목적: 슬관절 전치환술 후 진균에 의한 감염은 드물지만 심각한 합병증으로 알려져 있으며 그 치료에 있어 항진균제 투여 기간이나 수술의 방법에 대해서는 이견이 있다. 이에 저자들은 인공 관절 주위 진균 감염에 대한 치료 결과 및 임상경과를 알아보고자 하였다. 대상 및 방법: 2006년부터 2017년까지 슬관절 전치환술 후 진균 감염으로 계명대학교 동산의료원에서 2단계 재치환술을 시행받은 10명의 환자를 같은 기간 비진균 인공 슬관절 주위 감염으로 치료받은 119명의 환자와 비교하였다. 임상적 평가는 2단계 재치환술에 의한 감염 조절의 실패율과 감염의 재발률 및 정주용 항생제 및 항진균제 사용기간, 2단계 재치환술 후 관절 운동 범위, 한국형 슬관절 점수(Korean knee score, KKS), 적혈구 침강속도(erythrocyte sedimentation rate, ESR), C-반응성 단백질(C-reactive protein, CRP)을 평가하였다. 결과: 진균이 동정된 군 총 10예 중 7예(70.0%)에서 감염조절의 실패를 보였고 일반 감염군 119예 중 7예(5.9%)에서 감염조절의 실패를 보였다(p=0.04). 진균이 동정된 군에서는 정주용 항진균제의 평균 사용기간은 15.3주로 일반 감염군에서의 정주용 항생제 평균 사용기간인 6.2주에 비해 9.1주 더 길었다(p<0.001). 감염 조절술 시행 전 슬관절 운동 범위는 두 군에서 모두 증가하였다(p=0.265). 최종 추시 시 KKS는 일반 감염군에서 평균 71.01점으로 나타났고 진균이 동정된 군에서 61.3점이었다(p=0.012). ESR과 CRP는 두 군에서 모두 감소하였으나 CRP만 두 군 간에 유의한 차이를 보였다(p=0.007). 결론: 슬관절 전치환술 후 발생한 인공관절 주위 진균 감염에 대한 수술적 치료는 일률적으로 양호한 결과를 보이지 않으므로 감염 조절술 시행 시 항진균제 혼합 시멘트 삽입과 재치환술 후 경구용 항진균제 사용을 고려하는 것이 필요하다.
목적: 슬개 건을 이용한 전방 십자 인대 재건술 시, 이식 건을 경골 쪽에서 고정할 때 일반적으로 슬관절의 굴곡 각도는 신전 위치에서 시행한다. 저자들은 슬괵 건을 이용한 전방 십자 인대 재건술 시, 이식 건을 고정 할 때 슬관절의 굴곡 각도가 수술 후 슬관절 안정성과 운동 범위에 미치는 영향을 알아보고자 하였다. 대상 및 방법: 2002년 5월부터 2003년 1월까지 슬괵 건을 이용한 전방 십자 인대 재건술 시, Rigidfix system(Mitek Product, Johnson and Johnson, USA)와 intrafix를 이용하여 고정하였다. 전방 십자 인대 재건술을 시행 받은 39명 중 추시 관찰이 되지 않은 2명을 제외한 37명의 환자를 대상으로 하였다. 평균 추시 기간은 14개월(범위: $13{\sim}25$개월) 이었다. 완전 신전 위 또는 20도 굴곡 위의 슬관절 굴곡 각도의 결정은 수술 순서에 따라서 교대로 선택하였다. 내측 반월상 연골의 절제 2예, 내측 반월상 연골의 봉합 2예 외측 반월상 연골의 봉합 1예에서 시행하였다. 