Coben법에 의한 한국 아동의 두개안면골의 성장 변화에 관한 누년적 연구 (A LONGITUDINAL ROENTGENO-CEPHALOMETRIC STUDY ON THE CRANIOFACIAL DEPTH AND HEIGHT BY COBEN'S METHOD)

  • The morphology and growth changes of the Craniofacial Complex are important in orthodontics and cephalometrics for analysis and evaluation of facial bone growth are widely used. The author analyzed the data using lateral cephalometric roentgenogram of 75 Korean male and 50 Korean females age of 6 to 10 with normal occlusion to provide informations-relative rates of facial bone growth of Korean which is to be contributed in Korean standard. The results were as follows: 1. Means, standard deviation and coefficient of variation of Korean children were obtained. 2. The item which showed significent difference between male and female was craniofacial height in absolute dimension. 9. No difference of sex was showed in increment of craniofacial height and depth. 4. Among the craniofacial depth increments, the lower facial depth dimension increased most, midfacial depth dimension increased less, and cranial depth dimension increased the least. 5. The horizontal body of mandible showed rapid growing tendency more than did the ascending ramus.

소양인, 태음인의 표준 3차원 얼굴 모델링 개발 및 그 특성에 관한 연구 (Studies on the Modeling of the Three-dimensional Standard Face and Deriving of Facial Characteristics Depending on the Taeeumin and Soyangin)

  • Objectives This study was aimed to find the significant features of face form according to the Taeeumin and Soyangin by analyzing the three-dimensional face information data. Also, making standard face of the Taeeumin and Soyangin was an object of this study. Methods We collected three-dimensional face data of patients aged between 20~45 years old diagnosed by a specialist of Sasang constitutional medicine. The data were collected using a 3D scanner, Morpheus 3D(Morpheus Corporation, KOREA). Extracting a face feature point total of 64, was set to 332 pieces(height, angle, ratio, etc.) of each variable between feature points. ANOVA test were used to compare the characteristics of subjects according to the Taeeumin and Soyangin. Results When not to consider gender, the Taeeumin and Soyangin were different from the 18 items(3 items in the ear, 9 items in the eye, 1 item in the nose, 1 item in the mouth, 4 items in the jaw). When to consider gender, the Taeeumin and Soyangin men were different from the 6 items(1 item in the ear, 2 items in the nose, 3 items in the face). And the Taeeumin and Soyangin women were different from 17 items(1 item in the ear, 10 items in the eye, 2 items in the nose, 1 item in the mouth, 3 items in the face). Conclusions These results show Taeeumin's face(both men and women) width of the right and left is larger than the length of the top and bottom. Compared to men of Soyangin, men of Taeeumin has greater wings of the nose. Compared to women of Soyangin, women of Taeeumin has longer length of the eye. Soyangin's face(both men and women) length of the top and bottom is larger than the width of the right and left. Compared to men of Taeeumin, men of Soyangin has smaller wings of the nose. Compared to women of Taeeumin, women of Soyangin has more stereoscopic facial features at the top and bottom of the lateral face. Also, by accumulating three-dimensional face data, this study modeled the standard facial features by Taeeumin and Soyangin. These results may be helpful in the development of Sasang constitutional diagnostics utilizing the characteristics of the facial form at later.

일반인이 선호하는 얼굴의 미적 단위별 형태와 비율 연구 (A Study on Preferred Morphologic Feature and Proportion of Facial Aesthetic Subunit by Korean General Public)

  • Purpose: As the influence of mass media increases, the general standard of attractiveness or beauty of a face also changes. The primary purpose of the study is to find out the factors of the attractive and beautiful face recognized by public. Methods: We picked out standard model photography and operated with Adobe$^{(R)}$ Photoshop$^{(R)}$ and Monariza$^{(R)}$ virtual plastic surgery program. The contour of face, eye, nose, forehead, zygoma, chin and proportion of upper, middle, lower face were changed. The interview survey was conducted through structured standard photo for 310 respondents. That was utilized in the final analysis. Multiple regression analysis was executed by SPSS 12.0. It was used to deal with statistical data and all the other necessary analysis. Results: According to general characteristics of the respondents, many differences were found in preferred face and facial aesthetic subunits. The younger generation preferred the lozenge and inverted triangle shape contour. The respondents over 40 of age preferred the egg shape contour. In chin and zygoma contour, the respondents at the age of 20 preferred distinctly small chin and relatively small lower face. On the other hand, the respondents over 40 of age preferred the wide zygoma relatively. In the proportion of upper, middle, lower face, 51.0% of respondents answered 1 : 1 : 1. If they want to have an aesthetic operation, they preferred protruding forehead. Also they preferred the small chin and V-shaped chin in frontal view. Conclusion: Many respondents preferred to have a plastic surgery for the better facial subunit. The statistical evidence from this study suggests that the harmony and balance of facial aesthetic subunits make attractive and beautiful face.

