반응성 애착장애아와 발달성 언어장애아의 의사소통 의도 비교 연구 (A COMPARISON STUDY ON THE COMMUNICATIVE INTENT OF CHILDREN WITH REACTIVE ATTACHMENT DISORDER AND DEVELOPMENTAL LANGUAGE DISORDER)

  • 본 연구는 반응성 애착장애(reactive attachment disorder:향후 RAD라 칭함)집단과 발달성 언어장애(developmental language disorder:향후 DLD라 칭함)집단을 대상으로 외관상 유사한 사회성 문제를 지니고 있으나, 타인과의 사회적 접촉의 시도라는 좀 더 근본적인 의사소통의 의도에서는 집단간에 어떤 차이를 보이는지 알아보기 위해 생활 연령과 언어 연령으로 두 집단을 짝지어 의사소통 의도의 빈도와 주로 사용하는 의사소통 의도의 유형 및 발달 단계를 비교하였다. 그 결과는 DLD아동의 의도 표현 방법이 RAD아동보다 세련되고 정교화 되었음을 보여 주었으며, DLD아동이 RAD아동보다 의사소통하려는 의도를 더 많이 가지고 있음을 나타냈다. 그리고 두집단이 사용하는 의사소통 의도의 내용을 살펴보았더니, DLD아동은 사회적 상호작용>공유적 주의>행동 통제의 순으로, RAD아동은 행동 통제>사회적 상호작용>공유적 주의의 순으로 의사소통하려는 의도를 나타냄으로써, 주로 사용하는 의도의 내용 범주간에도 두 집단간에 차이를 보였다. 또한, 사용한 의사소통 의도의 다양성 비교에서도 총 12가지의 의사소통 의도에서 DLD아동이 RAD아동보다 더 다양한 수의 의사소통 의도를 나타냈다. 이들 결과를 종합해 볼 때, DLD아동보다는 RAD아동에게서 의사소통의도로 잰 사회적 결함의 심각성이 더 드러났음을 알 수 있다.

FCST의 음양균형장치를 활용한 뚜렛장애의 증례보고 (Tourette's Disorder Cases Managed by Yinyang Balancing Appliance of FCST, a TMJ Therapy for the Balance of Meridian and Neurological System)

  • Objectives: Therapeutic effect of Yinyang Balancing Appliance of functional cerebrospinal therapy (FCST) for meridian and neurologic yinyang balance was observed in Tourette's Disorder. Methods: Two Tourette's Disorder cases were managed with the Yinyang Balancing Appliance on temporomandibular joint (TMJ), combined with acupuncture. Clinical outcome measurement was based on subjective measures and clinical observations. Results: The patients showed positive changes even after the initial treatment and this effect maintained over the follow-up period. Conclusions: Although it is not clear the effect is a sustaining or temporary in its nature, a positive effect was observed and further clinical and biological research on FCST is expected.

  • Objectives : This study was carried out to investigate the effects of Chuna & General Oriental Method on Temporomandibular Joint Disorder Patient(Functional Lateral Mandibular Displacement) with Many Clinical Symptoms. Methods : The therapies were Chuna, acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring the length from the end of frenulum labii superioris to the frenulum labii inferioris, Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements: the length from the end of frenulum labii superioris to the frenulum labii inferioris changed from 10-7mm to 0-2mm, the VAS was changed from 10 to 0-2 and many clinial symptoms improved. Conclusions : These results suggusted that Chuna & General Oriental Method effected for Temporomandibular joint disorders caused many clinical symptoms.

하악전돌증 환자의 악교정 수술후 하악과두의 위치변화에 대한 연구 (A CEPHALOMETRIC AND PANORAMIC ANALYSIS OF THE CHANGES OF THE CONDYLAR POSITION AFTER ORTHOGNATHIC SURGERY)

  • Purpose: This study was aimed at analysis of the changes in the condyle position in subjects with mandibular setback osteotomies Method: Twenty patients were evaluated retrospectively for their changes in the condyle position who underwent surgical mandibular setback using bilateral sagittal split osteotomies with a manual condyle repositioning technique and rigid fixation. The cephalometric and panoramic analysis was performed preoperatively, 1 week, 6 months, and 1 year postoperatively. And postoperative noise, temporomandibular joint pain, and mouth opening were clinically examined 2 months, 4 months, 6 months, 8 months, 10 months, and 12 months postoperatively. Result: The condyles rotated posteriorly and laterally immediately after surgery, and they returned to the preoperative position during follow-up period but it is not significant. The statistical analysis (Paired t-test) showed no significant effects in postsurgical stability. The changes in the condyle position didn't have a significant harmful influence on temporomandibular joint disorder. Conclusion: A careful surgical mandibular setback using manual condyle repositioning and fixation technique will move condyle minimally and that will decrease the relapse and temporomandibular joint disorder.

