Trends of Dental Treatment under General Anesthesia and Patterns of Repeated General Anesthesia in Children and Adolescents with Severe Disabilities (소아청소년의 전신마취 하 치과 치료의 추세변화 및 치과 영역 중증 장애 유무에 따른 전신마취 재시행 양상에 대한 연구)

  • The aim of this retrospective study was to evaluate the demographic characteristics of pediatric dental patients who underwent dental treatment under general anesthesia (DTGA) at the Seoul National University Dental Hospital from January 2011 through December 2020 and compare the patterns of repeated DTGA between dental patients with severe disabilities (DSD) and non-DSD (healthy or medically compromised patients without DSD). There were 1,857 DTGAs among 1,719 patients (mean age = 5.1 years; males = 59.3%; ASA 2 or above = 52.9%; DSD = 26.8%). Overall, 6.6% of patients underwent repeated DTGA, and the rate of repeated DTGA over a 10-year period was 7.4%. ASA 2 or above (p < 0.0001) and DSD (p < 0.0001) were more likely to undergo repeated DRGA compared to ASA 1 and non-DSD. At both GA1 and GA2, DSD received significantly more restorative treatment on permanent teeth than non-DSD (p = 0.002, p < 0.0001, respectively). There has been an increasing demand for DTGA in pediatric dentistry over the last 10 years. Regular check-ups and preventive oral health care are necessary for pediatric dental patients with severe disabilities to reduce the possibility of repeated DTGA.

Rehabilitation with orthognathic surgery and orthodontic treatment in patient with severe occlusal disharmony: A case report (심한 교합 부조화를 보이는 환자에서 악교정수술 및 교정치료를 동반한 구강회복: 증례 보고)

  • The occlusal treatment including prosthetic treatment should be considered when the pathologic symptom was observed with the excessive discrepancy between the centric relation occlusion (CRO) and the maximum intercuspal position (MIP). Through careful diagnosis, the malocclusion and interarch relationship can be analyzed, and occlusal adjustment, restorative treatment, orthodontic therapy, or orthognathic surgery can be performed depending on the degree of disharmony. The patient in this case report complained the unstable occlusion and loss of masticatory function that had been occurring for several years. At the time of the visit, the patient showed severe occlusal disharmony, with only the upper right second molar contacting the lower jaw at the maximum intercuspal position. Based on the analysis of the occlusion, it was difficult to solve the problem with just occlusal adjustment or restorative treatment. In addition, the patient had the skeletal class II malocclusion between the upper and lower jaws. Therefore, for resolving the severe skeletal class II malocclusion, pre- and post-orthodontic treatment, bilateral sagittal split ramus osteotomy (BSSRO) was performed. After that, the occlusal adjustment was performed for stable occlusion, and the missing teeth area was restored with dental implants. During the follow-up period, a periodic follow-up visits and additional occlusal adjustments were performed to achieve a stable centric occlusion and harmonious anterior and lateral guidance. As a result, the final prosthodontic treatment was completed, and the patient's masticatory function was restored.

Estimation of Mandibular Third Molar Development Using the Correlation in Dental Developmental Stages (치아 발육 단계의 상관관계를 이용한 하악 제3대구치 발육 평가)

  • This study aims to confirm the average chronologic age according to the developmental stages of the mandibular canine (L3), first and second premolars (L4, L5), and second and third molars (L7, L8) in children and adolescents, and to confirm the developmental stage of L3, L4, L5, and L7, which can estimate the development of L8. A total of 1,956 digital panoramic radiographs of healthy individuals aged between 6 and 15 years who visited Seoul National University Dental Hospital from January 2019 to December 2020 were selected. The developmental stages of L3, L4, L5, L7, and L8 on both sides were evaluated using the dental maturity scoring system proposed by Demirjian and Goldstein. The average age at which the follicle of L8 was first observed was around 9.34 ± 1.35 years and varied from 6 to 12 years. The possibility of agenesis of L8 was high when no traces of L8 were observed after the following stages: L3, L4, and L5 at the developmental stage F and L7 at the developmental stage E; the age was about 10 years. In estimating the development of L8, when only one tooth was considered, estimation accuracy with L5 was the highest, and there was no significant difference when all four teeth were included. This study showed the age distribution according to the developmental stages of L3, L4, L5, L7, and L8 in children and adolescents and confirmed the developmental stages of L3, L4, L5, and L7, which can be used to estimate the development of L8.

