The purpose of this study was diagnosed to grasp the needs for the elderly-elderly care activity in rural areas. The research was conducted in questions and case studies in 7 farming villages. The major findings of this study as follows: First, the senior citizens in rural areas were not in good health, and the illnesses were revealed to be arthritis, high blood pressure, and diabetes. At the occurrence of an illness, they complained about the discomfort in doing daily life routines and expected eir spouses or offsprings to te care of them. Elders in the rural community responded that they had difficulty and financial problems in family maintenance. Leisure activities included watching TV, playing card/board games, or spending time without any specific activities. Second, recipients of elderly-elderly care expected to receive services from a woman in her 50s $1{\sim}2$ hours, once to twice a week. Third, the service providers wanted to be elderly-elderly care takers, hoping to work once a week for $2{\sim}3$ hours. They wanted the education for the elderly-elderly care to be once a week for 3 hours, with understanding the elderly, consultation, preventing senile dementia, and health as the contents.
Purpose : This study was to find the effects of conservative therapy and sling therapy program of strength exercise and Gymnic Ball therapy program of stabilizer exercise and to find the effects of pain relieve through visual analogue scale(VAS) in chronic low back pain patients (male; 18, female;22). Methods : The measurement of pain was performed to both group by VAS from pre treatment to one to seven weeks. Results : 1. By the result of one-way ANOVA, Both group was the statistical significance in VAS during seven weeks(p<.05). 2. By the result of paired t-test, Both group was not statistical significance between the values of pre treatment and treatment after six and seven weeks but was statistical significance one week to seven weeks(p<.05). Conclusion : It was concluded that low back pain would be relieved by the progressive exercise and education for low back pain, and that the interventions would be helpful to prevent additional injuries by increasing the flexibility and muscular endurance.
Purpose: The purpose of this study was to examine the social determinants of health (SDH) associated with health-related quality of life (HQOL) among the community-dwelling elderly, based on the conceptual framework of the Commission on Social Determinants of Health (CSDH). Methods: A survey was conducted with 199 elders aged 65 years or older sampled through convenient sampling. Data were collected through face to face interviews by trained interviewers using a structured questionnaire, which included scales of the residential area, social support, sense of community, social network, health behaviors, and HQOL. Data were analyzed by ANOVA and stepwise multivariate regression. Results: Major SDH affecting HQOL included the participants' residential area, sense of community, and health behavior. The HQOL of the elderly residing in the B area with a low rate of basic livelihood security recipients was higher than that of those residing in other areas. Conclusion: CSDH framework was useful to determine the factors associated with HQOL among the community-dwelling elderly. In addition to their health behavior, their sense of community was found to be a SDH of HQOL, indicating the need of health promotion programs tailored to the characteristics of residential areas and strategies to enhance involvement in community activities.
Purpose: The purpose of this study was to identify differences in health status and the utilization of long-term care service between urban and rural aged residents in Korea. Methods: Through convenience sampling, 1,405 elders (829 from urban areas and 576 from rural areas) were selected during March 1 to May 31 in 2004. All the subjects agreed to participate and filled out the survey questionnaire after signing the consent form. The instruments utilized in this study were the impairment of physio-sensory function, ADL IADL, cognitive function, and psycho-social function scale. This instrument was developed by modifying the scab developed by Gurland & Wilder (1984). Data was analyzed using the SPSS Win program. Results: There were significant differences in economic status, duration of living and type of medical insurance between rural and urban elderly(p<.05). Physio-sensory functions (t=4.53. p<.001), ADL (t=3.61. p<.001), IADL (t=2.45, p=.014), cognitive functions (t=-2.63. p=.024) and psycho-social functions (t=3.69. p<.001) were significantly different between the two groups. The utilization of long-term care facility in the urban elderly was significantly higher than that in the rural elderly ($x^2=10.14$, p<.001). Conclusion: Considering these findings. the need for long-term care should be assessed by residence characteristics. Because of different utilization of long term care facility according to the elderly's needs, long-term care services should be considered the residence characteristics.
Objectives: Despite the recent emphasis on a patient-centered chronic care model, few studies have investigated its use in older adults in South Korea. We explored how older Korean adults perceive and cope with their chronic illness. Methods: We conducted focus group interviews in Seoul, Korea in January 2010. Focus groups were formed by disease type (hypertension and type 2 diabetes) and gender using purposive sampling. Inclusion criteria were patients aged 60 and over who had been diagnosed with diabetes or hypertension and received care at a community health center for at least six months prior to participation. Interview data were analyzed through descriptive content analysis. Results: Among personal factors, most participants felt overwhelmed when they received their diagnosis. However, with time and control of their acute symptoms using medication, their worry diminished and participants tended to denying being identified as a patient or sick person. Among socio-familial factors, participants reported experiencing stigma with their chronic illness and feeling it was a symbol of weakness. Instead of modifying their lifestyles, which might interfere with their social relationships, they resorted to only following their medicine regime prescribed by their doctor. Participants also reported feeling that their doctor only prescribed medications and acted in an authoritative and threatening manner to induce and reinforce participants' compliance with treatment. Conclusions: For successful patient-centered management of chronic illnesses, supportive environments that include family, friends, and healthcare providers should be established.
