The Effect of Heel Height on Lumbar Sagittal Curvature at Standing Posture (기립자세에서 신발 굽의 높이가 요추부 시상만곡각에 미치는 영향)

  • Some segment or segments of the body must compensate for the heel, and the higher the heel the greater the compensation. Such compensation was once generally thought to take place in the lumbar region and therefore to increase the lumbar lordosis. The purpose of this study is to analyze changes of lumbar sagittal curvature in barefoot and 6cm 12cm high-heel stance. We selected 19 subjects(11 males, 8 females} without history of lower back pain, significant spinal abnormality. And lateral view X-ray of lumbar region from T12 to S1 was taken of each individual. On each X-ray film, lumbar lordotic angle lumbosacral angle and lumbar segmental angles were measured by Cobb method. We drew the following interpretations from the analysis of measured variables of the lumbar region. 1. In comparison of barefoot 6cm heel 12cm heel stance, lumbar lordotic angle had a tendency to decrease according as the heel height was higher. The change in lumbar lordosis measured in high-heel stance was inconsistent with clinical forkelord of hyperlordosis in wearers of high-heeled. 2. Lumbar lordotic angle from T12 to L5 showed sex difference, and was more lordotic in female(p<0.05). 3. There was no sex difference in lumbosacral angle and lumbar segmental angles(p>0.05). 4. There was a significant correlation between lumbar lordotic angle and lumbosacral angle(r>0.60).

  • Purpose : The purpose of this study is to investigate that the Effects of Pain, Lumbar Flexibility and Abdominal Obesity of Patient with Low Back Pain and Abdominal Obesity after Lumbar Strengthening Exercise. Methods : The selected subjects of this study were 17 patients who had diagnosis on low back pain and body mass index was more than $25kg/m^2$ and lumbar length was more than 90cm. They divided into a control group with 10 patients doing lumbar stabilization exercise and an experimental group with 7 patients ding simple lumbar strengthening exercise in order to compare degrees of oswestry disability index and lumbar stability, lumbar length was measured before the exercise, in 4 weeks and 8 weeks after exercises respectively. Result : lumbar strengthening exercise on patients with low back pain and abdominal obesity affected improving function of lumbar and decrease of pain with abdominal obesity. Conclusion : Then lumbar stabilization exercise than simple strengthening exercise affected decrease of Visceral fat tissue on abdominal obesity.

  • Lumbar discs are one of the first parts of the musculoskeletal system to age in the human body. Lumbar disc for females start to age at twenty years of age and completely age between fifty and sixty. The number of lumbar herniated intervertebral disc patients are increasing rapidly; however, studies on protective inner wear are insufficient. This study was conducted to develop protective inner wear equipment that retains an aesthetic appreciation of the body. The methods and procedures of this study are as follows. First, foreign and domestic lumbar pads were researched and analyzed by visiting medical shops and internet shopping malls that sell lumbar pads. Second, the survey was conducted on forty patients with disc disease in hospitals located in Seoul and surrounding suburbs. The following are the study results and observations. First, the lumbar pads selling on the market had a variety of designs, textiles, and fabrics. The result of the research was to develop additional inner protective equipment for lumbar herniated intervertebral disc patients. Second, the survey results of the forty patients with disc disease were that housewives were the most vulnerable. All patients had experience purchasing lumbar pads. The prevention of pain and stress were the most selected responses for the motivation to buy lumbar pads. Most individuals said that they used lumbar pads for less than 1 year. Dissatisfaction came from fabric, activity and design. 'Being uncomfortable to wear' was the most common complaint. 'Protection and how easy the clothes were to wear' were the most selected for the necessity of developments. The Inner wear shape was the most selected in shape. Except for six respondents, all favored the lumbar protective inner wear design.

