수은 고노출 지역 초등학생의 수은노출수준과 노출요인 연구 (A Study on Mercury Exposure Levels among Schoolchildren and Related Factors in High Mercury Exposure Areas in Korea)

  • Objectives: Blood mercury levels among adults living in certain areas of the Gyeongsang Provinces have been shown to be very high (Kunwee County $29.6{\mu}g/L$, Yeongcheon-city $26.7{\mu}g/L$). The purpose of this project was to determine mercury exposure levels in schoolchildren and factors related with their mercury levels in high mercury exposure areas identifyed by the 2007 Korea National Environmental Health Survey. Methods: From June to September 2010, 1,097 students from grades 3 to 6 at 19 elementary schools participated in this study, including 294 students from 10 elementary schools in Kunwee County, 529 students from Yeongcheon City, 122 students from two elementary schools in Pohang City, North Gyeongsang Province, and 152 students from two elementary schools in Ulsan Metropolitan City. Biological samples from schoolchildren, including whole blood, urine and hair, were collected to measure total mercury at the time of a health check up. Information about children was collected by questionnaire. Total mercury concentrations in blood were measured using the Direct Mercury Analyzer 80 with the gold-amalgam collection method. Results: The mean mercury levels were $2.70{\mu}g/L$ in 1,091 blood samples, $2.25{\mu}g/g-creat.$ in 820 urine samples and $1.03{\mu}g/g$ in 1,064 hair samples. Blood mercury levels in the schoolchildren was slightly higher than the result of $2.4{\mu}g/L$ from a 2006 survey of elementary school children on exposure and health effects of mercury by the National Institute of Environmental Research. However, 0.3% and 4.5% of participants exceeded the reference level of blood mercury by CHBMII ($15{\mu}g/L$) and the US EPA ($5.8{\mu}g/L$), respectively. The reference level of urine by CHBMII ($20{\mu}g/L$) was exceeded by 0.4% of participants. As factors, residence period in the study areas, residence type, father's education level and income all showed significant associations with mercury level in the biological samples. The number of dental amalgam sides showed an association with urine mercury. Fish intake preference and fish intake frequency were important factors in mercury levels. In particular, intake of shark meat and recent intake of shark meat were associated with higher mercury levels. In this regard, participation in the performance of an ancestral rite showed a relation with higher mercury levels. Conclusion: The intake of shark meat was very important factor to high mercury exposure level. It is recommended to monitor and manage students with high mercury exposures who exceeded CHBM II and EPA guidelines, and include blood mercury testing in the Children's Health check up for this province.

  • Objectives: According to the 2007 Korea National Environmental Health Survey (KNEHS), some areas in the Gyeongsang Provinces showed very high blood mercury levels in adults. We conducted this project to investigate any related health effects in children due to mercury levels in these areas. Methods: In total, 1,097 students between grades 3 and 6 at 19 elementary schools were recruited from four areas with high mercury exposure as identified by the KNEHS. Total mercury levels in biological samples were compared with health check-ups performed on the schoolchildren. Biological monitoring, supported by questionnaires, a computerized neurobehavioral test, a posturography test and a personality test, were applied. Results: Triglycerides showed a significant relation with mercury in blood, urine and hair. Total mercury concentrations were divided into two groups: upper and lower concentration groups based on the median value. In the computerized neurobehavioral test, the upper blood mercury group showed a greater reaction time for color-word vigilance (p<0.05). In the posturography test, the intensity value of the tremor test showd high significant relations with mercury levels (p<0.01). In the personality test, self-consciousness, misdeeds and family relationships showed significant differences between the upper and lower urine mercury groups (p<0.01), and specific reactions, ego resilience and hyperactivity also showed some differences (p<0.1). Conclusion: Some items in the neurobehavioral test, posturography test and personality test showed significant relations with biological mercury levels. Therefore, monitoring and appropriate management of students showing high mercury levels are recommended in order to reduce their mercury exposure.

노인의 환경인식과 주관적 건강과의 관련성 (Association between Environmental Perception and Subjective Health Status of Older Adults)

  • Objectives: The purpose of this study is to examine the level of community environmental perception of older adults and to identify the association between environmental perception and subjective health status among older persons. Methods: Analyses are based on data from Statistics Korea 2016 Social Survey. This study analyzed a total of 8,193 older adults aged 65 and over, and examined the association between environmental perception on air, water, soil, noise and vibration, and subjective health status using multiple regression analysis. Results: Study findings shown that environmental perception on water and soil had significant association with subjective health status of older adults. Specifically, older adults reported worse health status when they had more negative perceptions of water and soil environments in their communities. Conclusions: Results of this study suggested that efforts to enhance safety of drinking water and soil environments, and continuous research to identify relationship between environment and elderly health shoud be needed.

