Development of Knee Pain Diagnosis Questionnaire and Clinical Study of Diagnostic Correspondent Rate (슬통 진단용 설문지개발 및 진단 일치도 평가연구)

  • Objectives : This study is perfomed for preparation of oriental medicine clinical guidelines for drawing up the standards of oriental medicine demonstration and diagnosis classification about the knee pain. Methods : Statistical analysis about Crane's-knee wind(鶴膝風), arthralgia syndrome(痺症), knee injury(膝傷), gout arthritis(痛風), Youk jeol poung(歷節風) classified experts' opinions about knee pain patients by Delphi method is conducted by using oriental medicine diagnosis questionnaire. The result was classified by using linear discriminant analysis(LDA), diagonal linear discriminant analysis(DLDA), diagonal quadratic discriminant analysis(DQDA), K-nearest neighbor classification(KNN), classification and regression trees(CART), support vector machines(SVM). Results : The results are summarized as follows. 1. The result analyzed by using LDA has a hit rate of 81.65% in comparison with the original diagnosis. 2. The result analyzed by using DLDA has a hit rate of 63.3% in comparison with the original diagnosis. 3. The result analyzed by using DQDA has a hit rate of 65.14% in comparison with the original diagnosis. 4. The result analyzed by using KNN has a hit rate of 74.31% in comparison with the original diagnosis. 5. The result analyzed by using CART has a hit rate of 75.23% in comparison with the original diagnosis when the test of selected 13 significant questions based on analysis of variance was performed. 6. The result analyzed by using SVM has a hit rate of 87.16% in comparison with the original diagnosis. Conclusions : Statistical analysis using oriental medicine diagnosis questionnaire on knee pain generally turned out to have a significant result.

The Short-Term Effects of Difference Frequency of Transcutaneous Electrical Nerve Stimulation on Pain Relief using c-fos Expression in Spinal Cord with Knee Osteoarthritis Rats

  • PURPOSE: The purpose of this study was to determine the effect of different frequencies (4Hz and 100Hz) of transcutaneous electrical nerve simulation (TENS) on pain relief using c-fos expression in the spinal cord of rat osteoarthritis to investigate the appropriate frequency for pain relief. METHODS: Total of 30 Sprague-Dawley rats was used and randomly divided 2 groups according TENS frequency and applicate the TENS during 3 period (3 days, 7 days, 10 days). The induction of osteoarthritis by 3mg monosodium iodoacetat was injected into the right knee joint of rats. Three days later, commercially available TENS unit was used for stimulation was set to 20minutes on 3, 7, 10 days after surgery. Western blot analysis system was used to detect immunoreactive proteins. The thickness of the bands were photographically measured by Scion Image. RESULTS: When investigating the c-fos expression of TENS on spinal cord in OA knee over 10 days, between-groups differences in c-fos expression reached a significant level by day 10. For within-groups comparisons, the c-fos expression decreased significantly across days in low- and high-frequency TENS groups. CONCLUSION: Whether at low- and high-frequency, the TENS as a therapy obtained beneficial effects of pain relief and TNES at high-frequency is more beneficial effects on the pain relief when TENS applied at injury site.

Relationship between the Severity of Radiographic Features and Degree of Pain and Dysfunction in Patients with Knee Osteoarthritis (슬관절 골관절염 환자의 방사선학적 소견의 심각성과 통증 및 기능장애수준 간에 상관성)

  • PURPOSE: This study aimed to assess the relationship between the severity of radiographic features and pain and function in patients with knee osteoarthritis (KOA). METHODS: Seventy-eight subjects (14 men, 64 women) with KOA, between the ages of 41 and 83 years (mean age, 61.29 years), were included. All the subjects diagnosed with KOA were scored for severity of radiographic KOA according to the Kellgren-Lawrence (K/L) grade, visual analogue scale (VAS), knee joint range of motion (ROM), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), maximum muscle power (MMP), and sit-to-stand (STS) and one-leg standing (OLS) tests. Associations among the K/L grade, diagnosis, pain, and function were examined by correlation analysis. RESULTS: There were no significant differences between the K/L grade, and the VAS, STS test time, and WOMAC scores (p>.05). There were no significant differences between the K/L grade, bilateral ROM, MMP, and left OLS test time (p>.05). However, there was a significant difference between the K/L grade and right OLS test time (p<.05). The K/L grade was negatively correlated with the left OLS test time(r=-.24, p<.05) and with the right OLS test time (r=-.307, p<.01). CONCLUSION: These results suggest that radiographic KOA was not associated with pain, knee MMP, ROM, and STS test time, but had a weak negative correlation with OLS test time.

