Background: Paraben is a widely used substance with a preservative effect found in various materials such as food, medicine, personal care products, and cosmetics. Objectives: This study was conducted to identify the level of urinary paraben concentrations (i.e., methyl-, ethyl-, and propyl-) among Korean adults and to explore the factors related with the exposure levels. Methods: We analyzed the third period (2015~2017) of the Korean National Environmental Health Survey (KoNEHS). R statistical software (version 4.1.1) was used to estimate representative values for the whole population with weight variables to reflect sampling design. Whether urinary concentrations tended to increase as the level of paraben exposure-related characteristics increased was tested and Ptrend was calculated using general linear models. Results: Urinary concentrations of all three parabens (i.e., methyl-, ethyl- and propyl-) were higher in women than in men (Ptrend<0.0001, 0.008, and <0.0001), and the values of methylparaben and propylparaben tended to increase as the age of subjects increased (Ptrend<0.0001, and <0.0001). Urinary concentrations of methylparaben and propylparaben were associated with intensity of exercise (Ptrend<0.001, and 0.004), and that of propylparaben was higher in non-smokers (Ptrend=0.01). In terms of paraben exposure-related variables, urinary concentrations of parabens (i.e., methyl-, ethyl- and propyl-) increased as the daily average frequency of teeth-brushing (Ptrend<0.0001, 0.03 and 0.0001), the frequency of use of hair products (Ptrend=0.005, 0.05 and 0.04), the frequency of use of makeup products (Ptrend<0.001, 0.001 and <0.001), and the frequency of use of antibacterial products (Ptrend=0.005, 0.02 and 0.02) increased. Conclusions: In our study, urinary concentrations of all three parabens are associated with gender, teethbrushing, hair products, make-up products, and antibacterial products. Methyl- and proyl-parabens were associated with age and intensity of exercise, and propyl-paraben was associated with smoking.
Objectives: In order to investigate the relation between environmental pollution and its effect on human health in large-scale industrial complex regions, monitoring studies have been conducted since 2003. The main purpose of this Gangneung study is to produce background data which can be compared with several large-scale industrial complex regions such as Ulsan, Sihwa, Banwol, Gwangyang, Yeosu, Pohang and Cheongju Daesan industrial complexes. Methods: In this Gangneung study, 1,007 local residents were recruited and the framework of this study was designed to evaluate and monitor health effects associated with low-level but long-term exposure to environmental pollutants. A survey was performed based on personal questionnaires, medical check-ups, and allergy skin prick tests for twelve common allergens. The authors reviewed the prevalence rate of skin allergens in Gangneung compared with in large-scale industrial areas. The results are summarized as follows. Results: The diagnosis prevalence and medical treatment prevalence of asthma were lower than in industrial complex regions, but allergic rhinitis was higher. The allergy skin test prevalence was 26.0% and the most common allergens were dust mites (D. pteronysinus 16.7% and D. farinae 16.3%). Conclusions: The positive findings of the skin prick test were similar with large-scale industrial complex regions in Korea. Further study is required to find the cause of the regional differences in skin prick prevalence.
A national survey for enteric virus contamination in treated water and its source water was performed from March to November 1999. The water samples were subjected to virus filtration at the major water plants producing over 10$^5$ tons treated water per day. Twenty surveyed sites encompass most of heavily populated residential area except for Seoul and Pusan. (omitted)

Objectives: Environmental pollution is a significant global issue. Both objective (scientifically measured) environmental pollution and perceived levels of pollution are important predictors of self-reported health. The purpose of this study was to compare the associations between perceived environmental pollution and health in China, Japan, and South Korea. Methods: Data were obtained from the East Asian Social Survey and the Cross-National Survey Data Sets: Health and Society in East Asia, 2010 (n=7938; China, n=3866; Japan, n=2496; South Korea, n=1576). Results: South Koreans perceived environmental pollution to be the most severe, while Japanese participants perceived environmental pollution to be the least severe. Although the Japanese did not perceive environmental pollution to be very severe, their self-rated physical health was significantly related to perceived environmental pollution, while the analogous relationships were not significant for the Chinese or Korean participants. Better mental health was related to lower levels of perceived air pollution in China, as well as lower levels of perceived all types of pollution in Japan and lower levels of perceived noise pollution in South Korea. Conclusions: Physical and mental health and individual socio-demographic characteristics were associated with levels of perceived environmental pollution, but with different patterns among these three countries.