임상적 평가는 이학적 검사인 Lachman검사, Pivot shift검사와 재건술 후의 Lysholm점수, IKDC( international knee documentation committee)평가 기준, KT-1000관절 계측 결과를 이용하였다. 결과: 최종 추시 상 수술 후 Lysholm 점수는 93.1점(범위: $65{\sim}98$점), IKDC 평가 기준에 의한 최종 평가 상 정상(A) 26예, 거의 정상(B) 10예, 비정상(C) 1 예이었으며, 심한 비정상은 (D)은 없었다. 최종 추시 상 KT-1000 관절계를 이용한 최대 도수 부하 검사 상 건 측 과의 차이는 평균 2.5 mm 이였다. 술 후 시행한 Lachman 검사상 정상 32예, 1 등급 4예, 2 등급 II 1예 이었고, Pivot shift 검사 상 정상 34예, 1 등급 2예, 2 등급 1예 이었다. KT-1000 관절계를 이용한 최대 도수 부하 검사 상 건 측 과의 차이는 20도 고정을 한 경우는 평균 2.3 mm, 완전 신전 위에서 고정한 경우는 평균 2.7 mm이였다(P<0.05). 수술 후 1년 슬관절의 운동 범위는 20도 굴곡 위에서 고정한 경우에 1례 에서 5도의 굴곡 구축이 있었고, 신전 위에서 고정한 경우는 2례 에서 건측 보다 10도의 굴곡 제한이 있었다. 결론: 자가 슬괵 건을 이용한 전방 십자 재건술 시, 경골 부의 고정 시 슬관절의 굴곡 각도 따른 슬관절의 전방 안정성에 차이는 없었으나, 신전 위에서 고정하는 것이 과도한 이식 건의 장력으로 인한 슬관절 굴곡 구축을 예방하는데 도움이 될 것으로 사료된다.
목적: 고정형 슬관절 단일 구획 치환술의 5년 추시 결과로 임상적 및 방사선적 결과, 합병증 발생 빈도를 알아보고자 하였다. 대상 및 방법: 2003년 5월부터 2011년 8월까지 고정형 슬관절 단일 구획 치환술을 시행한 25명, 26예를 대상으로 하였다. 평균 연령은 63.5세였으며 남자가 3명(3예), 여자가 22명(23예)이었으며 술 전 진단은 골관절염 23예, 골괴사 3예였다. 평균 추시기간은 평균 67개월(60-149개월)이었다. 임상적으로 술 전과 술 후의 미국슬관절학회 슬관절 점수 및 기능점수, 관절 운동 범위를 조사하였고, 방사선적으로 슬관절 기립 전후방 사진과 측면 사진, 투시기 촬영을 통하여 술 후의 정렬상태와 골용해 여부를 조사하였다. 결과: 임상적 결과로 슬관절 점수는 평균 42.0점에서 87.9점으로, 기능점수는 57.5점에서 85.0점으로 향상되었다(p<0.001). 술 전 관절 운동 범위는 평균 $132.9^{\circ}$에서 술 후 $132.5^{\circ}$로 큰 차이를 보이지 않았다. 방사선적 결과로 대퇴경골각은 술 전 평균 내반 $0.5^{\circ}$에서 술 후 외반 $2.2^{\circ}$로 측정되었다. 방사선 투과성 선(radiolucent line)이 관찰되는 경우가 2예 발견되었으며 그 중 1예는 삽입물이 안정적이었으나 다른 1예는 단일 구획 치환술 시 대퇴슬개관절의 관절염을 확인하였고, 추시상 슬관절 내측 관절선의 동통 및 슬관절의 미만성 통증이 있어 슬관절 전치환술을 권유하였다. 그 외 골용해, 감염, 술 후 강직, 탈구 등의 합병증은 없었다. 결론: 고정형 슬관절 단일 구획 치환술의 중기 추시 결과는 임상적 및 방사선적으로 만족스러운 결과를 보였다.
