Ligament Injuries of Knee in the Recreational Skiers (스키에 의한 슬관절 인대 손상)

  • Purpose: This study is to analyse the injury patterns of knee ligament and the factors influencing ligament injuries of knee, and to evaluate the changes of knee function and activity after ski injury. Materials and Methods: Thirty cases of ligament injuries of knee were studied with a questionaire, stress radiographs, magnetic resolution imaging, and physical examination. Mean age was 28.6 years old and mean follow up was 29.6 months. Clinical evaluation of Lysholm knee scoring scale was used for knee function and Tegner activity score was used for activity change after ski injury. Results: The anterior cruciate ligament (ACL) injury accompanied with medial collateral ligament(MCL) injury was most in 11 cases and isolated MCL injury was the next in 9 cases. The common types of injury mechanism were Phantom foot phenomenon (13 cases, 43$\%$) and valgus external rotation injury (12 cases, 40$\%$), which constitute 83$\%$ of all case. At the last follow up, the mean Lysholm score was 93.4 and mean Tegner activity score was 4.2. The reduced Tegner activity score after injury was 1.9. Among several injury groups, the evaluation of knee function and activity was best in the isolated MCL injury group and worst in the ACL injury accompanied with MCL injury group. The factors to influence ski injury were participation to ski class, release of binding, and skiing long time more than 2 hours. Conclusions: Lysholm score at last follow up revealed good grading, but sports activity after ski injury was reduced when compared with pre-injury state. It seems to need a active, systemic sports rehabilitation program after sports injury.

The Effects of Landing Height and Distance on Knee Injury Mechanism (착지의 높이와 거리가 무릎 부상 메카니즘에 미치는 영향)

  • Various jumping and landing motions are shown during sports event. But most previous studies have not considered landing height and distance simultaneously. The purpose of this study was to identify the effects of landing height and distance on knee injury mechanism. Fourteen male(age: $28.86{\pm}1.99$ yrs, height: $177.00{\pm}4.69$ cm, weight: $76.50{\pm}6.41$ kg) participated in this study. The subjects attempted drop landing task onto the ground from 30 cm to 45 cm heights and to 20 cm to 40 cm distances. The results were as follows. First, higher drop landing height and longer distance showed greater degree of maximal knee flexion and valgus. Second, higher drop landing height and longer distance showed greater maximal knee extension moment and varus moment. Third, higher drop landing height and longer distance showed larger maximal knee absorption power. Lastly, higher drop landing height showed increased Peak GRF. Landing height was more related to the cause of injury, which was indicated by increased maximal knee extension moment, peak GRF and maximal knee absorption power. Landing distance was also associated with increased knee valgus moment and absorption power during landing. These results suggest that landing height and distance may be the cause of injury.

Management of Multiple Ligament Injured Knee (슬관절 다발성 인대 손상의 치료)

  • Multiple ligament knee injury is defined as rupture to at least two of the four major knee ligament structures. Three or four knee ligament injury results in knee dislocation as complete disruption of the integrity of the tibiofemoral articulation. In multiple ligament knee injury, vascular and neurologic assessment should be performed meticulously and systematically. Emergency surgery should be needed if arterial injury is suspected. Surgical treatment rather than conservative management should be done and early surgery might be better than delayed surgery. Reconstruction of ACL and PCL, repair or reconstruction of MCL, and reconstruction of posterolateral corner are recommended, although many debates have occurred. Multiple ligament knee injury requires more aggressive management than single ligament knee injury.

The Evaluation of Usefulness New Assistant Device to Observe Posterior Cruciate Ligament Rupture and Patellofemoral Joint Injury in Emergency Patient (응급환자에서 후방십자인대 손상 및 슬대퇴 관절을 관찰하기 위한 보조기구 제작 및 유용성 평가)

  • This study evaluates usefulness of the developed assistant device by taking projection of patellofemoral joint in emergency patients who were doubt posterior cruciate ligament injury in knee joint. The subjects of experiment were patients who visited Eul-Ji University Hospital due to knee injury from January 2006 to December 2006. Seventeen patients, who took the Knee post stress view, Knee merchant view, Knee Seo's view to use assistant device and Knee MRI. To make assistant device of $170{\times}50{\times}70\;cm$, we evaluated its usefulness by measuring posterior dislocation of tibia. Seo's view is more accurate to make judgment of posterior cruciate ligament injury than original knee post stress view. Interval difference of posterior dislocation of original knee post stress view is $6.17{\pm}3.04$ and Seo's view is $8.74{\pm}4.47$. The results show injury of patellofemoral joint, vertical fracture of patella and posterior cruciate ligament injury by taking a projection using Seo's view. Therefore, it is useful to take projection earlier than talometer and MRI in emergency patients who were doubt posterior cruciate ligament injury in knee joint.