Objectives: The purpose of this study was to assess excavator drivers in order to determine the proportion showing subjective symptoms of musculoskeletal disorders and to evaluate the association between the symptoms and characteristics of the subjects. Methods: A questionnaire survey was conducted to collect the information on participants' age, average working hours per day, duration of career (years), work-related musculoskeletal symptoms, and more. The final dataset included 141 excavator drivers. Frequency and percentage were summarized and then the associations between the characteristics of the subjects and symptoms were evaluated as odds ratios (ORs) and 95% confidence intervals (CIs) using multiple logistic regression analysis. Results: Seventy-six percent of the currently reported musculoskeletal symptoms. Those who have musculoskeletal symptoms attributed their symptoms to unstable posture at work, mental stress or vibrations. There was a significant association between symptoms of musculoskeletal disorders and career of excavator drivers (P for trend=0.04). Conclusions: The results of this study suggest that the prevalence of musculoskeletal disorders may be high among excavator drivers and warrants well-designed studies on work-related disorders among excavator drivers.
Exactly 25 years after the Chernobyl nuclear disaster, 11th of March 2011 the Fukushima nuclear accident occurred in Japan and was ranked at level 7 same to the Chernobyl. A Korean and Japanese joint civil survey was conducted around Fukushima on April 13-17. The radiation survey data clearly shows a large hotspot area between 20 km and 50 km radius north and west direction from the accident reactors, with the highest radiation recorded being 55.64 ${\mu}Sv/hr$ in the air, 99.89 ${\mu}Sv/hr$ in the surface air, and 36.16 ${\mu}Sv/hr$ in a car, respectively. 3.65 ${\mu}Sv/hr$ in the air and 6.89 ${\mu}Sv/hr$ in the surface air were detected at the playground of an elementary school in Fukushima City. Spring came with full cherry blossoms in Fukushima, but it was silent spring of radiation contamination. Interviews with Fukushima nuclear refugees reveal serious problems about Japanese nuclear safety systems, such as there was no practical evacuation drill within 1-10 km and no plan at all for 10-30 km areas. Several reforms items for Korean nuclear safety system can be suggested: minimization of accident damage, clear separation of regulatory and safety bureaus with a new and independent administrating agency, community participation and agreement regarding the safety system and levels, which is the major concern of 80% Korean. To tackle threats of nuclear disaster in neighboring nations like China, a new position entitled 'Ambassador for nuclear safety diplomacy' is highly necessary. The nuclear safety of Korea should no longer be the monopoly of those nuclear engineers and limited technocrats criticized as a 'nuclear mafia'.

Background: Phthalates are endocrine disrupting chemicals that are widely used in the production of items of daily life such as in polyvinylchloride plastics, insecticides, and medical devices. This study aimed to determine the association between phthalate exposure and shellfish consumption using data from the Korean National Environmental Health Survey (KoNEHS) cycle 3 (2015-2017), which is a nationally representative survey. Methods: In this study, we analyzed the KoNEHS cycle 3 data of 3,333 (1,526 men and 1,807 women) adults aged more than 19 years. Data related to the variables of sociodemographic factors, health-related behaviors, dietary factors, seafood consumption frequency, and urinary phthalate metabolites concentrations were collected. The concentrations of urinary phthalate metabolites of all the participants were divided into quartiles to define high and low concentration groups based on the 75th percentile concentration. A χ2 test was conducted to analyze the distribution of independent variables. To analyze the relationship between shellfish consumption and phthalate exposure, the odds ratios (ORs) were calculated using logistic regression analysis. Results: Total adults with shellfish consumption frequency of over once a week showed the following adjusted ORs for high concentrations of the following metabolites compared with the group that consumed shellfish once a week or less: 1.43 (95% confidence interval [CI]: 1.01-2.06) for mono-(2-ethyl-5-oxohexyl) phthalate (MEOHP), 1.43 (95% CI: 1.01-2.03) for mono-(2-ethyl-5-carboxypentyl) phthalate (MECPP), 1.57 (95% CI: 1.10-2.24) for ∑di-2-ethylhexyl phthalate (∑DEHP), 2.01 (95% CI: 1.46-2.77) for mono-carboxyoctyl phthalate (MCOP), 1.56 (95% CI: 1.11-2.18) for mono-carboxy-isononly phthalate (MCNP), and 2.57 (95% CI: 1.85-3.56) for mono (3-carboxypropyl) phthalate (MCPP). Conclusions: The concentrations of urinary phthalate metabolites (MEOHP, MECPP, ∑DEHP, MCOP, MCNP, and MCPP) were higher in adults with a higher frequency of shellfish consumption.
