Prevalence of bony septa, antral pathology, and dimensions of the maxillary sinus from a sinus augmentation perspective: A retrospective cone-beam computed tomography study

  • Purpose: Sinus elevation procedures have become a routine and reliable way to gain bone volume in the edentulous maxilla for dental implant placement. Presence of bony septations and pathology in the maxillary sinus often cause complications leading to graft or implant failure or both. The aim of this study was to retrospectively evaluate the prevalence of pathology, direction of the septa, and sinus width measured at 2 mm, 5 mm, and 10 mm from the sinus floor in maxillary sinuses using cone-beam computed tomography (CBCT). Materials and Methods: Seventy-two sinuses from 36 random preoperative CBCT scans referred for implant therapy were retrospectively evaluated for the number, prevalence, and direction of bony septations and presence of pathology. Width of the sinus was also measured at 2 mm, 5 mm, and 10 mm from the sinus floor to account for the amount of bone available for implant placement. Results: Maxillary sinus septa were found in 59.7%. Presence of a single septum was noted in 20 sinuses (27.7%), followed by two septa in 17 sinuses. The most common direction of the septum was the transverse direction. Retention pseudocyst and mucosal thickening were the most commonly seen abnormality/pathology. Conclusion: Based on the high prevalence of septa and sinus pathology in this sample, a preoperative CBCT scan might be helpful in minimizing complications during sinus augmentation procedures for dental implant therapy.

Porphyromonas gingivalis와 Tannerella forsythia의 응집반응 (Coaggregation between Porphyromonas gingivalis and Tannerella forsythia)

  • Dental plaque, a biofilm consisting of more than 500 different bacterial species, is an etiological agent of human periodontal disease, It is therefore important to characterize interactions among periodontopathic microorganisms in order to understand the microbial pathogenesis of periodontal disease. Previous data have suggested a synergistic effect of tow major periodontal pathogens Porphyromonas gingivalis and Tannerella forsythia in the periodontal lesion. In the present study, to better understand interaction between P. gingivalis and T. forsythia, the coaggregation activity between these bacteria was characterized. The coaggregation activity was observed by a direct visual assay by mixing equal amount (1 ${\times}$ $10^9$)of T. forsythia and P. gingivaJis cells. It was found that the first aggregates began to appear after 5-10 min, and that the large aggregates completely settled within 1 h. Electron and epifluorescence microscopic studies confirmed cell-cell contact between two bacteria. The heat treatment of P. gingivalis completely blocked the activity, suggesting an involvement of a heat-labile component of P. gingivalis in the interaction. On the other hand, heat treatment of T. forsythia significantly increased the coaggregation activity; the aggregates began to appear immediately. The coaggregation activity was inhibited by addition of protease, however carbohydrates did not inhibit the activity, suggesting that coaggregation is a protein-protein interaction. The results of this study suggest that coaggregation between P. gingivalis and T. forsythia is a result of cell-cell physical contact, and that coaggregation is mediated by a heat-labile component of P. gingivalis and T. forsythia component that can be activated on heat treatment.

불소함유 접착테잎의 법랑질 침식 예방효과 (PREVENTIVE EFFECT OF ADHESIVE TAPE SUPPLEMENTED WITH NaF ON ENAMEL EROSION IN VITRO)

  • 본 연구는 불소함유 접착테잎의 법랑질 침식 예방효과를 평가하기 위해건강한 소의 치아를 절단하여 아크릴 주형에 매몰하고 정중선에 홈을 형성하여 대조면과 불소 제제를 도포할 실혐면으로 나누었다. 시편을 무작위로 18개씩 4군으로 나누었다. 1군은 APF gel를 도포하고, 2군은 불소 바니쉬를 도포하였으며, 3군과 4군은 5% NaF를 각각 첨가한 methyl cellulose와 poly vinyl alcohol 테잎을 부착하였다. 시편을 콜라에 5분, 증류수에 10분씩 6회 번갈아 처리하며 5일 동안 반복하였다. 시간 경과에 따라 표면미세경도를 측정하고 5일후 각 시편을 절단하여 침식병소의 깊이를 측정하여 다음과 같은 결과를 얻었다. 1. 표면미세경도 값은 5일째만 제외하고 2군, 3군, 4군의 실험면이 각각의 대조면에 비해 컸다 (p<0.05). 2군, 3군, 4군의 실험면 역시 5일째만 제외하고 1군의 실험면에 비해 표면미세경도가 컸다 (p<0.05). 3군과 4군 사이에는 표면미세 경도의 유의한 차이가 없었다(p>0.05). 1군의 실험면과 대조면은 표면미세경도의 유의차가 없었다(p>0.05). 2. 침식병소의 깊이는 2군, 3군, 4군의 실험면이 각각의 대조면에 비해 컸다(p<0.05). 2군, 3군, 4군의 실험면은 1군의 실험면에 비해 침식병소의 깊이가 얕았다 (p<0.05). 2군, 3군, 4군의 실험면 사이의 침식병소의 깊이는 유의차가 없었다(p>0.05). 상기 결과로 보아 새로 개발된 불소함유 접착테잎은 기존의 불소 바니쉬와 같은 법랑질 침식 예방효과가 분명하게 나타나, 법랑질 침식에 감수성이 높은 어린이 환자에게 임상적인 예방법으로서 활용할 수 있는 가능성이 높다고 사료된다.

