Analysis of Oral Health Status for the Elderly

  • This study aims to analyze the oral health status of the elderly. Study subjects were 9,340 elderly aged over 65 who took the health examination (the first) for the local insured when the National Health Insurance Corporation carried out its survey from January to December, 2002. The subjects took an oral examination and filled in the questionnaire. Major results from the analysis are as follows: 1. Analysis of Oral Health Behavior For oral health behavior, 38.2% of total subjects had visited a dental hospital (or clinic) in the last one year in the order of the elderly of big cities (48.3%), the elderly of medium cities (43.9%), and the elderly of rural areas (29.0%)(P<0.001). Elderly men had a higher rate than elderly women, and the younger age had a higher rate(P<0.01). For experience of oral prophylaxis, 12.3% of the total elderly had experienced it in the order of the elderly of big cities (18.8%), the elderly of medium cities (16.0%), and the elderly of rural areas (6.4%) (P<0.001). For elderly men, the younger age had a higher rate of oral prophylaxis. The number of toothbrushing in order was twice(47.5%), once (26.7%), three times (25.0%), and none (0.7%). The younger age brushed their teeth more often (P<0.001). 2. Analysis of Oral Health Status The rate of caries was 10.6% of the elderly surveyed. By area, the elderly of rural areas had a higher rate of caries than the elderly of cities (p<0.001) and elderly men were higher than elderly women (p<0.001). By age, many elderly aged over 80 had more than two caries. For missing teeth, the elderly of rural areas had a higher rate than the elderly of cities (p<0.001) and the older age had a higher rate(p<0.001). The rate of periodontal disease was 43.2% of the total elderly. By area, the elderly of big cities (46.2%) had a higher rate of periodontal disease than the elderly of medium cities (39.4%) and rural areas (43.6%)(p<0.001), and elderly men (46.4%) were higher than elderly women (40.2%)(p<0.001). By age, the lower age had a higher rate of peridontal disease (p<0.001). Dental abrasion was observed in 16.9% of the total elderly. The elderly of cities (21.0%) had a higher rate than the elderly of rural areas (12.0%)(p<0.001) and elderly men (21.3%) were higher than elderly women (12.8%)(p<0.001). Also the lower age had more dental abrasion symptoms (p<0.001). For needing a denture, the rate among the elderly was 48.5% and was higher for the elderly of rural areas(20.9%), than the elderly of big cities(7.0%) and medium cities (10.5%)(p<0.001). For the rate of denture wearing, the elderly of rural areas(41.8%) were higher than the elderly of big cities (27.7%) and medium cities (28.2% )(p<0.001). For the relation of drinking and smoking to oral health, the elderly who had a higher frequency of drinking, had a higher rate of caries (p<0.001)periodontal disease(p<0.001) and missing teeth(p<0.001) Smokers had a higher rate of caries (p<0.001), periodontal disease (p<0.05), and missing teeth (p<0.001) than nonsmokers.

Performance of Occupational Health Services by Type of Service : Cost Benefit Analysis (사업장 보건관리 사업의 형태별 수행성과 분석 -비용편익 분석을 중심으로-)

