부산지역(釜山地域) 개업치과(開業齒科)의 치과보철물(齒科補綴物)에 관(關)한 만족도(滿足度) 조사(調査)(I) - 일부정식 치과 보철물에 관하여 - (A Study on Satisfaction of Practicing Dentists about Prosthesis in Pusan Part I : About Fixed Prosthetic appliances)

  • Fixed prosthetic appliance is the restorations of damaged teeth with cast metal or porcelain, and of replacing missing teeth with fixed, or cemented prosthesis. Successfully treating a patient by means of fixed prosthodontics requires the thoughtful combination of dentist and dental laboratory technician. In order to know the satisfaction of dentisits about fixed prosthetic appliances-porcelain, precious and non precious metal-, this study was conducted for 69 practicing dentisis in Pusan during Fed. to Mar. 1988 using a questionnaire method and was analyzed by the use of percentage, $X^2$-test The results are obtained as follow: 1. General characteristics of the respondents, The rate of age distribution of practcing dentists responded was as follow : 31$\sim$35 age group is 29.0%, 51$\sim$55age group 18.8%, 41$\sim$45 age group and 46$\sim$50 group each 11.6%, 36$\sim$40 age group and 56$\sim$60 age group 8.7%, over 60 age group 7.3%, below 30 age group 4.3% respectively. The tate of term of practice experience distribution of dentists responded was as follow : 6$\sim$10 year group 26.1%, 25$\sim$25year group 18.8%, 11$\sim$15 years group and 16~20 years group each 13.0%, below 5 years group and over 30 years group each 10.2%, 26$\sim$30 years group 8.7% in order. The rate of respondents’ degree was follow : bachelor group 81.2%, doctor group 13.0%, master group 5.8% in order. The rate of respondents’completed medical specialist course was as follow : non-complete group 75.4%, complete group 24.6%. The rate of having own laboratory in respondents’clinic was as follow : no having own laboratory group 58.0%, having own labiratory group 42.0% The rate of being lab. technician in respondent;s clinic was as follow: non-being lab. technician group 91.3%, being lab. technician group 8.7%. The rate of transactional commercial dental lab. numbers was as follow : I dental lab. group 40.6%, 2 dental lab. group 30.4%, 3 dental lab. group 20.3%, production at own lab. group 8.7% 2. Cognitions about the respondent's job, About cognition of patient numvers, there were neither many nor few group 62.3%, few group 17.4%, many group 15.9%, too many group 4.4% in order.(P<.01) As compared with the ratio of dentists to patient prothetic treatment, there were moderate level group 46.4%, high level group 33.6%, low level group 18.8% in order.(P<.01) By job satisfaction of respondents, the highest percentage group was satisfied group 44.9%, neither astisfied nor disstified group 33.3%, dissatisfied group 14.5%, much satisfied group 7.3%(P<.01). As compared with the level of prosthetics price, the were moderate level group 71.0%, low level group 23.2%, high level group 5.8%(P<.01) 3. Satisfaction and assessment of dissatisfied factor of the fixed prosthetic appliance, As compared with the satisfaction of al prosthetic appliance, there were neither satisfied nor dissatisfied group 59.4%, satisfied group 20.3% dissatisfied group 18.8%, much satisfied group 1.5% in order.(P<.01). About precious and non precious metal crown and bridge, there were neither satisfied nor dissatisfied group 60.9%, satisfied group 24.6%, dissatisfied group 11.6%, much satisfied group 2.9% in order.(P<.01) The reat of respondent;s satisfaction about porcelain crown and bridge was as follow : neither satisfied nor dissatisfied group 43.5%, dissatisfied group 33.3%, satisfied group 21.7%, much satisfied group 1.5%(P<.01). The rate of respondent's dissatisfied factor item of precious and non precious metal crown and bridge was as follow : inadequate margin fit item 39.1% respondent, inadequate occlusion item 24.6%, inadequate adaptation item 23.2%, poor anat omic form item 15.9%, inadequate contact item 31.9%, improper polishing item 4.4%, the rest item 11.6% respectively. The rate of respondent's dissatisfied factor item of porcelain crown and bridge was as follow : inadequate margin fit item 52.2%, inadequate occlusion item 2.9%, inadequate adaptation item 10.1%, poor anatomic form item 11.6%, improper polishing item 1.5%, shade mismatching item 71%, the rest item 7.2% respectively. The rate of cognition of respondents’ about cause of dissatisfied factor on fixed prosthetic appliance by item was as follow : deficiency of sincerity item 55.1%, deficiency of knowledge about work item 47.8%, deficiency of experience item 15.9%, poor quality of using material item 5.8%, the rest item 2.9% respectively.