Effect of the restorative technique on load-bearing capacity, cusp deflection, and stress distribution of endodontically-treated premolars with MOD restoration

  • Objectives: To evaluate the influence of the restorative technique on the mechanical response of endodontically-treated upper premolars with mesio-occluso-distal (MOD) cavity. Materials and Methods: Forty-eight premolars received MOD preparation (4 groups, n = 12) with different restorative techniques: glass ionomer cement + composite resin (the GIC group), a metallic post + composite resin (the MP group), a fiberglass post + composite resin (the FGP group), or no endodontic treatment + restoration with composite resin (the CR group). Cusp strain and load-bearing capacity were evaluated. One-way analysis of variance and the Tukey test were used with ${\alpha}=5%$. Finite element analysis (FEA) was used to calculate displacement and tensile stress for the teeth and restorations. Results: MP showed the highest cusp (p = 0.027) deflection ($24.28{\pm}5.09{\mu}m/{\mu}m$), followed by FGP ($20.61{\pm}5.05{\mu}m/{\mu}m$), CR ($17.62{\pm}7.00{\mu}m/{\mu}m$), and GIC ($17.62{\pm}7.00{\mu}m/{\mu}m$). For load-bearing, CR ($38.89{\pm}3.24N$) showed the highest, followed by GIC ($37.51{\pm}6.69N$), FGP ($29.80{\pm}10.03N$), and MP ($18.41{\pm}4.15N$) (p = 0.001) value. FEA showed similar behavior in the restorations in all groups, while MP showed the highest stress concentration in the tooth and post. Conclusions: There is no mechanical advantage in using intraradicular posts for endodontically-treated premolars requiring MOD restoration. Filling the pulp chamber with GIC and restoring the tooth with only CR showed the most promising results for cusp deflection, failure load, and stress distribution.

소아에서 치은 과증식에 대한 cyclosporine의 효과를 연구하는 새로운 방법 (A Novel Method to Study the Effects of Cyclosporine on Gingival Overgrowth in Children)

  • 소아에서 사이클로스포린에 의해 유발되는 치은과증식의 병인을 규명하기 위한 이전의 연구에서는, 개체 간의 차이를 유발할 수 있는 요인을 완전히 배제하지 않았다. 본 연구는 개체 별로 생물학적 변이의 영향을 통제한 실험 모델에 의해 사이클로스포린이 1형 교원질의 대사에 미치는 영향을 알아보았다. 5주령의 수컷 Sprague-Dawley rats 5마리에 6주 동안 사이클로스포린을 위장으로 투여하였다. 사이클로스포린 투여 전과 투여 후 2, 4, 6주째에 쥐의 하악구치부에서 치은을 채취하였다. 20개의 치은표본으로부터 치은섬유모 세포를 추출하였다. 개체 별로 사이클로스포린을 투여하기 전 채취한 치은섬유모세포의 반은 대조군, 나머지 반은 실험실 내 약물 처리한 실험군(Tt)으로 하였다. 사이클로스포린 투여 후 2, 4, 6주째 채취한 치은섬유모세포는 각각의 실험군으로서 T2, T4, T6으로 명명하였다. 면역형광법을 시행하여 모든 치은모세포 내의 1형 교원질의 존재를 확인하였다. 각 개체 별로 대조군과 실험군에서 1형 교원질의 유전자와 단백질 발현의 변화를 분석하기 위해서 각각 Real-time polymerase chain reaction과 Western blotting을 시행하였다. 사이클로스포린을 처리하기 전과 후를 한 개체내에서 비교하여 1형 교원질의 변화를 평가하였다. 대조군과 실험군에서 1형 교원질의 유전자 발현에서는 유의한 차이가 없었다. 모든 개체에서 실험실 내 사이클로스포린 처리한 치은섬유모세포의 1형 교원질의 단백질 발현이 증가한 경우, 사이클로스포린을 투여한 후 채취한 치은섬유모세포에서도 1형 교원질의 단백질 발현이 증가하였다. 본 연구에서 시행한 개체별로 발생할 수 있는 생물학적 변이를 모두 통제한 실험 방법은 유용하였다. 1형 교원질 이외에 다른 요인들에 대한 후속 연구 및 인간에서의 심도 있는 연구가 필요하다.

