40대 이상 농촌 및 중소도시 성인의 식품섭취 패턴 (Pattern)과 질환별 유병위험도 - 한국인유전체역학조사사업 일부 대상자에 대해 - (Dietary Patterns and Prevalence Odds Ratio in Middle-aged Adults of Rural and Mid-size City in Korean Genome Epidemiology Study)

  • Recently, dietary pattern analysis was emerged as an approach to examine the relationships between diet and risk of chronic diseases. This study was to identify groups with population who report similar dietary pattern in Korean genome epidemiology study (KoGES) and association with several chronic diseases. The cohort participants living in Ansung and Ansan (Gyeonggi province) were totally 10,038. Among those, 6,873 subjects with no missing values in food frequency questionnaire were included in this analysis. After combining 103 food items into 17 food groups, 4 dietary factors were obtained by factor analysis based on their weights. Factor 1 showed high factor loadings in vegetables, mushrooms, meats, fish, beverages, and oriental-cereals. Factor 2 had high factor loadings in vegetables, fruits, fish, and factor 3 had high factor loadings in cereal-oriental, cerial-western and snacks. Factor 4 showed positive high factor loadings in rice and Kimchi and negative factor loadings in mushrooms and milk and dairy products. Using factor scores of four factors, subjects were classified into 3 clusters by K-means clustering. We named those 'Rice and Kimchi eating' group, 'Contented eating' group, and 'Healthy and light eating' group depending on their eating characteristics. 'Rice and Kimchi eating' group showed high prevalence in men, farmers and 60s. 'Contented eating' group and 'Healthy and light eating' group had high prevalence in women, people living in urban area (Ansan Citizen), with high-school education and above, and a monthly income of one million won and more. 'Contented eating' group appeared lower distribution proportion in the sixties and 'Healthy and light eating' group does higher in the fifties. 'Contented eating' versus 'Rice and Kimchi eating', odds ratio for hypertension, diabetes, metabolic syndrome and obesity significantly decreased after adjusting age and sex (OR=0.64, 0.73, and 0.85 respectively, 95% CI). Although our results were from a cross-sectional study, these imply that the dietary patterns were related to diseases.

  • 본 연구 결과 우리나라 40세 이상 성인에서 FBDI와 당뇨병 발생률 간의 상관성은 없었으나 당화혈색소 수준과 상관관계를 가지고, 이러한 경향은 여성이거나 운동을 하지 않는 사람, 그리고 비만인 경우 영향을 더 많이 받는다는 결과를 도출해냈다. 이는 앞으로 우리나라 당뇨병 관리 측면에서 염증을 고려한 다각화된 임상영양치료의 발판이 되리라 사료된다. 또한 추후 장기간의 추적 연구를 통해 식사염증지표와 당뇨병의 발생에 대한 추가적인 연구가 필요한 것으로 보이며, 당뇨병의 심각성에 관한 연구는 거의 없는 실정이기 때문에 추후 연구가 반드시 필요하다고 사료된다. 본 연구를 발판 삼아 추가적인 연구를 기반으로 임상현장에서 식사염증지표가 당뇨병 관리 도구로 활용될 수 있기를 기대하는 바이다.

비만도와 신체활동이 성인의 고혈압에 미치는 영향 (The Effects of the Obesity and Physical Activity on the Prevalence of Hypertension in Korean Adults)

  • This study was conducted to assess joint effect of body mass index (BMI) and physical activity with risk of hypertension in middle-aged Koreans. The effects of BMI and physical activity on risk of hypertension were studied using data set of 10,020 subjects aged 40~69 years in a Korean Genome and Epidemiology Study (KoGES). About 31.8% of men and 30.2% of women had hypertension. Higher BMI was associated with increased risk of hypertension in men and women. However, diverse association of physical activity with hypertension was only detected in women. Inactive women with a BMI${\geq}25$ were more likely to have hypertension than active women with a BMI<23 (ORs=3.96, 95% CI; 2.77~5.67). The present study indicates that regular physical activity and weight control can reduce risk of hypertension in Korean middle-aged women.

