Randomized, Double-blind, and Placebo-controlled Human Trial to Evaluate the Efficacy and Safety of Allium Fistulosum L. Root Extract on Improvement of Child Height Growth: Study Protocol (총백추출물의 어린이 키 성장에 대한 유효성 및 안전성을 평가하기 위한 무작위배정, 이중눈가림, 위약 대조 인체적용시험: 인체적용시험 프로토콜)

  • Objectives The purpose of the study is to evaluate the safety of the Allium Fistulosum extract in children and its effectiveness in height growth. Methods This study is randomized, double-blind, placebo-controlled trial. The participants are children between the 3rd and 25th percentiles in height, and between the ages of 5 and 12 years. They are randomly assigned to treatment group or control group. The treatment group will take 5 g (1 g as Allium Fistulosum extract) for 24 weeks, 1 time a day. The control group will take the 5 g (0 g as Allium Fistulosum extract) of placebo for 24 weeks, 1 time a day. The primary outcome is change in height, and the secondary outcomes are growth rate, height standard deviations, Insulin-like growth factor-1 (IGF-1), Insulin-like growth factor binding protein-3 (IGFBP-3), IGF1-1/IGFBP-3 ratio, growth hormone, bone age, osteocalcin, and Z-score for growth. Results This protocol has been approved by the institutional review board (IRB) of Daejeon Korean Medicine Hospital of Daejeon University (IRB No. DJDSKH-20-BM-15), and registered in the Clinical Research Information Service (CRIS) (Registry No. KCT0005981). Conclusions This study will provide clinical information about the effectiveness and safety of Allium Fistulosum extract in children for their growth.

Effect of pectoralis major myofascial release massage for breastfeeding mothers on breast pain, engorgement, and newborns' breast milk intake and sleeping patterns in Korea: a randomized controlled trial

  • Purpose: Supportive interventions to improve breastfeeding practice are needed in nursing. This study investigated the effects of pectoralis major myofascial release massage (MRM) on breast pain and engorgement among breastfeeding mothers and on breast milk intake and sleep patterns among newborns. Methods: Breastfeeding mothers who had delivered between 37 and 43 weeks and had 7-to 14-day-old newborns were recruited from a postpartum care center in Gunpo, Korea. Participants were randomized to the MRM or control group. The outcome variables were breast pain and breast engorgement among breastfeeding mothers and breast milk intake and sleep time among newborns. The experimental treatment involved applying MRM to separate the pectoralis major muscle and the underlying breast tissue in the chest. After delivery, the first MRM session (MRM I) was provided by a breast specialist nurse, and the second (MRM II) was administered 48 hours after MRM I. Results: Following MRM, breast pain (MRM I: t=-5.38, p<.001; MRM II: t=-10.05, p<.001), breast engorgement (MRM I: right, t=-1.68, p =.100; left, t=-2.13, p=.037 and MRM II: right, t=-4.50, p<.001; left, t=-3.74, p<.001), and newborn breast milk intake (MRM I: t=3.10, p=.003; MRM II: t=3.09, p=.003) differed significantly between the groups. Conclusion: MRM effectively reduced breast engorgement and breast pain in breastfeeding mothers, reducing the need for formula supplementation, and increasing newborns' breast milk intake. Therefore, MRM can be utilized as an effective nursing intervention to alleviate discomfort during breastfeeding and to improve the rate of breastfeeding practice (clinical trial number: KCT0002436).

An Observational Multi-Center Study Protocol for Distribution of Pattern Identification and Clinical Index in Parkinson's Disease (파킨슨병 변증 유형 및 지표 분포에 대한 전향적 다기관 관찰연구 프로토콜)