Statistical Relationship between Sawtooth Oscillations and Geomagnetic Storms (Sawtooth 진동 현상과 지자기 폭풍의 통계적 관계)

  • We have investigated a statistical relationship between sawtooth oscillations and geomagnetic storms during 2000-2004. First of all we selected a total of 154 geomagnetic storms based on the Dst index, and distinguished between different drivers such as Coronal Mass Ejection (CME) and Co-rotating Interaction Region (CIR). Also, we identified a total of 48 sawtooth oscillation events based on geosynchronous energetic particle data for the same 2000-2004 period. We found that out of the 154 storms identified, 47 storms indicated the presence of sawtooth oscillations. Also, all but one sawtooth event identified occurred during a geomagnetic storm interval. It was also found that sawtooth oscillation events occur more frequently for storms driven by CME $({\sim}62%)$ than for storms driven by CIR $({\sim}30%)$. In addition, sawtooth oscillations occurred mainly $({\sim}82%)$ in the main phase of storms for CME-driven storms while they occurred mostly $({\sim}78%)$ during the storm recovery phase for CIR-driven storms. Next we have examined the average characteristics of the Bz component of IMF, and solar wind speed, which were the main components for driving geomagnetic storm. We found that for most of the sawtooth events, the IMF Bz corresponds to -15 to 0 nT and the solar wind speed was in the range of $400{\sim}700km/s$. We found that there was a weak tendency that the number of teeth for a given sawtooth event interval was proportional to the southward IMF Bz magnitude.

Study on the knowledge regarding caries prevention among pregnant women (임부의 구강건강관리지식에 대한 조사 연구 -영유아기 구강관리 내용 중심 고찰 -)

  • It is important to practice preventive method to control oral disease. Dental caries tendency has been decreased in developed countries, but early childhood caries(ECC, BBTD) became serious dental problems in many countries. To slove these problems, more positive and definite prenatal programs on infants and child are needed. Mostly the control of oral health was affected by a mother's knowledge and behavior in this stage. According to many studies, prenatal education is the most effective method to promote oral health in children. But in Korea, there is little oral health education programs for pregnant women, nursing mothers, infants and children. The purpose of this study was to access the knowledge and opinions of caries prevention among gravid women. 330 pregnant women(primiparae=181, multiparae=149), aged 23 to 39 years were randomly selected at prenatal health education courses in Seoul, Korea, 2000. Face-to-Face interviews were conducted by a trained interviewer using questionnaires included demographics, attitude toward children's dental cares, knowledge of early childhood caries, knowledge concerning pregnancy and oral health. The obtained results were as follows; (1) Most respondents were very concerned about their children's oral health. (2) 58% of the respondents were unaware of early childhood caries and the proportion was significantly higher among primipara (p<0.05). (3) Only 43% of the respondents believed that improper breast feeding could develop ECC, the proportion was significantly higher among primipara(p<0.05). (4) 82% of the respondents reportedly believed that the fetus takes away calcium from the mother's teeth during pregnancy. (5) 71% of the respondents believed that dental treatment during pregnancy was unsafe. There is a relatively low level of accurate knowledge regarding infant nutritions, maternal need for oral health and dental caries prevention among pregnant women. Oral health education and promotion programs that are science-based are needed for gravid women.

A Study on Patterns or Reality of Oral Health: Government Employees and Private School Employees (구강건강행태 및 실태에 대한 조사연구 (공무원 및 사립학교 교직원 중심으로))

  • Our study shows the following results of the patterns and reality of oral health of 862 Government employees and private school employees who had received oral examinations from April to May, 2000. For the exposition of each group to dental service, the male groups(37.4%) were more exposed than the female ones(30.0%, p<0.05), while the scaling experience results indicated that more ages mean more scaling(the over 50 year group 27.1%, 41-50 group 23.9%, 31-40 group 20.5%, 21-30 group 10.8%, p<0.001). The data involving sweet food and refreshments revealed that 'positive respondents' in the male groups were 19.7% with 12.5% positive in the female ones, and 'negative respondents' were 32.3% and 51.0% in the male and female groups respectively(p<0.001). For the oral odor, the male groups(37.9%) are more severe than the female ones(25.5%, p<0.01). The results of dental caries revealed that 19.1% of the male and 13.9% of the female said 'yes'(p<0.05), with 25.1% of the male and 11.1% of the female suffering from missing teeth(p<0.001); 77.4% of the male and 51.0% of the female from periodontal disease (p<0.001).

Satisfaction with Dental Care in Orthodontic Patients (치열 교정환자의 치과 의료서비스 만족도에 관한 연구)

  • In parallel with social and economic progress, motives of patients to consult dentists have increased and diversified. An increasing number of people visit dental clinics to improve their facial appearance and masticating functions deteriorated largely due to malocclusion. There is also greater interest in and concern as well as demand for teeth alignment treatment among people. Under this situation, it is required to find and suggest ways to enhance quality and profitability of dental care in view of managerial needs for dentists to cope with changing circumstances. This study, thus, aims to measure satisfactions of orthodontics patients and to determine the elements influencing their satisfactions. For this aim, a survey was performed for 20 days from March 7th to 26th, 2005 by means of questionnaires distributed to randomly selected samples, resulting in following conclusions: 1. From comparison of satisfactions with dental service by their elements, higher satisfaction was reported from orthodontics patients in the element of dental hygienists(4.21). Least satisfaction, however, was found in the procedure for treatment and waiting hours(3.73). 2. By gender, male orthodontics patients turned out to be more satisfactory with dental service than females. In terms of intention for return visits, both male and female orthodontics patients showed higher satisfaction than non-orthodontics ones. 3. As for satisfaction with dental service by age of orthodontics and non-orthodontics patients, with regard to intention for return visit, orthodontics patients of all ages showed to have higher grade of satisfaction. 4. Regarding satisfaction with dental treatment by orthodontics and non-orthodontics patients by monthly income, Dental hygienist with intention for return visit, the higher the satisfaction of orthodontics patients. 5. In regard to satisfaction with dental treatment by education, higher satisfaction was expressed by orthodontics patients with elementary school education kept dentist, dental hygienist and internal and external environments. As a result from this study, we knew that dental service of the orthodontics patients is higher satisfaction than that of the non-orthodontics patients.