  • The US-NCAP was rated by the head and chest injury, but the new US-NCAP requires various dummy injury parts such as head, neck, chest, and femur. So, new restraint systems are needed. Particularly, the knee bolster must meet both unbelted and belted test condition requirements. This paper analyzed the dummy response of both test condition and suggested a knee bolster F-D requirement as well as a new knee bolster structure.

  • Objectives: The purpose of this study is to investigate the clinical application of Gamisopunghwalhyeal-tang to three patients traumatic knee joint injury. Methods: Patients are hospitalized at Dept. of Oriental Rehabilitation, Bu-Chun Jaseng Oriental Medicine Hospital, diagnosed as traumatic knee joint injury and treated mainly with herbal medicine ; Gamisopunghwalhyeal-tang. This study was measured by VAS(Visual Analogue Scale) score and walking time and Western Ontario and McMaster Universities(WOMAC) index score. Results: After taking Gamisopunghwalhyeal-tang, the patient's pain was controlled and increased time of walking on floor after treatment. VAS & WOMAC score were decreased. Conclusions: As seen in this three cases of traumatic knee joint injury, Gamisopunghwalhyeal-tang has a positive effect to control pain with traumatic knee joint injury.

The Clinical Report on 2 cases of Meniscal Injury patients with Knee Pain improved by Motion Style Treatment (슬관절 M.S.T.(Motion Style Treatment)를 이용한 반월상 연골판 손상으로 진단받은 슬통 환자 치험 2례)

  • Objectives : The purpose of this study is to report two cases of improvement in meniscal injury induced knee pain after M.S.T(Motion Style Treatment). Methods : Two patients diagnosed as meniscal injury were improved by M.S.T(Motion Style Treatment). This study was measured by Numerical Rating Scale(NRS) score and physical examination. Results : In this study, in the end of continuous M.S.T(Motion Style Treatment), patient's knee pain had improved. Numerical Rating Scale(NRS) were also decreased. Conclusions : As seen in this two cases of patients of knee pain who were diagnosed as meniscal injury, M.S.T(Motion Style Treatment) has a positive effect to control pain with knee pain who are diagnosed as meniscal injury.

The Effect of Burning Acupuncture Therapy on the Traumatic Injury of Medial Collateral Ligament : Report of Five Cases (가열식 화침을 이용한 외상성 내측측부인대 손상 치험 5례)

  • Objective : The purpose of these cases is to observe the effect of burning acupuncture therapy on the traumatic injury of medical collateral ligament. Methods : The patients were treated by burning acupuncture therapy to recover injury of medical collateral ligament. The progress evaluation of knee joint pain was measured by the knee society knee score(KSKS), function score (KSFS) and visual analog scale(VAS). Results : KSKS & KSFS were increased and VAS decreased in all cases. Conclusions : We had concluded that the burning acupuncture therapy can be effective to knee joint pain caused by traumatic injury of medial collateral ligament.

  • Although uncommonly encountered, knee dislocation is frequently associated with popliteal artery injury. It has been reported that all detected arterial injuries warrant intervention and open surgical management is the standard of treatment. We report here on a case of endovascular intervention for popliteal artery injury in knee dislocation, and then this was treated with ligament surgery with arthroscopic and open procedure.

The Study on critical Value of Kinematical Evaluation Variables of Lower Extremity Pronation in Biomechanical Evaluation of Running Shoes (운동화의 생체역학적 평가시 하지 회내운동의 운동학적 평가변인에 대한 상해 기준치 연구)

  • The purpose of this study was to find the relationship between Achilles tendon angle, angular velocity from 2D cinematography utilized to easily analyze the functions of shoes, ankle joint moment, knee joint moment, and hip joint moment from 3D cinematography utilized to predict the injury. Also, this study was to provide the optimal standard to analyze the injury related to the shoes. Subjects in this study were 30 university male students and 18 conditions (2 types of running speed, 3 of midsole hardness, 3 of midsole height) were measured using cinematography and force platform. The results were as following. 1) Hip joint abduction moment was effected by many variables such as running speed, midsole height, maximum achilles tendon angle, ground reaction force. 2) Knee joint rotational moment in running was approximately 1/10 - 1/4 times of the injury critical value and eversion moment was approximately 1/4 - 1/2 times of the injury critical value. 3) Ankle joint pronation moment in running was 1/3 - 1/2 times of the injury critical value. 4) Knee joint rotational moment was found to be irrelevant with maximum achilles tendon angle or angular velocity. 5) Pronation from running was thought to be relevant to rather eversion moment activity than rotational moment activity of knee joint. 6) Plantar flexion abductor of ankle showed significant relationship with the ground reaction force variable. 7) When the loading rate for ground reaction force in passive region increased, extensor tended to be exposed to the injury. Main variables in biomechanical analysis of shoes were impact absorption and pronation. Among these variables, pronation factor was reported to be relevant with knee injury from long duration exercise. Achilles tendon angle factor was utilized frequently to evaluate this. However, as the results of this study showed, the relationship between these variables and injury relating variable of knee moment was so important. Studies without consideration on this finding should be reconsidered and reconfirmed.
