In the total artificial heart(TAH), a transcutaneous information transmission system(TITS) is vely important to monitor the TAH status and detect the device failure, and repair the possible problems. First of all, the communication channel(skin) and method were simulated in terms of transmittance, scattering, reflection and absorption, then the system was designed with size reduction including low power consumption and reliability compared to the previous one. The informations are transmitted through the skin(approximately 1cm in depth) by frequency modulated near infrared(NIR) pulses using 780nm laser diodes as transmitters and photodiode as receiver with high speed and high spectral sensitivity. The logic high and low frequencies are 3MHz, 1MHz respectively. The system is a bidirectional data link for more than 38.4Kbps data rate, full-duplex with a bit error rate of less than $10^{-5}$.
In this paper we reviewed and stated the present state of our team's research of the moving-actuator type total artificial heart (MA-TAH). Especially, this paper showed new direction of researches. The integrated research of battery, telemetry, TET is required. And the real-time supervisory of the state of hemodynamic variables and MA-TAH is also needed. These requirement made us to think the supervisory control system (SCS). As functional abilities of the SCS require very high computational cost, the digital signal processor is an appropriate choice for our need.
The two major problems related to the blood flow in a floating type polymer valve are thrombus formation and hemolysis. It is well known that the shear stress in the fluid and flow separation around the valve are blamed for such disastrous phenomena. In this viewpoint, through study of the flow field around the valve is imperative to improve design of the valve. The aim of this study is to investigate the fluid flow around a floating type polymer valve. The numerical method employed in this study is the finite element software called ADINA. Incompressible viscous flow is assumed for blood using the assumption of Newtonian fluid. In this study, two prominent features of the axisymmetric flow around the floating type polymer valve are observed: jet-like flows observed near the gap between the conduit and the valve, and recirculating flow downstream of the valve. We also provided a detailed description of shear stress field according to the variation of flow conditions. The shear stress in fluid has its maximum value near the gap between the valve and the conduit.
As a part of electro-mechanical totally implantable artificial heart, a transcutaneous energy transmission system has been developed. By mutual magnetic induction between the first coil on the skin and the subcutaneously implanted second coil, the system transfers electrical power through the skin. This research aimed a minimizing the size of the implanted part as well as maximizing the transfer efficiency. When an air gap is 1$\sim$2cm, voltage gain and current gain low and it is hard to transfer energy due to large leakage flux. That is, the required input voltage and input current must be large compared with the output voltage and output current, respectively, This paper research the inverter topology and the control method in order to increase the voltage gain and the current gain. For this purpose, this inverter employs double tune to compensate the large leakage inductance of primary and secondary of the transcutaneous transformer. Intel's 80C196KD has been applied at the moving-actuator type total artificial heart ( MA-TAH ) from about early 1990th at our laboratory and it fulfilled its role successively. But the increased need of higher performance brought the other-type of the CPU, and the new challenge of ASIC to the application to the MA-TAH accelerated its need. Lumbrokinase, potent fibrinolytic enzyme purified from earthworm, was immobilized onto polyurethane valves using photoreaction, photoreactive polyallyl-amino as a photoreactive linker. For evaluation of blood compatibility, lumbrokinase immobilized polymer valves were assembled into the total artificial heart (TAH). This TAH was implanted to 60kg healthy lamb for 1-3 days with the cardiac output 5 L/min. In the control lamb, the valves were untreated, in ore other, only valves on the right were treated, and in the remaining animal, only those on the left. To facilitate the thrombus formation, low doses of heparin were administered. For evaluation of the immobilized lumbrokinase, thrombus formation, proteolytic and fibrinolytic activity was measured. This data shows that lumbrokinase-treated polyurethane valves lead to decreased thrombus formation in vivo, and that their biocompatibility is therefore higher than that of untreated valves.
A transcutaneoos information transmission system using transmitted infrared light for an implantable total artificial heart(TAH) an developed and the performance of the system was tested. In an in vitro test, transmittance of the developed system was measured through various thicknesses of pig shins. In an in vivo test, a loopback test using a personal computer was performed to determine the error rate and an experiment using a mock system was taken. In this experiment, within acceptable lateral and vertical displacements, the error rate of at least 3.8E-6 was guaranteed. Monitoring signals from the internal controller(Intel 87C196) of the mock system were successfully transmitted to the external controller(IBM PC) and the operating commands from the external controller were successfully conducted by the mock system. Communication was done in half duplex mode according to RS-232 protocol at the speed of 4800 bps.

배경: 개심술 후 공기 제거는 매우 중요한 시술이다. 그러나, 심장 내의 공기 분포, 시술로 인한 합병증, 심폐바이패스의 시간 연장으로 여전히 제한적이다. 인위적으로 동정맥루를 만들어 공기를 제거하고 그 효과를 경식도초음파로 확인하였다. 대상 및 방법: 심폐바이패스 완료 직전에 압력표준변환선을 동맥과 정맥회로에 설치해둔 마개에 연결하면 동맥과 정맥사이에 작은 동정맥루가 형성된다. 심장 내의 공기정도와 공기제거시간은 경식도 초음파와 압력표준변환선으로 확인하였다. 결과: 간단한 방법으로 심폐바이패스의 시간단축, 수분 내에 공기가 제거되는 것을 경식도초음파로 확인하였다. 결론: 동맥과 정맥 사이에 작은 연결관으로 동정 맥루를 만드는 것은 매우 간단하며 개심술 후 공기 제거와 공기색전증 예방에 효과적인 방법이다.
Various prosthetic heart valves have been developed and used clinically, but they have problems, such as thrombogenecity, hemoltsis, high cost and low durability. New types of trileaflet polymer heart valves have been developed in order to use them as inlet and outlet valves in a ventricular assist device. The aim of this study is to determine the hydrodynamic effectiveness of the newly designed trileaflet polymer valves and their feasibility for temporary use in the blood pumps. Trileaflet polymer valves are made of polyurethane, because of its good blood compatibility, high tonsil strength and good resistance to fatigue. An in vitro experimental investigation was perf'ormed in order to ev91ua1e hydrodynamic performance of the trileaflet polymer valves having different design and fabrication tech- niques. The St. Jude Medical valve (SJMV) and floating-type monoleaflet polymer valve (MLPV) were also tested The pressure drop across the valve, leakage volume, and the flow patterns mere investigated for valves. The result of comparative tests showed that the trileaflet polymer valves had a better hydrodynamic performance than the others. TPV which has two stable membrane shape showed the lowest back flow. The pressure hops of TPVs were lower than that of MLPV, but slightly higher than SJMV. The hydrodynamic performance of valves under the pulastile flow showed the similar results as steady flow. The velocity profiles and turbulent intensities were measured at the distal sites of valves using a hot-film anemometer. Central flow was maintained in trileaflet polymer valves, and the maximum turbulent intensities were lower in TPVs comparing to MLPV.
