국내 의료기관에서 시행된 부종 질환의 한의치료 연구 동향 (Research trend of Korean Medicine for edema in domestic medical institutions)

  • Objectives : The purpose of this study is to investigate the research trend of Korean traditional medicine for edema under Korean healthcare facilities. Methods : We collected data from search engines for research including domestic online database using the keywords edema, Hydrocele, emphysema and Pneumocranium Results : Total of 41 articles were about Lower body edema, General edema, edema of the eyelid, Lymphedema, Hydrocele, Bone marrow edema, hand edema, Brain edema, emphysema, Pneumocranium The treatment methods used were acupuncture and pharmacopuncture treatment, moxibustion treatment, herbal medicine treatment, cupping treatment, massage treatment, radio frequency treatment, and Chuna treatment. Twenty-nine studies used only oriental medicine treatment, and 12 studies combined oriental medicine and western medicine. the most commonly used measurement was Measurement of the perimeter of the area. Conclusions : This study shows the research trend of edema. Through the collected data, the treatment methods and for specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in edema.

수부냉증 진단을 위한 조갑모세혈관 혈류속도 측정 예비연구 (The Role of Nail-fold Capillary Blood Velocity in Evaluating Cold Hypersensitivity in the Hands: A Pilot Study)

  • This study aims to compare nail-fold capillary blood velocity (CBV) according to the presence or absence of cold hypersensitivity in the hands (CHH) using nail-fold capillaroscopy under cold stress conditions. Eleven participants were taken from the cohort study for observing long-term health status of the faculty of Semyung university. Seven of the participants displayed CHH while four showed signs of non-CHH. CBV between 0 and 60 seconds (S1) and between 240 and 300 seconds (S2) was measured under cold stress for 5 minutes, respectively, and the average value was used to compare non-CHH group and CHH group. Body mass index, Nogung (PC8) temperature and temperature difference between Nogung (PC8) and Hyeopbaek (LU4) were significantly lower in CHH group compared to non-CHH group. The S2-S1 CBV difference was negatively correlated with PC8 temperature and PC8-LU4 temperature, and positively correlated with cold pattern score and cold hypersensitivity visual analog scale. The CHH group had a lower CBV overall than non-CHH group, and the S2-S1 CBV difference was significantly higher in the CHH group than in non-CHH group. This study suggests that CBV measured by nail-fold capillarosopy might be a useful indicator of cold hypersensitivity properties.

소아암의 한의치료 임상 실제 - 중의 증례연구를 중심으로 (Traditional Chinese Medicine Treatment for Childhood Cancer - Review of Case Studies)

  • Objective : To investigate specific prescriptions, herbal medicines and treatment strategy, this study systematically reviewed case studies on traditional Chinese medicine (TCM) treatment for childhood cancer. Method : Literature searches were performed on database of China National Knowledge Infrastructure (CNKI). Studies published before December 2020 were categorized and analyzed according to the treatment strategy and we also investigated Chinese herbs used in the prescriptions. Results : Twenty-five studies including 17 case series and 8 single case reports were included in our review. Of the total 439 patients, 379 (85.7%) had leukemia and other myeloplastic disease. Conventional western medicine (CWM) treatment was also used with TCM based on the syndrome identification. Most studies reported favorable effect of combination treatment with CWM and TCM. Among 266 herbs in the studies, reinforcing herbs were more frequently used than anti-cancer herbs. Conclusion : Traditional herbal treatment for childhood cancer should be performed considering the syndrome identification as well as stage of CWM. Reinforcing herbs should be more focused because of the fragility and growth of the childhood.

경혈경락이론에 근거한 체외충격파 치료가 무릎 관절염에 미치는 영향: 체계적 문헌 고찰 (Extracorporeal Shock Wave Therapy with Meridian and Acupoint Theory for Knee Osteoarthritis: Systematic Review)

  • Objectives To determine the evidence of effectiveness and safety of extracorporeal shock wave therapy (ESWT) with meridian and acupoint theory for knee osteoarthritis. Methods By March 3, 2021, five foreign electronic databases and six Korean medical electronic databases were reviewed with the key words 'extracorporeal shock wave' and '(acupoint OR acupuncture point)'. This key words was set up to increase the sensitivity of the search. After the search, knee osteoarthritis study was selected based on the title and abstract and then included after full-texts were read. Results Five randomized controlled trials were eligible in our inclusion criteria. The meta-analysis of three studies showed positive results for the using ESWT with meridian and acupoint theory for knee osteoarthritis compared with the control group on efficancy rate, visual analog scale and lysholm knee score. but there remains a conundrum regarding the safety of ESWT in the treatment of knee osteoarthritis. Conclusions Most of studies showed ESWT with meridian and acupoint theory were statistically effective to knee osteoarthritis. However there are limitations that the number of selected studies was small, risk of bias was unclear. So use of ESWT with meridian and acupoint theory for knee arthritis has limited evidence compared to usual care.

파킨슨병 변증 유형 및 지표 분포에 대한 전향적 다기관 관찰연구 프로토콜 (An Observational Multi-Center Study Protocol for Distribution of Pattern Identification and Clinical Index in Parkinson's Disease)

  • Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD. 슬관절 전치환술 환자에 적용한 추나치료: 체계적 문헌고찰 및 메타분석 (Chuna Manual Therapy for Patients after Total Knee Arthroplasty: A Systematic Review and Meta-Analysis)

  • Objectives To assess the effectiveness of chuna manual therapy for patients with total knee arthroplasty. Methods We searched 9 electronic databases(KISS, NDSL, RISS, OASIS, KMBASE, Cochrane Library, PubMed, CNKI, Wangfang data) using the keywords 'total knee arthroplasty OR total knee replacement' and 'tuina OR chuna' to identify RCTs that investigated the effectiveness of Chuna manual therapy after total knee arthroplasty. Results 23 RCTs were selected based on inclusion criteria. The systematic review showed positive effect of using chuna manual therapy after total knee arthroplasty. Conclusions 23 studies reported that chuna manual therapy had significant effects on total knee arthroplasty. However, many studies were exposed to probable high risk of bias. Also, all of the studies were published in one overseas country. Therefore, further research is required using well-designed RCTs to support the effectiveness of chuna manual therapy.

