치매의 한방치료와 한양방 협진치료에 관한 전문가 집단 설문연구 (The Study on the Korean Traditional Medical Treatment and System of Collaborative Practice Between Korean Traditional Medicine and Western Medicine for Dementia: Based on Analysis of Questionnaire Survey in Professional Group)

  • Objectives : Recently, the elderly population and dementia prevalence have been rapidly increasing in Korea. Dementia is a concept of syndrome composed of characteristic symptoms. However, there have been no approved treatments for dementia with proven efficacy. Therefore, we investigated Korean traditional treatment being used in clinic and collaborative practice for dementia in the professional group. Methods : We conducted a survey using questionnaires to professors of oriental neuropsychiatry for a month. Items of the questionnaire consisted of overall Korean traditional medical treatment for dementia, which included herbal medicine, acupuncture, moxibustion, cupping, psychotherapy and others. Results : 1. It was shown that Yukmijihwangtanggamibang, Jowiseungcheongtang, Seonghyangjeonggisan have been used the most in herbal medicine. 2. Chechim, Saamchimbeop, and scalp acupuncture have been used the most in acupuncture treatment. 3. In Korean traditional psychotherapy, Ijeongbyeongi, Jieongoronyobeop, Gigongyobeop have been used, and effective in anxiety, agitation, improvement of comprehension for caregiver and environment. 4. Most professors answered that collaborative practice between Korean traditional medicine and western medicine has been needed for dementia. They suggested that herbal medicine, followed by acupuncture, and moxibustion can be used with western medicine simultaneously. Conclusions : The survey revealed that professors use herbal medicine and acupuncture as main treatments for dementia. They share perspective regarding the need of long period to treat dementia patients, and there are needs of collaborative practice for dementia.

우즈베키스탄에서의 한국 한의학 교육에 대한 검토: 일개 의과대학을 중심으로 (Reviewing the Education of Oriental Medicine in Uzbekistan: with Reference to the Data in a Medical Academy)

  • Objectives: The aim of this study was to investigate the current educational environment of Oriental medicine and to develop a future-oriented curriculum for traditional Korean medicine in Uzbekistan. The questionnaire used in this research was based on the current curriculum. Methods: This study was carried out by distributing questionnaires to 120 students (5th year in Tashkent medical academy in Uzbekistan) in order to examine the coursed's efficacy for the attainment of oriental medical knowledge. Results: 1. Among the respondents, 69.6% of them have already studied oriental medicine. 2. The respondents felt that the class in Oriental medicine was effective (88.3%). Moreover, they felt that understanding traditional Korean medicine was also beneficial (81.6%). However, 52.5% of them have responded that the class seemed difficult. 3. Among Uzbeki medical students, 64.9% of them have shown interest in acupuncture and moxibustion, 38.4% in traditional Korean herbology and pharmacognosy. 4. We also found that 63.8% of them have desired to have more specific studies in traditional Korean medicine. Conclusion: According to our results, developing adequate textbooks, teaching methods, and qualities of instructors should be considered as alternatives for the successful generalization of traditional Korean medicine in Uzbekistan.

  • King Se-Jong, who had put much effort into maintaining the national structure, published geological documents to reinforce his authority and through these documents could see the nationwide distribution of medicinal resources. Since then the regional geological documents published in each region included information on the local medical environment of those times. This study is one that focuses on how to specifically understand oriental medicine resources and the contents of ancient medicine by applying the appropriate geological information.