개발제한구역 관리에 대한 공무원 인식 분석 - 서울특별시를 대상으로 - (A Study on the Analysis of Officials' Cognition on the Management of Green Belt - With Special Reference to Seoul Metropolitan City -)

  • 본 연구는 개발제한구역 관리에 대한 담당 공무원들의 인식을 실태조사하여 향후 개발제한구역 관리제도 개선에 대한 기초 자료 제시를 목적으로 수행되었다. 사례조사는 2000년도 이래 서울특별시 개발제한구역 관련 발생된 민원분석과 공원녹지 담당 등 개발제한구역 관리 관련 공무원을 대상으로 본청 3개과, 25개 구청, 1본부, 5개 사업소의 관련 공무원을 대상으로 수행하였다. 연구과정은 1단계로 개발제한구역 관련 민원 접수 내용을 분석하고, 2단계로 그 내용을 중심으로 심층 면접하였다. 3단계로 1, 2단계에서의 도출된 문제점을 문항으로 하여 설문조사를 실시하여 분석한 결과, 다음과 같은 연구결과가 도출되었다. 첫째, $2000{\sim}2005$년까지 서울특별시에 제기된 개발제한구역 관련 민원은 시설물 및 건축물의 신 증 개축에 관련된 것이 가장 많았고, 무단적치, 토지형질변경 등의 순서로 빈도가 높았다. 둘째, 공무원 인터뷰 결과, 이러한 3가지 종류의 문제가 관리상 어려움이 많은 것으로 나타났다. 셋째, 특히 관리상 문제 가 많이 발생하는 점은 축사. 콩나물 재배사 등 농 수산용 시설물 및 건축물의 불법전용, 농 수산물 가공창고의 타 용도로의 불법 전용, 주택 또는 근린생활시설 건축 시 필로티(pilotie)를 건축하고 이를 불법적인 타용도로 전환하는 행위, 넷째, 불법적 토지형질 변경 및 무단 적치 등이 심각한 것으로 나타났다. 이러한 연구 결과는 개발제한구역의 지정 및 관리에 관한 특별조치법 제12조 제1항 제1호 나목과 다목, 제4호, 제7호, 제8호 등과, 이 와 관련한 동법시행령 제13조(허가대상 건축물 또는 공작물의 종류 등)와 관련하여 농 수산업용 창고 등 허가 시 '임차 토지는 경작 면적에 미포함 시켜야 한다는 점', 또한, '농 수산업용 창고 허가 시 경작 면적이 일정면적 미만일 경우엔 불허가해야 한다는 점', 특히, '농 수산업용 창고 등 허가 시 $10,000m^2$ 이하의 경작 면적지에 대해서도 면적 기준에 따라 창고의 허용 면적을 지정해야 한다는 점'에 대한 개선이 논의되어야 할 필요성이 있음을 시사하고 있다.

서울시 보건소 물리치료사들의 지역사회중심재활에 대한 인식 (A Recognition for community Based Rehabilitation by Public Health Center Physical Therapists in Seoul Province)

  • To survey the recognition of Community Based Rehabilitation(CBR) by Public Health Center(PHC) Physical Therapists in Seoul Province. This study analysed general characteristic of Physical Therapists and cognition, participation, education, problems and requirements for CBR. The subjects of this study were 31 therapists who work at PHC in Seoul Province and Seoul City Childerns Municipal Hospital. Data was collected for 18 days from April 13 to 31, 1998 and results computed from 25 responses. First, Female therapists outnumber males. The age category 30 to 35 old represented 44.0% of the total number studied. 48.0% of therapists had worked more than 7 years and therapists usually 84.0% worked from 3 to 4 years at the PHC. Junior colleage graduates were 72.0%, and respondents with family responsibility were 52.0%. Second, Recently pain control and therapeutic exercises has become the major requirement of patients at PHC(56.0%). If the offered expanded services, a rehabilitation programe at PHC(36.0%) was highly. When asked about a working guide book, nobody had one. Third, 64.0% of therapists were aware of the present level of CBR, 28.0% of them had gotten an information from newspaper and mass media. Fourth, For CBR, 72.0% had positive ideas in which they could participate. 56.0% wished to compose a team of various rehabilitation specialists. 52.0% wished for the meeting day to be flexible and 64.0% desired to meet once or twice a week. Fifth, Almost all(92.0%) desired more educational opportunity about CBR. Example, once or twice a year(48.0%) and training period of 4 to 7 day(44.0%). They wanted to learn theory and parctice together(88.0%) and 48.0% wanted national mutirehabilitation center as a educational managing organization. Sixth, The most important requirement for accomplishing CBR by Physical Therapists was job security and adequate number of therapists(48.0%). Required number of physical therapists in the PHC was 3(52.0%). The cooperative agency should be the distict govement(32.0%) and community welfare center of the disabled persons(32.0%). Factors inhibiting the execution of CBR were lack of physical therapists(56.0%), and equipments for pain control and for therapeutic exercises(68.0%).