목적: 주관절의 퇴행성관절염에 대해 측와위에서 견인 후 관절경을 이용한 수술적 치료를 시행하고 그 결과와 술식의 유용성을 보고하고자 한다. 대상 및 방법: 12개월 이상 추시가 가능하였던 주관절의 퇴행성관절염 21례를 대상으로 하였다. 남자가 15례 여자가 6례였으며 평균 연령은 47세, 우세지가 14례였다. 모든 례에서 수술은 전신 마취하에 측와위에서 10파운드의 견인 상태로 관절내 활액막염, 골성 유리체, 관절의 전방 및 후방의 골극을 제거 하였다. 수술 후 최대 신전 상태에서 고정을 하였으며 2일부터 CPM을 이용한 운동을 시행하였다. 결과: 수술전 관절 운동 범위는 평균 $30{\sim}115$도였으며 수술 후 평균 $5{\sim}130$도로, 신전과 굴곡 각각 25도 16도, 전체적으로 41도 증가하였다. 통증에 대한 VAS상 7.5점에서 2.3점으로 감소, 기능 만족도는 1.8에서 9.0으로 증가하였다. 합병증은 2례로 1례는 척골 신경의 이상 감각증상이 있었으나 보존적 치료에 소실되었고, 1례는 수포가 형성되었으나 치유되었다. 결론: 주관절 관절경 수술시 측와위에서의 견인은 넓은 시야 확보에 도움을 주었다. 간단한 견인 방법으로 주관절의 퇴행성 관절염에 대해 관절경 수술을 시행한 바 통증의 의 해결과 관절 운동 범위 회복에 있어 우수한 결과를 얻을 수 있었다.

Purpose: The diagnostic relevancies and characteristics and of clinical methods in the diagnosis of internal derangement(ID) were tested by comparing the results of them with those of magnetic resonance imaging(MRI). Methods: 75 patients(150 temporomandibular joints; TMJs), who were suspected to have ID by clinical diagnoses, were included. Clinical diagnoses including mouth opening pathway and TMJ sound were conducted and MRI takings were done. Accuracies, sensitivities, specificities, positive predictive values, and negative predictive values of clinical diagnosis, mouth opening pathway, and TMJ sound were calculated by comparing with diagnoses with MRIs. Results: Accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of clinical diagnosis were 59.3%, 83%, 49%, 81%, and 51%. They were 59%, 82%, 25%, 73%, and 35% for mouth opening pathways. Although deviation was somewhat accurate for representing disc displacement with reduction(ADDWR), other discrepancies on opening pathways were not clinically relevant. Accuracy, sensitivity, specificity, positive predictive value, and negative predictive value of clicking sounds were 85%, 49%, 78%, 85%, and 37%. TMJs with crepitus were only three. But all TMJs with crepitus were diagnosed to have disc displacement without reduction(ADDWOR). Conclusion: When compared with diagnoses with MRIs, clinical diagnoses for ID were not so accurate. But they were suitable for screening tests for ID. Opening pathways and TMJ sounds were not so relevant in the diagnoses of IDs and so it was concluded that considerations for other factors must be included in the diagnoses of IDs.
Background: We prospectively compared the response to blind and ultrasound-guided glenohumeral injection of corticosteroids for treatment of shoulder stiffness. Methods: A total of 77 patients with shoulder stiffness between April 2008 and March 2012 were recruited. Patients were randomized to receive either a blind (group 1, n=39) or ultrasound-guided (group 2, n=38) glenohumeral injection of 40 mg triamcinolone. The clinical outcomes and shoulder range of motion (ROM) before injection, at 3, 6, and 12 months after injection and at the last follow-up were assessed. The same rehabilitation program was applied in both groups during the follow-up period. Results: There was no significant difference in demographic data on age, sex, ROM, and symptom duration before injection between groups (p>0.05). There were no significant differences in ROM including forward flexion, external rotation at the side, external rotation at $90^{\circ}$ abduction, and internal rotation, visual analogue scale for pain and functional outcomes including American Shoulder and Elbow Surgeons score, Simple Shoulder test between the two groups at any time point (p>0.05). Conclusions: Based on the current data, the result of ultrasound-guided glenohumeral injection was not superior to that of blind injection in the treatment of shoulder stiffness. We suggest that ultrasound-guided glenohumeral injection could be performed according to the patient's compliance and the surgeon's preference. Once familiar with the non-imaging-guided glenohumeral injection, it is an efficient and reliable method for the experienced surgeon. Ultrasound could be performed according to the surgeon's preference.
