Several reports have described the increased perioperative risks as well as surgical techniques for performing various procedures in obese patients. However, few reports have addressed the effects of obesity on routine arthroscopic procedures known to be common and safe. Describing the case of a partial meniscectomy performed in a 17-year-old male with a body weight of 120 kg, we review literatures published to the present and provide technical problems and possible complications that can occur when performing routine knee arthroscopy in bariatric patients.
Objective: The purpose of this study was to assess the validity of the global rating of knee function as a measure of participation restrictions experienced during activities of daily living and sports by patients with a history of anterior cruciate ligament reconstruction (ACLR). Design: Cross-sectional study. Methods: Forty-three subjects (33 males, 10 females, age=20.3 ± 1.3 years), at a mean of 31.2 ± 14.4 months following ACLR, participated in this study. During testing, subjects were first asked to provide a global rating of function by assessing their level of knee function on a 0 to 100 scale, with 0 points representing complete loss of function due to their knee injury and 100 points representing their level of function prior to their knee injury. After providing a global rating of function, subjects completed the Knee Outcome Survey (KOS) Activities of Daily Living Scale (ADLS) and Sports Activity Scale (SAS), which served as the measure of participation restrictions in this study. Results: Pearson product correlations revealed moderate relationships between the global rating of function and the ADLS (r=0.66, p<0.0001) and SAS (r=0.69, p<0.0001). Conclusions: The global rating of knee function provides a valid measure of participation restrictions experienced during activities of daily living and sports by patients with a history of ACLR.
Purpose : The purpose of this prospective study was to evaluate second look arthroscopic finding after osteochondral autograft transfer(OAT) for the treatment of the chondral defect of the knee. Materials & Methods : Fourteen out of forty cases underwent second look arthroscopy after the OAT for the treatment of the chondral defect of the knee. The average age of patients was 34 years. Preoperatively, magnetic resonance imaging was performed in eleven cases and chondral defect was confirmed in nine cases. The site of the chondral defect located at medial femoral condyle at seven, trochlea in four, and lateral fomoral condyle in three cases. The average size of the condral defect was $13\times10mm$, maximum $22\times20mm$. The number of graft was one in eight, two in three, three in two, and five in one case. The average follow-up period was twenty-one months. Second look arthroscopy was performed at six months in almost cases. Results : After the OAT, $86\%$ of cases showed that the consistency of grafted cartilage was firm and $93\%$ of cases showed that grafted cartilage was well incorporated with surrounding cartilage. Conclusion : The results of this study suggest that OAT can be one of treatment modality in the chondral defect of the knee.

Giant cell tumor is most frequently found in juxtaarticular region, and difficult to treat because of local recurrence. Although primary resections reduce recurrence, the joint function will be markedly impaired. Techniques involving physical adjuncts(high speed burr and electric cauterization), acrylic cement or en bloc resection with VFG(vascularized fibular graft) have been employed to reduce local recurrence. From October 1984 to April 1994, twenty-nine patients diagnosed as giant cell tumor were treated at department of Orthopaedic Surgery, School of Medicine, Kyung Hee University. There were eleven men and 18 women, ranging in age from 17 to 52 years(mean: 34 years). The average follow-up period was four years and five months. The location of the lesion was around the knee in 15, distal radius in three, femoral head in three, and others in eight patients. Fifteen patients around the knee joint were treated with several modalities; curettage with bone graft in five, curettage with cement filling in three, curettage with bone graft and physical adjuncts in five, en bloc resection with VFG in one and en bloc resection with arthroplasty in one patient. The functional results, according to the Marshall's knee score, were excellent in one, good in two, and fair in two after the curettage with bone graft, good in three after the curettage with bone cement filling, excellent in one, good in four after the curettage with bone graft and physical adjuncts, and good in two after the en bloc resection with VFG or arthroplasty. Three patients had local recurrence among 15 patients with giant cell tumor around knee. Vascularized fibular graft around wrist joint provided good functional restoration without local recurrence in all three patients who had giant cell tumor in distal radius. Although there is no statistical significance, it seems that curettage with bone graft using physical adjuncts or acrylic cement reveals better results than simple curettage with bone graft. Excellent functional result were obtained without local recurrence by using vascularized fibular graft after en bloc resection.
Purpose: The purpose of this study was to analyze the clinical result and the gait patterns for patients who had undergone the arthroscopic posterior cruciate ligament (PCL) reconstruction using Achilles allograft. Materials and Methods: Among the patient who had undergone the arthroscopic PCL reconstruction using fresh-frozen Achilles allograft between March 2004 to March 2005, we selected 12 patients who compliance to our rehabilitation program. Clinical result and gait analysis were carried out at 36 months postoperatively. There were measured by using range of motion (ROM), Lysholm knee score, Tegner activity score, IKDC score, posterior stress test, and posterior translation measured by using Telos stress arthrometer. Kinematic gait analysis was carried out using 3-dimensional gait analysis system. Results: The PCL reconstruction surgery yielded statistacally significant results in all of clinical evaluation. In gait analysis, the average knee flexion angle increased during stance phase and decreased during the swing phase, but was not statistically significant and there were no significant difference between both sides of their knees overall. Conclusion: Since there was no statistically significant difference between the injured and uninjured sides as a result of the gait evaluation of the patients who had arthroscopic reconstruction surgery on their PCL injured knee and conformed to the rehabilitation program, systemic and strict rehabilitation ought to be important in the reconstruction surgery.

