  • Nevoid basal cell carcinoma syndrome (NBCCS) is a rare autosomal genetic disease caused by a PTCH mutation. The disease is characterized by multiple basal cell carcinomas of the skin, multiple keratocystic odontogenic tumors (KCOTs) in the jaw, palmar and/or plantar pits, bifid ribs, ectopic calcification of the falx cerebri, and skeletal abnormalities. Early diagnosis is difficult in many cases because there may be a number of systemic symptoms. The purpose of this study is to report the case of a 12-year-old girl who was hospitalized with multiple KCOTs that occurred in the upper and lower jaws. Through characteristic clinical symptoms and radiologic findings, she was finally diagnosed as having NBCCS. This study also aims to organize the symptoms often observed in Korea using previously published case reports to provide useful information for the early diagnosis of NBCCS.

CAD/CAM splint based on soft tissue 3D simulation for treatment of facial asymmetry

  • Background: Most cases of facial asymmetry involve yaw deformity, and determination of the yaw correction level is very difficult. Methods: We use three-dimensional soft tissue simulation to determine the yaw correction level. This three-dimensional simulation is based on the addition of cephalometric prediction to gradual yaw correction. Optimal yaw correction is determined visually, and an intermediate splint is fabricated with computer-aided design and computer-aided manufacturing. Application of positioning devices and the performance of horseshoe osteotomy are advisable. Results: With this procedure, accurate repositioning of jaws was confirmed and patients obtained fairly good facial contour. Conclusions: This procedure is a promising method for a widespread, predictable treatment of facial asymmetry.

A HISTOLOGICAL STUDY ON THE CHANCES OF THE SURROUNDING TISSUE IN MANDIBULAR JOINT BY THE ORTHOPEDIC FORCE IN RATS (ORTHOPEDIC FORCE에 의한 백서악관절 주위조직의 변화에 대한 조직학적 연구)

  • Many different appliances have been used to move teeth or to attempt to inhibit or stimulate growth of the jaws. Especially, Orthopedic force was used to apply a constant force on the mandible. Various orthopedic forces bring the changes of surrounding tissue, growth and development on the mandibular joint. Author carried out this study to find the histological changes of the mandibular joint in rats when the mandible applied posterior displacement. For the purpose of this study, $009\times036inch$ closed coil spring and 030inch orthopedic wire were used between the post occipital region and the chin region in order to render the orthopedic forces, posteriorly. The experimental animals vu. used 1 month old (GroupI), 3 months old (Group II), and 12 months old (Group III) rats. Following results were obtained; 1. At the beginning of the experimental Group I, there were predominant increment of Cartilage layer in the posterior parts of condylar head. Especially, the mature chondrocyte zone was increased. 2. In the experimental Group II, the chondroblastic zone was decreased while it showed slightly increased mature cartilage zone. 3. In the experimental Group III, the mature chondrocyte zone was disappeared gradually and there was a tendency to reduce the endochondral bone formation in condylar head. 4. After 30 days of experimental Group I and in experimental Group II, there was a tendency to decrease the immature cellular zone in the glenoid fossa by pressure. 5. There were no histological changes in the articular disc by pressure.

CLINICAL AND RADIOLOGIC STUDY OF ODONTOGENIC KERATOCYST IN THE JAWS (악골에 발생한 치성각화낭종의 임상 및 방사선학적 연구)

  • The author has clinically and radiologically investigated 57 cases of odontogenic keratocyst in 47 patients consisted of 26 males and 21 females aged from.2 to 63 years, who were pathologically diagnosed as odontogenic keratocyst at infirmaries of dental colleges, Yonsei University and Seoul national university during 1965-1986. The results were as follows: 1. The peak incidence of the disease was on their teenagers (29.8%). The ratio of Male/Female was 1.23:1 and incidence rate of males showed higher than their counterpart. 2. The most frequent complaints were swelling in (65.9%) followed by pus discharge, unknown mass, pain, residual root. 3. The most common site was mandibular third molar and mandibular ramus region (15.8%) followed by mandibular body and ramus, mandibular third molar, mandibular anterior teeth. Incidence of this disease in mandible was higher than in maxilla. 4. The lesions not associated with adjacent teeth were (14.0%) and in the lesions associated with adjacent teeth (35.1%) showed root resorption, (50.9%) were without root resorption, (35.1%) showed tooth migration and (50.9%) were without tooth migration. 5. The border types of the lesions were scalloped type in (52.6%), smooth type in (47.4%) and morphological type were unilocular in (50.9%), multilocular in (49.1%). 6. The radiologic cyst type of the lesions were follicular type (42.1%) followed by primordial, unclassified odontogenic, residual, lateral periodontal, median mandibular, globulomaxillary type.

  • Odontodysplasia is a rare developmental anomaly of tooth formation in which hard tissue are affected. Odontodyplasia affects the primary and permanent dentitions in the maxilla, mandible, or both jaws. The maxilla is involved twice as often as the mandible. The condition is more common in female than in male patients and in the anterior than in the posterior regions. The clinical manifestation of odontodysplasia are hypoplasia and hypocalcification of the enamel and dentin of affected teeth. Teeth tend to be small and discolored, with short roots and widely open pulp canal. Delayed eruption of affected teeth with abscess formation is common. Radiographically teeth assume a faint radiolucent image ("ghost teeth"). enamel and dentin appear thin and are similar in radiodensity. The pulp chambers are often larger than normal, calcifications(pulp stone and denticle) are found within them. The etiology of regional odontodysplasia is unknown. However, several causes have been discussed, including somatic mutation, local circulatory disorders, local trauma, failure of migration and differentiation of neural crest cells, local infection.

