Benefits of adherence to the Korea Healthy Eating Index on the risk factors and incidence of the metabolic syndrome: analysis of the 7th (2016-2018) Korea National Health and Nutrition Examination Survey (제7기 (2016-2018년) 국민건강영양조사 자료를 이용한 식생활평가지수 준수와 대사증후군 위험요소 및 대사증후군 발생 관계 연구)

  • Purpose: The purpose of the study was to investigate whether adherence to the Korea Healthy Eating Index (KHEI) was associated with metabolic syndrome and risk markers. Methods: The participants included 8,345 adults, aged 20-59 years, who took part in the 7th Korea National Health and Nutrition Examination Survey (KNHANES). The data were analyzed using a complex-sample t-test, the Rao Scott χ2-test, and logistic regression analysis on the SPSS v. 26.0 software. The participants were divided into four groups by quartiles of KHEI scores. Results: The average KHEI score was 61.06 points out of 100, and the women's score (62.50 points) was significantly higher than that of men (59.63 points). The KHEI quartiles status showed significant differences by age (p < 0.001), household income (p < 0.001), smoking status (p < 0.001), and food security. Specifically, the KHEI quartiles in the men showed significant differences in education (p < 0.001) and economic activity (p < 0.001) whereas those of women showed significant differences in alcohol-consumption (p < 0.001), depression (p < 0.01) and eating-out (p < 0.001). As the KHEI scores increased, the proportion of subjects with an energy intake below the estimated energy requirement (EER) was lower, and significantly better levels of intake were observed for carbohydrate, protein, vitamin C, calcium, vitamin B1, vitamin B2, and niacin. The incidence of the metabolic syndrome risk factors, hypertriglyceridemia and hyperglycemia for men and hypertension, and hyperglycemia for women showed significant differences. The KHEI scores were inversely associated with abdominal obesity, hypertriglyceridemia, low high-density lipoprotein cholesterol, hyperglycemia, hypertension, and metabolic syndrome. Conclusion: Based on these results, we conclude that higher adherence to the KHEI was associated with lower metabolic syndrome risk factors and incidence of the metabolic syndrome.

Folate: 2020 Dietary reference intakes and nutritional status of Koreans (엽산: 2020 영양소 섭취기준과 한국인의 영양상태)

  • Folate, a water-soluble vitamin, acts as a coenzyme for one-carbon metabolism in nucleic acid synthesis and amino acid metabolism. Adequate folate nutritional status during the periconceptional period is known to prevent neural tube defects. In addition, insufficient folate intake is associated with various conditions, such as anemia, hyperhomocysteinemia, cardiovascular disease, cancer, cognitive impairment, and depression. This review discusses the rationale for the revision of the 2020 Korean dietary reference intakes for folate, and suggestions for future revisions. Based on the changes in the standard body weight in 2020, the adequate intake (AI) for infants (5-11 months) and the estimated average requirements (EARs) for 15-18 years of age were revised, but there were no changes in the recommended nutrient intakes (RNIs) and tolerable upper intake levels (ULs) for all age groups. Mean folate intake did not reach RNI in most age groups and was particularly low in women aged 15-29 years, according to the results of the 2016-2018 Korea National Health and Nutrition Examination Survey (KNHANES). The percentages of folate intake to RNI were lower than 60% in pregnant and lactating women, but serum folate concentrations were higher than those in other age groups, presumably due to the use of supplements. Therefore, total folate intake, from both food and supplements, should be evaluated. In addition, the database of folate in raw, cooked, and fortified foods should be further expanded to accurately assess the folate intake of Koreans. Determination of the concentrations of erythrocyte folate and plasma homocysteine as well as serum folate is recommended, and quality control of the analysis is critical.