Objective: This study was performed to estimate the number of those who used humidifier disinfectants (HDs) and experienced health effects from exposure to HDs in Korea between 1994 and 2011. Methods: A nationwide interview survey was conducted for the representative sample to identify the proportion of those who used HDs among the general population (n=3,001). Another online survey was conducted for those exposed to HDs to find the proportion of those who experienced health effects among those who were exposed to HDs (n=3,993). Statistics for population size by region and year (1994-2011) were used to estimate the cumulative number of those exposed to HDs and those who experienced health effects. In terms of the proportion of those exposed to HDs, those less than 30 years of age were excluded due to an issue related to information bias. Various approaches for estimation included the capture-recapture method for estimation of those who experienced health effects. Results: The cumulative proportion of those exposed to HDs was 6.7% among the general population, and the proportion of those who experienced health effects among those who were exposed to HDs was 13.9%. Based on these factors, it was estimated that 3.5 to 4.0 million people were exposed to HDs and 350 to 400 thousand experienced health effects at least requiring visiting a hospital. Conclusion: It is suggested that a nationwide representative sample may be essential for population size estimation of those exposed to environmental risk factors and of those who experienced health effects.
Objective: A significant association between blood lead levels and hypertension has been reported in many studies. The relationship between cadmium and hypertension has been debated as well. We aimed to study the association of lead, cadmium, and both with hypertension in the Korean general population. Methods: We examined 5,967 adult men and 6,074 women who participated in the Korea National Health and Nutrition Examination Survey III-VI (2005, 2008-2013 years). Logistic regression models were used to examine the relationship between blood lead concentration and blood cadmium concentration and hypertension using logtransformed blood lead and cadmium concentrations as independent variables after covariate adjustment. Results: Adjusted for general characteristics, the odds ratio of log-lead to hypertension was 2.71 (1.82-4.03), and log-cadmium to hypertension was 2.52 (1.83-3.47). Estimates were found to be statistically significant (p<0.001). When a multiple logistic model was applied, the odds ratio of log-lead and log-cadmium for hypertension were 2.24 (1.50-3.36) and 2.24 (1.62-3.10), respectively. The standardized estimate coefficients of log-lead and logcadmium for hypertension were 4.77 and 6.65, respectively. Conclusion: We observed the association of blood lead concentration, blood cadmium concentration, and both with hypertension. This study suggests that exposure to lead and exposure to cadmium are both risk factors for hypertension.
Objectives: Since 1998, the Korea National Health and Nutrition Examination Survey (KNHANES) has been conducted in order to investigate the health and nutritional status of Koreans. The food intake data of individuals in the KNHANES has also been utilized as source dataset for risk assessment of chemicals via food. To improve the reliability of intake estimation and prevent missing data for less-responded foods, the structure of integrated long-standing datasets is significant. However, it is difficult to merge multi-year survey datasets due to ineffective cleaning processes for handling extensive numbers of codes for each food item along with changes in dietary habits over time. Therefore, this study aims at 1) cleaning the process of abnormal data 2) generation of integrated long-standing raw data, and 3) contributing to the production of consistent dietary exposure factors. Methods: Codebooks, the guideline book, and raw intake data from KNHANES V and VI were used for analysis. The violation of the primary key constraint and the $1^{st}-3rd$ normal form in relational database theory were tested for the codebook and the structure of the raw data, respectively. Afterwards, the cleaning process was executed for the raw data by using these integrated codes. Results: Duplication of key records and abnormality in table structures were observed. However, after adjusting according to the suggested method above, the codes were corrected and integrated codes were newly created. Finally, we were able to clean the raw data provided by respondents to the KNHANES survey. Conclusion: The results of this study will contribute to the integration of the multi-year datasets and help improve the data production system by clarifying, testing, and verifying the primary key, integrity of the code, and primitive data structure according to the database normalization theory in the national health data.