An Algorithm to Guide Recipient Vessel Selection in Cases of Free Functional Muscle Transfer for Facial Reanimation

  • Background The aim of this study was to review the recipient vessels used in our cases of facial reanimation with free functional muscle transfer and to identify patient variables that may predict when the facial vessels are absent. From this we present a protocol for vessel selection in cases when the facial artery and/or vein are absent. Methods Patients were identified from November 2006 to October 2013. Data was collected on patient demographics, facial palsy aetiology, history of previous facial surgery/trauma and flap/recipient vessels used. A standard operative approach was adopted and performed by a single surgeon. Results Eighty-seven eligible patients were identified for inclusion amongst which 98 hemifaces were operated upon. The facial artery and vein were the most commonly used recipient vessels (90% and 83% of patients, respectively). Commonly used alternative vessels were the transverse facial vein and superficial temporal artery. Those with congenital facial palsy were significantly more likely to lack a suitable facial vein (P=0.03) and those with a history of previous facial surgery or trauma were significantly more likely to have an absent facial artery and vein (P<0.05). Conclusions Our algorithm can help to guide vessel selection cases of facial reanimation with free functional muscle transfer. Amongst patients with congenital facial palsy or in those with a previous history of facial surgery or trauma, the facial vessels are more likely to be absent and so the surgeon should then look towards the transverse facial vein and superficial temporal artery as alternative recipient structures.

Facial Nerve Repair following Acute Nerve Injury

  • Background Acute facial nerve iatrogenic or traumatic injury warrants rapid management with the goal of reestablishing nerve continuity within 72 hours. However, reconstructive efforts should be performed up to 12 months from the time of injury since facial musculature may still be viable and thus facial tone and function may be salvaged. Methods Data of all patients who underwent facial nerve repair following iatrogenic or traumatic injury were retrospectively collected and assessed. Paralysis etiology, demographics, operative data, postoperative course, and outcome were examined. Results Twenty patients underwent facial nerve repair during the years 2004 to 2019. Data were available for 16 of them. Iatrogenic injury was the common category (n = 13, 81%) with parotidectomy due to primary parotid gland malignancy being the common surgery (n = 7, 44%). Nerve repair was most commonly performed during the first 72 hours of injury (n = 12, 75%) and most of the patients underwent nerve graft repair (n = 15, 94%). Outcome was available for 12 patients, all of which remained with some degree of facial paresis. Six patients suffered from complete facial paralysis (50%) and three underwent secondary facial reanimation (25%). There were no major operative or postoperative complications. Conclusion Iatrogenic and traumatic facial nerve injuries are common etiologies of acquired facial paralysis. In such cases, immediate repair should be performed. For patients presenting with facial paralysis following previous surgery or trauma, nerve repair should be considered up to at least 6 months of injury. Longstanding paralysis is best treated with standard facial reanimation procedures.

안면마비 환자에 대한 Sunnybrook Facial Grading System의 적용 실태 분석 : 후향적 관찰연구 (The Application State of the Sunnybrook Facial Grading System for Facial Palsy Patients : A retrospective study)

  • Objectives : Among the assessment tools for evaluating facial function, the House-Brackmann scale is used as a standard tool, but it has some shortcomings. The Sunnybrook Facial Grading System can assess the after effects of facial palsy and facial movement by each part of the face. By understanding the application state of this Sunnybrook Facial Grading System, we intend to analyze the relationship between House-Brackmann scale score and Sunnybrook Facial Grading System score so that we can examine the advantages of the Sunnybrook Facial Grading System as a more accurate tool. Methods : We screened both inpatients and outpatients who visited the Facial Palsy Center at Kyung Hee University Hospital for Korean medical treatment and were evaluated with the Sunnybrook Facial Grading System from December 2015 to October 2016. A total of 159 out of 166 patients were studied, including basic characteristics and missing data. We used descriptive statistics for general features of patients and SPSS Ver.18 for statistical analysis. Results : House-Brackmann scale and Sunnybrook Facial Grading System have high negative correlation through Pearson Correlation Coefficient with a score of -0.884. Analyzing outlier data resulting from relation analysis between the House-Brackmann scale and the Sunnybrook Facial Grading System showed many outliers when the damaged state of each part of the face is different. Conclusion : Sunnybrook Facial Grading System can make up for faults of the House-Brackmann scale, which is inferior in accuracy when each damage status of each part of the face is different. Sunnybrook Facial Grading System performs a detailed assessment of facial function and sequelae of facial palsy easier than the House-Brackmann scale.