  • Objectives: This study investigated the pattern identification (PI) and clinical index of Parkinson's disease (PD) for personalized diagnosis and treatment. Methods: This prospective observational multi-center study recruited 100 patients diagnosed with PD from two Korean medicine hospitals. To cluster new subtypes of PD, items on a PI questionnaire (heat and cold, deficiency and excess, visceral PI) were evaluated along with pulse and tongue analysis. Gait analysis was performed and blood and feces molecular signature changes were assessed to explore biomarkers for new subtypes. In addition, unified PD rating scale II and III scores and the European quality of life 5-dimension questionnaire were assessed. Results: The clinical index obtained in this study analyzed the frequency statistics and hierarchical clustering analysis to classify new subtypes based on PI. Moreover, the biomarkers and current status of herbal medicine treatment were analyzed using the new subtypes. The results provide comprehensive data to investigate new subtypes and subtype-based biomarkers for the personalized diagnosis and treatment of PD patients. Ethical approval was obtained from the medical ethics committees of the two Korean medicine hospitals. All amendments to the research protocol were submitted and approved. Conclusions: An objective and standardized diagnostic tool is needed for the personalized treatment of PD by traditional Korean medicine. Therefore, we developed a clinical index as the basis for the PI clinical evaluation of PD.   • 최근 유한연료의 고갈로 인해 세계 유가가 불안정 됨으로서 대체 에너지에 대한 연구가 많이 진행 되고 있다. 특히 압전 소자를 이용한 에너지 하베스팅은 압전 역효과를 이용한 것으로서 주변에서 무의미하게 버려지는 진동이나 바람, 열 에너지를 실 생활에 사용할 수 있는 전기 에너지로 변환할 수 있는 유망한 기술 중 하나이다. 이러한 에너지 하베스팅 기술은 일본과 같은 선진국에서 이미 지하철 및 일반 다리와 같이 진동이 극히 많은 곳에서 응용되고 있다. 이러한 에너지 하베스팅 기술을 응용 하려면 전압출력 계수($g_{33}$)가 높아야 한다. 이것은 압전 d 상수와 유전상수에 영향을 많이 받는 것으로 알려져 있다. 현재가지 응용되는 압전 하베스팅 조성은 Pb(Zr,Ti)$O_3$ (PZT)를 기초로한 세라믹이 응용되고 있다. Pb(Zr,Ti)$O_3$ (PZT) 세라믹은 Morpohotropic phase boundary(MPB)에서 전기기계 결합계수 (kp) 와 기계적 품질계수 (Qm) 이 각각 0.5와 500으로 우수한 특성을 나타낸다. 또한 큐리온도 (Tc) 도 $400^{\circ}C$로 온도 안정성 또한 높다. 하지만 $1000^{\circ}C$ 이상에서 소결하는 PbO는 소결 중 급격한 휘발로 환경적 오염 뿐 아니라 특성의 저하를 야기시킨다. 그래서 몇몇 나라에서는 그 사용을 제한하고 점차적으로 사용을 줄여 나가고 있는 동시에 PbO가 첨가되어 있지 않은 Lead-Free 세라믹의 연구가 많이 진행되고 있다. Lead-Free 세라믹 중 alkaline niobate를 기초로 한 페로브스카이트 구조의 ($Na_{0.5}K_{0.5})NbO_3$ (NKN) 은 PbO를 기초로 한 세라믹을 대체할 유망한 후보자 중 하나이다. 하지만 NKN세라믹의 K 성분의 조해성 및 고온에서의 휘발로 인해 일반 적인 소결 방법으로는 고밀도의 세라믹을 얻기 매우 어렵다. 그래서 Hot pressing, Hot forging, RTGG(Reactive Template Grain Growth), SPS(Spark plasma Sintering)와 같은 특별한 소결 법을 이용하거나 $K_8CuNb_4O_{23}$(KCN) 이나 $K_{5.4}Cu_{1.3}Ta_{10}O_{29}$(KCT) 등을 첨가하여 그 소결성을 향상 시키는 방법도 있다. 또한 압전 d상수를 향상 시키기 위해 $Nb_2O_5$나, $La_2CO_3$, $CeO_2$, $Li_2CO_3$ 등을 치환함으로써 압전 d상수를 향상 시켜 전압출력 계수를 높이는 연구 또한 많은 보고가 되어 있다. 특히 $Li_2CO_3$의 첨가는 일반 적인 소결 방법으로도 밀도의 조밀함을 향상 시켜 그에 따른 높은 유전율과 전기기계 결합계수, 압전 d상수를 가져 많은 연구가 되어지고 있다. 그래서 본 연구에서는 일반적인 ($K_{0.5}N_{0.5})_{1-x}Li_x(Nb_{0.96}Sb_{0.04})O_3$ + 0.2mol%$La_2O_3$ + 1.2mol%$K_8CuNb_4O_{23}$ 세라믹에 x(=Li) 치환에 따른 유전 및 압전특성을 조사하였다.

