Review of Ethical and Scientific Aspects of Acupuncture Clinical Trials Pointed Out by Institutional Review Board a Part of Development Process of the Acupuncture and Moxibustion Clinical Research Guideline (침구임상시험 가이드라인 개발과정에서 IRB의 윤리적·과학적 지적사항 사례 조사 연구)

  • Objectives : To improve quality of clinical research for acupuncture and moxibustion, a guideline for clinical research protocol is needed. While developing a guideline for acupuncture and moxibustion clinical research, we reviewed the ethical and scientific aspects of protocols pointed out by the institutional review board. This will offer practical assistance to the researchers. Methods : Ethical and scientific aspects of acupuncture and moxibustion research protocols reviewed by Kyung Hee University Korean Medicine Hospital Institutional Review Board were gathered and reviewed. Results : Ethical and scientific aspects of protocol review was reported. Conclusions : The example of review will be helpful for new researchers when developing acupuncture and moxibustion clinical research protocol.

Effects of Pharmacoacupuncture for Obesity: A Systematic Review and Meta-Analysis (비만에 대한 약침의 효과: 체계적 문헌고찰과 메타분석)

  • Objectives: Obesity is a chronic metabolic disease that negatively affects quality of life and health. It is associated with various disease and its prevalence is on the rise. Although various treatments for obesity are used, they have several adverse effects. Pharmacoacupuncture is a widely used method for treating various diseases and has the advantage of having fewer adverse effects. This systematic review and meta-analysis evaluate the efficacy of pharmacoacupuncture in treating obesity. Methods: Eight electronic databases were searched from their inception to August 2023. Only randomized controlled trials were included in this review. Intervention groups included pharmacoacupuncture. Control groups had no limitation. The primary outcome was body mass index (BMI), and the secondary outcomes were body weight (BW), waist circumference (WC), and adverse effects. Continuous outcome data were presented as mean differences (MD) with 95% confidence intervals (CI). Results: Four randomized controlled trials (RCTs) involving 139 participants were included in this systematic review and meta-analysis. Results revealed that BMI ((MD)=-0.98, 95% CI -1.84 to -0.11, Z=2.22, P=0.03, I2=0%), WC (MD=-4.29, 95% CI -7.51 to -1.08, Z=2.62, P=0.009, I2=38%) were significantly improved in the intervention groups compared to control groups. Adverse effects were reported in two studies. Conclusions: This systematic review and meta-analysis suggests that pharmacoacupuncture is an effective and safe therapy for simple obesity. Further detailed studies should be conducted on effects of pharmacoacupuncture for obesity to increase reliability.

Primary intraosseous squamous cell carcinoma mimicking periapical disease: a case report

  • Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare carcinoma, which arises within the jaws without connection to the oral mucosa and presumably develops from a remnant of odontogenic epithelium. We present a case of solid type PIOSCC in a 52-year-old male patient complaining of dull pain on his left lower molar. In this case, early stage PIOSCC mimicking a periapical lesion might lead to a one-year delay in treatment due to the misdiagnosis of osteomyelitis after extraction of the third molar. The clinical, radiological, and histologic features are described. In this case, there was initial radiographic evidence for PIOSCC mimicking a periapical lesion. Incautious radiographic interpretation and treatment procedures had delayed the correct diagnosis and resulted in extensive bony destruction during the patient's disease progression.

The Clinical Effect on Acupuncture Treatment of Postpartum Hypogalactia: A Systematic Review of Randomized Controlled Trials (산후 유즙분비부전에 대한 침 치료 효과 : 체계적 문헌 고찰)

  • Objectives: The purpose of this systematic review is to overview and evaluate the effect on postpartum hypogalactia after acupuncture treatment. Methods: Relevant randomized controlled trials (RCTs) were identified by database searches in Embase, Pubmed, CENTRAL, CNKI, KISS. Searching keywords were "Hypogalactia" or "Hypolactaion" and "Acupuncture". After searching qualified articles, we independently assessed the risk of bias according to the Cochrane Rob criteria. Results: Among the 81 articles five RCTs were finally selected. Four of the selected were statistically significant. One of them was not statistically significant because it was comparative study of chinese medicine and acupuncture treatment. The quality of the evidence for specific comparisons ranged from low to high. Conclusions: Our systematic review found encouraging. Acupuncture treatment would be effective way to increase insufficient milk supply on postpartum hypogalactia.

A Case of Restless Legs Syndrome Patient with Numbness in Both Soles Using Gyejibokryeong-hwan (하지불안증후군으로 인한 양 발바닥 저림에 대한 한의 치험 1례)

  • Objective: In this case report, we address the case of a 68-year-old woman with numbness in both soles due to restless legs syndrome. Methods: A patient was treated with Korean medicine including herbal medication Gyejibokryeong-hwan during 73 days as an outpatient. We evaluated the improvements of symptoms by use of the International Restless Legs Scale (IRLS) and checking subjective symptoms of patient. Results: After 73 days of Korean medicine treatment, there was improvement in patient's symptoms. IRLS score improved from 12 to 8, and subjective symptoms improved to about 30% compared to the first time. Conclusions: This study suggested that Korean medicine treatments might be effective in RLS patient recovery.

  • Expensive herbs such as ginseng are always a possible target for fraudulent labeling. New mountain ginseng strains have occasionally been found deep within mountain areas and commercially traded at exorbitant prices. However, until now, no scientific basis has existed to distinguish such ginseng from commonly cultivated ginseng species other than by virtue of being found within deep mountain areas. Polymerase chain reaction (PCR) analysis of the internal transcribed spacer has been shown to be an appropriate method for the identification of the most popular species (Panax ginseng) in the Panax ginseng genus. A single nucleotide polymorphism (SNP) has been identified between three newly found mountain ginseng (KGD4, KGD5, and KW1) and already established Panax species. Specific PCR primers were designed from this SNP site within the sequence data and used to detect the mountain ginseng strains via multiplex PCR. The established multiplex-PCR method for the simultaneous detection of newly found mountain ginseng strains, Korean ginseng, and foreign ginseng in a single reaction was determined to be effective. This study is the first report of scientific discrimination of "mountain ginsengs" and describes an effective method of identification for fraud prevention and for uncovering the possible presence of other, cheaper ginseng species on the market.

Insilico profiling of microRNAs in Korean ginseng (Panax ginseng Meyer)

  • MicroRNAs (miRNAs) are a class of recently discovered non-coding small RNA molecules, on average approximately 21 nucleotides in length, which underlie numerous important biological roles in gene regulation in various organisms. The miRNA database (release 18) has 18,226 miRNAs, which have been deposited from different species. Although miRNAs have been identified and validated in many plant species, no studies have been reported on discovering miRNAs in Panax ginseng Meyer, which is a traditionally known medicinal plant in oriental medicine, also known as Korean ginseng. It has triterpene ginseng saponins called ginsenosides, which are responsible for its various pharmacological activities. Predicting conserved miRNAs by homology-based analysis with available expressed sequence tag (EST) sequences can be powerful, if the species lacks whole genome sequence information. In this study by using the EST based computational approach, 69 conserved miRNAs belonging to 44 miRNA families were identified in Korean ginseng. The digital gene expression patterns of predicted conserved miRNAs were analyzed by deep sequencing using small RNA sequences of flower buds, leaves, and lateral roots. We have found that many of the identified miRNAs showed tissue specific expressions. Using the insilico method, 346 potential targets were identified for the predicted 69 conserved miRNAs by searching the ginseng EST database, and the predicted targets were mainly involved in secondary metabolic processes, responses to biotic and abiotic stress, and transcription regulator activities, as well as a variety of other metabolic processes.
