우리나라 재래염소의 질병에 관한 연구-문헌조사 (Studies on The Diseases of the Korean Native Goat-A Review)

  • The Korean native goat has been extensively used in the experimental work. Compared to those experimental work employing the goat, reports on their diseases have been scarcely documented. In the present communication, authors reviewed the literatures pertaining to the diseases of the Korean native goat. Diseases of the respiratory system, the alimentary tract, the liver, the musculoskeletal system, the skin, the urogenital system, the nervous system, the eye, and the heart were reviewed. And then metabolic, toxic, and parasitic diseases were reviewed. Of those diseases reviewed, diseases of the heart and kidney, and metabolic and toxic diseases were wholly artificially-induced. Except the parasitic diseases, a considerable number of diseases of other organ systems were also artificially-induced. And the parasitic diseases reported were confined to the internal parasites. Authors feel that the known occurrence of diseases would help diagnose new disease in the goat.

소아 및 성인 마제신 환자들의 임상적 특징과 비교 (Clinical Manifestations and Characteristics in Patients with Horseshoe Kidney)

  • 목적: 마제신은 신장융합(renal fusion)을 가진 가장 흔한 신기형이다. 저자들은 단일병원에서 관찰된 마제신 환자들의 동반 질환을 포함한 임상적 특징을 조사하고자 하였다. 방법: 2000년 1월부터 2012년 12월까지 부산백병원에서 영상의학적 방법에 의해 마제신으로 진단된 환자 43명을 대상 (소아군 14례, 성인군 29례)으로 후향적으로 의무기록을 분석하였다. 또한 소아군과 성인군의 자료를 비교 분석하였다. 결과: 전체 환자의 진단당시의 평균 연령의 중간값은 34세이었고, 성별은 남자 17명 여자 26명 이었다. 소아군(<18세, 14명)의 평균연령은 $6.7{\pm}6.2$세, 남자가 5명이 있었다. 진단 시 혈뇨를 보인 1명을 제외한 대부분 동반 질환의 평가 도중 우연히 발견되었다. 동반된 질환으로는 터너증후군이 5례(36%)로 가장 많았고, 동반된 비뇨기계 기형으로는 2명(14.2%)에서 신우요관협착을 동반하였다. 신기능은 마지막 추적검사까지 모두 정상이었다. 성인군에서 진단된 29명의 진단 당시 연령 중간값은 48세이었고, 남자가 12명 여자 17명이 있었다. 진단 시 다른 질환의 평가 중 18명이 우연히 발견되었으며, 11명은 혈뇨 또는 결석으로 인한 복통 등이 있었다. 동반질환으로는 터너증후군이 5례로 가장 많았으며, 5명에서 신우요관협착이 동반되었고, 그 외 수신증, 과민성 방광 등을 동반하였다. 마지막 추적검사에서 6명(21%)에서 신기능감소(혈청Cr>1.5)가 관찰되었다. 결론: 마제신은 우연히 발견되는 경우가 대부분으로, 동반된 단일 질환으로는 터너증후군이 가장 많았다. 성인기로 이행 후 다양한 비뇨기과적 합병증의 위험이 높으므로 지속적인 추적 관찰이 필요하다.

How to delay the progression of chronic kidney disease: focusing on medications

  • Patients with chronic kidney disease (CKD) bear a significant financial burden and face numerous complications and higher mortality rates. The progression of CKD is associated with glomerular injury caused by glomerular hyperfiltration and oxidative stress. Factors such as uncontrolled hypertension, elevated urine protein levels, anemia, and underlying glomerular disease, contribute to CKD progression. In addition to conservative treatment, several medications are available to combat the progression of CKD to end-stage kidney disease. Renin-angiotensin-aldosterone system blockers could slow the progression of CKD by reducing glomerular hyperfiltration, lowering blood pressure, and decreasing inflammation. Mineralocorticoid receptor antagonists inhibit the mineralocorticoid receptor signaling pathway, thereby attenuating inflammation and fibrosis. Sodium-glucose cotransporter 2 inhibitors exhibit protective effects on the kidneys and against cardiovascular events. Tolvaptan, a selective vasopressin V2-receptor antagonist, decelerates the rate of increase in total kidney volume and deterioration of kidney function in patients with rapidly progressive autosomal dominant polycystic kidney disease. The protective effects of AST-120 remain controversial. Due to a lack of evidence regarding the efficacy and safety of these medications in children, it is imperative to weigh the benefits and adverse effects carefully. Further research is essential to establish the efficacy and safety profiles in pediatric populations.