  • The data about dental caries obtained from 1438 preschool children (male 797, female 641) in Seoul aged from 3 to 6 were analyzed by their sexes, ages, jaws and teeth surfaces. The results were as follows. 1. dmf rate : 92.63% 2. dmft index : 6.06 dmft rate 30.81 % dmfs index : 11.82 dmfs rate 12.02 % 3. d,m & f rate d rate: 79.45% m rate: 8.15% f rate : 12.40 % 4. dft index : 5.94 5.   • The author have studied on the pulp responses to the electric pulp stimulator (Pyo-Cure)(R) in the primary molar age from 4 to 11 of the 186 children (male 100, female 86). The total numbers of teeth used in this study were 1096. The results were as follows ; 1. The difference in responses between the teeth of the male and female was not statistically significant. 2. The difference in responses between the teeth of the right and left sides was not statistically significant. 3. The difference in responses between the teeth of the upper and lower jaws was not statistically significant. 4. The responses of the teeth in the older age is higher than its of the early age. 5. Inspite of advanced root resorption of the primary molars, except severe root resorption, the responses of the teeth was higher according to increasing the age. 6. The responses of the teeth were decreased severely in the primary molars as it becomes exfoliation time.

2 Clinical Reports of Schzophrenic Patients with TD (전광 환자의 지연성 운동장애 Tardive Dyskinesia 치험 2례)

  • Tardive Dyskinesia(TD) may be defined as a syndrome consisting of abnormal, stereotyped involuntary movements, which occurs relatively late in the course of drug treatment and in the etiology of which the drug treatment is a necessary factor like schizophrenia, The frequently involved parts of the body in the schizophrenic patients who have TD were tongue, upper extremity, lips and perioral area, jaws, lower extremity, muscle of facial expression, trunk respectively. The last few years have seen a number of well-conducted clinical, biochemical, neuropathological, and therapeutic studies in patients with TD, but there is no satisfactory treatment for the majority of patients with TD. I used herb medicines and some acupoints like GV20(百會), Extra-point(鎭靜), Extra-point(間谷), Liv20(行間), of the chennel of Gall bladder to treated 2 schizophrenic patients with TD from December 2003 to May 2004. The patients with TD were somewhat improved. Nonetheless for just 2 cases, I might thought that this acupuncture therapy could be helpful to treat schizophrenic patients with TD.However, further studies should be followed.

THE STATISTICAL STUDY OF INTERDENTAL SPACE IN THE PRIMARY DENTITION (유치열(乳齒列)의 치간공극(齒間空隙)에 관(關)한 통계학적(統計學的) 연구(硏究))

  • The purpose of this study was to statistically evaluate the spacing of primary dentition according to whether or not a space existed and measurement of amount of space in 205 children (male 114, female 91) from 2 to 6 years of age. The results were as follows; 1. In the incidence of interdental space on each region, spacing between the primary lateral incisors and primary canine in maxilla and between the primary canine and 1st primary molar in mandible showed the highest percentage, on the other hand, spacing between the 1st and 2nd primary molars in both jaw showed the lowest percentage. 2. In the kinds of interdental space, the state of primate space and other space together showed the highest percentage, and in the degree of spacing, $S_2$ showed the highest percentage in both jaw. 3. In the incidence of primate space, maxillary primate space showed higher percentage than mandibular primate space in bilateral case. 4. In the type of arch form, spaced arch showed higher percentage than closed arch and about the half of closed arch showed crowding. 5. In both Jaw, available space showed incisor segments, canine segments, and premolar segments in order of amount, and there were not statistically significant differences of sex or jaws in the amount of available space.

Design and fabrication of microgripper using thermal actuator and SU-8 (열 구동 엑츄에이터와 SU-8을 이용한 마이크로 그리퍼 설계 및 제조)

  • A microgripper using thermal actuator and SU-8 polymer was designed and fabricated to manipulate cells and microparts. A chip size of a microgripper was 3 mm ${\times}$ 5 mm. The thermally actuated microgripper consisted of two couples of hot and cold arm actuators. The high thermal expansion coefficient, 52 $ppm/^{\circ}C$, of SU-8 compared to silicon and metals, allows the actuation of the microgripper. Thickness and width of SU-8 as an end-effector were 26 ${\mu}m$ and 80 ${\mu}m$, respectively. Initial gap between left jaw and right jaw was 120 ${\mu}m$. The ANSYS program as FEM tool was introduced to analyze the thermal distribution and displacement induced by thermal actuators. $XeF_2$ gas was used for isotropic silicon dry etching process to release SU-8 end-effector. Mechanical displacements of the fabricated microgripper were measured by optical microscopy in the range of input voltage from 0 V to 2.5 V. The maximum displacement between two jaws of a microgripper Type OG 1_1 was 22.4 ${\mu}m$ at 2.5 V.