The Comparative Analysis of Clinical Characteristics and Surgical Results between the Upper and Lower Lumbar Disc Herniations

  • Objective : There are differences in the clinical characteristics and surgical results between upper (L1-2 and L2-3) and lower (L3-4, L4-5, and L5-S1) lumbar disc herniations. We conducted this study to compare the clinical features and surgical outcomes between the two types of lumbar disc herniations. Methods : We retrospectively reviewed the clinical features of patients who underwent microdiscectomies from 2008 to 2012. We evaluated the clinical characteristics such as age, preoperative autonomic dysfunction, the presence or absence of previous lumbar surgery and fusion required during surgery. Visual Analogue Scale (VAS) scores about back pain and leg pain were evaluated preoperatively and at the final follow-up. Results : Upper lumbar group (n=15) was significantly older than lower lumbar group (n=148). The incidence of autonomic dysfunction was significantly higher in upper lumbar group. The number of patients with a previous lumbar surgery was significantly greater in upper lumbar group. There was no statistical significance for fusion required during surgery between two groups. Both groups showed a significant decrease in the VAS scores of leg pain. VAS scores of back pain were significantly decreased in lower lumbar group. But this was not seen in upper lumbar group. Both groups showed significant improvement of Oswestry Disability Index score. Conclusion : Upper lumbar group had different clinical characteristics from those of lower lumbar group and these include older age, a higher incidence of autonomic dysfunctions and a higher incidence of patients with previous lumbar surgery. There were no significant differences in surgical outcomes, except for back pain, between two groups.

A Fatigue Analysis on Lumbar Extension Muscle during Repeated Trunk Extention Exercise for Chronic Lumbar Pain (만성요통환자의 반복체간운동 시 요부신전근 피로 분석)

  • The purpose of this study were analysed fatigue character of lumbar extension Muscle during repeated trunk flexion-extension motion we used EMG multi-telemeter (WEB 5000, Nihon Koden, Japan), Medex lumbar extension machine(Ocala, USA). We evaluated 20 persons on chronic low back pain group and 20 persons on control group. We analysed the quantitative variables IMF, MF, decreased ratios of MF to investigate fatigue. As a result, the following conclusion was drawn : 1. Each lumbar IMF of CLBP was L5> L3> L1 in the order of their size. CON group was similar quantitative and same order. But the relationship of two groups were not significantly. 2 The IMF within lumbar position(L1-L3, L1-L5, L3-L5) of two groups was significantly all positions(p<.05). 3. The MF shifting at lumbar position of two groups during repeated trunk flexion-extension motion was decrease shifting pattern with increase repeating motion, especially CLBP group was suddenly dropping than CON at all lumbar positions(L1, L3, L5) on early stage motion(34 set). 4. The lumbar fatigue of CLBP was higher than CON during repeated trunk flexion-extention motion, so CLBP supposed more tired than CON. And it was significantly within two groups at lumbar positions(L1, L3 : p<.05, L5 : P<.01).

Suggestion of Sacral Lumbar joint distraction technic with prone position in Chuna Manual Therapy (추나요법에서 복와위 요천관절 신연기법에 대한 제언)

  • Objectives : This study aimed to introduce a new sacral lumbar joint distraction technique in the prone position in Chuna manual therapy for effective lumbar distraction. Methods : The patient was placed in the prone position. Next, clinicians contacted the sacrum and L1, L2, L3, L4 and L5 spinal processes. Then they were pulled in the direction of the head and leg respectively while pressing down with 30% force to maintain the lumbar lordosis. This is a more effective and safer lumbar distraction technique according to the latest knowledge. Conclusions : This technique can be used for lumbar disc herniation in addition to relaxing the existing lumbar muscles, and is suggested to be used as a safe and effective Chuna manual therapy.

The Changes of Range of Motion of Lumbar Region and Muscle Activities of Lumbar Extensor During Trunk Motions Between Subjects With Low Back Pain and Healthy Subjects (만성요통환자와 정상인의 체간 운동시 요추 신전근의 근활성도와 관절운동범위의 변화)

  • The purpose of this study was to compare the electromyography (EMG) activities of the lumbar extensor muscles during motion of trunk flexion-extension and compare range of motion (ROM) with a 3-dimensional motion analysis system of the lumbar region between subjects with chronic low back pain (CLBP) and healthy subjects during the trunk flexion-extension, trunk rotation and trunk lateral flexion cycle. Thirty CLBP subjects and thirty healthy subjects were included. We measured the root mean square (RMS) value of the lumbar extensor muscles from resting, standing, lumbar flexion and return position. The RMS ratio was normalized from maximal EMG activity of the lumbar extensor muscles during trunk motion. The results of this study showed that the RMS ratio of the lumbar extensor was significantly higher in CLBP subjects than healthy subjects during all of trunk motion (p<.05). The ratio of the highest RMS value during flexion and extension was higher in CLBP subjects than in healthy subjects (p<.05). The ROM of the lumbar region was significantly lower in CLBP subjects than healthy subjects during trunk flexion-extension, trunk rotation and lateral flexion cycle. The relationship between the RMS ratio for full lumbar flexion and the ROM of lumbar flexion was not correlated significantly. CLBP subjects have both decreased ROM of the lumbar region and higher muscle activities of the lumbar extensor muscle than healthy subjects.