Production of 3,4-dihydroxyphenyl-L-alanine by Using the ${\beta}$-Tyrosinase of Citrobacter freundii Overexpressed in Recombinant Escherichia coli. (재조합 대장균에서 과발현된 Citorbacter Freundii KCTC2006 유래의 ${\beta}$-Tyrosinase를 이용한 3,4-Dihydroxyphenyl-L-alannine의 생산)

  • By using the ${\beta}$-tyrosinase of Citrobacter freundii KCT2006, which was cloned and overexpressed in Escherichia coli, 3,4-dihydroxy phenyl-L-alanine (L-DOPA) was synthesized efficiently from pyrocatechol, sodium pyruvate, and ammonium acetate. Optimal temperature and pH for the reaction were determined to be about 18$^{\circ}C$ and 8.5, respectively. The effects of substrate concentrations were also examined at different concentrations of ammonium acetate, sodium pyruvate, and pyrocatechol. Ammoniumacetate and sodium pyruvate increased the reaction rate until the concentrations reached to 300mM and 50mM, respectively. Although pyrocatechol showed the optimal concentration at 20mM, it was controlled between 20mM and 50mM to avoid the depletion of substrate during the enzymatic synthesis. Meanwhile the synthetic rate was improved about 20% when ethanol was included in the reaction solution. Based on above results, a reaction medium for the productin of L-DOPA was prepared and incubated with 1 unit/ml of ${\beta}$-tyrosinase. Pyrocatechol and sodium pyruvate was added to the reaction solutin intermittently to avoid the substrate depletion during the enzymatic reaction. After 24 hour of reaction, 31.6g/l of L-DOPA was accumulated in the reaction solution as soluble and precipitated ones and the conversion yield was about 85.2%.

Expression of a $\beta$-1,3-Glucanase Gene from Bacillus circulans in B. subtilis and B. megaterium (Bacillus subtilis와 Bacillus megaterium에서의 $\beta$-1,3-glucanase 유전자의 발현)

  • A Bacillus circulans KCTC3004 $\beta$-1,3-glucanase gene contained in a recombinant plasmid pLM460 derived from subcloning the original recombinant plasmid pLM530 was trasferred into a new shuttle vector plasmid pLMS1180 by ligating linearized DNAs of pLM460 and pUB110. B. subtilis RM125 and B. megaterium ATCC14945 transformed with pLMS1180 produced the $\beta$-1,3-glucanase substantially. Most of the enzyme was produced during the exponential growth period. The maxium activities of the $\beta$-1,3-glucanase produced by the Bacillus transformants were compared with that of the B. circulans gene donor strain. The B. subtilis RM125 (pLM1180) enzyme showed the activity 14 times higher than that of the gene donor cells, followed by the B. megaterium ATCC14945 (pLMS 1180) enzyme with activity 5 times higher than that of the gene donor cells. While E. coli secreted about 7% of the produced enzyme, B. subtilis excreted the enzyme into the medium wholly and B. megaterium about 97% of the total product. The SDS-PAGE of this enzyme produced in E. coli (pLMS1180), B subtilis (pLMS1180) or B. megaterium (pLMS1180) indicated a molecular weight of 38,000. The enzymes overproduced in three different host cells hydrolyzed laminarin to produce mainly laminaribiose, laminaritriose, and laminarioligosaccharides. The plasmid pLMS1180 was stable in B. megaterium, E. coli, but was unstable in B. subtilis.

Effects of Acupuncture Stimulation on the Radial artery's Pressure Pulse Wave in Healthy Young Participants: Protocol for a prospective, single-Arm, Exploratory, Clinical Study

  • Introduction: This study aims to investigate the effects of acupuncture stimulation on the radial artery's pressure pulse wave, along with various hemodynamic parameters, and to explore the possible underlying mechanism of pulse diagnosis in healthy participants in their twenties. Methods and analysis: This study is a prospective, single-arm, exploratory clinical study. A total of 25 healthy participants, without regard to gender, in their twenties will be recruited by physicians. Written informed consent will be obtained from all participants. The participants will receive acupuncture once at ST36 on both sides. The radial arterial pulse waves will be measured on the left arm of the subjects by using an applicable pulse tonometric device (KIOM-PAS). On the right arm (appearing twice), electrocardiogram (ECG), photoplethysmogram (PPG), respiration and cardiac output (CO) signals, will be measured using a physiological data acquisition system (Biopac module), while the velocity of blood flow, and the diameter and the depth of the blood vessel will be measured using an ultrasonogram machine on the right arm (appearing twice). All measurements will be conducted before, during, and after acupuncture. The primary outcome will be the spectral energy at high frequencies above 10 Hz ($SE_{10-30Hz}$) calculated from the KIOM-PAS device signal. Secondary outcomes will be various variables obtained from the KIOM-PAS device, ECG, PPG, impedance cardiography modules, and an ultrasonogram machine. Discussion: The results of this trial will provide information regarding the physiological and the hemodynamic mechanisms underlying acupuncture stimulation and clinical evidence for the influence of acupuncture on the pressure pulse wave in the radial artery. Ethics and dissemination: This study was approved by the Institutional Review Board (IRB) of Kyung Hee University's Oriental Medical Center, Seoul, Korea (KOMCIRB-150818-HR-030). The study findings will be published in peer-reviewed journals and presented at national and international conferences. Trial registration number: This trial was registered with the Clinical Research Information Service (CRIS) at the Korea National Institute of Health (NIH), Republic of Korea (KCT0001663), which is a registry in the World Health Organization's (WHO's) Registry Network.