소양인(少陽人) 병리론(病理論)에 관한 고찰(考察) (The Study on the Pathology of Soyangin in Sasang Constitutional Medicine (SCM))

  • 1. Objectives: This research was proposed to find out the pathology of Soyangin in Sasang Constitutional Medicine(SCM). 2. Methods: The related contents of the pathology of Soyangin were selected in Je-Ma Lee's literatures such as "Dongmu-YuGo(東武遺稿)"(DYG), "Donguisusebowon-SaSangchobongyun(東醫壽世保元四象草本卷)"(DSS), "Donguisusebowon-GabObon(東醫壽世保元甲午本)"(DGO), "Donguisusebowon-ShinChukbon(東醫壽世保元辛丑本)"(DSC), and the research was written in order to find out the physiology and pathology of Soyangin in SCM. 3. Results and Conclusions: The chronical change of pathologic concept in Soyangin diseases as follows : Pathology in Soyangin diseases was much Hot Qi(熱氣), and more ascending Qi, less descending Qi in DYG, DSS. In "Discourse on the viscera and bowels" of DGO and DSC, Soyangin has a circulation of Water-Food Hot Qi of Spleen Group(脾黨) and Water-Food Cold Qi of Kidney Group(腎黨). Exterior Disease(表病) was the injury of Exterior-Qi such as mouth-hip Qi(口膀胱氣) by Anger-Nature-Qi(怒性氣), and Interior Disease(裏病) was the injury of Interior-Qi such as kidney-large intestine Qi(腎大腸氣) by Sorrow-Emotion-Qi(哀情氣). All diseases of Soyangin are caused by insufficient Cool Yin Qi(陰淸之氣) in Kidney Group(腎黨), so the pathology of Soyangin was focused on Requisite energy(保命之主) and each small viscera and bowels(偏小之臟). In this viewpoint, the schema of Soyangin diseases such as Soyangsangpoong-syndrome(少陽傷風證), Kyulhyung-syndrome(結胸證), Mangeum-syndrome(亡陰證), Hyungkyukyeol-syndrome(胸膈熱證), Sogal-syndrome(消渴證) and Eumhuoyeol-syndrome(陰虛午熱證) were designed to explain the mechanism of each syndrome.

고령의 만성콩팥병 환자에서 발생한 초기 신경학적 악화가 동반된 후방순환뇌경색 치험 1례 (A Case Report on Posterior Circulation Cerebral Infarction with Early Neurological Deterioration in an Elderly Patient with Chronic Kidney Disease)

  • Objectives: This study investigated the effect of Korean medicine on an elderly patient with posterior circulation cerebral infarction, chronic kidney disease (CKD), and early neurological deterioration (END). Methods: The patient, who already had CKD, was treated with Korean medicine, comprising herbal medicine, acupuncture, moxa, and cupping combined with Western medicine (antiplatelet, diabetes) and physical therapy. A manual muscle test (MMT) and a modified Barthel index (MBI) were used to observe the treatment effects, and blood tests were performed to check estimated glomerular filtration rate (eGFR), creatinine and blood urea nitrogen (BUN), which represent renal function. Results: After the treatment, MMT, MBI, and renal function scores had increased. Conclusions: This study suggests that Korean medicine can effectively treat posterior circulation cerebral infarction with END in CKD, but further studies should be conducted.