임상 간호사의 당뇨병에 대한 지식, 중요도 및 인지도 조사 (Research into Clinical Nurses' Knowledge, Importance and Perception of Diabetes Mellitus)

  • The purpose of this study is to determine the clinical nurses' knowledge of DM and diabetes patient education aptitude so as to provide a basis for clinical nurse training with respect to diabetes patient education program. The data has been collected through the questionnaires of 42 items from the Knowledge on DM and each 16 item from the Importance on the elements of diabetes patient and the Recognition for clinical nurses, respectfully. 166 nurses from a general hospital who had participated in clinical nurse training in Sep. 28 and Oct. 4, 2001 were subject to respond the questionnaires. Analysis has been done by using statistical method such as percentage, average, standard deviation, t-test, ANOVA, Duncan test and Pearson correlation coefficients. The findings are as follows: 1. Clinical nurses' knowledge levels of diabetes 1) The average level of nurses' knowledge about diabetes is 29.37 (right answer- finding rate: 70%), which is intermediate. 2) Amongst the nurses classified by their knowledge levels about diabetes, the group with less than one year career and that with more than 5 year careers are found to have higher knowledge levels. The item with the highest right answer-finding rate was 'Please find the wrong out of the following examples about foot care'. Meanwhile, the item with highest incorrect answer-finding rate was 'what does blood sugar control aims for amongst gestational diabetes?'. 2. Clinical nurses' importance and perception levels of educational training about diabetes 1) There were no differences amongst nurses' importance level about diabetes. 2) Nurses usually had high scores(4.30) in terms of the items related to the importances about educational training. 3) There were quite high recognitions of general characteristics and symptoms about diabetes, amongst the nurse cohorts working more than one year and less than 5 years, and over 5 years, the group belonging to the internal department, that having the previous experiences of dealing with diabetes, and that having their diabetic relatives and other close people. Meanwhile, strangely, the group who identified themselves as 'not good at treating diabetes' had a high recognition level of educational training about diabetes. 3. Relationship between knowledge levels and importance & perception levels of diabetes 1) The higher knowledge about diabetes nurses had, the more importance they recognized. 2) It is found that there was no relationship between knowledge and perception of diabetes. 3) The more importance about diabetes nurses had, the higher perception they obtained. In conclusion, there is an urgent need for systematic educational programs about diabetes including technical aspects, in order to upgrade and improve nurses knowledge levels. In addition, re-educational training should be provided at regular intervals. Further, we believe the nurses with high knowledge about diabetes and interests in the provision of educations for patients can be far more confident, and in return, patients can have better self-management about diabetes obtained through educations. Based on the above-mentioned findings, we would like to make the suggestion: re-evaluation about nurses' knowledge and cognition levels should be carried out after job training programs about diabetes.

고등학생의 고령화사회와 고령친화산업에 대한 인식 (A Study on the High School Students' Cognition of the Aging Society and the Silver Industry)

