Clinical characteristics and progress of Kawasaki disease patients who had early treatment with intravenous immune globulin (가와사끼병에서 면역글로불린 조기 투여군의 임상적 특성 및 치료 경과)

  • Purpose : To determine the optimal time of high dose intravenous immune globulin (IVIG) treatment, we analysed the clinical characteristics and progress of a group of Kawasaki disease patients who had early treatment with IVIG. Method : A retrospective study was conducted of 188 patients with Kawasaki disease who were admitted to Yeungnam University Medical Center from January 2000 to December 2005. All patients were treated with a high dose IVIG and high dose aspirin for the initial acute phase treatment. The early treatment group consisted of 94 patients who received treatment before 5 days of fever, and the conventional group consisted of 94 patients who were treated on or after day 5. The patients' sex, age, laboratory findings, total duration of fever, duration of fever after initial IVIG, need for additional IVIG and coronary artery status were noted. Result : There were no significant differences between the two groups in sex ratio and age. No significant differences were noted in the level of WBC count, ESR, CRP, serum albumin, LDH, total duration of fever and coronary abnormality. But the value of ALT($151.8{\pm}17.3$ vs. $81.9{\pm}13.4$, P=0.002), duration of fever after initial IVIG ($3.8{\pm}0.5days$ vs. $2.1{\pm}0.2days$, P=0.003), and rate of additional IVIG (15.9% vs. 6.3%, P=0.037) were significantly higher in the early treatment group. There was no significant difference in initial dose of IVIG, but dosage of aspirin was lower in early treatment group (P=0.037). Conclusion : There is no evidence that early treatment of IVIG has greater efficacy in preventing cardiac sequelae than conventional treatment. In addition, early treatment is likely to result in a greater requirement for additional IVIG treatment.

A Change of Adrenal Androgen and Cortisol in Kawasaki Disease (가와사끼병에서 부신 안드로겐과 코티솔의 변화)

  • Purpose : Endocrine and immune systems are connected and interdependent. Adrenal glands play an important role in this network and control the balance between serum levels of dehydroepiandrosterone sulfate(DHEAS) and cortisol. These steroids have an antagonistic effect on the T cell progression into Th1 and Th2 cells and on the induction of correlated interleukins. Therefore we evaluated the role of adrenal androgen and cortisol as immune modulators in Kawasaki disease( KD) with changes of T cell immunity. Methods : From April to August in 2001, we examined serum DHEAS and 24 hour urine free cortisol(F) before administration of immunoglobulin and steroids by radioimmunoassay in 14 KD patients. It's clinical severity was determined by Harada score and coronary lesion. Results : The age of the patient group ranged from 4 months to 4 years; its average age was 2.3 years. Three patients(21.4%) were below 1 year, 2(14.3%) between 1 and 2 years, 5(35.7%) between 2 and 3 years, 4(28.6%) between 3 and 4 years of age. Male to female ratio was 1:1.3. DHEAS was significantly decreased in patients($11.1{\pm}6.0{\mu}g/dL$) more than controls($81.6{\pm}13.3{\mu}g/dL$)(P<0.05). Twenty-four hour urine free cortisol was significantly increased in patients($36.9{\pm}21.9{\mu}g/dL$) more than controls($13.6{\pm}5.5{\mu}g/dL$)(P<0.05). Ratio of DHEAS/F was decreased remarkably in patients($0.33{\pm}0.20$) more than controls($6.65{\pm}2.56$)(P=0.016). There was no difference between ratio of DHEAS/F and Harada score, but its ratio was very low in patients with coronary aneurysm. Conclusion : These data demonstrate that there are changes of DHEAS and cortisol in acute stage of KD and the dis-equilibrium between two steroids may be relevant in the T cell immune response induction of Kawasaki disease. These changes support the use of DHEAS/F ratio as one of the predictive factors of coronary arteries complication.

Applications and a View of Gas Separation by Membranes in Japan (일본에서의 기체분리막의 현황 및 응용)

  • The development of separation technology is an important research subject as is clear from its role in the Japanese government's research and development program for basic technology for the next generation(1981~1990). Japan is poor not only in mineral resources but also in energy resources and if a sudden change occurs in oil producing facility or an accident occurs in a nuclear power plant, then energy policy must undergo changes and economic foundations may collapse. Japan has already experienced this. Although, oil prices are stable at present and Japan can import oil at low cost due to the yen appreciation, Japan needs to promote development work for any new energy crisis that may come in the future. This has been the motive for gas separation membrane development in Japan. The study of gas permeation through polymer membranes, which is the basis for membranes for gas separation, at Japanese universities began many years ago, but interest in membranes for gas separation was aroused mainly by the Government. The development of gas separation membranes in Japan started with membranes for oxygen separation on an industrial scale.