  • Occupational health services in Korea have been operated as dual types : one is operated by occupational health care manager and the other is health care agency without their own personnel. The performance of occupational health service should be different due to the variety of characteristics of health care manager and workplace, qualification of health care manager. This study is to analyze performance of occupational health care services with a particular consideration of job performance shape and efficiency, based on comparing those two types of health care management to show on the basic data for the settlement of more qualitative health care management system at workplace. For this study, total 391 places in Seoul and Inchon city area ; 154 places (39.4%) managed by designated health care manager and 237 places (60.6%) by the agency with their commission are selected as research samples. Tools for data collection are questionnares that have been investigated during the period of 20 September 1993-20 December 1993. Those data are compared with percentiles, mean, standard deviation and B/C ratio using SPSS PC program. Conclusions observed from the tests and each comparison could be summerized as follows : 1. Occupational health care have been accomplished at workplaces with designated people than with agencies people, and coverage rate of the occupational health care services has differences, due to management types. The reason of these results is due to visit only one or two times monthly by the agencies, while their own health care manager obsess, at the workplaces all the times. 2. Most of the expense for environmental control of all health care services expenditures shows that there is almost no fundamental improvement because more expenses are needed for procuring personal protective equipment and measuring work environment instead of environmental improvement. 3. It is investigated how much the cost of occupational health care services needs per worker, and calculated how much the cost needs per service hour per worker. The results from this show that the cost of occupational health services at workplaces with their own managers used less than the cost of health care agencies, eventually the former gives better services with less cost than the latter. 4. Benefit/Cost ratio is also produced by total benefit/total cost. The result from the above way reads 4.57 as a whole, while their own manager having workplaces reads 4.82 and the agencies do l.56. Even if their own manager performing workplaces spent more cost, this system produces more benefit than the agencies management. 5. The B/C ratio for medical organization such as local clinic, health care center and pharmacy shows more than or equal to at the workplaces controlled by the agencies. It is inferred that benefit would be much less than the cost used, with so being inefficient. 6. It is assumed that the efficiency ratio of health education is equal to reduction rate of workers medical organization visit. Estimated reduction rate 5%, 10%, 15%, show that the efficiency ratio of health education have an effect on producing benefits. It is estimated that more benefit can be produced if more qualitative education will be provided for enhancing health care efficiency. 7. Results of this study cannot be generalized because there are large scale of deviation in case of workplaces with less than 300 full time workers, but B/C ratio reads 2.69 as a whole and 3.25 at workplaces with their own health care manager are higher than 1.63 at the workplaces manged by the agencies. Finally, all the benefit concerning health care services could not be quantified, measured and shown on the value of money. This is a reason that a considerable part of benefits are so underestimated. This is also thought that measurement tools should be developed for measuring benefits of health care services with a comprehensive quantification. in the future. It is also expected that efficiency of occupational health care services should be investigated using cost-effectiveness analysis.

  • In recent years, microtremor array observations have been used for estimation of shear-wave velocity structures. One of the methods is the conventional spatial autocorrelation (SPAC) method, which requires simultaneous recording at least with three or four sensors. Modified SPAC methods such as 2sSPAC, and linear array methods, allow estimating shear-wave structures by using only two sensors, but suffer from instability of the spatial autocorrelation coefficient for frequency ranges higher than 1.0 Hz. Based on microtremor measurements from four different size triangular arrays and four same-size triangular and linear arrays, we have demonstrated the stability of SPAC coefficient for the frequency range from 2 to 4 or 5 Hz. The phase velocities, obtained by fitting the SPAC coefficients to the Bessel function, are also consistent up to the frequency 5 Hz. All data were processed by the SPAC method, with the exception of the spatial averaging for the linear array cases. The arrays were deployed sequentially at different times, near a site having existing Parallel Seismic (PS) borehole logging data. We also used the imaginary part of the SPAC coefficients as a data-quality indicator. Based on perturbations of the autocorrelation spectrum (and in some cases on visual examination of the record waveforms) we divided data into so-called 'reliable' and 'unreliable' categories. We then calculated the imaginary part of the SPAC spectrum for 'reliable', 'unreliable', and complete (i.e. 'reliable' and 'unreliable' datasets combined) datasets for each array, and compared the results. In the case of insufficient azimuthal distribution of the stations (the linear array) the imaginary curve shows some instability and can therefore be regarded as an indicator of insufficient spatial averaging. However, in the case of low coherency of the wavefield the imaginary curve does not show any significant instability.

  • The purpose of this study is to analyze the management performance of hospitals by analyzing the ratio of stability, profitability ratio, and growth rate through the financial ratios of medical institutions using accounting information disclosure data of medical institutions, financial status table and profit and loss statement. The main goal is to analyze and analyze financial statements of medical institutions' accounting information in 2016 and 2017, analyze the difference and analyze the general characteristics and financial ratios by type, type and size of medical institutions, The financial characteristics of medical institutions were identified. The ratio of stability, profitability, and growth rate through financial ratios were compared and analyzed. In addition, we analyzed the correlation between the medical profit margin, the total asset profit margin, the medical profit margin rate, and the net profit margin of the medical institutions through the financial ratios of accounting information disclosure data of medical institutions. The main results are as follows: First, the size of the hospital and the size of the debt through the change of assets, liabilities and capital of the financial statement are increasing, the size of own capital is relatively decreased, and the management performance is getting worse It is showing. Second, the increase in average medical revenues in the income statement is small, and the average increase in net profit is small. Thus, medical institutions were able to confirm the difficulty in creating profits through medical activities. In addition, there was a large difference in the debt ratio, the stability ratio, and the profitability ratio of the general hospitals and the general hospitals according to the types of medical institutions, and the difference in the average financial ratios of national and public hospitals, school corporation hospitals, I could confirm. The correlation between independent variables in the correlation was -0.904 between the capital ratio and the total assets turnover ratio, -0.800 between the labor cost ratio and the hospital income ratio, and -0.631 between the labor cost ratio and the foreign profit ratio. In order to improve the management deterioration of hospitals by using accounting information disclosure data of medical institutions, it is necessary to have a large effect on the net profit margin of the medical care and the net profit margin of the total assets.