The purpose of this study was to test whether Yoga exercise, one of muscle relaxation, helps to relieve chronic low back paln. Sample were selected from nurses who were worked at K medical center in the period from March 18 to April 23, 1996. The sample size was fifteen. The research design was one-group pretest-posttest design. To test the major research question, this study had the following procedure. The pretest included measuring individuals' degree of pain and discomfort depending on the scope and types of an range of motion, and their pain by their activity of daily living(ADL). The treatment was conducted twice every other week. The reason why this study had twice observations was to control history effect and maturation which treat internal validity in the research design of this study. In this research design, a treatment was to expose Yoga exercise to samples. The exercise was taken in 30 minutes per day for four days in a week (Two consecutive days and twice every other day). The posttest included re-measuring the individuals' the degree of pain and discomfort, and their pain by their ADL. Several hypotheses concerning effect of Yoga exercise was analyzed by the paired t-test, comparing the difference scores between pre and post tests. The results of this study was as follows. The first hypothesis that the post-treatment group taking the Yoga exercise had the pain score lower than the pre-treatment group was supported(t=3.31, p=.005). The second hypothesis that the discomfort score of the post-treatment group had lower than does that of the pre-treatment group was supported(t=2.75, p=.016). The third hypothesis that the post-treatment group had the pain score by ADL lower than does the pre-treatment group was supported(t=5.52, p=.000). In summary, this study examined the effect of a yoga exercise, one of muscle relaxation, on those who were suffered from chronic low back pain. The effect measured by the degree of pain and discomfort with a visual analog scale was statistically significant. The degree of pain according to postures in ADL also showed statistical significance. These findings showed that a yoga exercise was effective to alleviation of chronic low back pain. A pretest-posttest control group design, however, needs to get more accurate results since the design satisfies Internal validity. Consequently, yoga as an exercise cure improves an interaction between muscles and articulations, and performance of ADL for those suffered from chronic low back pain. Furthermore, this positive impact may be an effective method as nursing intervention for their physical, mental, emotional, social, and psychological recovery.
This study was performed to investigate the masicatory efficiency in patients with temporomandibular disorders (TMDs), especially internal derangement of temporomandibular (TM) joint. For this study, 26 patients after treatment and 33 dental students who had no signs and symptoms of TMDs were selected as the patients group and as the normal group, respectively. Mean treatment duration of the patients was. 5.1 months. Verbal rating scale(VRS) and Visual analogue scale(VAS) were used for recording of subjective symptoms. Treatment index (VAS Ti) derived from VAS was calculated for evaluation of treatment progress and clinical examination was also performed for objective symptoms. BioEGN(Bioresearch Inc., Milwaukee, USA) was used for observation chewing movement pattern on peanut, caramel candy, and gum chewing. Chewing time in second and symptoms after chewing were recorded, and pattern of chewing stroke between in affected side and in contralateral side or between in right in left side were compared, and especially, gum chewing pattern between before and after treatment were also compared in the patients group. The data obtained were analysed by SPSS windows program and the results of this study were as follows : 1. Subjective symptoms evaluated by VAS showed no difference between the two groups, but those by VRS showed slight difference for TM joint pain, head not neck symptoms, and chewing ability. 2. There were no difference at the level of subjective symptoms between the subgroups divided by treatment duration of five months in the patients group. However, value of VAS Ti of pain was higher in subgroup of long treatment duration than that of subgroup of short treatment duration. 3. There were no difference in chewing time for peanut or caramel candy between the two groups, but on caramel candy chewing, the patients group complained slight discomfort after swallowing. Chewing velocity and range of motion on gum chewing after treatment in the patients group showed significant difference and greatly improved compared to those to before treatment, and which were not differ from those of normal subjects. In conclusion, treatment of temporomandibular disorders about for five months would greatly improve chewing ability and movement pattern in most of the patients with TMDs.
