medicine for one month. Intracerebral hemorrhage symptoms were evaluated using the Korean version of the Modified Barthel Index (K-MBI), the Manual Muscle Test (MMT), and brain CT images. Results: Following Pyung-Hyung acupuncture and herbal medicine, K-MBI, MMT, and brain CT image results improved. Conclusion: This case showed that Pyung-Hyung acupuncture and herbal medicine effectively treated intracerebral hemorrhage with hemiplegia.

하지 강직으로 인한 보행장애를 동반한 특발성 파킨슨병 환자에 대한 두침 및 추나 치료 임상증례 보고 (A Case Report of Scalp Acupuncture and Chuna Manual Therapy for a Patient with Idiopathic Parkinson's Disease with Walking Disorders due to Lower Extremity Rigidity)

  • The purpose of this study is to report the case of a patient with idiopathic Parkinson's disease treated with scalp acupuncture and Chuna manual therapy. A total of 10 sessions of scalp acupuncture and Chuna manual therapy were performed. At the beginning of the treatment, the Korean version of modified Barthel index (K-MBI) was 25 points, and the range of motion (ROM) of both knee joints was 150° in passive flexion, while active movement was impossible. In addition, both knee joints were flexed and contracted at -40° on the right and -30° on the left. Over the course of treatment, the ROM of both knee joints improved to -20° on the right and -10° on the left. The patient was even able to maintain a standing position and was allowed to walk approximately 10 steps with support. Temporary improvement was observed as a result of applying scalp acupuncture treatment and Chuna manual therapy to a patient whose symptoms were gradually worsening.

뇌졸중 환자의 일상생활 및 인지기능 회복에 대한 의·한의 협진 재활치료의 효과 (Effect of East-West Integrative Rehabilitation on Activities of Daily Living and Cognitive Functional Recovery in Stroke Patients: A Retrospective Study)

  • Objectives This study was conducted to verify the effectiveness of east-west integrative rehabilitation therapy on activity of daily living and cognitive functional recovery in stroke patients by comparing with integrative rehabilitation therapy group and conventional rehabilitation therapy group in a single institution. Methods The medical records of 106 stroke patients hospitalized in Department of Rehabilitation Medicine, Dongguk University Bundang Oriental Hospital from January 1, 2017 to February 28, 2019 were reviewed. After screening and dividing it into conventional rehabilitation (CR) group and integrative rehabilitation (IR) group, Korean version of Modified Barthel Index (K-MBI), functional independence measure (FIM), clinical dementia rating-sum of boxes (CDR-SB) were statistically analyzed. Results IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.001) and there was a statistically significant difference in K-MBI and CDR-SB score changes than CR group (p<0.05). And chronic patient of IR group showed significant improvement in K-MBI, FIM, and CDR-SB after treatment (p<0.01) and there was a statistically significant difference in CDR-SB score changes than CR group (p<0.05). In particular, the earlier the treatment initiation time, the more the improvement in function and when the treatment started within 2 years from the onset and patients took acupuncture and pulsed electromagnetic therapy, all scales significantly improved (p<0.001). Conclusions IR showed more improvement on activities of daily life and cognitive functional recovery than CR in this study.