Bias Correction for GCM Long-term Prediction using Nonstationary Quantile Mapping (비정상성 분위사상법을 이용한 GCM 장기예측 편차보정)

  • The quantile mapping is utilized to reproduce reliable GCM(Global Climate Model) data by correct systematic biases included in the original data set. This scheme, in general, projects the Cumulative Distribution Function (CDF) of the underlying data set into the target CDF assuming that parameters of target distribution function is stationary. Therefore, the application of stationary quantile mapping for nonstationary long-term time series data of future precipitation scenario computed by GCM can show biased projection. In this research the Nonstationary Quantile Mapping (NSQM) scheme was suggested for bias correction of nonstationary long-term time series data. The proposed scheme uses the statistical parameters with nonstationary long-term trends. The Gamma distribution was assumed for the object and target probability distribution. As the climate change scenario, the 20C3M(baseline scenario) and SRES A2 scenario (projection scenario) of CGCM3.1/T63 model from CCCma (Canadian Centre for Climate modeling and analysis) were utilized. The precipitation data were collected from 10 rain gauge stations in the Han-river basin. In order to consider seasonal characteristics, the study was performed separately for the flood (June~October) and nonflood (November~May) seasons. The periods for baseline and projection scenario were set as 1973~2000 and 2011~2100, respectively. This study evaluated the performance of NSQM by experimenting various ways of setting parameters of target distribution. The projection scenarios were shown for 3 different periods of FF scenario (Foreseeable Future Scenario, 2011~2040 yr), MF scenario (Mid-term Future Scenario, 2041~2070 yr), LF scenario (Long-term Future Scenario, 2071~2100 yr). The trend test for the annual precipitation projection using NSQM shows 330.1 mm (25.2%), 564.5 mm (43.1%), and 634.3 mm (48.5%) increase for FF, MF, and LF scenarios, respectively. The application of stationary scheme shows overestimated projection for FF scenario and underestimated projection for LF scenario. This problem could be improved by applying nonstationary quantile mapping.

An Empirical Model for Forecasting Alternaria Leaf Spot in Apple (사과 점무늬낙엽병(斑點落葉病)예찰을 위한 한 경험적 모델)

  • An empirical model to predict initial disease occurrence and subsequent progress of Alternaria leaf spot was constructed based on the modified degree day temperature and frequency of rainfall in three years field experiments. Climatic factors were analized 10-day bases, beginning April 20 to the end of August, and were used as variables for model construction. Cumulative degree portion (CDP) that is over $10^{\circ}C$ in the daily average temperature was used as a parameter to determine the relationship between temperature and initial disease occurrence. Around one hundred and sixty of CDP was needed to initiate disease incidence. This value was considered as temperature threshhold. After reaching 160 CDP, time of initial occurrence was determined by frequency of rainfall. At least four times of rainfall were necessary to be accumulated for initial occurrence of the disease after passing temperature threshhold. Disease progress after initial incidence generally followed the pattern of frequency of rainfall accumulated in those periods. Apparent infection rate (r) in the general differential equation dx/dt=xr(1-x) for individual epidemics when x is disease proportion and t is time, was a linear function of accumulation rate of rainfall frequency (Rc) and was able to be directly estimated based on the equation r=1.06Rc-0.11($R^2=0.993$). Disease severity (x) after t time could be predicted using exponential equation $[x/(1-x)]=[x_0/(1-x)]e^{(b_0+b_1R_c)t}$ derived from the differential equation, when $x_0$ is initial disease, $b_0\;and\;b_1$ are constants. There was a significant linear relationship between disease progress and cumulative number of air-borne conidia of Alternaria mali. When the cumulative number of air-borne conidia was used as an independent variable to predict disease severity, accuracy of prediction was poor with $R^2=0.3328$.