  • Purpose: This study examined the nutrient intake and dietary habits based on breakfast consumption in Korean adolescents. Methods: The data of the 2013 ~ 2015 Korea National Health and Nutrition Examination Survey (KNHANES) were used in this study. The analysis included 1,281 adolescents aged 12 to 18 years. The subjects were divided into two groups (EBF: eating breakfast, n = 911; SBF: skipping breakfast, n = 370). Results: The EBF group was significantly younger than the SBF group. In addition, the EBF group had a significantly lower frequency of skipping dinner than the SBF group. The EBF group consumed significantly higher levels of most nutrients except for vitamin A than the SBF group. The percent of nutrient intake under the EAR (estimated average requirements) in the EBF group were also significantly lower than that in the SBF group. Moreover, the NAR (nutrient adequacy ratio) and MAR (mean adequacy ratio) of the EBF group were significantly higher than those of the SBP group. The intake of cereal, sugars and sweeteners, vegetables, and eggs in the EBF group was significantly higher than that in the SBF group. Conclusion: The habit of skipping breakfast in adolescents leads to a decrease in the total nutrient intake and imbalanced dietary habits. Breakfast consumption in adolescence is very important. Therefore, more studies will be needed to develop nutrition education systematically in Korean adolescents.

  • Purpose: The purpose of this study was to estimate intakes of macronutrients and antioxidant vitamins according to the Hansik consumption rate among Korean adults. Methods: Using data from the 2007~2012 Korea National Health and Nutrition Examination Survey, a total of 33,069 subjects aged over 19 years old were included in this study. We estimated individual daily Hansik consumption rates and intakes of macronutrients and antioxidant vitamins, including vitamin A and its subgroup such as retinol, ${\alpha}$-carotene, ${\beta}$-carotene, and ${\beta}$-cryptoxanthin, vitamin C, and vitamin E, by linking food consumption data with the nutrient and antioxidant vitamin database of commonly consumed foods. Results: Around 75% of subjects consumed Hansik in over 75% of their daily total consumed food. The most frequently consumed Hansik was cabbage kimchi (1.57 times/day), followed by multigrain rice (0.86 times/day) and white rice (0.80 times/day). The household income level and education level was inversely associated with the Hansik consumption rate. There was a positive relationship between Hansik consumption rate and vitamin A, ${\beta}$-carotene, ${\beta}$-cryptoxanthin, and vitamin C intakes. On the other hand, Hansik consumption rate was inversely associated with energy and fat intake. Conclusion: The results of this study suggest that Hansik consumption could provide more antioxidant vitamins and less energy and fat. Thus, further research will be needed to analyze the association between Hansik and health effects.

Evaluation of Diet Quality according to Self-Rated Health Status of Korean Middle-Aged Women -Based on 2008~2009 Korean National Health and Nutrition Examination Survey- (한국 중년 여성의 주관적 건강상태에 따른 식사의 질 평가 -2008~2009 국민건강영양조사 자료 활용-)

  • The purpose of this study was to evaluate the diet quality in Korean middle-aged women based on their self-rated health status (SRH) as well as to investigate the relationship between SRH and the diet quality. For the study, we obtained data for analysis from the combined 2008~2009 Korean National Health and Nutrition Examination Survey (KNHANES). The study subjects were divided into two groups-'Good' group (${\geq}3.06$, n=622) and 'Bad' group (<3.06, n=1,092)- on the basis of the average SRH value (3.06). The women in the 'Good' group had achieved a higher education level and had a significantly high monthly average income (P<0.05), whereas women in the 'Bad' group had a larger waist circumference (P<0.05). In addition, women in the 'Good' group engaged in more physical activities (P<0.01). In comparison with the women in 'Bad' group, women in the 'Good' group were found to have a superior nutritional status, with a nutrient adequacy ratio (NAR) of protein (P<0.05), calcium (P<0.05), iron (P<0.05), vitamin A (P<0.05), vitamin $B_2$ (P<0.05), and niacin (P<0.01). The mean adequacy ratio (MAR) of women in the 'Good' group was also significantly higher than that of the women in the 'Bad' group. The index of nutritional quality (INQ) of protein (P<0.05), iron (P<0.05), vitamin $B_1$ (P<0.05), and niacin (P<0.05) in women belonging to the 'Good' group were also significantly higher than that in the women assigned the 'Bad' group. The results of regression analysis on the relevance between the diet quality and SRH, MAR, and INQ showed a significantly positive relevance (P<0.001), even after adjusting for disturbance factors. The results of this study show that diet quality seems to be the important factor for improving SRH of women. Therefore, we suggest that developing dietary guidelines and educating women about the guidelines would improve health of middle-aged women in Korea.