Effect of shear wall location in rigid frame on earthquake response of roof structure

  • The purpose of this study is to investigate the effect of the shear wall location in rigid frames on the dynamic behavior of a roof structure due to vertical and horizontal earthquake motions. The study deals with a gabled long span beam supported by two story rigid frames with shear walls. The earthquake response analysis is carried out to study the responses of the roof: vibration mode, natural period, bending moment and horizontal shear force of the bearings. The study results in the following conclusions: First, a large horizontal stiffness difference between the side frames is caused by the shear wall location, which results in a large vertical vibration of the roof and a large shear force at the side bearings. Second, in this case, the seismic design method for ordinary buildings is not useful in determining the distribution of the static equivalent loads for the seismic design of this kind of long span structures.

  • Recently, AC variable-speed motors are used for many steel rolling mill drive systems, because of their low maintenance and enhanced control performance. We have been applied GTO inverters for these AC motor drive systems since 1993. We have developed world largest 6-inch diameter GTO and large capacity 3-level GTO inverter up to 20000(kVA). As an example, in this paper, we describe the main circuit, system arrangement and control features of the 6-inches GTO inverters to drive rougher mills for hot strip mill of Pohang Iron & Steel Co., Ltd. The motor capacity is 6000(kW), and it's overload is 250(%).

A CLUSTER SURVEY AROUND THE UNIDENTIFIED EGRET SOURCES

  • Based on optical galaxy data, we executed a systematic search for galaxy clusters around the 15 steady unidentified EGRET GeV gamma-ray sources in high Galactic-latitude sky ([b] > $30^{\circ}$). We found a strong correlation with 3.7$\sigma$ level between close cluster pairs (merging cluster candidates) and the unidentified EGRET sources, though, in contrast, no correlation with single clusters. This result implies that merging clusters of galaxies are a possible candidate for the origin of high galactic-latitude, steady unidentified EGRET gamma-ray sources.

  • The self-mixing type semiconductor laser Doppler velocimeter(SM-LDV) is applied to measure two simultaneously moving targets with different velocities in the same direction as a prototype target for multiscatterers. The measured beat waveform is found to be a composite wave of each beat waveform measured fran each of only moving target. In the composite waveform, each one-cycle wave has a feature of the sawtooth wave. This fact shows a possibility to discriminate the flow direction of fluid containing multiscatterers with distributed velocities by cooperating an improved version of the direction discrimination circuit already devised by the authors.

  • Since 1996, KEPCO has been operating a wide range lightning detection system, LPATS, and been accumulating relative application techniques and statistical analysis skills. So, KEPRI already has its own basis to develope more accurate advanced detection technology and references to do comparative study. For three-dimensional imaging of lightning channels, UHF/VHF antenna systems were installed at 2 sites. The distance between two sites is about 30 km. These systems were used the AOA(Angle of Arrival) methods for lightning detection. In this paper, we would like to introduce about our system and its results.

  • The objectives of this paper are twofold. First is to introduce a model for representing documents with semantics relatedness using rough sets but with tolerance relations instead of equivalence relations (TRSM). Second is to introduce two document hierarchical and nonhierarchical clustering algorithms based on this model and TRSM cluster-based information retrieval using these two algorithms. The experimental results show that TRSM offers an alterative approach to text clustering and information retrieval.

  • In this paper, a new procedure for selecting weighting matrices in linear discrete time quadratic optimal control problems (LQ-problem) is proposed. In LQ problems, the quadratic weighting matrices are usually decided on trial and error in order to get a good response. But using the proposed method, the quadratic weights are decided in such a way that all poles of the closed loop system are located in a desired area for good responses as well as for stability and values of the quadratic cost functional are kept less then a specified value. The closed loop systems constructed by this method have merits of LQ problems as well as those of pole assignment problems. Taking into consideration that little is known about the relationship among the quadratic weights, the poles and the values of cost functional, this procedure is also interesting from the theoretical point of view.