한국 표준 얼굴 데이터를 적용한 3D 가상 얼굴 성형 제작 시스템 설계 및 구현 (Designing and Implementing 3D Virtual Face Aesthetic Surgery System Based on Korean Standard Facial Data)

  • 본 논문은 한국 표준 얼굴 데이터를 바탕으로 제작한 3D 얼굴 모델을 활용하여 성형 수술 전후의 모습을 비교하여 성형 수술의 만족도를 높이는 웅용 프로그랭인 3D 가장 얼굴 성형 시스댐에 대한 연구 및 구현이다. 이 연구를 위하여 2D 개인 얼굴 이미지와 3D 한국인 표준 모델을 활용하여, 2D 이미지 상의 사용자의 실제 얼굴과 똑같은 3D 얼굴 모델을 구현한다. 본 연구에서 제안하는 3D 가상 얼굴 성형 시스템은 얼굴 모텔의 기본이 되는 3D 한국인 표준 얼굴 모델에 2D 개인 얼굴 이미지로부터 추출한 개인 특성 정보를 적용함으로서 3D 얼굴 모델링 시스템을 구현하고 3D 얼굴의 사실성올 높이는 텍스처 맴핑 및 구축된 얼굴을 활용하여 성형 수술 후의 얼굴의 변화를 나타내는 3D 얼굴 성형 시스댐 등으로 구성되어 있다. 제안된 3D 가상 얼굴 성형 시스템은 정확한 개인 얼굴 모델링 방법을 제공하고, 성형 전후의 얼굴을 미세하게 조정하는 기능을 부여함으로써 현존하는 유사 시스템에 비교하여 성형 수술의 만족도와 안정성을 높이는 결과를 나타내었다.

안면비대칭 진단을 위한 하악골 3차원영상 계측기준치에 관한 연구 (A study on the 3-D standard value of mandible for the diagnosis of facial asymmetry)

  • 기존의 2차원적 진단자료는 상의 확대 및 왜곡으로 인해 형태나 크기의 정량적 평가시 부적절한 결과를 야기할 수 있어 안면비대칭의 정확한 진단과 치료계획 수립을 위해서는 3차원영상 진단의 도입이 필요하다. 본 연구는 안면비대칭 진단시 3차원적 분석에 도움이 되는 기준자료를 얻고자 정상교합자를 대상으로 하악골 3차원 영상 계측항목의 평균값과 좌우차이의 정상범주를 알아보기 위하여 시행되었다 뚜렷한 안면비대칭을 보이지 않는 성인 정상교합자 남녀 30명씩 총 80명을 대상으로 두경부 전산화단층사진을 촬영한 후 컴퓨터프로그램을 이용한 각각의 단면 촬영영상 정보를 이용하여 3차원 입체영상을 재구성한 다음, 하악골 비대칭시 좌우차이를 보일 수 있는 계측항목으로 ramus length를 나타내는 5개 condylar neck length를 나타내는 1개, mandibular body length를 나타내는 8개 등 15개의 거리 계측항목과, gonial angle을 나타내는 4개. frontal ramal inclination을 나타내는 2개. lateral ramal inclination을 나타내는 2개 등 8개의 각도계측항목, 총 23개의 계측항목을 설정한 후, 좌측과 우측을 구분하여 컴퓨터 상에서 3차원계측치를 구하고 좌우차이값을 산출하였다. 본 연구결과 얻어진 정상교합자의 하악골 3차원영상 계측항목의 좌우차이값은 안면비대칭 환자의 진단 기준치로 유용하게 사용될 수 있을 것이다.

  • This study investigated differences in children's facial, behavioral, and verbal expressions of pride and shame by their age, and type and level of task difficulty when they succeeded or failed on tasks. The subjects were 55 4-year-old children from 2 day-care centers and 37 7-year-old children from 1 elementary school. Each child participated in pride and shame assessment sessions. Data were analyzed by means, standard deviations, repeated measures ANOVA, and paired t-test. When they succeeded on tasks, 4-year-olds showed more pride in behavioral expression than 7-year-olds. More pride in facial, behavioral, and verbal expressions were shown when subjects succeeded on difficulty than on easy tasks. Interaction effect for behavioral expression of pride was found between age and type of task difficulty. In verbal expression of pride, interaction effect was found between age, and type and level of task difficulty. When they failed on tasks, 7-year-olds showed more shame in behavioral and verbal expressions than 4-year-olds. More shame in facial, behavioral, and verbal expressions were shown when subjects failed on easy than on difficulty tasks. Interaction effect for facial expression of shame was found between age, and type and level of task difficulty.

The Functionality of Facial Appearance and Its Importance to a Korean Population

  • Background Many people have an interest in the correction of facial scars or deformities caused by trauma. The increasing ability to correct such flaws has been one of the reasons for the increase in the popularity of facial plastic surgery. In addition to its roles in communication, breathing, eating, olfaction and vision, the appearance of the face also plays an important role in human interactions, including during social activities. However, studies on the importance of the functional role of facial appearance. As a function of the face are scare. Therefore, in the present study, we evaluated the importance of the functions of the face in Korea. Methods We conducted an online panel survey of 300 participants (age range, 20-70 years). Each respondent was administered the demographic data form, Facial Function Assessment Scale, Rosenberg Self-Esteem Scale, and standard gamble questionnaires. Results In the evaluation on the importance of facial functions, a normal appearance was considered as important as communication, breathing, speech, and vision. Of the 300 participants, 85% stated that a normal appearance is important in social activities. Conclusions The results of this survey involving a cross-section of the Korean population indicated that a normal appearance was considered one of the principal facial functions. A normal appearance was considered more important than the functions of olfaction and expression. Moreover, a normal appearance was determined to be an important facial function for leading a normal life in Korea.