혈중수은 노출에 따른 일부 초등학생의 건강영향에 관한 연구 (Health Effects of Mercury Exposure on Some School Children in Korea)

  • As mercury absorbed into body can cross the blood-brain barrier and react with DNA and RNA. Central nervous system has been known to be affected especially in children. But it was very difficult to know the influences of chronic low-does Hg exposure on the health. Although many studies investigated the affect, most of results were still disparate. In order to investigate the health effects of mercury exposure, several test were conducted for some Korean school children. The general health effects were investigated using blood test, Posturography and computer-based neurobehavioral test was done to examine the affect of Hg into neural responses. About 400 children were chosen for blood test whose blood Hg level were upper and lower 10% of population participated in the nationwide Hg exposure survey. The concentration of calcium and creatine, the number of white and red blood cell showed statistical significance with Hg exposure in blood test. Another 36 children were selected from the same participants for the posturography and neurobehavioral test. The intensity and center frequency of hand tremor which were related to unconsciousness also showed distinct significances. Any general relations with Hg exposure were not found in all test including computer-based neurobehavioral test.

  • This study was carried out to investigate inflow stream of Chungju intake reservoir and in catchment area, run-off loading amount, distribution of Nitrogen and phosphorus, N/P ratio, correlationship between T-N and chlorophyll a, correlationship between T-P and chlorophyll a, and study of trophic state. Field survey was carried out from June to August, 1985, for the purpose of finding out the nitrogen and phosphorus concentration in Chungju intake reservoir.

  • Objectives: In many previous studies, endocrine disruptors (EDCs) have been found to affect delays in puberty. Various EDCs have been reported on, but there have been only limited epidemiologic studies on the effects of exposure to environmental heavy metals and pyrethroid pesticides on puberty delay. Therefore, the aim of this study was to investigate the association of exposure to environmental mercury, cadmium, and pyrethroid pesticides with delayed puberty in children based on national survey data that represents Korean children. Methods: We selected 450 children at the ages of 9-11 years old from the third Korean National Environmental Health Survey (3rd KoNEHS). The relations of urinary Hg, Cd, and 3-PBA with pubertal development were evaluated using multiple logistic regression analyses. Results: Urinary Hg levels were significantly associated with delayed puberty in boys [testicle development: OR=0.41 (95% CI: 0.20, 0.84); genitalia development: OR=0.35 (95% CI: 0.15, 0.81)]. Girls with higher Hg levels were more likely to experience delayed menarche [OR=0.23 (95% CI: 0.06, 0.90)]. We observed a significant 49% reduction in odds for menarche per increasing unit of urinary cadmium levels [OR=0.51 (95% CI: 0.24, 1.01)]. In addition, urinary 3-PBA showed a negative association with genitalia development in boys and menarche in girls [genitalia development: OR=0.73 (95% CI: 0.55, 0.96); menarche: OR=0.56 (95% CI: 0.32, 1.00)]. Conclusions: The results of this study support the hypothesis that exposure to environmental mercury, cadmium and pyrethroid pesticides may affect puberty delays. Additional evidence needs to be obtained through further prospective studies.

  • We live in an age of information. We also live in an era of rising environmental awareness. As a result, efficient and judicious use of environmental information has become a prerequisite to planning and management of environmental affairs in contemporary societies. During the past few decades, we have recorded a phenomenal rate of growth. and experienced an array of environmental problems such as pollution, destruction of natural environment, and ecological disturbances. In response to these problems we has embarked on a study of Environmental Information System(EIS). This study has four major components. First, it describes the current status of EIS in Korea. Second, it provides a survey of EIS in other countries, specifically USA and Japan. Third, the study also presents an application of GIS(Geographic Information System)in Korea, USA, and Japan. Finally, the study outlines strategies to develop EIS in Korea.

한우 농가의 농작업 안전보건 실태 및 안전관리 수준 조사 (A Survey on the Current Status of Safety and Health and of Safety Management Levels among Korean Native Cattle Farms)

