Indirect short resin post Crown을 이용한 유전치부 심미수복 (ESTHETHIC RESTORATION OF PRIMARY ANTERIOR TEETH BY INDIRECT SHORT RESIN POST CROWN : CASE REPORT)

  • Children who have severely destructive anterior primary dentition, as in nursing-bottle caries, in trauma, in rampant caries and in developmental defects, present the dentist with one of the most perplexing situations in dentistry. Especially, children with managed behavor difficultly is very severly situations. This paper reported a new technique for the utilization of resin post and strip crown to indirect methods on severely destructive primary anterior teeth. We name it "indirect short resin post crown" in this paper. Indirect technique be considered to be a simple, a retentive, intensive, color-stable and esthetic restoration.

Comparative evaluation of pain perception following topical application of clove oil, betel leaf extract, lignocaine gel, and ice prior to intraoral injection in children aged 6-10 years: a randomized control study

  • Background: In the field of dentistry, topical anesthetics play an important role in reducing pain during needle pricks. The anesthetic property of betel leaves remain unexplored, even though they have been widely used for the treatment of various ailments. The purpose of this study was to compare and evaluate pain perception following topical application of lignocaine gel, clove gel, ice, and newly developed betel leaf extract gel during intraoral injection in children. Methods: Sixty children aged 6-10 years who met the inclusion criteria were divided into four groups. Topical anesthetic agents, 2% lignocaine (Lox-2% Jelly, Neon, Mumbai, India), 4.7% clove gel (Pain Out Dental Gel, Colgate Palmolive India Ltd, Solan, India), 10% betel leaf extract gel, and ice were applied to each group for one min, followed by administration of infiltration anesthesia. Pain perception was analyzed during needle insertion. The Wong Bakers FACES pain rating scale (WBFPRS) was used for subjective assessment and the Sound, Eye, Motor (SEM) scale for objective assessment. Recorded values were tabulated and subjected to appropriate statistical analysis using SPSS software with a P value set at 0.05. Results: The clove oil and betel leaf groups demonstrated the highest WBFPRS scores, followed by the ice and lignocaine groups. The clove, betel leaf extract, and ice groups showed equal and highest SEM scores, followed by the lignocaine group. The mean WBFPRS and mean SEM scores were statistically significant. Conclusion: Betel leaf extract gel is effective in reducing pain and can act as an alternative topical anesthetic agent.

  • Usage of the rubber dam has been advocated by countless number of dentists. The advantages of the rubber dam such as the following are well-known 1. Moisture control. 2. Improved field of vision. 3. Ease of approach. 4. Soft tissue retraction and Injury prevention. 5. Prevention of aspiration of materials or instruments. 6. Shortened chair time. 7. Induction of nasal breathing during administration of $N_2O-O_2$ sedation. Recent reports indicate the rubber dam can protect the dental staffs from the infection when treating HBV or HIV positive patients. Also, improved moisture control and freeing of both hands allowed by the rubber dam makes it very useful when bonding orthodontic brackets. This case study presents the various clinical application of the rubber dam on patients visiting SNUDH dept. of pediatric dentistry to emphasize the importance of its use in pediatric dentistry.

  • Three cases of treated permanenet teeth are presented. A horizontal root fracture was treated by endodontic treatment in which coronal segment only was done. A replanted incisor is ankylosed because of false restorative method and long extraoral period. Resorption of traumatized bone can be seen in the third case. The prognosis of traumatized incisor depends on proper restoration, adequate amergency treatment and periodic follow-up check.

  • During the eruption of permanent teeth, Traumatic root fractures in young permanent incisors are rare. They occur most commonly in the maxillary central incisors of male patient and are frequently seen in the coronal third of the root. Permanent incisors are very important in terms of esthetics as well as of function, and so conservative treatment is advisable. It is important to maintain vitality of pulp to achieve better result. Location of the fracture line determines the Prognosis. No clinical change were seen in this three case. A case of central incisor with apparent healing of a root fracture without any tratment is presented in this paper. Long term clinical observation is required periodically.

  • Face mask is an extraoral appliance which used to protract maxilla, and can help in the correction of moderately severe class IlI malocclusions by the anterior displacement of the maxilla and maxillary dentition, and possibly restricting or changing the direction of the growth of the mandible. In three cases the results were followed. 1. Anterior crossbite was corrected 2. Maxilla & maxillary dentition were displaced forwardly. 3. Lingual tipping of the mandibular incisors, and backward & downward rotation of mandible were performed. 4. Acceptable improvement in the class III profile was performed.

  • Inverted maxillary incisor is a state in which the maxillary incisor rotates to the upward position. The present report provides two examples of correction of inverted maxillary incisors with surgical intervention & orthodontic appliance. Through surgical exposure & direct bonding of lingual button, the central incisor were brought into proper eruption path with elastic traction. The case 1 & 2 were both treated successfully. The results showed the good position of treated teeth and satisfactory esthetics and adequate width of keratinized gingiva were achieved. Careful differential diagnosis procedure is needed in order to avoid dissatisfactory results and the treatment approaches taken in this case report provided an esthetic and functional results.

  • Anterior openbite is defined as the lack of contacts between the functional occluding teeth on vertical line at centric occlusion and classified into functional and skeletal anterior openbite based on its causes and characteristics. Anterior openbite causes masticatory, speech, and esthetic problems in the growing children and difficulties in diagnosis, treatment, and the prediction of its prognosis. We are reporting on the treatment of anterior openbite in the growing children and the results follow as : 1. In the growing children with anterior openbite, the overbite could be increased by the treatment according to its causes and characteristics. 2. The prognosis is not determined by the presence or severity of oral habit but the skeletal tendency of the patient.

  • The prevalence of congenital teeth missing is about 2-10% of population and it is more common in permanent dentition than primary dentition. In the patients with congenital missing teeth symptoms such as conical tooth, prolonged retention of primary tooth, tooth attrition. and deficient alveolar bone growth can be accompanied. In this paper the results of clinical observation of the congenital teeth missing brothers and their familial history are presented.

  • As one of the developmental disturbances in shape of teeth, an accessory root may involve any teeth, but has very rare frequency in the deciduous teeth. Because it is possible to diagnose the accessory root only by use of the radiographs, it is important to detect this root early by carefully examining the radiographs. The accessory root produces several problems such as abnormal exfoliation of deciduous teeth, eruption abnormality of successive permanent teeth, infection due to residual root during tooth extraction, and incomplete pulp treatment, and therefore it is necessary to have a continuous follow check-up to prevent these problems.