Factors Influencing the Regular Oral Check-Ups: Based on the Data of the 2014 Korea National Health and Nutrition Examination Survey (우리나라 경제활동자의 구강검진 수진 영향요인: 국민건강영양조사 제6기 2차년도(2014) 자료를 바탕으로)

  • This study aimed to investigate the factors influencing the decision on whether to receive regular oral check-ups among the national health insurance beneficiaries with income and aged over 20 years in Korea. This was a cross-sectional study, using the data from the 2014 6th Korea National Health and Nutrition Examination Survey (KNHANES VI-2), conducted by the Korea Centers for Disease Control and Prevention. The final sample included 2,843 subjects who participated in the health examination survey and oral check-ups. The IBM SPSS Statistics ver. 20.0 was used for the statistical analysis, based on the complex sampling design. The Rao-Scott chi-square test was used to verify the difference in the distribution of independent variables with regard to oral check-ups. The binary logistic regression analysis was used to determine the influencing factors. The ratio of beneficiaries who received oral check-ups was less than half of those who received medical examinations. The Rao-Scott chi-square test, revealed significant differences in the age group (p<0.05) of the predisposing factors, toothache within a year (p<0.001), untreated dental caries (p<0.001) of the need factors, personal income level (p<0.001), educational level (p<0.001), type of medical insurance (p<0.001), and private health insurance (p<0.01) of the enabling factors. In the binary logistic regression analysis, factors influencing oral check-ups were age group, personal income level, educational level, toothache within a year, and untreated dental caries. These findings show a variety of factors influencing the utilization of regular oral check-ups.

Influence of Menopause on Breast Cancer in South Korean Adult Women (한국 성인 여성의 폐경 여부가 유방암에 미치는 영향)

  • The purpose of this study was to investigate breast cancer its menopause in South Korean adult women using data from the seventh Korea national health and nutrition examination survey(KNHANES VII-2) 2017. The subjects were 3,414 people in total. The Data were analyzed by frequency, ${\chi}^2$ test and multiple logistic regression analysis using SPSS complex samples Win 25 program. According to the age of the subjects, breast cancer was 1.7% higher in women aged 50~59 years than aged 40~49 years, and 1.7% was significantly more frequent in women who had no drinking frequency than in almost everyday. Menopausal age was significantly higher in women under 44 years of age with breast cancer than in those over 50 years of age. According to menopause, postmenopausal breast cancer was 1.7% more than before menopause. When adjusted for age and alcohol drinking frequency, premenopausal adult women in Korea had significantly lower breast cancer frequency by 0.03 compared with postmenopausal women. Based on these results, it is important to pay attention to the management of breast cancer in young women aged 44 years or younger and multidisciplinary efforts are needed to reduce the ever-increasing prevalence of breast cancer.

Vitamin D intake, serum 25OHD, and bone mineral density of Korean adults: Based on the Korea National Health and Nutrition Examination Survey (KNHANES, 2011) (한국 성인의 비타민D 섭취량과 혈중 25OHD 농도 및 골밀도와의 관련성 : 2011 국민건강영양조사 결과 재분석)

  • Purpose: The purpose of this study was to estimate dietary intake of vitamin D and the relationship between serum 25-hydroxyvitamin D (25OHD) concentration and bone mineral density (BMD) in Korean adults using the 2011 data from the Korea National Health and Nutrition Examination Survey. Methods: Daily intake of vitamin D and ratio of subjects that consumed less vitamin D than adequate intake (AI) were estimated in 4,879 Korean adults. The relationship between daily intake of vitamin D and serum 25OHD and BMD were analyzed. Results: Average daily intakes of vitamin D were $3.84{\pm}0.23{\mu}g/day$ for men and $2.22{\pm}0.11{\mu}g/day$ for women. Approximately 72~97% of men and 80~99% of women consumed less than the AI of vitamin D for Koreans. Serum 25OHD concentration increased with age, and the ratios of serum vitamin D deficiency (< 20 ng/mL) were 47.8~81.1% for men and 59.4~92.8% for women. Average intake of vitamin D was higher in subjects aged < 50 yr than in those ${\geq}50yr$, but lower in serum 25OHD concentration. In subjects aged < 50 yr, serum 25OHD was higher in subjects that consumed $10{\mu}g/day$ of vitamin D than in those that consumed less than $5{\mu}g/day$. In female subjects aged ${\geq}50yr$, average intake of vitamin D was associated with higher bone mineral density. Conclusion: It was found that dietary intake of vitamin D could increase serum 25OHD concentration in young adults and bone mineral density in old women. Therefore, nutrition policies for enriched foods with vitamin D and nutrition education to consume more vitamin D-rich foods are needed to ameliorate vitamin D status of the Korean population. Adequate intake for Korean population aged < 50 yr might be adjusted upwardly up to $10{\mu}g/day$.

