The Effectiveness of Health Promotion Program for the Elderly (노인 건강증진 프로그램의 효과 분석)

  • Purpose: This study was conducted in order to identify the effect of a community based health promotion program for elders in Korea. Methods: The PICO-SD (Participants, Intervention, Comparison, Outcome, and Study Design) strategy was established. A search of the electronic bibliographic database of NDSL, RISS, KMbase, and KoreaMed etc. was performed and 28 articles met inclusion criteria. Results: Both exercise and health education were in most programs and were more effective than one of the two. The health education included stress management, mental health, health promotion lifestyle, chronic disease, and medication. Various exercises such as walking, stretching, gymnastics, rhythmic activity, muscle strength, and joint exercise were performed. The programs for elders showed an association with significantly improved muscular strength, flexibility, depression, quality of life, subjective health status, satisfaction with life, self-efficacy, and health promotion behavior. Conclusion: Intervention programs including both exercise and health education are effective in improving health promotion behavior and physical and psychological health status in elders. Therefore, these results could provide useful guidelines for development of effective health intervention programs for the elderly.

  • Helicobacter pylori plays an important role in the pathogenesis of chronic gastritis, peptic ulcer disease, gastric cancer, and gastric mucosa-associated lymphoid tissue lymphoma. In Korea, the guidelines for the diagnosis and treatment of H. pylori infection in adults were revised in 2013. The European Helicobacter and Microbiota Study Group and Consensus panel released the fifth edition of the Maastricht Consensus Report for the management of H. pylori infection in 2015, and the European Society of Paediatric Gastroenterology, Hepatology and Nutrition and the North American Society of Paediatric Gastroenterology, Hepatology and Nutrition released the updated joint guidelines for children and adolescents in 2016. Considering these recommendations and recent progress in our research and that of other research teams, this study aimed to discuss the diagnostic strategies for H. pylori infection in children and adolescents.

  • The role of botulinum toxin as a therapeutic agent is expanding rapidly in otolaryngology. Botulinum toxin is a protease that blocks the release of acetylcholine from nerve terminals. Its effects are transient and nondestructive, and largely limited to the area in which it is administered These effects are also graded according to the dose, allowing for individualized treatment of patients and disorders. Botulinum toxin has been used primarily to treat disorders of excessive or inappropriate muscle contraction. In the field of otolaryngology, these include spasmodic dysphonia, oromandibular dystonia, and blepharospasm, vocal tics and stuttering, cricopharyngeal achalasia, various tremors and tics, hemifacial spasm, temporomandibular joint disorders and a number of cosmetic applications. Botulinum toxin treatment has recently begun to show some benefit in the control of pain from migraine and tension headache. It may also prove useful in the control of autonomic dysfunction, as in Frey syndrome, sialorrhea, and rhinorrhea. In over 20 yews of use in humans, botulinum toxin has accumulated a considerable safety record, and in many cases represents relief for thousands of patients unaided by other therapy.

Surgical Strategies for Prevention and Treatment of Airway Aspiration in Head and Neck Cancer Patients (두경부암 환자에서 기도 흡인의 예방과 치료를 위한 수술 전략)

  • Postoperative airway aspiration is not uncommon in patients with head and neck cancer. Airway aspiration has serious consequences, such as swallowing disorders, nutrition-related health problem, or reducing the quality of life due to maintenance of tracheal or nasogastric tubes. The postoperative oropharyngeal defect due to the surgery may interfere with normal swallowing reflex, or the laryngeal dysfunction caused by radiation therapy may cause severe airway aspiration, which may lead to complications such as dyspnea and pneumonia. Complete removal of the disease is also important in the treatment of head and neck cancer, but it is necessary to select a method to avoid and predict the occurrence of airway aspiration according to the treatment method. The most important factor to prevent airway aspiration after surgery is to preserve the proper volume of the oropharynx and to preserve at least one of the cricoarytenoid joint function. It is also the most effective way to reduce additional complications by seeking appropriate surgical treatment according to airway aspiration status. The purpose of this study is to review the operative methods that can induce airway aspiration and consider the prevention and treatment strategy through review of the literature.