Introduction : This study compared the clinical results with biceps rerouting fer the isolated posterolateral instability (PLI) and for the PLI combined with PCL injuries. Methods : 21 cases of isolated PLI (group I) and 25 cases of PLI combined with PCL rupture were included in the study. The PLI was reconstructed by modified biceps femoris rerouting technique with PCL reconstructions performed prior to the PLI correction in cases of combined injury The clinical results were reviewed and analyzed. Results : Pre-operatively positive reverse pivot shift test turned negative in 43 cases post-operatively. Increased preoperative external rotation thigh foot angle (ERTFA) showed significant differences between the two groups and all fell within normal limits post-operatively At a mean follow-up of 40.3 months, the average Lysholm knee score and. The Hospital for Special Surgery Knee Ligament Score for group I and group II revealed above 90 points without statistically significant difference between the groups. 3 cases of tenodesis failure developed and re-operation was performed. Discussion and Conclusion : The advantages of modified Clancy technique include reduced surgical damages to the iliotibial band and fixation of the biceps tendon at the isometric position. The modified biceps rerouting technique is recommended for the reconstruction of both isolated and combined PLI except in patients with severe damages at the attachment of biceps tendon.
Purpose : To document the change in the temperature of the knee joint at the initiation and conclusion of an arthroscopic procedure and correlate this temperature change with other intra-operative variables. Material and Methods : Temperature measurements were performed in 40 consecutive patients(42 cases) that underwent arthroscopic surgery. Temperature measurements were taken at the initiation of the procedure, before and after inflation of the tourniquet. The last measurement was recorded at the end of the surgical procedure. Results : The mean knee joint temperature evaluated before inflation of the torniquet was $35.1{\pm}1.0$, at the end of surgery, $24.6{\pm}1.5^{\circ}C$. The mean temperature change observed from the beginning to the end of the procedure was $10.5^{\circ}C$. The student t test showed a statistically significant difference of the initial joint temperature(p<0.01) between the patient with no and+1 effusion / between the patient with no and +2 effusion. The temperature at the end of the procedure was found to be statistically low correlated (p<0.01) with the lower temperature of the irrigant and the lengthening of the arthroscopic procedure. Conclusion : Consideration should be given to maintaining the saline irrigant to more physiologic temperature to protect the articular cartilage from any possible temperature induced damages.
Deep vein thrombosis and pulmonary embolism are serious fatal complication after orthopedic surgery. Most of them occur after large operation such as total knee/hip arthroplasty. Some reports about deep vein thrombosis after simple arthroscopic procedure were on the Western literatures, but there was no report on the Korean literature. So we report two cases of pulmonary embolism after knee arthroscopic procedures.

Purpose: The purpose of this research was to identify factors that influence health-related quality of life of patients who have had knee or hip arthroplasty. Factors included sleep disorders, pain, activities of daily living, and depression. Methods: The participants in this research were 97 patients within one year after receiving knee or hip arthroplasty surgery and who regularly visited a hospital outpatient department. Data were analyzed using descriptive statistics, t-test, ANOVA, and stepwise regression analysis with the SPSS program. Results: Sleep disorders, pain, activities of daily living, and depression had significant correlations with health related quality of life. Factors influencing health-related quality of life among arthroplasty patients were the length of time since surgery, sleep disorders, activities of daily living, and depression. These factors explained 58% of the participants' health-related quality of life. Conclusion: The results show that various factors can affect patients' health-related quality of life following joint replacement surgery. The findings indicate that sleep disorders and depression should receive continued attention after the arthroplasty. It is also necessary to develop appropriate nursing intervention programs to lower depression and increase patient participation in daily living activities.
