The purpose of this study was to determine the effects of weight shift training with joint mobilization on the ankle joint passive range of motion (PROM), balance capacity and gait velocity in hemiplegic patients. Fourteen subjects were randomly assigned to either the experimental group (EG) or the control group (CG), with seven subjects in each group. The EG received weight shift training with joint mobilization in the paretic leg's subtalar joint in order to increase ankle dorsiflexion. The CG received general physical therapy training. Both groups received training five times a week over a period of two consecutive weeks. The figures for PROM of ankle dorsiflexion on the paretic leg, the functional reach test (FRT), the timed up and go (TUG) test, and gait velocity were recorded both before and after the training sessions for both groups. The EG's results in gait velocity, the FRT and the TUG test improved after training (p<.05). The PROM of ankle dorsiflexion improved both in the EG and the CG (p<.05), the EG demonstrated a significantly higher increase (p<.05) than that of the CG. The results of this study suggest that increased joint mobilization positively affects balance and gait velocity of hemiplegic patients. Further studies with a greater sample size are necessary in order further prove the accuracy of the results of this study.

This study was conducted to examine the effect of joint mobilization on forward head posture and to propose an effective method to improve correct body posture and balance. A total of eight patients from a Maitland-applied group (n=8) received mobilization therapy to increase the mobility of the cervical joint, with Maitland's movement grade III intensity for 30 seconds of treatment and 30 seconds of rest, for ten intervals, three times a week for four weeks. The craniovertebral angle (CVA) changes before and after the intervention with the Maitland technique were measured as $56.85{\pm}2.31^{\circ}$ before, $63.23{\pm}2.23^{\circ}$ two weeks after, and $64.98{\pm}1.27^{\circ}$ four weeks after joint intervention. There were significant CVA changes before and after the Maitland technique (P <.05). The results of this study suggest that the Maitland technique is useful for improving the head vertebral angle in patients with forward head posture.
Objective: The purpose of this study was to investigate the effects of joint mobilization on foot pressure, ankle moment, and vertical ground reaction force in subjects with ankle instability. Method: Twenty male subjects (age, $25.38{\pm}3.62yr$; height, $170.92{\pm}5.41cm$; weight, $60.74{\pm}9.63kg$; body mass index (BMI), $19.20{\pm}1.67kg/m^2$) participated and underwent ankle joint mobilization. Weight-bearing distribution, ankle dorsi/plantar flexion moment, and vertical ground reaction force were measured using a GPS 400 and a VICON Motion System (Oxford, UK), and subsequently analyzed. SPSS 20.0 for Windows was used for data processing and paired t-tests were used to compare pre- and post-mobilization measurements. The significance level was set at ${\alpha}$ = .05. Results: The results indicated changes in weight-bearing, ankle dorsi/plantar flexion moment, and vertical ground reaction force. The findings showed changes in weight-bearing distribution on the left (pre $29.51{\pm}6.31kg$, post $29.57{\pm}5.02kg$) and right foot (pre $32.40{\pm}6.30kg$, post $31.18{\pm}5.47kg$). There were significant differences in dorsi/plantar flexion moment (p < .01), and there were significant increases in vertical ground reaction forces at initial stance (Fz1) and terminal stance (Fz2, p < .05). Additionally, there was a significant reduction in vertical ground reaction force at midstance (Fz2, p < .001). Conclusion: Joint mobilization appears to alter weight-bearing distribution in subjects with ankle instability, with resultant improvements in stability.