Bovine teeth에 대한 수 종 상아질 접착제의 미세인장결합강도 (MICROTENSILE BOND STRENGTH OF DENTIN BONDING ADHESIVES ON BOVINE TEETH)

  • 전통적인 3단계 total-etching 시스템인 제 4세대 상아질 접착제로부터 술식의 단순화를 이루며 진화한 제 5, 6세대 및 최근에 소개된 제 7세대 상아질 접착제를 상아질 표면처리방식과 적용단계별로 구분하여 bovine teeth 상아질에 대한 미세인장결합강도를 측정하여 그 효용성을 비교 평가하였다. Bovine incisor의 상아질 표면을 노출시키고, 적절한 표면처리 후 제조사의 지시에 따라 5종의 상아질 접착제; 제 4 세대로 Scotch $Bond^{TM}$ Multipurpose(3M ESPE, USA), $Adper^{TM}$ Single Bond 2(3M ESPE, USA), 제 5세대 $Clearfil^{(R)}$ SE Bond(KURARAY, JAPAN), 제 6세대 AQ $Bond^{TM}$(SUN MEDICAL, JAPAN), 제 7세대 $Clearfil^{(R)}$ tri-S Bond(KURARAY, JAPAN)를 각각 적용하여 미세인장결합강도를 측정하고 접착계면의 파절양상을 관찰하였다. 연구결과 미세인장결합강도는 $Clearfil^{(R)}$ SE Bond군에서 가장 높았으며 다음으로 Scotch $Bond^{TM}$ Multipurpose군, $Adper^{TM}$ Single Bond 2군, AQ $Bond^{TM}$군, $Clearfil^{(R)}$ tri-S Bond군의 순으로 나타났다. 계면의 파절양상은 상대적으로 미세인장강도가 높게 나타난 Scotch $Bond^{TM}$ Multipurpose, $Adper^{TM}$ Single Bond2, $Clearfil^{(R)}$ SE Bond군은 레진의 응집성 파절이 많이 발생하였고 상대적으로 미세인장강도가 낮은 AQ $Bond^{TM}$, $Clearfil^{(R)}$ tri-S Bond 군의 경우에는 대부분 접착성 파절이 관찰되었다. 이상의 결과를 종합하면 상아질 접착제의 미세인장결합강도는 각 접착시스템별로 다소의 차이를 보이며, 특히 제 6, 7세대 상아질 접착제가 이전 세대보다 낮은 것으로 나타났다.

마취된 흰쥐 해마신경세포에서 Neurobiotin 이온주입으로 인한 신경세포의 생리적 특성의 변화 (CHANGES IN ELECTROPHYSIOLOGICAL PROPERTIES OF NEUROBIOTIN-LABELED PYRAMIDAL CELLS OF HIPPOCAMPUS RECORDED IN VIVO)

  • 마취된 흰쥐를 사용하여 해마의 CA영역에 위치한 피라밋세포들의 세포막 특성을 in vivo의 세포내 기록법에 의해서 관찰한 후 2.5% neurobiotin을 세포내 기록용 미세전극에 채워 세포내로 충진시킨후 충진전과 동일한 실험순서로 반응을 다시 관찰하고 ABC kit를 이용하여 면역조직염색을 행하여 형태학적인 관찰을 하였다. 피라밋세포의 세포내 반응 특성은 높은 휴지막 세포막전위, 낮은 input resistance 그리고 큰 활동전위를 가졌다. neurobiotin 충진 전 후에 따른 세포막 특성의 변화는 sustained AHP의 duration과 amplitude, input resistance, 그리고 세포외 및 세포내 자극에 따른 AP 수에서 유의한 차이를 보였고(P<0.05), 세포외 자극에 의한 억제는 주로 전반부에 나타났으며 CA3 영역에 위치한 이 세포의 형태학적 관찰 결과 세포체는 피라밋층에서 분명한 피라미드 형태를 띄고 있었고 기저 및 선단 가지가 각각 백색판층 및 섬유방-분자층까지 뻗어 있었으며 축삭은 겉질을 향해 기저가지돌기면에서 수직으로 뻗어 있었다. 해마의 주세포인 피라밋세포의 세포막 특성과 세포내 염색지시체(marker)로 주로 쓰이는 neurobiotin에 의해 세포막 특성중 일부가 변화됨을 알 수 있었고, 뇌내의 신경세포연결망이 완전히 보존되어 세포들 사이의 시냅스관계를 추측할 수 있는 in vivo 실험 모델이 응용될 수 있음을 제시하였다.