Dental Caries Risk Can Be Predicted by Simply Measuring the pH and Buffering Capacity of Saliva (치아우식과 연관된 타액의 pH와 완충력)

  • This study examined the relationship between the quantity of Streptococcus mutans and Lactobacillus spp. related to dental caries and the degree of acidity in saliva. A total of 240 saliva samples were taken from 80 subjects at the faculty of dentistry in Skopje, Macedonia. The saliva samples were taken by stimulating saliva production stimulation with paraffin chewing. However, no stimulation was applied when obtaining the samples used for measuring the pH. The data showed that in the caries group, S. mutans in 1 ml of saliva formed colonies with confluent growth (CFU > $10^6$ and $10^4-10^5$) in 100% of samples, whereas the Lactobacillus spp. formed colonies with confluent growth in 78.3%. In contrast, no colonies with confluent growth (CFU > $10^6$ and $10^5$) were found in the control group (with healthy intact teeth). In the caries group, the pH of the saliva was slightly acidic (pH = 5.90 - 6.50) and the buffering capacity was very low (below 0.7 ml of saliva per min). On the other hand, the pH of the saliva in the control group was neutral (pH 7.01 - 7.7) and the buffering capacity was high (over 1 ml of saliva per min). The increased number of S. mutans and Lactobacillus spp. in 1 ml of saliva (above $10^5$ CFU or more) from the CRT (Caries Risk Test, Vivadent, Liechtenstein) bacteria test can indicate an increased caries risk as well as slightly higher acidity of the saliva. Overall, these results reveal that the caries risk can be predicted by simply measuring the pH and buffering capacity of saliva, and can be used to monitor the effect of dental hygiene practices with the aim of preventing dental caries.

Health Status and Health Behavior according to Perception Oral Malodor (일부 대학생의 구취자각에 따른 건강상태 및 건강행동)

  • In this study, a survey was conducted to check the health status and health-related behavior according to the subjective oral malodor of some university students targeting 1,490 students who are attending N university in Cheonan. Based on the result to confirm the awareness of oral malodor and systemic diseases status, there were many cases to be realized such as sinus infection, chronic rhinitis, asthma, gastrointestinal disorders and xerostama among the people who think that they have bad breath, not the people who think that they don't have bad breath (p<.05). Based on the result to confirm the subjective oral health condition according to awareness of oral malodor status, it was shown that there were more respondents who have some oral conditions such as 'there are tooth to be treated', 'there is food stuck between tooth' and 'the gums are often swollen' among the people who realize that they have bad breath, not the people who think that they don't have bad breath (p<.05). It was shown that there were more cases to be brushed teeth everyday among the people who realize that they don't have bad breath than the people who think that they have bad breath (p<.05) and there were more cases to be used dental floss and to be brushed tongue everyday among the people who think that they don't have bad breath (p<.01).

A Survey of Oral Health State, Oral Health Awareness and Dental Treatment perception for Middle Schoolers in Hwaseong (화성시 중학생의 구강건강 실태, 인식 및 치과치료에 관한 개별반응 조사)

  • The purpose of this study was to examine the oral health state, oral health awareness and oral health behavior of middle school students in a bid to raise concerns about oral health problems in a local community and help ensure the efficiency of oral health programs. The subjects in this study were 662 seventh graders in 14 different middle schools located in the city of Hwaseong. The findings of the study were as follows: Frist, In terms of indices related to dental caries, DMFT index numbered 2.0. Secondly, as to subjective oral health state, 22 percent of the boys and 10.9 percent of the girls found their own oral cavity to be healthy, and the gap between them and the others was statistically significant different(p=.000). Thirdly, in regard to tooth-related habits, the girls brushed their teeth more often than the boys(p=.000). Fourthly, for snack-intake habits, the largest number of the boys ate between meals once(42.1%), and the second largest group of them did it two or three times(31.3%). Fifthly, regarding how they felt about dental treatment, the greatest number of the boys and girls replied there's nothing special when they were asked about their mood in case of seeing a dentist next day. The second largest group answered they would get a little nervous. As to mood while waiting for treatment, the greatest number of the boys and girls answered they would get a little nervous. In regard to mood while lying on a dental chair, the largest group replied they would be nervous. Concerning mood while lying on a dental chair for scaling, most of them answered they would get nervous.