The Price-discovery of Korean Bond Markets by US Treasury Bond Markets by US Treasury Bond Markets - The Start-up of Korean Bond Valuation System - (한국 채권현물시장에 대한 미국 채권현물시장의 가격발견기능 연구 - 채권시가평가제도 도입 전후를 중심으로 -)

  • This study tests the price discovery from US Treasury bond markets to Korean bond markets using the daily returns of Korean bond data (CD, 3-year T-note, 5-year T-note, 5-year corporate note) and US treasury bond markets (3-month T-bill, 5-year T-note 10-year T-bond) from July 1, 1998 to December 31, 2003. For further research, we divide full data into two sub-samples on the basis of the start-up of bond valuation system in Korean bond market July 1, 2000, employing uni-variate AR(1)-GARCH(1,1)-M model. The main results are as follows. First the volatility spillover effects from US Treasury bond markets (3-month T-bill, 5-year T-note, 10-year T-bond) to Korean Treasury and Corporate bond markets (CD, 3-year T-note, 5-year T-note, 5-year corporate note) are significantly found at 1% confidence level. Second, the price discovery function from US bond markets to Korean bond markets in the sub-data of the pre-bond valuation system exists much stronger and more persistent than those of the post-bond valuation system. In particular, the role of 10-year T-bond compared with 3-month T-bill and 5-year T-note is outstanding. We imply these findings result from the international capital market integration which is accelerated by the broad opening of Korean capital market after 1997 Korean currency crisis and the development of telecommunication skill. In addition, these results are meaningful for bond investors who are in charge of capital asset pricing valuation, risk management, and international portfolio management.

An Analysis of the Cognition of Professionals Regarding the Validity of Planting Design Change that Occurred in the Landscape Construction of a Major Private Company (민간기업 조경공사에서 나타나는 식재설계 변경 타당성에 대한 전문가 인식 분석)

  • This study analyzes the validity of the type classification of the type and design changes of apartment landscaping planting construction design changes that were completed in the private sector, efficiently manages the design changes that are displayed over landscaping planting work in general in the future, and performs research by placing the object underlying the presentation. The results are as follows. First, the percentage that occurred in the planting construction of design changes that have occurred in the apartment landscaping construction was carried out in the private sector and accounted for 61.8%. This indicates that part of the planting is a major design change. Second, as the cause of such a design change to be those associated with the field conditions such as lack of main construction period. In particular, due to a change in oral, appeared 7-48 times design changes of one review design change approval is complex, design changes of planting construction had shown a feature that occurs in multiple simultaneous. Third, the 7 types of Design Changes in planting design were delineated as 'design changes for consideration of the user', 'design changes for image improvement', 'design changes for ease of maintenance', 'design changes due to the mismatch of design statement', 'design changes due to the relationship with the engineering species of other', 'design changes due to lack of field study', and 'design changes due to the consideration of feasibility.' Fourth, 'design changes for consideration of the user' and 'design changes for image improvement' were found in more than half of the frequency of the overall changes. This differed from the results shown in public corporations. Fifth, if planting construction design change process, private companies, it was found that is showing the approval of the practice after the previous construction of the construction cost savings due to construction time. However, in the case of a public corporation, these exhibited a different aspect from the private sector and show a design change procedure that reflects the changes after the design change events in the field have occurred. The above results, the type of landscaping works in planting design change of public enterprises, regardless of the private sector, is the same in the seven types, the main reason of and procedures for design changes, indicating that there are other respects. In design change, it may be desirable to apply becomes liquidity rationality and efficiency of the dimension, depending on the nature of the landscape construction.