  • In order to understand Changeologically on the meaning of FCST's TMJ (temporo-mandibular joint) treatment procedure, Yi, Shike, Bi, Gen trigrams concerning the jaw and change by treatment were analyzed from the viewpoint of semiotic context of hexagon and holistic interpretation on disease. Yi is meant by jaw but actually indicates mouth made by maxilla and mandible, and it's characters are related with nourishing by aliment and words. But when we eat and speak in the daily life, jaw does not nourish properly it's own body by bad habit or postures. For the treatment of this ill state, there needs punishment and correction symbolized with Shike. Shike has fourth nine meaning obstacles between the two strong lines in the upper and lower end, and so it has the function of mastication and get rid of the fourth nine metaphorically indicating subluxation of axis using CBA and auxiliary measures of four movement or laughing methods. Bi expresses the achievement and effects of consecutive mastication process implicating normalized manifestation of jaw and its linked spinal function. Gen symbolizes removing selfish motive or partiality in advance and reaches the best state of the saint righteously self-nourishable human being.

  • Health care big data is thought to be a promising field of interest for disease prediction, providing the basis of medical treatment and comparing effectiveness of different treatments. Korean government has begun an effort on releasing public health big data to improve the quality and safety of medical care and to provide information to health care professionals. By studying population based big data, interesting outcomes are expected in many aspects. To initiate research using health care big data, it is crucial to understand the characteristics of the data. In this review, we analyzed cases from inside and outside the country using clinical data registry. Based on successful cases, we suggest research method for evidence-based Korean medicine. This will provide better understanding about health care big data and necessity of Korean medicine data registry network.

  • Headache disorders, one of most common disease in general population, have been developed according to many versions of international classifications. The primary headaches are those in which no consistently identified organic cause can be determined. It is divided into the following categories: (1) migraine, (2) tension-type headache, (3) cluster headache and other trigeminal autonomic cephalalgias, (4) other primary headaches. This review described a diagnosis of primary headache disorders based on International Classification of Headache Disorders (ICHD)-3 beta criteria.

Lumbar Synovial Cyst Associated with Retrolitheis -A Case Report- (척추 후방 전위증을 동반한 요추부 활막낭종 1례)

  • In advant of high-resolutional CT and MRI, it is not so difficult to detect the lumbar synovial cyst, however the unusual disease should be included in the differential diagnosis of cauda equina compression syndromes. The case of a 55-year-old man who had a low back pain and severe radiating pain on both legs showed a retrolisthesis at L3-4 and L4-5 and epidural enhansing mass just around L4-5 facet joint on MRI. Microscopically a lining of synovial tissue was demonstrated. After surgery of the total resection of synovial cyst and stabilization, the patient's presenting symptoms were improved.

  • I. PURPOSE AND SIGNIFICANCE OF RESEARCH The presentation of research direction for compositive study and application of oriental medicine is urgently demanded. Since there is few investigation for the interest in basic and clinical field of oriental medicine, and we hope that compositive study and concentrative research between instruction in college and local clinics is established by this research. II. RESEARCH CONTENT ${\cdot}$ Analysis of survey in basic field of oriental medicine as instruction curriculum. ${\cdot}$ Analysis of survey in clinical field of oriental medicine as the classification of disease. III. RESULT The result of investigation shows that the most interested field is acupuncture and moxibustion in clinical field and diagnostics in basic field of oriental medicine. And oriental medicinal clinic doctors are concerned about prescription and a newly-developed techniques such as new acupuncture therapy, new machines. They hope that a study of oriental medicine in clinical field be accomplished with the special clinics as leader. According to the previous results, we can work out the foundation of compositive study and concentrative research between instruction in college and local clinics, and on this ground it is considered that we can grope for the presentation of research direction of local clinics.

Malignancies in children who present with bone pain (뼈의 통증이 있을 때 감별해야 할 악성종양)

  • Bone pain in a child could be associated with cancer as an initial manifestation of the disease. The childhood malignancies that frequently present bone pain are leukemia, neuroblastoma, and primary bone tumors such as osteosarcoma and Ewing sarcoma. Persistent bone or joint pain associated with swelling, mass, or limitation of motion implies underlying serious causes. Systemic manifestations such as lymphadenopathy, hepatosplenomegaly, fever, fatigue, night sweat, and laboratory abnormalities are also suggestive of malignancy. The index of suspicion tends to be low since less than 1% of children who complain of bone pain are diagnosed as cancer. Nonetheless, pediatricians should be alert to the possibilities of cancer since early detection and prompt treatment might reduce mortality.