The relationship between past smoking period and tooth loss in Korean elderly (한국 노년층의 과거흡연기간과 상실치아의 연관성)

  • The purpose of this study was to evaluate the relationship between past smoking period and tooth loss in Korean elderly based on the data the Sixth Korea National Health and Nutrition Examination Survey (2013-2015). The study subjects were 4,037 elderly people (1,737 men and 2,300 women) over 65 years old. The statistical analyses were conducted by applying complex sample analysis technique using SPSS/WIN 22.0(${\alpha}=0.05$). The average number of tooth loss in Korean elderly was 11.14. The analysis result of the relationship of the tooth loss according to the past smoking period, the risk of tooth loss was more than twice higher in over 30 years past smoking period group than in under 10 years past smoking period group. In conclusion, the long term past smoking were associated with tooth loss in Korean elderly. Therefore, it seems to need a continuous oral health care through non-smoking policy and education.

Association between Glycemic Index, Glycemic Load, Dietary Carbohydrates and Diabetes from Korean National Health and Nutrition Examination Survey 2005 (2005 국민건강영양조사 자료 분석을 통한 한국 성인 남녀의 식이 중 Glycemic Index, Glycemic Load 및 탄수화물 섭취 수준과 당뇨 발병과의 관련성 연구)

  • The purpose of this study was to establish an association between glycemic index (GI), glycemic load (GL), dietary carbohydrates and diabetes with the context of the current population dietary practice in Korea. The subjects of 3,389 adults (male 1,430, female 1,959) were divided into normal (serum fasting glucose < 100 mg/dL), impaired glucose tolerance (100 ${\leq}$ serum fasting glucose < 126 mg/dL), diabetes (serum fasting glucose > 126 mg/dL) by serum fasting glucose. Anthropometric and hematologic factors, and nutrient intakes, dietary glycemic index (DGI), dietary glycemic load (DGL) were assessed. Multiple logistic regression model was used to determine the odds ratios (ORs) and 95% confidence intervals for relationship of DGI, DGL, carbohydrates intakes, and diabetes. DGI and DGL were not significantly correlated with impaired glucose tolerance and diabetes. However, the risk of impaired glucose tolerance and diabetes showed a tendency to increase as increase of DGI after multivariate adjustment (age, education, income, region area, diabetes family history, smoking, drinking, exercise, energy intake) in male. The risk of impaired glucose tolerance and diabetes showed a tendency to increase in the DGI 71.1-74.8 after multivariate adjustment in female. DGL was inversely related to impaired glucose tolerance and diabetes in male. In female, however, DGL was positively related to impaired glucose tolerance and diabetes. In particular, the risk of diabetes increased positively in level of DGL 260.5, and remained after multivariate adjustment (Q5 vs Q1:2.38, 0.87-6.48). When percent energy intakes from carbohydrates were more than 70%, the risk of impaired glucose tolerance and diabetes increased in both male and female. In particular, when percent energy intakes from carbohydrates were more than 69.9%, the risk of diabetes increased positively in male (Q4 vs Q1:2.34, 1.16-4.17). In conclusion, above 70% energy intakes from carbohydrates appeared to be a risk factor of diabetes. It seemed that the meal with high GI and GL value must be avoided it. And also, the macronutrients of the meal must be properly balanced. In particular, it may be said that it is a preventive way for treatment of the diabetes to avoid eating carbohydrates of much quantity.