A Surface Treatment Technique for Interim Crown Fabricated by Three-Dimensional Printing with Digital Light-Processing Technology

  • Purpose: The technique introduced in this study describes a technique for surface treatment that applies a photocuring resin to the surface of an interim crown fabricated by three-dimensional (3D) printing without a conventional polishing method. The purpose of this study was to evaluate marginal and internal fit and the intaglio surface trueness of interim crowns after surface treatment of 3D-printed crowns for clinical application. Materials and Methods: An interim crown was fabricated using a 3D printer with digital light-processing technology, and the surface support was removed. After the posttreatment process, the resin was thinly applied to the surface of the interim crown and polymerized to solve the esthetic problem of the surface without the conventional polishing process. In addition, the marginal and internal fits were measured to verify the clinical use of this technique, and the trueness was evaluated to confirm the deformation of the inner surface according to the technical application of the outer surface of the interim crown. The difference before and after the evaluation by a statistical method was verified using an independent t-test (α=0.05). Result: There was no significant difference in the marginal and internal fit before and after the application of this technique (P>0.05). There was no significant difference in intaglio surface trueness before and after the application of this technique (P=0.963). Conclusion: There was no change in the marginal and internal fit or in intaglio surface trueness of the interim crowns to which this technology was applied. This surface treatment technique is a more convenient method for interim crowns fabricated using 3D-printing technology without the conventional polishing process.

Maxillary ameloblastoma in an 8-year-old child: A case report with a review of the literature

  • Ameloblastoma is a benign locally invasive tumor with a high tendency to recur. It is considered rare in the pediatric population, with most cases diagnosed in the third to fifth decades of life. Approximately 80% of ameloblastomas occur in the molar and ramus region of the mandible, while 20% of cases occur in the maxillary posterior region. This report presents a case of plexiform ameloblastoma in an uncommon location in an 8-year-old child. The lesion was initially thought to be a dentigerous cyst, based on its location and radiographic appearance. The clinical and radiographic features, histopathology, and treatment of solid, plexiform, maxillary ameloblastoma are reviewed, with an added emphasis on a literature review of ameloblastoma in children. This report emphasize the importance of long-term follow-up, since recurrence may occur many years after initial tumor removal.

한국인 아동에서의 IL-1 유전자 다변성 연구 (The IL-1 Gene polymorphisms in Korean Children)

  • The severe form of chronic periodontitis(CP) has been reported to be strongly associated with the presence of allele 2 of composite IL-1B(+3954) and IL-1A(+4845) genetic polymorphisms(genotype positive). However, other studies have reported conflicting findings. These might have resulted from differences in ethnic background and disease entities. The aim of this study was to determine the distribution of IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN(VNTR) genetic polymorphisms in children as a future Korean population. The study population consisted of 92 children from the Dept. of Pediatric Dentistry, Chonnam National University Hospital. Genomic DNA was obtained from buccal swab. The IL-1A(+4845), IL-1B(+3954), and IL-1B(-511) genes were genotyped by amplifying the polymorphic region using multiplex polymerase chain reaction(PCR), followed by restriction enzyme digestion and gel electrophoresis. IL-1 RN(VNTR) polymorphism were then evaluated by PCR amplification and fragment size analysis in agarose gel. The allele 2 frequency was 41.3%, 4.3%, 47.8%, and 9.9% for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN respectively. The frequency of genotype with allele 2 carriage for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN was 77.1%, 7.6%, 63.0%, and 15.2% respectively. The allele 2 frequency in IL-1B(+3954) was significantly higher in female than in male population(p<0.05). The negative association was shown between the presence of allele 2 in IL-1B(-511) and in IL-1B(+3954), and the carriage rate of IL-1B(+3954) allele 2 tended to lower in IL-1B(-511) allele 2(P=0.056). Only 7.3% of children carried the composite genotype of IL-1A(+4845) and IL-1B(+3954). These results suggest that the polymorphism of IL-1B(+3954) and the positive composite genotype was relatively rare in Korean population.