Association between ESR1 rs1884051 polymorphism and dietary total energy and plant protein intake on obesity in Korean men

  • ESR1 has been listed in the Human Obesity Gene Map as candidate gene associated with obesity. Thus, in this study, we investigated the effect of the ESR1 rs1884051 polymorphism on obesity-related variables, together with their modulations by dietary intake in Korean men. The obesity-related variables and dietary intake of 3,039 Korean men aged 40-59 years from KoGES database were analyzed. Body weight (P = 0.007), BMI (P = 0.003), waist-hip ratio (= 0.011), fat body mass (P = 0.010), and body fat percentage (P = 0.040) were significantly lower in subjects with the minor T allele of ESR1 rs1884051 than in subjects carrying the C allele. Moreover, the rs1884051 T allele was associated with a decreased risk of obesity prevalence (P = 0.040). Among the subjects whose total energy intake was below the median, carrier of the minor T allele of ESR1 rs1884051 had a lower BMI (P = 0.003) when compared with subjects carrying the C allele. In addition, among subjects whose plant protein intake was above the median, carrier of the minor T allele of ESR1 rs1884051 had a lower BMI (P = 0.044) compared with subjects carrying the C allele. Our findings demonstrate that there is a significant association between the ESR1 rs1884051 variant and obesity-related variables and this association can be potentially modified by dietary energy and plant protein intake.

  • Wire-suspensions in the conventional actuators mechanically support the moving part and guarantee the accuracy of the actuator without tangential tilt actuation. However, such a suspension configuration has considerable stiffness in the tangential tilt direction with two additional wire beams for the tangential tilt. Thus, we performed a design sensitivity analysis for the wire-suspension stiffness of 4-axis actuator and controlled the main parameters such as distance among wire-suspensions and wire-suspension length to allow tangential tilt flexibility. The elasticity of frame PCB that supports the moving part by wire-suspensions was also exploited to improve the flexibility of wire-suspension in the tangential tilt direction. A novel suspension structure was devised by establishing eight wire-suspensions at both sides of the moving part for electrical connection to coils. The magnetic circuit according to the proposed 4-axis actuator using multi-polar magnet and coils was also suggested for the generation of electromagnetic forces in the focusing, tracking, radial and tangential tilt directions.

Dietary Patterns and Nutrient Intakes of Individuals with Circulatory Diseases: Ansan-Ansung Cohort Data from the Korean Genome and Epidemiology Study

  • Recently, there is growing interest in studying the dietary patterns that affect the risk of circulatory system diseases (CSDs). We investigated the relationship between CSDs and dietary patterns through a follow-up study in Korea (2001-2016). The participants of this study were obtained from the Korean Genome and Epidemiology Study (KoGES). This study was a large community-based cohort study (the Ansan-Ansung areas) conducted to assess the effects of various factors, especially diet, on the onset of chronic diseases among the Korean population aged 40-69 yrs. Baseline data were collected from 2001 to 2002, and follow-up studies were performed every 2 yrs, with over 7 follow-up studies performed (2015-2016). Three dietary patterns were identified using factor analysis: "vegetable and seafood (men)/soup and stew (women)" pattern, "sweet foods and breads-rice cake" pattern, and "multigrain rice and cooked white rice" pattern. None of the dietary patterns were significantly associated with the risk of CSDs in either men or women. Our follow-up study is meaningful as it investigated whether the dietary patterns of individuals according to sex affects the development of CSDs.