  • 본 연구는 고등학교 학생들을 대상으로 고령화사회와 고령친화산업에 대한 인식도를 조사하여 다가오는 초고령 사회의 충격에 대비하기 위한 교육의 방향 설정에 기초 자료를 제공하고자 하는데 목적이 있다. 이를 위해 광주광역시 고등학생 642명을 대상으로 설문조사를 실시하였으며, 수집된 자료는 SPSS/PC WIN 14.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차를 산출하였고, $x^2$과 t-test로 분석하였다. 먼저, 노인에 대한 인식은 가족 관계 영역과 심리적 영역에서 대체로 긍정적인 반면 신체적 영역에서는 다소 부정적으로 나타났고 고령화사회에 대한 지식은 전반적으로 낮은 수준이었다. 성별과 조부모 접촉 경험에 따른 고령화사회에 대한 인식의 차이를 살펴보면 노인에 대한 인식은 조부모 방문 횟수에 따라 유의한 차이가 나타났고 고령화사회에 대한 지식은 성별에 따라 유의한 차이가 나타났다. 둘째, 고령친화산업에 대한 인식은 고령친화산업이 필요하다고 생각하고는 있으나, 고령친화산업을 본인의 직업으로는 선호하지 않는 것으로 나타났고 고령친화산업 중 가장 발달되어야 한다고 생각하는 분야는 '요양 서비스' 분야를 선호하였다. 고령친화산업에 대한 인식의 차이를 살펴보면, 고령친화산업의 필요성과 전망, 발달되어야 한다고 생각하는 고령친화산업 분야는 성별에 따라 유의한 차이가 나타났고, 고령친화산업 분야의 직업 선택 의사는 조부모 방문 횟수에 따라 의한 차이가 나타났다. 셋째, 고령화사회 및 고령친화산업에 대한 교육 방향은 노인관련 교육의 필요성을 대부분 느끼고 있었고 기술 가정과에서 직업 교육과 노인 이해 교육을 함께 실시해야 한다는 의견이 높게 나타났다. 고령화사회 및 고령친화산업에 대한 교육 방향의 차이를 살펴보면, 고령친화산업 발달에 대비한 노인 관련 교육의 필요성에 대해서는 성별에 따라, 고령친화산업 관련 교육 내용에 있어서는 성별, 조부모 동거 경험, 조부모 방문 횟수에 따라 유의한 차이가 나타났다.

The results are abtained as follow- 1. By age The degree of Satisfaction of items about all graduate dental technician were "41$\sim$45 age" group 26.53%), "46$\sim$50 age" group 22.45%, "36$\sim$40 age" group 18.36%, "31$\sim$35 age" group 12.24%, "51$\sim$55 age" group and "56$\sim$60 age" group 8.16% 60$\sim$age group 4.08% in order. 2. By Job Career. The Highest degree item out of Satisfaction about all graduated dental dental technician was 21$\sim$25 years group 28.5%, 16$\sim$20 years 20.4%, 11$\sim$15 years group 12.24%, 26$\sim$20%, 6$\sim$10 years group 4.08%, below 5 years group 2.04% in order. 3. By managing term of respondent's dental laboratory. The improvement policy and prblematic issues presonted by the owner and operatore of atriticial dental manufacturing plants are as shown below First; The viewpoint that they contribute to the improvement of people's oral health in terms of their mission as artificial dental technicians. Second: The cultivation of basic technical ability to cope with clinical practice upon graduation Third; They require guidance in study and research in their filed of profession, that tray may beable to estaclish a from theory.