Objectives : The purpose of this study was to confirm effects of oral exercise program intended to improve the condition of salivary hypofunction and to provide basic data for development of oral health program for the elderly Methods : The subjects were 125 elderly women who resided in Gyeong-In area and were aged 65 years and older. During a 2-month period, an oral exercise was conducted twice a week. A survey was conducted by interviewing the selected elders to determine the state of their subjective dry mouth. Unstimulated whole saliva was measured before and after the oral exercise program. Results : There was a significant improvement in subjective dry mouth symptoms, severity scores of dry mouth after the oral exercise, and there were affected by age and income level. There was a significant increase in unstimulated whole-saliva after the oral exercise program. There was positive interrelationship between dry mouth Symptom improvement level and inconvenience improvement level, and between inconvenience improvement level and the increase of unstimulated whole-saliva. Conclusions : This study showed the effects of the oral exercise program. It is suggested that this oral exercise program has positive effects on the condition of dry mouth. For the explicit improvement effect on the condition of dry mouth, the oral exercise program needs to be carried out in consideration of the elderly's general characteristics.
This study was conducted to investigate risk factors for senile dementia as well as care givers' stresses and thier needs for nursing care. It was done using a retrospective survey. A convenience sample or In senile dementia patients and l20 nor-mal elders in a rural area was used. The tools used in the study were the MMSE-K(Mini-Mental State Examination-Korea) for dementia screening test and a questionaire developed by the research team. Data were collected through home visits by Com-munity Health Practitioners. Data were analyzed using descriptive statistics, T-test, and Chi-square test. The findings are as follows : 1. There were significant differences in age, marital status, and religions between the two groups. 2. There was a significant difference in smoling behavior between the two groups. 3. There was a significant difference in past his-tory of cancer between groups. 4. There was a significant difference in past and present elderftmily relationship between the two groups. 5. There were significant differences in intellectual activities, assuming major role in family and seeking other's help in daily life troubles between the two groups. 6. There were significant differences in stress factors such as child problem, family conflict, health problem and illegal behavior between the two groups. 7. The major problems out by families in caring for dementia patient were catastrophic reactions, dirtiness, mood change, devouring and tremor. The most serious problems faced by families was dirtiness. with catastrophic reactions, sleep distrubance, changeableness, and a suspcio-usness following. The care givers expressed chronic fatigue, anxiety, tension, depression, disorder in daily life, shamefulness, blame from neighbours and guiltiness. 8. There is need for geriatric hospitals, nursing homes, burden sharing, and counselling or education for family care givers. A replicate study in the urban area is recommended to validate the findings of this study. To explore the impact of stress in life and ‘han’ on senile dementia, a qualitative study is recommended.
Purpose: This research used quantitative research to identify differences in death consciousness between pre and post education. The study was also designed to further understanding of the effects of nursing education by using a qualitative analysis to examine hospice education experience. Method: This study a one group pre-post test design. Results: 1 The mean score for the students' death consciousness before the hospice education averaged $2.15{\pm}\;.33$, a medium level for death Consciousness. The Score after education was $2.25{\pm}\;.36$, that is, there was higher score for death consciousness after education. 2. The result of classification, giving their names and categorizing the experience of being in a coffin shown to be self-reflection, regret, recognition to death, death as discontinuation of life, the last closing from everything, death as a sad and cruel event, death as another world, specialist intuition of nursing. Conclusion: This research provided an opportunity for nursing students to consider death earnestly and realistically through hospice nursing education. We also discovered affirmative changes in the students' viewpoint of death, students who in future clinical practice may work with elders. We also found increases in motives to develop ability to present effective aid to dying patients.
최근 원격진료의 필요성과 사용자의 서비스 요구사항이 증가함에 따라 보다 편리한 원격진료 서비스 제공 방법에 대한 연구가 다양한 분야에서 진행 중이다. 본 논문에서는 기존의 원격진료시스템과는 다르게 무선통신센서를 이용한 CPS(Cyber Physical System)기반의 음성통신과 생체정보시스템을 연동하여 거동이 불편한 고령자는 편리하게 무선으로 정보를 송수신하고, 원격지의 의료인을 위해 전달 받은 생체정보와 음성대화를 통해 진단의 정확도를 높일 수 있으며, 고령인의 건강정보를 선택적으로 저장하고 추후 의료인이 조회하여 상시 건강관리가 가능한 통합원격의료서비스를 제안한다. 본 연구를 통해 고령사회로 진입한 우리나라 상황에 맞는 고령친화적인 원격진료시스템을 구축하여 실버타운에 거주하는 고령인들을 위한 고령친화적인 의료서비스를 제공할 수 있을 것이다.
Purpose: The purpose of this study is to investigate effects of an oral self-care program on oral dryness and oral health related quality of life in the elderly. Methods: The current research design isa nonequivalent control group protest-posttest design with two groups of elders from two different senior centers in D city. Tools for the research include paper of salivary absorption (WF41-1850, Whatman, Germany), 6 items for subjective oral dryness and OHIP-14 for oral health related to quality of life. The data were analyzed to test three hypotheses by using the SPSS 22.0 program. Results: The three hypotheses were significantly accepted. The subjective oral dryness score was lower in the experimental group than in the control group (t=-2.45, p=.022). Salivary absorption was higher in the experimental group than in the control group (t=3.83, p<.001).Oral health related quality of life was better in the experimental group than in the control group (t=-2.11, p=.044). Conclusion: The oral self-care program has appeared to be effective on oral dryness and an oral health related quality of life in the elderly. Therefore, it can be suggested that the oral self-care program is applicable as an intervention program motivating the elderly to maintain oral health.