  • Background: Alzheimer's disease (AD) is a chronic neurodegenerative disease that causes disorientation, mood swings, problems with language, and difficulty remembering recent events. Acetylcholinesterase inhibitors (AchEIs) and memantine have been used to slow the course of the disease, but they can neither modify its progression nor prevent disease onset. Previous studies have suggested that Kami-guibi-tang (KGT) could be beneficial for supporting cognitive function in AD patients, but few clinical trials have been published. This pilot study aimed to evaluate the effect of KGT in improving cognitive function in AD patients. Methods: The study will be a randomized, placebo-controlled, double-blind, single-center trial conducted using subjects diagnosed with mild AD by neurologists. Study subjects will be randomly assigned to either a treatment or control group. The treatment group will receive KGT granules for 24 weeks, while the control group will receive placebo granules. AchEI administration will be maintained in both groups during the entirety of the study. Subjects will be assessed using the following exams: the Seoul Neuropsychologic Screening Battery (SNSB) for cognitive function; brain magnetic resonance imaging (MRI) for brain metabolite, neurotransmitter, and cerebral blood flow (CBF) measurements; the Korean version of Quality of Life-Alzheimer's Disease (KQol-AD) for quality of life; the Caregiver-Administered Neuropsychiatric Inventory (CGA-NPI) for neurobehavioral symptoms; blood tests for amyloid and tau proteins and general blood parameters; and electrocardiography (ECG) before and after taking the medication. Discussion: Our findings will provide insight into the feasibility of large-scale trials to consolidate evidence for the efficacy of KGT for dementia treatment. Development of the Dredged Sediments Management System and Its Managing Criteria of Debris Barrier (사방댐 준설퇴적물 관리시스템 개발 및 관리기준 제안)

  • The dredged sediment management system was developed to have an objective, quantitative and scientific decision for the optimum removal time of dredged sediments behind debris barrier and was set up at the real site. The dredged sediment management system is designed and developed to directly measure the dredged sediments behind debris barrier in the field. This management system is composed of Data Acquisition System (DAS), Solar System and measurement units for measuring the weight of dredge sediments. The weight of dredged sediments, the water level and the rainfall are measured in real time using the monitoring sensors, and their data can be transmitted to the office through a wireless communication method. The monitoring sensors are composed of the rain gauge to measure rainfall, the load cell system to measure the weight of dredged sediments, and water level meter to measure the water level behind debris barrier. The management criteria of dredged sediments behind debris barrier was suggested by using the weight of dredged sediments. At first, the maximum weight of dredged sediments that could be deposited behind debris barrier was estimated. And then when 50%, 70% and 90% of the maximum dredged sediments weight were accumulated behind debris barrier, the management criteria were divided into phases of Outlooks, Watch and Warning, respectively. The weight of dredged sediments can be monitored by using the dredged sediment management system behind debris barrier in real time, and the condition of debris barrier and the removal time of dredged sediments can be decided based on monitoring results.

Suggestion on the Dredging Time of Sediments Behind Debris Barrier Using Rainfall Data (강우자료를 이용한 사방댐 배면 퇴적물의 준설시기 선정)

  • The rainfall intensity-duration curve (I-D curve) was used for selecting the dredging time of sediments behind a debris barrier which is located at the study area in Inje-gun, Kangwon Province. The I-D curve was newly suggested by using the data of rainfall-induced landslides for about 30 years from June to September in Kangwon Province. According to the monitoring results, the landslides have been not occurred during the monitoring period of the dredged sediments management system at the study area, and also all of the rainfall events were located below the I-D curve. The weight of the dredged sediments measured at the management system in the field was increased but the weight increment was small. It means that the increase of the dredged sediments was not the effect of landslide but the effect of soil erosion at the ground surface due to heavy rainfall. The weight of the dredged sediments behind a debris barrier could be known in real time using the rainfall data measured at the management system. Also, when the I-D curve is used with the management system, it is possible to select the optimum dredging time for sediments behind debris barrier.