Purpose: The purpose of this study was to assess the 2-year follow-up results of patients with a trimalleolar fracture, who had undergone an anterior incision cannulated screw fixation of the posterior malleolar fragment, which had more than 25% of articular involvement or had no cortical continuity with the distal tibia. Materials and Methods: Among 28 patients with a trimalleolar fracture who had undergone fixation of the posterior malleolar fragment between February 2005 and February 2010, 14 patients, who underwent an anterior incision cannulated screw fixation of posterior malleolar fragment and were followed-up for more than 2 years, were selected. The postoperative clinical and radiological findings immediately and at the 1- and 2-year follow-up were compared. The clinical findings were evaluated as American Orthopaedic Foot and Ankle Society (AOFAS) score. The radiological assessment was evaluated as the maintenance of reduction, period to bone union, and the presence of nonunion, malunion, and complications. Results: The clinical outcome by mean AOFAS score revealed 83.0 points in the group with preoperative displacement below 2 mm and 80.7 points in the group with preoperative displacement above 2 mm postoperatively. The mean AOFAS score was 91.7 and 93.1 points in the group with preoperative displacement below 2 mm on 1- and 2-year follow-up, respectively, and 89.8 and 91.7 points in group with the preoperative displacement above 2 mm on 1- and 2-year follow-up, respectively. After a 2-year follow-up among 14 cases selected for this study, 13 cases showed an excellent reduction state and only 1 case (7.1%) showed a displacement of more than 2 mm. No complication were encountered in the group with preoperative displacement below 2 mm. On the other hand, among 8 patients in the group with preoperative displacement above 2 mm, there were 3 with limitations of the range of motion of the ankle joint (37.5%) and 1 post-traumatic arthritis (12.5%) at the 2-year follow-up. Conclusion: Anterior incision cannulated screw fixation of the posterior malleolar fragment could be a valuable method for the treatment of trimalleolar fractures that provides satisfactory results.
목적: 전위성 쇄골 간부 골절의 치료로 관혈적 정복술 후 LCP를 사용하여 내고정을 시행한 환자를 대상으로 방사선학적, 임상적 결과를 분석하여 전위성 쇄골 간부 골절의 치료에 있어서 LCP의 유용성을 알아보고자 하였다. 대상 및 방법: 2003년 5월부터 2004년 11월까지 본원에서 쇄골 간부 골절에 대해 LCP를 이용한 관혈적 정복, 금속내 고정술을 시행하고 6개월 이상 추시가 가능하였던 26례를 대상으로 하였다. 수술 후 최종 평가는 방사선 결과와 fang s criteria를 이용한 임상적 결과로 분석하였다. 결과: 수술 후 평균 9.3주에 모든 예에서 지연유합 없이 골유합 되었으며 임상적으로 Kangs criteria에 따라 구분한 결과, 우수 이 상이 22례로 나타났다. 특히, 견관절 운동은 상완골 골절이 동반된 두 경우를 제외한 24례에서 2.9주내에 정상 범위로 빠른 회복을 보였다. 합병증으로는 견관절 운동 장애 2례, 수술 절개 부위의 켈로이드 형성 1례였으며 그 이외에 다른 주요 합병증은 없었다. 결론: 전위성 쇄골 간부 골절에서 LCP를 이용한 관혈적 정복 및 금속내 고정술은 기존의 금속판에 비해 수술 후 합병증을 줄이고 효과적인 골유합과 조기 견관절 운동에 도움을 줄 수 있는 좋은 치료 방법 중 하나라고 사료된다.

① NDSL에서 제공하는 모든 저작물의 저작권은 원저작자에게 있으며, KISTI는 복제/배포/전송권을 확보하고
있습니다.
② NDSL에서 제공하는 콘텐츠를 상업적 및 기타 영리목적으로 복제/배포/전송할 경우 사전에 KISTI의 허락을
받아야 합니다.
③ NDSL에서 제공하는 콘텐츠를 보도, 비평, 교육, 연구 등을 위하여 정당한 범위 안에서 공정한 관행에
합치되게 인용할 수 있습니다.
④ NDSL에서 제공하는 콘텐츠를 무단 복제, 전송, 배포 기타 저작권법에 위반되는 방법으로 이용할 경우
저작권법 제136조에 따라 5년 이하의 징역 또는 5천만 원 이하의 벌금에 처해질 수 있습니다.
제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에
당 사이트 홈페이지를 통하여 회원에게 공지합니다.
② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)
회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
