Objectives : In this study, we investigated the trends of cohort project in domestic and overseas, and suggested the necessity of cohort on korean medicine and the direction of research. Methods : The papers which include cohort in the title and published by 2015 were searched using PubMed. In order to study the history of cohort in domestic and overseas, research trend was investigated based on the websites of each cohort projects and traditional medicine searching engine named OASIS. Results : Using PubMed, 42,802 papers were searched. Most of the studies over 50 % of total articles, however, were conducted in these 6 years. Nevertheless, cohort studies are being actively proceed, the korean medicine based on cohort studies are nonexistent. Therefore, further discussion about cohort study on korean medicine is necessary to gather physiological index and clinical index of korean medicine consistently. Conclusions : Systematically constructed cohort on korean medicine would be the foundation which collects information, such as gene information, bio information, dietary habit, life style, and Pattern identification (辨證), and integrates them. The analysis of massive clinical materials could suggest the basis of Prevention and treatment of korean medicine considering the difference between personal.
알츠하이머와 같은 치매 유병률의 세계적인 증가추세로 치매예방에 대한 정부의 관심이 증가하고 있다. 본 연구의 목적은 최근 국내에서 발표된 치매예방프로그램 논문을 수집, 분석, 종합하여 체계적으로 문헌고찰하기 위한 것이다. 체계적인 문헌고찰을 위하여, 자료검색은 KoreaMed, Riss4u 및 9개의 간호학회지에 게재된 논문을 대상으로 하였고 '치매예방'과 '인지기능'을 키워드로 전체검색 하였다. 2000년부터 2014년 사이에 게재된 치매예방 연구논문을 대상으로 하였다. 총 250편의 연구 논문이 검색되었고 자료선정기준에 따라 최종 29편의 논문이 선별되었다. 치매예방프로그램의 종류를 분석한 결과, 운동프로그램이 전체 29편 중 11편(37.9%), 통합프로그램 9편(31.0%), 그리고 인지강화프로그램 4편(13.8%)이었다. 대부분의 연구(19편, 65.5%)는 65세 이상 노인을 대상으로 하였고 75세 이상 노인을 대상으로 한 연구는 2편이었다. 본 연구결과를 토대로 후기노인에 대한 효과적인 치매예방과 적절한 중재를 제공하기 위한 치매예방프로그램의 개발이 필요하다.
본 연구는 국내 혈액투석환자의 이행 관련 선행연구들을 체계적으로 고찰하여 이행 측정도구와 관련변인을 파악하고, 중재의 효과크기를 분석하여 혈액투석 환자를 위한 근거중심의 실무프로그램 개발 및 향후 연구 방향을 제시하기 위하여 시도되었다. 10개의 데이터베이스에서 검색한 437편의 연구 중 37편의 연구를 최종 선택하였으며, 5편의 중재연구를 메타 분석하였다. 연구결과, 사용된 이행 측정도구는 11개의 원 도구로부터 도출된 것으로 나타났으며, 관련변인은 총 20개 변인이 검색되었다. 중재방법은 개별교육프로그램, SMS, 이메일 및 전화 상담을 이용한 자가간호프로그램, 지지간호프로그램, 약물교육프로그램 등으로 분류되었고, 임의효과모형(random effect model)을 이용하여 분석한 중재의 효과크기는 1.04(95% CI: 0.60, 1.49, p<.00001)로 중재프로그램의 효과가 큰 것으로 나타났다.
Purpose: This study was done to describe the research trend of Complementary and Alternative Medicine(CAM) in Korea and internationally. Method: A Systematic review of CAM related literature published from 2000 to 2005 was done. A comprehensive search by using 34 CAM modalities as search terms to identify all articles dealing with CAM was undertaken from 4 databases: PubMed, RICHIS, KoreaMed, and Riss4U. A Total of 588 abstracts and 223 original articles were reviewed by two independent reviewers using systematic analysis guidelines. Data regarding the types of study design, characteristics of subjects and researchers, and classification of CAM modalities was recorded. The outcome measures of CAM were identified. Results: CAM research has been rapidly increasing worldwide in the last decade. In Korea, nursing(53.5%) followed by medicine(42.7%) occupied the majority of CAM research in comparison to medicine(42.9%) and disciplines in CAM(25.0%), like homeopathy, internationally. Prevailing research designs in Korea were experimental and quasi-experimental, however, randomized clinical trials were found. Internationally, survey research was dominant, with the exception of randomized clinical trials that were mostly an experimental design. Conclusion: The findings suggest that a robust research design in CAM research is needed to accumulate strong scientific evidence and to adopt nursing interventions from CAM modalities.