PURPOSE: The purpose of the present study was to apply joint mobilization in a sitting position and in a prone position to patients with acute mechanical neck pain and compare the immediate treatment effects in these two positions. METHODS: After the baseline was assessed, 46 patients were randomly assigned to two groups: experimental group I ($n_1=23$) for joint mobilization in the sitting position and experimental group II ($n_2=23$) for joint mobilization in the prone position at the symptomatic cervical level. The patients in both groups received treatment by unilateral posterior-anterior gliding for 30 seconds per trial, 10 trials per session, for a total of 5 minutes, and two trials of 10 active extending motions with distraction per trial. RESULTS: In the Wilcoxon signed-rank test, all the pain and physical function variables were significantly improved after intervention in both groups (p<.05). In the Mann-Whitney U test, which compared the differences before and after the intervention between the two groups, experimental group I showed significant improvement over experimental group II in resting pain (p<.01), satisfaction with the treatment (p=.01), left rotation (p<.01) and CCFE (p<.01). In the analysis of covariance results, experimental group I showed significant improvement over experimental group II in the most painful motion pain (p<.01) and the most painful quadrant motion pain (p<.01). CONCLUSION: These outcomes suggest that joint mobilization should be applied in sitting positions for patients with acute mechanical neck pain that feel pain during sustained positions, extension or rotation.

Background: Surgery has been known as an inefficient approach to reduce back pain in patients with lumbar spinal stenosis; therefore, non-surgical treatments are necessary. However, there has been little research to analyze the effect of non-surgical treatments on lumbar spinal stenosis pain. Objective: To identify the effectiveness of 2 physiotherapeutic treatment approaches to relieve pain due to lumbar spinal stenosis. Design: Randomized controlled trial Methods: The participants were 36 lumbar spinal stenosis patients who were randomized in the joint mobilization group (JMG) and transcutaneous electrical nerve stimulation group (TENSG). Joint mobilization (JM) was conducted at the posteroanterior joint in the spinous process of the lumbar spine with stenosis. Transcutaneous electrical nerve stimulation (TENS) was applied on the lumbar spine with stenosis at a high frequency and intensity. Results: Visual analog scale (VAS) pain score significantly decreased in both groups, and the VAS value decreased more after JMG than that after TENSG. The pain thresholds of both groups also significantly increased, and that of JMG increased more compared to TENSG. In both the groups, significant improvements in VAS and pain thresholds were found, and JMG showed better results than TENSG. Conclusions: JM and TENS showed significant relief in both pain threshold and painpain, and JM showed more advanced relief compared to TENS.
The techniques of joint mobilization and traction are used to improve joint mobility or to decrease pain by restoring accessory movements to the shoulder joints and thus allowing full, nonrestriced, pain-free range of motion. In the glenohumeral joint, the humeral head would be the convex surface, while the glenoid fossa would be the concave surface. The medial end of the clavicle is concave anterioposteriorly and convex superioinferiorly, the articular surface of the sternum is reciprocally curved. The acromioclavicular joint is a plane synovial joint between a small convex facet on lateral end of the clavicle and a small concave facet on the acromion of the scapula. The relationship between the shape of articulating joint surface and the direction of gliding is defined by the Convex-Concave Rule. If the concave joint surface is moving on a stationary convex surface, gliding occur in the same direction as the rolling motion. If the convex surface is moving on a stationary concave surface, gliding will occur in an opposite direction to rolling. Hypomobile shoulder joints are treated be using a gliding technique.
Background: Posterior shoulder tightness, which is a problem mainly seen in patients with shoulder impingement syndrome, disrupts the scapulohumeral rhythm between the humerus and scapulae. Objects: The aim of this study was to compare the effects of joint mobilization and stretching on shoulder muscle activity and internal rotation range of motion (ROM) of the glenohumeral joint in patients with impingement syndrome with posterior shoulder tightness. Methods: The research subjects included 22 in-patients with impingement syndrome with posterior shoulder tightness. They were randomly divided into two groups: one group (12 patients) was treated with joint mobilization and the other group (10 patients) was treated with stretching for the posterior shoulder tightness. Each treatment was performed five times a week for two weeks, and there were 15 sessions for each treatment. The ROM of the internal rotation and muscle activities of shoulder joint were evaluated pretest and posttest in each group. Electromyography data were collected from the upper, middle, and lower trapezius and serratus anterior during shoulder abduction of 90°, 120°, and 150°. Results: Both the joint mobilization and stretching groups showed significant decreases in muscle activity in the upper, middle, and lower trapezius on the posttest (p < 0.05). There was a significant difference in serratus anterior at 150° (p < 0.05), but there was no significant difference between group in post-hoc analysis (p > 0.025). The internal rotation ROM was significantly increased in the stretching group compared to that in the joint mobilization group (p < 0.025). Conclusion: This study found that both joint mobilization and stretching for posterior shoulder tightness were effective in muscle activity during arm abduction, also in order to increase internal rotation ROM of shoulder joint, stretching was effective in patients with impingement syndrome with posterior tightness.