Structural Behavior of the Buried flexible Conduits in Coastal Roads Under the Live Load (활하중이 작용하는 해안도로 하부 연성지중구조물의 거동 분석)

  • Soil-steel structures have been used for the underpass, or drainage systems in the road embankment. This type of structures sustain external load using the correlations with the steel wall and engineered backfill materials. Buried flexible conduits made of corrugated steel plates for the coastal road was tested under vehicle loading to investigate the effects of live load. Testing conduits was a circular structure with a diameter of 6.25m. Live-load tests were conducted on two sections, one of which an attempt was made to reinforce the soil cover with the two layers of geo-gird. Hoop fiber strains of corrugated plate, normal earth pressures exerted outside the structure, and deformations of structure were instrumented during the tests. This paper describes the measured static and dynamic load responses of structure. Wall thrust by vehicle loads increased mainly at the crown and shoulder part of the conduit. However additional bending moment by vehicle loads was neglectable. The effectiveness of geogrid-reinforced soil cover on reducing hoop thrust is also discussed based on the measurements in two sections of the structure. The maximum thrusts at the section with geogrid-reinforced soil cover was 85-92% of those with un-reinforced soil cover in the static load tests of the circular structure; this confirms the beneficial effect of soil cover reinforcement on reducing the hoop thrust. However, it was revealed that the two layers of geogrid had no effect on reducing the overburden pressure at the crown level of structure. The obtained values of DLA decrease approximately in proportion to the increase in soil cover from 0.9m to 1.5m. These values are about 1.2-1.4 times higher than those specified in CHBDC.

A study of SCM strategic plan: Focusing on the case of LG electronics (공급사슬 관리 구축전략에 관한 연구: LG전자 사례 중심으로)

  • Most domestic companies, with the exclusion of major firms, are reluctant to implement a supply chain management (SCM) network into their operations. Most small- and medium-sized enterprises are not even aware of SCM. Due to the inherent total-systems efficiency of SCM, it coordinates domestic manufacturers, subcontractors, distributors, and physical distributors and cuts down on cost of inventory control, as well as demand management. Furthermore, a lack of SCM causes a decrease in competitiveness for domestic companies. The reason lies in the fundamentality of SCM, which is the characteristic of information sharing, process innovation throughout SCM, and the vast range of problems the SCM management tool is able to address. This study suggests the contemplation and reformation of the current SCM situation by analyzing the SCM strategic plan, discourses and logical discussions on the topic, and a successful case for adapting SCM; hence, the study plans to productively "process" SCM. First, it is necessary to contemplate the theoretical background of SCM before discussing how to successfully process SCM. I will describe the concept and background of SCM in Chapter 2, with a definition of SCM, types of SCM promotional activities, fields of SCM, necessity of applying SCM, and the effects of SCM. All of the defects in currently processing SCM will be introduced in Chapter 3. Discussion items include the following: the Bullwhip Effect; the breakdown in supply chain and sales networks due to e-business; the issue that even though the key to a successful SCM is cooperation between the production and distribution company, during the process of SCM, the companies, many times, put their profits first, resulting in a possible defect in demands estimation. Furthermore, the problems of processing SCM in a domestic distribution-production company concern Information Technology; for example, the new system introduced to the company is not compatible with the pre-existing document architecture. Second, for effective management, distribution and production companies should cooperate and enhance their partnership in the aspect of the corporation; however, in reality, this seldom occurs. Third, in the aspect of the work process, introducing SCM could provoke corporations during the integration of the distribution-production process. Fourth, to increase the achievement of the SCM strategy process, they need to set up a cross-functional team; however, many times, business partners lack the cooperation and business-information sharing tools necessary to effect the transition to SCM. Chapter 4 will address an SCM strategic plan and a case study of LG Electronics. The purpose of the strategic plan, strategic plans for types of business, adopting SCM in a distribution company, and the global supply chain process of LG Electronics will be introduced. The conclusion of the study is located in Chapter 5, which addresses the issue of the fierce competition that companies currently face in the global market environment and their increased investment in SCM, in order to better cope with short product life cycle and high customer expectations. The SCM management system has evolved through the adaptation of improved information, communication, and transportation technologies; now, it demands the utilization of various strategic resources. The introduction of SCM provides benefits to the management of a network of interconnected businesses by securing customer loyalty with cost and time savings, derived through the consolidation of many distribution systems; additionally, SCM helps enterprises form a wide range of marketing strategies. Thus, we could conclude that not only the distributors but all types of businesses should adopt the systems approach to supply chain strategies. SCM deals with the basic stream of distribution and increases the value of a company by replacing physical distribution with information. By the company obtaining and sharing ready information, it is able to create customer satisfaction at the end point of delivery to the consumer.