The epidemiology of human group A rotavirus was analyzed by examining genotypic data acquired from 1989 to 2009 in South Korea. This information was derived from all the available published articles on rotavirus studies in South Korea, retrieved from both the PubMed and KoreaMed databases. Four common G types (G1, G2, G3, and G4) and three common P types (P[8], P[4], and P[6]) accounted for approximately 93% and 99% of the rotavirus reports, respectively. The G9 type was frequently detected after 2000, and because of this prevalence, it is considered to be the fifth most important G type rotavirus after the G1-G4 genotypes. Less common G types of the virus such as G12, G11, and G10 were detected in some geographic settings, and it is important to consider the context of these subtypes and their epidemiological significance. The P[9] virus genotype was observed in the study and has been discussed in many other studies; however, the P[3], P[10] and P[25] genotypes were rarely detected in the epidemiological research. In general, the distributions of the G and P genotypes showed temporal and geographical fluctuations, and a nationwide rotavirus vaccine program that targeted these genotypes demonstrated effectiveness in protecting against the circulating rotavirus strains. However, further analysis is needed to determine the true long-term effectiveness of these vaccines; the analysis should also consider the unexpected effects of vaccinations, such as vaccineinduced diseases, herd immunity, and changes in host susceptibilities.
Purpose: As the number of gastric cancer survivors is increasing and their quality of life after surgery is being emphasized, single-port surgery is emerging as an alternative to conventional gastrectomy. A novel multi-degree-of-freedom (DOF) articulating device, the ArtiSential® device (LivsMed, Seongnam, Korea), was designed to allow more intuitive and meticulous control for surgeons facing ergonomic difficulties with conventional tools. In this study, we evaluated the feasibility of this new device during single-port laparoscopic distal gastrectomy (SP-LDG) for early gastric cancer (EGC) patients. Materials and Methods: Consecutive patients diagnosed with EGC who underwent SP-LDG with ArtiSential® (LivsMed) graspers between April 2018 and August 2020 were enrolled in the study. The clinical outcomes were compared with those of a control group, in which prebent graspers (Olympus Medical Systems Corp) were used for the same procedures. Results: Seventeen patients were enrolled in the ArtiSential® group. There was no significant difference in operative time (205.4±6.0 vs. 218.1±9.9 minutes, P= 0.270) or the quality of surgery, in terms of the number of retrieved lymph nodes (49.5±3.5 vs. 45.9±4.0, P=0.473), length of hospital stay (15.4±2.0 vs. 12.4±1.3 days, P=0.588), and postoperative complications (40.0% vs. 41.2%, P=0.595), between the ArtiSential® group and the control group. Conclusions: The new multi-DOF articulating grasper is feasible and can be used as an alternative for prebent graspers during SP-LDG.
Objectives: This study aimed to assess the clinical evidence supporting the use of acupuncture (AC) for premenstrual syndrome (PMS). Methods: We searched randomized controlled trials (RCTs) that used AC for PMS in 9 databases (PubMed, EMBASE, Cochrane Library, CINAHL, CNKI, CiNii, SCIENCE ON, KoreaMed, OASIS) from their inception to July 2022. Results: We identified 1,168 studies, of which 15 RCTs met the inclusion criteria. The risk of bias in the included studies was relatively unclear or high. Meta-analysis of 3 RCTs showed that the AC group had a significantly higher effective rate than the Medroxyprogesterone group (RR 1.48, 95% CI (1.23, 1.78), p<0.0001). Meta-analysis of 2 RCTs showed that there was no statistically significant difference between the AC group and the Sham AC group in the effective rate (RR 4.72, 95% CI (0.88, 25.36), p=0.07). In a review of individual studies, the AC group was more effective than the control group in terms of the effective rate, symptom scale, quality of life, adverse events, and recurrence rate. Conclusions: The AC group was more effective than general treatments groups such as sham AC, western medicine, and dietary supplements, and there were no serious adverse events. However, the evidence on the effectiveness and safety of acupuncture for PMS was inconclusive due to the small number of included studies and low quality. Therefore, systematic reviews based on more rigorously designed randomized clinical trials are needed in the future to properly evaluate the effect of AC on PMS.