악관절 질환 환자에 대한 초기치료의 효과: 상담 및 투약

  • ㆍPurpose: This study was performed to investigate the initial conservative treatment for TMD patients using careful counselling and medication prospectively. ㆍMaterials and Methods: Careful counselling and medication were performed in 51 TMD patients and 27 patients had follow-up check 2 months or more. Diagnosis of TMD was based on medical history and, physical and radiographic examination. TMD included masticatory disorder, internal derangement, degenerative joint disease, inflammatory joint disorder. and problems resulting from extrinsic trauma. All patients had chief complaints of TMJ pain, mouth-opening limitation. joint noise, and/or referred pain. We counselled and explained to the patient about the pathogenesis, etiologic factors, diagnosis and treatment plan for abut 10 minutes. We prescribed nonsteroidal anti-inflammatorv analgesic(Somalgen) and amitriptyline 10mg per day for 2 weeks. We informed the patient of the attention sheet and taught self-exercise of jaw. The patient were assessed by answering the questionnaire of subjective evaluation of TMD & maxillofacial pain. Questionnaire of an activity limitation. Questionnaire of a jaw function, and Questionnaire for the evaluation of TMD. ㆍResults: In questionnaire for the evaluation of TMD, 88.5% of 26 patients answered that the treatment was efficacious. 71.4% of 21 patients answered no problem in everyday life. There were significant differences between pretreatment and final follow-up in the evaluation of the subjective pain in the following sections: opening widely, chewing, resting, morning, masticatory muscle, and temporal portion(SAS program, paired T-test, P = 0.05). ㆍConclusions: Considerate counselling and proper medication could be significantly effectve in the initial treatment of TMD.

대학생의 생활 스트레스가 측두하악관절 장애에 미치는 영향 (Influence of life stress on Temporomandibular joint disorders in undergraduate students)

  • Purpose: The purpose of this study was to investigate the factors contributing to temporomandibular joint disorder symptoms. Methods: The survey was conducted from May 1 to June 10 2017 against college students(120 males and 123 females) in Daejeon and Gangwon who understood the purpose of this study and agreed to participate. Results: First, The most frequent symptom of temporomandibular joint disorders is noises from the joint (male 45%, female 61.8%), followed by headache or neck pain (male 25.8%, female 52.8%). There was shown a significant difference by gender in all items except for jaw dropping, poor alignment of teeth and trauma(p<0.05). Second, Looking into the correlation between stress and subjective symptoms of Temporomandibular joint disorders, The stress of Temporomandibular joint disorders showed more significant relation with realistic issues such as academic task, economic burden (e.g., treatment cost), future career, and life values rather than aspect of social relationship(p<0.01). Conclusion: As a result of analyzing variables to understand the influence of stress on temporomandibular joint disorders, the accumulated number of bad habits, stress and temporomandibular joint disorders showed a significantly positive correlation with one another (p<0.05).   • This study was intended to review comprehensively the articles related to temporomandibular joint disorder patients and to analyze psychoanalytically the case with temporomandibular dysfunction and bruxism as a symptom of hysterical conversion disorder. As a result, the author found that , in the aspect of psychoanalysis, the temporomandibular problem and bruxism was a kind of symptom of emotional conflict being converted to the face. I would like to emphasize that the referral process and the ability to work as a therapeutic team including dentist and psychiatrist can enhance the probability of successful treatment for several types of patients.

  • Fibromyalgia is a syndrome characterized by chronic pain in the skeletal system accompanied by stiffness, sleep disturbance, fatigue, and psychiatric problems, such as anxiety and depression. Fibromyalgia commonly affects orofacial health, presenting with a variety of oral manifestations, including temporomandibular disorder, xerostomia, glossodynia, and dysgeusia. Therefore, oral healthcare providers need to be aware of this clinical entity to effectively manage oral symptoms and provide proper oral self-care modification and education on the nature of fibromyalgia. This review focuses on the epidemiology, pathophysiology, clinical manifestation, diagnosis, orofacial concerns, and treatment of fibromyalgia.

  • Objectives The purpose of this research is to investigate recent clinical studies of Temporomandibular Joint Disorder in China. Methods We investigated recent clinical studies of Oriental Medicine therapies in traditional Chinese medical journals for Temporomandibular Joint Disorder through CNKI search. 20 clinical articles published from 2010 to 2015 were analyzed. This study examined the authors, published years, types of study designs, criteria for diagnosis and evaluation, periods, purposes of study and classified articles by techniques of treatment. Results Most of articles were classified as RCT. TMD was diagnosed by symptoms in a high proportion of articles. The criteria for evaluation that most frequently used were grading scale, but there was a lack of objectivity. The techniques of treatment were Tuina, acupuncture, herb medicine, electrotherapy, splint, PNF, congnitive behavior therapy, laser therapy, magnetic therapy. Conclusions In order to develop treatment of TMD in the Korean medicine, clinical studies for various therapies on a high level and cooperative studies between medical communities are needed.