  • Objectives The purpose of this study was to investigate the effectiveness of Chuna manual therapy (CMT) in the treatment of simple obesity. Methods We used seven databases to perform a literature search using the keywords "Chuna OR Tuina" AND "Obesity." We selected randomized clinical trials in which Chuna was used as the main method for treating simple obesity. The risk of bias was assessed by three independent researchers using the Cochrane risk of bias. Results A total of 14 RCTs were selected and analyzed. The results of the meta-analysis showed a significantly higher efficacy rate in the patient group treated with CMT as the main treatment than that in the control group. Conclusions The results of this systematic review suggest that CMT is effective in the treatment of simple obesity. However, given the limitations such as the small number of studies and the high risk of bias in the studies, additional research is needed to lend further support to this claim.

기허가 한약제제를 대상으로 한 인지장애 치료 유효 약물 탐색 - 인지장애 임상연구를 위한 예비연구 (Exploring the Effective Herbal Prescription for Cognitive Disorder Treatment among Licensed Herbal Medicines in Korea - A Preliminary Study for Clinical Trial of Cognitive Disorders)

  • It is necessary to investigate the efficacy of commercial Korean herbal medicine for cognitive disorder. The purpose of this study is to select candidates among licensed herbal medicines that are expected to be effective in the treatment of cognitive disorders and can be used in future clinical trial. From laboratory experiments, we first found individual single herbal drugs that could be effective for cognitive disorder, and then through experts recommendation, selected five priorities of single herbal drug and obtained the information of the best herb as a combination of each herbal drug. To derive the final herbal prescriptions, we searched the KFDA drug information system for licensed herbal medicines containing each drug and its combination. As a result of laboratory experiments and experts recommendation, we found that the five effective single herbal drugs for cognitive disorders. They are Ginseng Radix, Acori Graminei Rhizoma, Cyperi Rhizoma, Coptidis Rhizoma, Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata(Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata were tied for $5^{th}$). And licensed herbal prescriptions derived from the planned search are Palbohoichoon-tang, Taehwa-hwan, Bosim-hwan, and so on. Among these, in consideration of the feasibility of research and possibility of success in development, Yukgunja-tang, Samhwangjichul-hwan can be selected as future study subjects. Through experimental studies and expert recommendations, we have derived herbal prescriptions that can be effective in treating cognitive disorders from licensed herbal medicines.

동측 반맹과 안면실인증을 호소하는 뇌경색 환자 한방복합치료 1례 (A Case of Combined Korean Medicine Treatment for Homonymous Hemianopia and Prosopagnosia after Ischemic Stroke)

  • ■ Background Patients with posterior cerebral artery infarction could complain of visual field defects or prosopagnosia that does not recognize a person's face. However, there has been no standardized treatment for these symptoms. ■ Case report A 57-year-old male patient complained homonymous hemianopia and prosopagnosia after posterior cerebral artery infarction. After combined Korean medicine treatment for 49 days, subjective visual field was improved and the discomfort associated with visual field defect and the disability of recognizing people was decreased. Evaluations were took place by assessing visual field using Automated Perimetry and Confrontation visual field exam. Discomfort caused by visual field defects or prosopagnosia was evaluated by visual analog scale. The patient was treated with acupuncture, moxibustion, and herbal medications. ■ Conclusion The present case report suggests that combined Korean medicine treatment might be effective to resolution of homonymous hemianopia and prosopagnosia after stroke.

노인등록연구 사례를 통한 임상평가지표 선정 과정 및 검증된 설문도구 승인 경험의 공유 (Sharing Experiences in Selecting Clinical Outcome and Approving Validated Questionnaires : Insights from an Elderly Registry Study)

  • Objectives: Underpinned by the context of a Korean traditional medicine cohort study on healthy aging, this research primarily aims to guide the selection of Clinical Outcome Assessments (COAs) for elderly healthy aging patient registry research, offering insights into the selection process; and secondly, to streamline the resource-intensive process of obtaining permissions for validated COAs, benefiting future traditional Korean medicine clinical researchers. Methods : In this study, we identified outcomes through a review of previous studies, followed by a process involving expert consultations to select the final outcomes. Subsequently, for the selected outcomes that were Clinical Outcome Assessments (COAs) developed tools, we searched in commercial databases to confirm the availability of Korean versions and the necessity of obtaining permissions. Finally, we obtained permissions for their utilization and, when needed, acquired the original instrument questionnaire through payment. Results: Through a literature review of existing observational studies, a total of 57 outcomes were selected, with 19 of them identified as COA instruments. Upon verifying usage permissions for these 19 instruments, it was found that 17 required author-specific permissions, and among these, 2 needed a purchase as they were commercially available. Conclusion: This study provides a detailed overview of outcome selection and permission acquisition for elderly patient registry research. It underscores the importance of Clinical Outcome Assessment (COA) tools and the rigorous approval process, aiming to enhance research reliability. Continuous verification of COA information is essential, and future research should explore Core Outcome Set (COS) development through consensus-building approaches like Delphi studies.