Effects of Taping Therapy on Knee Pain and Depression in Older Adult Women (테이핑요법이 여성노인의 무릎통증과 우울감에 미치는 영향)

  • This study examined the effects of taping therapy on knee pain and depression in older adult women. A nonequivalent control group pre-post design was used. A total of 63 older adult women in the senior welfare center were recruited and assigned to the experimental group (31) or control group (32). Taping therapy was provided to the experimental group three times a week for two weeks. Data were collected before and after the intervention program. The data were analyzed using frequency, percentage, Chi-square test, and t-test with SPSS Statistics Win 21.0 program. Knee pain (t= -9.91, p<0.001) and depression (t= -4.59, p<0.001) were decreased significantly in the experimental group. The results show that taping therapy is an effective nursing intervention for decreasing knee pain and depression in older adult women.

Effect of Kinesio Taping on Isokinetic Muscular Function and Pain of Knee Joint in Elderly Women (키네시오 테이핑이 고령자의 슬관절 등속성 근 기능 및 통증에 미치는 영향)

  • The aim of this study were to evaluate the effect of kinesio taping on isokinetic muscular function and pain of knee joint. The subjects for this study were twelve elderly women($65{\sim}70$years) suffered by knee joint pain over more than three years. The taping methods performed on 3 times a week for 8 weeks for improvement of isokinetic muscular function. Also, the change of visual analogue scale(VSA) and varval rating scale(VRA) has been measured before and post taping 8 weeks. Paired t-test was performed for data analysis at the 0.05 level of significance. After 8 weeks, the results of the study are as follow; First, peak torque, peak torque % BW, total work and ipsilateral balance ratio were significantly improved. And, work fatigue, visual analogue scale and verval rating scale were decreased significantly after 8 weeks taping. The results suggest that applying kinesio taping on knee joint may improve isokinetic muscular function of elderly women. Therefore kinesio taping method was effective for pain relief and prevention of knee joint pain in the elderly women.

A Comparative study of Warm needling and Bee Venom Pharmacopuncture on Osteoarthritis of the Knee - a Randomized Controlled Trial - (퇴행성슬관절염에 대한 봉약침과 온침효과 비교)

  • Objective This study was performed to investigate whether Bee Venom Pharmacopuncture(BVP) could be a more effective modality than Warm Needling(WN) in relieving pain and symptoms of knee osteoarthritis(OA). Design Prospective, randomized and controlled clinical trial. Setting Single center trial in Korea Patients 49 volunteers with knee OA participated in the study. All the participants were screened through an inclusion and exclusion criteria. 33 participants were completed the clinical trial. Intervention The subjects were randomly assigned to one of two groups. One group received BVP(n=18), while the other group received WN(n=15). Sixteen sessions of BVP or WN were given at the pain region of the problematic knee for 8 weeks. Primary outcome measure is the Korean translation of Western Ontario and McMaster Universities Osteoarthritis Index scores(Korean WOMAC, KWOMAC). Secondary outcome measure is the physical health scores based on the 36-Item Short-Form Health Survey(SF-36) and Patient Global Assessment(PGA). KWOMAC and SF-36 were measured third (baseline, 4 and 8 weeks). PGA was measured twice(4 and 8 weeks). Results BVP group showed significant decrease compared to WN group in pain, function and total scores of KWOMAC according to the Mann-Whitney U-test. In the PGA, BVP group, compared to WN group, showed a significant increase. Conclusions BVP was more effective in relieving pain of knee OA than WN. These findings suggest that BVP is a promising alternative for treating knee OA.