  • Objectives: The present study was conducted to investigate farm work environments among farmers and examine the level of management of safety and health, and to subsequently produce study result to serve as foundational data for the development of guidelines on safety and health as part of the improvement of farming work environments among farmers raising Korean native cattle. Methods: The present study conducted a survey on farm work environments and the management of safety and health with 407 farmers engaged in Korean native cattle farming in selected regions in eight provinces. It also visited 10 farmers to verify the current status of farm work. Results: The survey results showed that 16.4% of the respondents experienced safety-related accidents due to farm work. The locations of the accidents were inside the cattle shed (71.4%) and facilities outside the cattle shed (19.6%). The types of accident showed collision with animals (35.7%), collision or contact with obstacles (27.1%), and musculoskeletal accidents due to heavy object handling (12.9%). The causes of the accidents were cattle (38.3%), cultivators and tractors (25.4%), facility tools in cattle sheds (9.0%), and slippery floors (6.0%). The damaged areas were hand (21.0%), spine (19.8%), lower limb (18.5%), and foot (17.3%). A self-diagnostic survey on respiratory diseases showed that 11.5% of the respondents experienced respiratory-related symptoms. The survey on safety and health during farm work showed that wearing personal protective equipment and response to emergency situations, which were needed to prevent safety-related accidents, were relatively low compared to the level of recognition of the need and awareness of safety issues. Furthermore, the field survey identified the current status of safety and health issues such as prevention management of collision accidents with cattle, how to handle heavy objects, and wearing of personal protective equipment. Conclusions: The present study identified safety-related accidents and problems in the management of safety and health among Korean native cattle farmers. In order to address the problem, it is necessary to not only provide guidelines on safety and health management which are appropriate to the characteristics of Korean native cattle farming work, but also to study the development of personal protective equipment.

  • Background: Pyrethroid insecticides account for more than 30% of the global insecticide market and are frequently used in agricultural settings and residential and public pest control among the general population. While several animal studies have suggested that exposure to pyrethroids can alter glucose homeostasis, there is only limited evidence of the association between environmental pyrethroid exposure and diabetes in humans. Objectives: This study aimed to report environmental 3-phenoxybenzoic acid (3-PBA) concentrations in urine and evaluate its association with the risk of diabetes in Korean adults. Methods: We analyzed data from the Korean National Environmental Health Survey (KoNEHS) Cycle 2 (2012~2014) and Cycle 3 (2015~2017). A total of 10,123 participants aged ≥19 years were included. Multiple logistic regressions were used to calculate the odds ratios (ORs) for diabetes according to log-transformed urinary 3-PBA levels. We also evaluated age, sex, education, monthly income, marital status, alcohol drinking, physical activity, urinary cotinine, body mass index, and sampling season as potential effect modifiers of these associations. Results: After adjusting for all the covariates, we found significant dose-response relationships between urinary 3-PBA as quartile and the prevalence of diabetes in pooled data of KoNEHS Cycles 2 and 3. In subgroup analyses, the adverse effects of pyrethroid exposure on diabetes were significantly stronger among those aged 19~39 years (p-interaction<0.001) and those who consumed high levels of cotinine (p-interaction=0.020). Conclusions: Our findings highlight the potential diabetes risk of environmental exposure to pyrethroids and should be confirmed in large prospective studies in different populations in the future.

뇨중 파라벤 농도에 영향을 미치는 요인에 관한 연구: 제3기 국민환경보건기초조사 자료 분석 (A Study on the Factors Affecting Urinary Paraben Concentration: An Analysis of the Third Korean National Environmental Health Survey (KoNEHS) Data)

  • Background: Paraben is a widely used substance with a preservative effect found in various materials such as food, medicine, personal care products, and cosmetics. Objectives: This study was conducted to identify the level of urinary paraben concentrations (i.e., methyl-, ethyl-, and propyl-) among Korean adults and to explore the factors related with the exposure levels. Methods: We analyzed the third period (2015~2017) of the Korean National Environmental Health Survey (KoNEHS). R statistical software (version 4.1.1) was used to estimate representative values for the whole population with weight variables to reflect sampling design. Whether urinary concentrations tended to increase as the level of paraben exposure-related characteristics increased was tested and Ptrend was calculated using general linear models. Results: Urinary concentrations of all three parabens (i.e., methyl-, ethyl- and propyl-) were higher in women than in men (Ptrend<0.0001, 0.008, and <0.0001), and the values of methylparaben and propylparaben tended to increase as the age of subjects increased (Ptrend<0.0001, and <0.0001). Urinary concentrations of methylparaben and propylparaben were associated with intensity of exercise (Ptrend<0.001, and 0.004), and that of propylparaben was higher in non-smokers (Ptrend=0.01). In terms of paraben exposure-related variables, urinary concentrations of parabens (i.e., methyl-, ethyl- and propyl-) increased as the daily average frequency of teeth-brushing (Ptrend<0.0001, 0.03 and 0.0001), the frequency of use of hair products (Ptrend=0.005, 0.05 and 0.04), the frequency of use of makeup products (Ptrend<0.001, 0.001 and <0.001), and the frequency of use of antibacterial products (Ptrend=0.005, 0.02 and 0.02) increased. Conclusions: In our study, urinary concentrations of all three parabens are associated with gender, teethbrushing, hair products, make-up products, and antibacterial products. Methyl- and proyl-parabens were associated with age and intensity of exercise, and propyl-paraben was associated with smoking.