대사증후군 성인의 흡연 및 음주 상태가 대장암 발생 위험에 미치는 영향: 전향적 코호트 연구 (Incidence of Colon Cancer Related to Cigarette Smoking and Alcohol Consumption in Adults with Metabolic Syndrome: Prospective Cohort Study)

  • Purpose: The purpose of this study was to identify the impact of cigarette smoking and alcohol consumption on the incidence of colon cancer in adults with metabolic syndrome. Methods: This study employed a longitudinal study design and utilized secondary data drawn from the Korean Genome and Epidemiology Study (KoGES). The data of a sample of 2,327 adults with metabolic syndrome tracked every two years from 2001 to 2014 were used in this study. Statistical data analyses of the frequency, number of cases per 100,000 person-years, log-rank test, Kaplan-Meier curve, and Cox's proportional hazards regression were performed using IBM SPSS statistics version 24. Results: During the observation period, the number of colon cancer cases was 46, and the total person-years were 252,444. The incidence of colon cancer was higher in current, over 10 pack-year smokers when compared to non-smokers (hazard ratio=3.38, 95% confidence interval=1.09~8.42). Conclusion: Excessive and long-term smoking should be avoided to prevent colon cancer, especially in adults with metabolic syndrome, since it might exacerbate the risk factors of colon cancer. Particularly, health professionals need to provide individualized smoking cessation interventions to those at high risk of colon cancer.

Replication of Interactions between Genome-Wide Genetic Variants and Body Mass Index in Fasting Glucose and Insulin Levels

  • The genetic regulation of glucose and insulin levels might be modified by adiposity. With regard to the genetic factors that are altered by adiposity, a large meta-analysis on the interactions between genetic variants and body mass index with regard to fasting glucose and insulin levels was reported by the Meta-Analyses of Glucose- and Insulin-related trait Consortium (MAGIC), based on European ancestry. Because no replication study has been performed in other ethnic groups, we first examined the link between reported single-nucleotide polymorphisms (SNPs) and fasting glucose and insulin levels in a large Korean cohort (Korean Genome and Epidemiology Study cohort [KoGES], n = 5,814). The MAGIC study reported 7 novel SNPs for fasting glucose levels and 6 novel SNPs for fasting insulin levels. In this study, we attempted to replicate the association of 5 SNPs with fasting glucose levels and 5 SNPs with fasting insulin levels. One SNP (rs2293941) in PDX1 was identified as a significant obesity-modifiable factor in Koreans. Our results indicate that the novel loci that were identified by MAGIC are poorly replicated in other ethnic groups, although we do not know why.

Association between ADIPOQ Gene Polymorphism rs182052 and Obesity in Korean Women

  • The association between adiponectin concentration and obesity have been reported and genetic variations of the ADIPOQ gene are known to influence the plasmatic concentration of adiponectin. Therefore, we investigated the effect of AIPOQ single nucleotide polymorphism (SNP) on obesity-related variables, and their modulation by dietary intakes in Korean women. The subjects consisted of 3,217 Korean women aged 40-59 years participating in the Korean Genome Epidemiology Study (KoGES). The general characteristics, anthropometric variables, serum blood profiles were measured. Dietary intake was analyzed using the Food Frequency Questionnaire. Subjects with the T allele of AIPOQ rs182052 showed significantly higher obesity-related variables such as weight (p=0.005), BMI (p<0.000), fat body mass (p=0.005), and waist-hip ratio (p=0.007) than those with the C allele. Moreover, the rs182052 T allele was associated with an increased risk of obesity prevalence (p=0.019). However, there were not any significant interactions observed between the genotype of ADIPOQ rs182052 and dietary intake on BMI and fat body mass. These findings suggest that the obesity-related variables may be more dominantly affected by the genotype of ADIPOQ rs182052 than dietary intake in middle aged Korean women.

결합 다단계 일반화 선형모형을 이용한 다변량 경시적 자료 분석 (The Use of Joint Hierarchical Generalized Linear Models: Application to Multivariate Longitudinal Data)

  • 경시적 자료는 각 환자마다 시간에 따라 반복 측정되는 코호트 연구 등에서 많이 쓰인다. 본 연구는 반응변수 간 상관성을 고려할 수 있는 결합 다단계 일반화 선형모형을 이용하여, 다변량 경시적 자료 분석을 수행하였다. 한국 유전체 역학 연구에서 실시한 코호트 자료를 적합하고 결과를 해석한다. 조건부 아카이케 정보 기준을 이용하여 모형 선택을 하고, 변량효과들의 추정치들을 설명한다.