The Effect of Posteroanterior Joint Mobilization Applied to The 3rd Lumbar Vertebra Movement of Adjacent Segmental in Healthy Individuals

  • Objective: The purpose of this study was to characterize the movement of adjacent segments that occurred when posteroanterior joint mobilization was applied to the 3rd lumbar and thereby to provide basic clinical data. Design: Randomized controlled trial design. Methods: While the subjects were receiving posteroanterior joint mobilization of the 3rd lumbar vertebra, LD (lumbar displacement), LID (lumbar intervertebral distance), LIA (lumbar intervertebral angle), and LLA (lumbar lordosis angle) were measured using X-ray, and the data were then analyzed. Changes before and after posteroanterior joint mobilization were analyzed using a paired-sample t-test, and a one-way ANOVA of variance was performed to determine the difference between segmental movements. In addition, Pearson's correlation analysis was performed to determine the correlation between segmental movements. Results: The results revealed that there was a significant change in LD before and after the application of joint mobilization of the 3rd lumbar in all lumbar vertebrae (p<0.000), among which the 2nd lumbar vertebra, an adjacent segment, showed the most significant change. A significant change in LIA angle was observed in all segments (p<0.000), with the most significant change observed between the 2nd and 3rd lumbar vertebrae. There was a significant change in LLA before and after the application of posteroanterior joint mobilization (p<0.000). The correlation analysis showed a high correlation between displacement of the 3rd lumbar vertebra and displacement of all the other lumbar vertebrae. Conclusions: The presence of kinematic movements of the lumbar vertebrae when segmental movements were generated through the application of posteroanterior joint mobilization of the 3rd lumbar vertebra.

Psoas Compartment Block for the Relief of Lumbar and Left Thigh Pain after Operation of Second Lumbar Compression Fracture -A case report- (척추 수술 후 발생된 요부 및 좌측 대퇴전부 통증 치료를 위한 대요근 근구 차단술의 효과 -증례 보고-)

  • Psoas compartment block has been used to provide anesthesia and analgesia of hip joint. This block is advocated for relief of pain of various origins in the thigh, leg and lumbar area. A-40-year-old women complained of pain in the left thigh and lumbar area after operation of the second lumbar vertebral compression fracture. To relieve pain, caudal block was performed. This block reduced in lumbar pain but left thigh pain persisted. So, we were performed psoas compartment block using mepivacaine and dexamethasone, which relieved the pain in the left thigh and lumbar area. We recommend psoas compartment block as useful and simple method for patients with thigh and lumbar area pain, especially when the epidural block is not feasible.

Effect of thoracic kyphosis and lumbar lordosis by Korean Medical treatment involving The Jeongham Therapy: 8 cases (정함요법을 포함한 한의학적 치료방법이 흉추후만 요추 전만에 미치는 영향: 환자 8례)

  • Objectives : To introduce the Jeongham Therapy which means a mandibul-rectifying treatment. And to show meaningful changes of thoracic kypkosis and lumbar lordosis by the Jeongham Therapy. Methods : We reviewed 8 patients having abnormal thoracic kyphosis and lumbar lordosis who were treated at the JeongHam Korean Medical Clinic. We measured thoracic kyphosis and lumbar lordosis by 3D Body Analyzer 'IBS 2000'. Results : Between before and after the Jeongham Therapy, statically significant differences were found in the thoracic kyphosis and lumbar lordosis. And in these values of measurements, the thoracic kyphosis and lumbar lordosis diminish after the Jeongham Therapy. Conclusion : These results suggest that the Jeongham Therapy is a meaningful therapy to diminish the thoracic kyphosis and lumbar lordosis.