A late onset solitary mediastinal cystic lymphangioma in a 66-year-old woman who underwent kidney transplantation

  • Lymphangioma is a congenital abnormality of the lymphatic system detected primarily in early childhood. There are rare reports of mediastinal lymphangioma in older adults. We hereby report on a 66-year-old female patient who underwent kidney transplantation 20 years previously and who developed pathologically confirmed solitary mediastinal lymphangioma 1 year ago. Chest radiography showed a mediastinal nodule, which was not observed 2 year previously, therefore she was referred to the pulmonary division. She had no symptoms, and chest computed tomography demonstrated a 25-mm, well-defined, low-density nodule located at the anterior mediastinum. The size of the nodule had increased from 25 mm to 34 mm 1 year later, and it was completely resected via video-assisted thoracic surgery. The histological diagnosis was cystic lymphangioma. Therefore, we recommend that clinicians consider cystic lymphangioma as a possible diagnosis even in older patients with a mediastinal cystic mass that shows progressive enlargement.

Pediatric Acute Kidney Injury: Focusing on Diagnosis and Management

  • Acute kidney injury (AKI) is common in critically ill children, and is associated with increased mortality and long-term renal sequelae. The definition of pediatric AKI was standardized based on elevation in serum creatinine levels or decrease in urine output; accordingly, epidemiological studies have ensued. Although new biomarkers appear to detect AKI earlier and predict prognosis more accurately than traditional markers, they are not frequently used in clinical setting. There is no validated pharmacological intervention for AKI, so prevention and early detection are the mainstays of treatment. For high risk or early stage AKI patients, optimization of volume status and blood pressure, avoidance of nephrotoxins, and sufficient nutritional support are necessary, and have been demonstrated to be effective in preventing the occurrence of AKI and improving prognosis. Nevertheless, renal replacement therapy is needed when conservative care fails.

급속진행성 사구체 신염이 동반된 막성신증 1례 (A case of Crescentic Glomerulonephritis Superimposed on Membranous Glomerulonephropathy)

  • Idiopathic membranous nephropathy is one of the most common causes of nephrotic syndromes in adults but rare in childhood. The occurrence of crescentic glomerulonephritis and membranous nephropathy in a patient is rare. This report describes a patient who initially was diagnosed as a membranous glomerulonephropathy at age 12 years and subsequently developed a crescentic, rapidly progressive glomerulonephritis at age 19 years. The patient responded to methylprednisolone pulse therapy and he maintained with partial remission and normal renal function with residual proteinuria.

도축한우의 신장에 대한 병리학적 연구 (Abattoir survey of kidney lesions in Korean cattle)

  • To examine the prevalence and types of kidney lesions in Korean cattle, a survey was carried out at a abattoir in september 2000 and January 2001. The collected kidneys were examined grossly and histopathologically and investigated for the patterns of renal diseases with considering season and sex. Of 735 cattle (99 bulls and 246 cows in September and 140 bulls, 3 steers, and 247 cows in January) surveyed, 301 (41%) cattle had various renal lesions. The most common prominent finding was focal or multiple interstitial nephritis (36.2%). Other lesions included focal or multiple renal cyst (9.1%), infarction (0.1%), hemorrhage (0.3%), and renal atrophy (0.3%). Microscopically, main pathological lesions were acute or chronic interstitial nephritis with moderate infiltration of neutrophil or lymphocyte, respectively. Finally, the kidney lesions are more or less correlated with the meat quality, especially in female Korean cattle.

Obesity and chronic kidney disease: prevalence, mechanism, and management

  • The prevalence of childhood obesity is increasing worldwide at an alarming rate. While obesity is known to increase a variety of cardiovascular and metabolic diseases, it also acts as a risk factor for the development and progression of chronic kidney disease (CKD). During childhood and adolescence, severe obesity is associated with an increased prevalence and incidence of the early stages of kidney disease. Importantly, children born to obese mothers are also at increased risk of developing obesity and CKD later in life. The potential mechanisms underlying the association between obesity and CKD include hemodynamic factors, metabolic effects, and lipid nephrotoxicity. Weight reduction via increased physical activity, caloric restriction, treatment with angiotensin-converting enzyme inhibitors, and judicious bariatric surgery can be used to control obesity and obesity-related kidney disease. Preventive strategies to halt the obesity epidemic in the healthcare community are needed to reduce the widespread deleterious consequences of obesity including CKD development and progression.