한약제제 허가기준 개선방안 연구(I) (A Study on Amendment of Approval Specifications for Traditional Korean Medicines)

  • This study was conducted to improve the current regulation guidelines for developing traditional Korean medicines with effectiveness and productivity, in order to cope with the upcoming ICH on specifications on oriental herbal drugs. Also, major purposes of this study are to motivate R&D and to pioneer foreign markets for domestic herbal drug companies. First, after examining concepts of traditional Korean medicines and comparing the numbers and differentiation of herbal drugs registered on Pharmacopeia among Korea, China, and Japan, the current new drug development requirements for traditional Korean medicines were reviewed in detail, followed by comparison of foreign regulation systems including USA, EU, China, and Japan. Second, empirical cases on failure of development for new traditional Korean medicines under the current regulation system in the domestic companies including Dong-A, Kwangdong, and Samchondang, were collected and analyzed. As a result, hanbangsaengyak, the new category for traditional Korean medicines was newly developed on the basis of scientification of data between saengyak and hanyak, from the perspectives of harmonization between oriental medicine and western medicine and of balance between food and drug, in terms of industrialization, publicity, modernization, and effectiveness of administration. In addition, the new regulation requirements for the new hanbangsaengyak preparations were discussed by establishing principles of reinforcing preclinical test and of simplifying clinical trials in Korea. Finally, the further researches to articulate the complete specifications for pre-clinical and clinical requirements for traditional Korean medicines were strongly suggested.

  • Background : Currently, the absence of a unified terminology and proper translation about Traditional Korean Medicine is a problem that should be solved. Although Traditional Korean Medicine has undergone steady growth in popularity during the past years, the Westerner's poor access to information has been drawback to its reception in the West. Objective : This research is aimed to seek system for standardization of stroke terminology and proper translation into English. Conclusion : We proposed the method of making unified terminology and translation about stroke in Traditional Korean Medicine.

북한 고려의학 학술 저널에 대한 저자 및 키워드 네트워크 분석 (A Network Analysis of Authors and Keywords from North Korean Traditional Medicine Journal, Koryo Medicine)

  • Objectives : This study seeks to grasp the current status of Koryo medical research in North Korea, by focusing on researchers and research topics. Methods : A network analysis of co-authors and keyword which were extracted from Koryo Medicine - a North Korean traditional medicine journal, was conducted. Results : The results of author network analysis was a sparse network due to the low correlation between authors. The domain-wide network density of co-authors was 0.001, with a diameter of 14, average distance between nodes 4.029, and average binding coefficient 0.029. The results of the keyword network analysis showed the keyword "traditional medicine" had the strongest correlation weight of 228. Other keywords with high correlation weight was common acupuncture (84) and intradermal acupuncture(80). Conclusions : Although the co-authors of the Koryo Medicine did not have a high correlation with each other, they were able to identify key researchers considered important for each major sub-network. In addition, the keywords of the Koryo Medicine journals had a very high linkage to herbal medicines.

무릎 내측측부인대 손상의 한의학적 치료에 대한 국내외 임상연구 동향 분석 (A Clinical Analysis to Study Effectiveness of Korean Medicine for Medial Collateral Ligament Injury of the Knee)

  • Objectives This study aimed to review clinical studies on traditional Korean medicine treatment for medial collateral ligament injury of the knee. Methods Clinical studies on Korean traditional medicine treatment of medial collateral ligament injury were conducted. We used five Korean online databases (OASIS, KISS, RISS, DBPia, and ScienceOn) and three foreign databases (PubMed, Cochrane Library, and CNKI). Out of 99 studies that were found, we excluded repeated articles, studies that were not related to Korean medicine, and those not relevant to the topic of the study. Results Ten randomized controlled trials and 20 case studies were selected. Eight traditional Korean medicine treatments, including acupuncture, herbal medicine, chuna, and herbal ointment, were used in these studies. The most commonly used treatment was found to be acupuncture. Conclusions Our study showed that traditional Korean medicine for medial collateral ligament injuries was effective. However, there were some limitations. Further clinical studies and randomized controlled clinical trials are needed for more evidence on Korean traditional medicine.