By born place, the dental laboratory heads’ satisfactory degree mean of items about all dental technician who graduated from junior college were Pusan group 3.55, Kyungnam group 2.94. The difference among the groups was significant enough(P<0.05). The satisfactory degree about woman dental technician graduated from junior college were Pusan group 3.45, Kyungnam group 2.72. 2. By age, The degree of satisfaction of items about all graduate dental technician were 30$\sim$34 age group 3.13, 40$\sim$44 age group 2.71. The degree of satisfaction about woman graduate dental technician were 30$\sim$34 age group 3.13, 40$\sim$44 age group 2.71. The degree of satisfaction about woman graduate dental technician were as follow : 35$\sim$39 age group 3.50, 45$\sim$49 age group 3.16, 30$\sim$34 age group 2.86, 40$\sim$44 age group 2.43. 3. By job career, the highest degree item out of satisfaction about all graduate dental technician was 6$\sim$10 years group 3.58, the other items were 1$\sim$15 years group 2.90, 16$\sim$20 years group 2.71. The satisfactory degree item about woman graduate dental technician was 6$\sim$10 years group 3.43, 16$\sim$20 years group 3.15, 11$\sim$15 years group 2.82 respectively. 4. By managing term of respondents’ dental laboratory, the satisfactory degree of items about all graduate dental technician were below 2 years group 3.25, 3$\sim$6 years group 3.14, 7$\sim$10 years group 2.93 in order. The satisfactory degree items about woman graduate dental technician were 11$\sim$14 years group 3.25, 3$\sim$6 years group 3.14 7$\sim$10 years group 2.80 in order. 5. By acquired qualification of licence test, the satisfactory degree of dental laboratory heads about all graduate dental technician were job career group 3.07, graduated from Dept. of Dental Technology group 3.00. The satisfactory degree about woman graduate dental technician were job career group 3.03 and graduated from Dept. of Dental Technology group 2.57. 6. By dental technician numbers of respondents’ dental laboratory. the satisfactory degree of items was as follow: 10$\sim$13 persons group is 2.86, 6$\sim$9 persons group 3.25, below 5 persons group 3.11. There was significant enough.(P<0.01). The satisfactory degree items about woman graduate dental technician were below 5 persons group is 3.00, 10$\sim$13 persons group 2.79, 6$\sim$9 persons group 2.67. 7. by the outlook of occupation, the satisfactory degree of dental laboratory heads about all dental technicians who graduated from junior college were neither good norbad group 3.20 bad group 2.59, good group 3.13. The difference among the groups was significant enough(P<0.01). The satisfactory degree about woman dental technicains graduated from junior college were good group 3.25, neither good nor bad group 2.80, bad group 2.75. The difference among the groups was significant enough.(P<0.01) 8. By occupational satisfaction, The satisfactory degree of items about all graduate dental technician were neither satisfied nor dissatisfied group 3.08, satisfied group 3.17, dissatisfied group 2.89. As compared with the items ut of satisfaction and improvement about all dental technician who graduated from junior college, the rate of satisfaction item was for as follow : dental technological theory is 5.21%, ability of adaptation and sincerity each 17.1%. The rate of improvement items was as follow : sincerity is 39%, ability of work and ability of basical work each 24.4%. 12. As compared with the items out of satisfaction and improvement about woman dental laboratory technician who graduated from junior college, the rate of satisfaction item was as follow : atmosphere of the office is 56.1%, sincerity 22%. The rate of improvement items was as follow : ability of adaptation is 36.6%, ability of work 31.7%.

fMRI를 이용한 망간 노출 용접공의 운동수행에 따른 뇌 활성도 평가 (Cerebral Activity by Motor Task in Welders Exposed to Manganese through fMRI)

  • Objectives: The purpose of this study is to analyze the effects of chronic exposure by welders to manganese (Mn) through an analysis of the degree of brain activity in different activities such as cognition and motor activities using the neuroimaging technique of functional magnetic resonance imaging (fMRI). The neurotoxic effect that Mn has on the brain was examined as well as changes in the neuro-network in motor areas, and the usefulness of fMRI was evaluated as a tool to determine changes in brain function from occupational exposure to Mn. Methods: A survey was carried out from July 2010 to October 2010 targeting by means of a questionnaire 160 workers from the shipbuilding and other manufacturing industries. Among them, 14 welders with more than ten years of job-related exposure to Mn were recruited on a voluntary basis as an exposure group, and 13 workers from other manufacturing industries with corresponding gender and age were recruited as a control group. A questionnaire survey, a blood test, and an fMRI test were carried out with the study group as target. Results: Of 27 fMRI targets, blood Mn concentration of the exposure group was significantly higher than that of the control group (p<0.001), and Pallidal Index (PI) of the welder group was also significantly higher than that of the control group (p<0.001). As a result of the survey, the score of the exposure group in self-awareness of abnormal nerve symptoms and abnormal musculoskeletal symptoms was higher than those of the control group, and there was a significant difference between the two groups (p<0.05, respectively). In the correlation between PI and the results of blood tests, the correlation coefficient with blood Mn concentration was 0.893, revealing a significant amount of correlation (p<0.001). As for brain activity area within the control group, the right and the left areas of the superior frontal cortex showed significant activity, and the right area of superior parietal cortex, the left area of occipital cortex and cerebellum showed significant activity. Unlike the control group, the exposure group showed significant activity selectively on the right area of premotor cortex, at the center of supplementary motor area, and on the left side of superior temporal cortex. In the comparison of brain activity areas between the two groups, the exposure group showed a significantly higher activation state than did the control group in such areas as the right and the left superior parietal cortex, superior temporal cortex, and cerebellum including superior frontal cortex and the right area of premotor cortex. However, in nowhere did the control group show a more activated area than did the exposure group. Conclusions: Chronic exposure to Mn increased brain activity during implementation of hand motor tasks. In an identical task, activation increased in the premotor cortex, superior temporal cortex, and supplementary motor area. It was also discovered that brain activity increase in the frontal area and occipital area was more pronounced in the exposure group than in the control group. This result suggests that chronic exposure to Mn in the work environment affects brain activation neuro-networks.