Purpose : The purpose of this study was to investigate the effects of Kaltenborn-Evjenth (KE) joint mobilization of the distal radioulnar joints (RUJ) and proximal RUJ in distal radius fractures (DRFs) on range of motion (ROM), grip strength (GS), and patient-written wrist evaluation (PRWE) in each group once, thrice, or fivefold. Methods : Forty-two subjects participated in this study. We divided the subjects with DRFs into groups applying KE concepts RUJ mobilization once, thrice, and fivefold. The patients' ROM and GS were measured using a joint goniometer and dynamometer, respectively. Pain and function were also assessed using a PRWE. In the statistical analysis, all data were tested for normality using the Shapiro-Wilk test, and paired t-tests were performed for within-group before-and-after comparisons of each intervention. One-way analysis of variance was used for between-group comparisons of differences. All statistical significance levels were set at α=.05. Results : There were significant differences in the ROM in all three groups before and after the intervention (p<.05), but there were no significant differences between the groups. There were significant differences in the GS in the three groups before and after the intervention (p<.05), but there were no significant differences between the groups. In the pain part of the PRWE, all three groups had significant differences before and after intervention (p<.05), but there was no significant difference between the groups. In the functional part of the PRWE, there were significant differences in the three groups before and after intervention (p<.05), but no significant difference occurred between the groups. Conclusion : Based on the aforementioned results, there were no significant between-group differences in ROM, GS, and PRWE (pain and function) after the application of the K-E joint mobilization to DRFs once, thrice, and fivefold. Nevertheless, there were significant within-group differences in all the above.
This paper tries to examine whether the application of joint mobilization to subjects who have the forward head posture due to malalignment in the cervical joint has influence on posture changes and functions in the cervical joint. The subjects were 39 students from G University in Gyeongsangbuk-do. The cervical joint mobilization was applied to 20 subjects and not to 19. The students with a cervical lordosis angle of $21^{\circ}C$ or less, an anterior weight bearing (AWB) of 15mm or greater, and a cervical extension ROM of $70^{\circ}C$ or less in terms of radiography were selected as subjects under their voluntary agreement. The patients actively performed the joint mobilization slowly 8 times per session while therapists continuously applied sustained accessory glide to their painful joints 3 times per week for 4 weeks along with the cervical expansion and flexion in SNAGS among other Mulligan's (1995) techniques. The measurement was carried out in terms of radiographic inspection and neck disability indexes. As a result of the experiment, it turned out that the subjects with the forward head posture had changes in the cervical AWB and ARA, the ranges of expansion and flexion, and the NDI(Neck Disability Index) after the intervention for the experimental group by applying cervical joint mobilization. There were no changes observed in the control group. In conclusion, the application of joint mobilization turned out to have influence on the improvement of cervical joint postures, and craniocervical region functions.
The purpose of this study was to investigate the effect of scapular stabilization exercise and thoracic joint mobilization on scapular position, scapular motion and shoulder pain and disability index(SPADI) in adults with scapular dysfunction. A total 42 subjects were divided into a scapular stabilization exercise group(A, n=14), a thoracic joint mobilization group(B, n=14) and scapular stabilization exercise with thoracic joint mobilization group(C, n=14) and performed exercise and joint mobilization three times per week for four weeks, total 12 times. As for changes in scapular position among the groups, group C showed statistically significant increase(p<.05). In changes of scapular motion among the groups, group C showed statically significant improvement in shoulder abduction 45° and 90° both(p<.05), In changes of SPADI among the groups, group C were showed statistically significant improvement(p<.05). In conclusion, scapular stabilization exercise and thoracic joint mobilization were effective on improving scapular position, movement and SPADI score in adults with scapular dysfunction.