한방의료기관의 한약 관리 및 가격 현황에 대한 연구 (A Survey of the Status of Management and Prices of Herbal Medicines: Traditional Korean Medicine Institutions)

  • Objectives: Comparison of the status of herbal medicine management carried out by traditional Korean medicine hospitals and clinics, and changes in the prices of the herbal medicines offered by them. Methods: A questionnaire survey was conducted for 126 traditional Korean medicine hospitals and 4,200 traditional Korean medicine clinics. Questionnaire sheets were dispatched to them by mail in 2008. Rate of response: 57 hospitals (45.2%) and 465 clinics (11.0%) Results: 1. Concerning the herbal medicines kept by them, the hospitals and the clinics kept 295 and 147 kinds of medicine, respectively, on average. As for their monthly expenditure on the purchase of herbal medicines, the hospitals spent an average of KRW 28.38 million while the clinics spent an average of KRW 1.64 million. 2. The average monthly expenditure of the clinics on the purchase of herbal medicines decreased by 32% from 2006, which can be interpreted as a reduction of their business. 3. The prices of herbal medicines for disease treatment ranged from KRW 140,000~190,000 per jae(dosage form) in the hospitals compared to KRW 140,000~168,000 in the clinics. 4. Compared to 1994, the prices of herbal medicines used for disease treatment have risen, whereas the prices of herbal medicines used as tonics have decreased. Conclusions: There are no conspicuous differences between the hospitals and the clinics in terms of the prices charged for herbal medicines, regardless of whether they are used to treat diseases or prescribed as tonics.

동결견의 한의학적 치료에 대한 국내 연구 동향 (The Domestic Trends of Traditional Korean Medicine Treatments on Frozen Shoulder)

  • Objectives : The purpose of this review is to investigate the trends of traditional korean medicine treatments on frozen shoulder. Methods : We investigated the studies on traditional korean medicine treatments for frozen shoulder via searching 6 Korean web databases. As a result, 32 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments, the instruments for assessment and ethical approvals. Results : 32 papers were published since 1989. The studies on traditional korean medicine treatments about frozen shoulder were mainly published in The journal of korean acupuncture & moxibustion medicine. 10 case series, 7 reviews, 6 non-randomized controlled trials, 5 randomized controlled trials and 4 case reports had been under research. In most of the research, the number of the cases were not enough. In 15 cases of the studies, various traditional korean medicine treatments were used to treat the symptoms. In 8 cases of the studies, one method was used to treat the symptoms. Range of motion(ROM) and visual analogue scale(VAS) were used as primary assessments. Among the 25 clinical studies, 5 of them were accepted by institutional review board(IRB). Conclusion : In this study, we analyzed the trends of traditional korean medicine treatments on frozen shoulder. Reviewing the domestic trends of studies on traditional korean medicine treatments for frozen shoulder and examining the strong and weak points of those treatments are essential for the future studies.

노간주나무 에탄올추출물의 지방 흡수 억제 및 지방분해 효능 (Anti-lipase and Lipolytic Activities of EtOH Extract from Juniperus rigida)

  • Obesity is an important risk factor that significantly increases mortality and disease rates in the cardiovascular disease, diabetes, and various diseases. So far, the most powerful way to inhibit fat absorption is pancreatic lipase inhibitors. In this study, we investigated the anti-obesity effect of the extract of Juniperus rigida. Juniperus rigida extract (JRE) had a inhibitory effect on pancreatic lipase activity ($IC_{50}$=8.63 ${\mu}g$/ml). In in vivo oil-emulsion loading test, this extract also inhibited the intestinal fat absorption. In addition, we measured inhibitory effects of JRE on activity of phosphodiesterase (PDE) and hormone sensitive lipase (HSL) among the important enzymes associated with lipolysis. JRE strongly inhibited PDE activity ($IC_{50}$=4.56 ${\mu}g$/ml), whereas inhibitory effect on HSL activity was very weak compared with orlistat. As a result, JRE inhibited the absorption of fat by inhibiting the activity of pancreatic lipase and induced lipolysis through inhibition of PDE activity. Therefore, we suggest that Juniperus rigida may be a potential therapeutic agent improving obesity.