Comparison of the Effects of Non-elastic Taping on Patellar tendon Pain, Knee Muscle Strength and Gait in Patients with Patellofemoral Joint Pain Syndrome (비탄력 테이핑이 슬개대퇴관절 통증증후군 환자의 슬개건 통증과 슬관절부 근력, 보행에 미치는 영향 비교)

  • Background: Ground repulsion or impact on the ground during daily activities, sports, or occupational activities may cause injury to the knee when walking. Non-elastic taping is effective in treating these problems in previous studies. Non-elastic taping strengthens the structure of the soft tissues of the injured knee joint to maintain constant tension, improves muscle rearrangement and function, and improves proprioception. Based on previous studies, we intended to see the therapeutic changes of non-elastic taping in patients with patellofemoral joint pain syndrome. Methods: The non-elastic taping application method was applied to the patient three times for five hours for one week. Non-elastic taping was applied to the patellar tendon with little space above the skin segment of the patellar femur, with both sides fixed by taping. Muscle strength and gait change were evaluated with non-elastc taping. Results: The knee flexion, extension strength and gait evaluation of the knee joint with inelastic taping showed significant differences after treatment. There was a significant difference in the comparison between the two groups after the treatment method was applied (p<.05). Conclusion: As a result, this study confirms that the non-elastic taping method applied for the treatment of patellar femoral joint pain syndrome is effective in the treatment.

Telephone Survey for Developing Clinical Trial Protocol on Individualized Acupuncture Treatment for Knee Osteoarthritis (퇴행성 슬관절염의 맞춤형 침구 임상시험 프로토콜 개발을 위한 전화조사)

  • Objectives : This study was accomplished in order to find out how Korean medical doctors take acupuncture prescription for knee osteoarthritis in clinical practice Methods : The survey questions were developed by the consensus from 4 professors and 10 residents who major in acupuncture & moxibustion for developing clinical trial protocol on individualized acupuncture treatment for knee osteoarthritis. 206 persons having more than 10 years experience were randomly selected from a list of Korean medical doctors. Telephone interviews with them were conducted by the well-trained interviewers between May 21 and May 27 in 2005. Results : 139 of 206 Korean medical doctors made an acupuncture prescription prior to treatment of knee osteoarthritis. 103 of 139 Korean medical doctors used both local and remote points, and 26 of them used remote points only, and 10 of them used local points only. In case of doctors who use remote points, the five element (45 of 129) or Saam (28 of 129) acupoints were used dominantly, and its was based on the pattern identification of the Jangbu organs related with the pain (56 of 129) or of the pain location (35 of 129). Conclusion : This study showed that Koran medical doctors prefer to use remote points such as five element or Saam acupoints based on the pattern identification of the Jangbu organs related with the pain or the pain location. These result may be useful for developing protocols for clinical trial on acupuncture on knee osteoarthritis.

Musculoskeletal Ultrasound Findings of Knee Pain Patients: A Retrospective Observational Study (근초음파 진단기기를 활용한 슬관절 통증 환자의 초음파 소견: 후향적 관찰연구)

  • The study aimed to understand the knee condition of patients with knee joint pain who visited an Korean Medicine hospital by performing musculoskeletal ultrasound. From July 1, 2021 to October 31, 2021, information on the gender, age, diagnosis, and ultrasound findings of knee joint pain patients were retrospectively collected. To examine the condition of the knee joint, images were taken in a total of three postures. All images were analyzed by an examiner. 7 cases of osteoarthritis, 1 case of medial collateral ligament rupture, and 1 case of quadriceps tendon calcification were collected. It was able to check the condition of articular cartilage, lipoma arborescens, and the formation of osteophytes through musculoskeletal ultrasound. The implementation of musculoskeletal ultrasound can enable Korean medicine doctors to give more active treatment for patients through accurate and scientific diagnosis. Based on more evidence, it is thought that the use of medical devices by Korean medicine doctors will need to be made in the future.
