A large osteoid osteoma of the mandibular condyle causing conductive hearing loss: a case report and review of literature

  • Osteoid osteomas are benign skeletal neoplasms that are commonly encountered in the bones of the lower extremities, but are exceedingly rare in jaw bones with a prevalence of less than 1%. This unique clinical entity is usually seen in younger individuals, with nocturnal pain and swelling as its characteristic clinical manifestations. The size of the lesion is rarely found to be more than 2 cm. We hereby report a rare case of osteoid osteoma originating from the neck of the mandibular condyle that grew to large enough proportions to result in conductive hearing loss in addition to pain, swelling and restricted mouth opening. In addition, an effort has been made to review all the documented cases of osteoid osteomas of the jaws that have been published in the literature thus far.

  • Cyst is a closed sac, having a distinct membrane and division compared to the nearby tissue. Most cysts in the oro-maxillofacial area are odontogenic origin and found most commonly by oral and maxillofacial surgeons. Enucleation is considered as a definitive therapy in the management of these cysts. However, to select the treatment method for large cyst, consideration of position of the cyst, size and accessibility to main structure are needed. Marsupialization is a minimal invasive technique to treat large odontogenic cyst and tumor without damage to adjacent important structures. This study was undertaken to describe three cases of odontogenic cysts and tumors of jaws that were successfully treated with enucleation after marsupialization.

마우스가드의 역할과 기능 (Role and Function of Mouthguard)

  • A mouthguard is a protective device normally worn on the upper jaw, to reduce injures to the teeth, jaws and surrounding soft tissues. It has a definite role in preventing injuries to the teeth and face and for this reason it is strongly recommended for all sporting activities where there is a risk of trauma to the teeth and associated structures. Mouthguards can be effective in reducing impact force to the teeth, and attributed to enhancement of postural control and muscle performance during teeth clenching. Although there is evidence that mouthguards reducing impact force to the teeth, and prevent orofacial trauma occurrence during sport practice, the influence of this device on athletic performance has not been systematically quantified. Nevertheless, wearing a dentally fitted laminated mouthguard of at least 3 mm thickness can be strongly recommended during sport practice.

  • The cutting process of materials is accompanied with the elastic and plastic deformation due to chucking forces in the boring process of thin holes on the turning. Upon removal of chucking forces at the end of process, the original shape is remained in the plastic deformation; on the other hand, it is modified in the elastic deformation due to spring back. Fixing materials by chucks on the turning has influence on roundness because the process is conducted with unbalanced distribution load induced from the fixing of three jaws. Moreover, the amount of spring back depends on the magnitude of fixing forces. We studied the change of roundness according to fixing forces as well as the method to reduce the influence of chucking forces.

  • Osteosarcoma is the most common malignant tumor of bone. The mean age of occurrence of osteosarcoma in the jaws is around 30, somewhat older than for other sites in the body. These lesions occur about equally in the maxilla and mandible. They most frequently develop in the body in the mandible, and the antrum and the posterior portion of alveolar ridge in the maxilla. We report a case of osteosarcoma in 35 years old female complaining swelling of the left cheek. Radiographic features showed cotton wool appearance in upper left posterior area. Histopathologic findings exhibited pleomorphic malignant osteoblasts and neoplastic osteoid.

가족성 거대 백악종 (Familial gigantiform cementoma)

  • Familial gigantiform cementoma is a rare fibre-cemento-osseous disease of the jaws which appears to be transmitted as an autosomal dominant trait with variable expressivity of the phenotype. A 7-year-old girl visited DKUDH complaining of the painless facial deformity. Clinically, significant facio-lingual expansion was observed at the left maxilla, left mandibular body and symphysis portion. Malposition of lower anterior teeth was found. Panoramic radiograph and CT scan showed the extensive expansile mixed lesion at maxilla and mandible. Bone scan revealed hot spot at the maxilla and left side of mandible. Histologic examination revealed moderately dense fibrous connective tissue with scattered masses resembling cementum. The patient's mother had a history of the mandibular resection due to benign tumor. Her younger brother had buccal expansion of right mandible. We report our finding of a family that has exhibited clinical, radiographic and histologic findings consistent with the familial gigantiform cementoma.

  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissue and involve, essentially, the same pathological process. They are usually classified into three main groups: periapical, florid, and focal cemental dysplasias depending on their extent and radiographic appearances. Radiographically, florid cemento-osseous dysplasia (FCOD) appears as dense, lobulated masses, often symmetrically located in various regions of the jaws. The best management for the asymptomatic FCOD patient consists of regular recall examinations with prophylaxis. The management of the symptomatic patient is more difficult. A case of FCOD occurring in a 52-year-old edentulous Korean female is reported which is rare with regard to race and sex.

  • Brown tumor is usually diagnosed at the terminal stage of hyperparathyroidism. Diagnosis of this tumor is confirmed by endocrinologic investigations along with clinical and radiographic examination. Radiographical differential diagnosis of this tumor includes central giant cell granuloma, aneurysmal bone cyst, metastatic tumor, multiple myeloma, and Paget disease. This report presents a rare case of multiple brown tumors occurring at the maxilla and mandible, which was initially misdiagnosed as central giant cell granuloma. Plain radiographs demonstrated multiple well-defined multilocular radiolucency. CT images showed soft tissue mass with low attenuated lesions, perforation of the lingual cortical plate, and a heterogeneous mass at the right thyroid lobe. These findings were consistent with parathyroid adenoma. The patient had hypercalcemia, hypophosphatemia, and elevated alkaline phosphatase level. Surgical excision of the tumor was performed. No recurrence was observed during a 28-month follow-up.

  • 지금까지 국내에서는 자치어만 기록된 청베도라치과 갈기베도라치(Scartella emarginata)의 성체 1개체와 유어 2개체가 제주도에서 처음으로 채집되었기에 이들을 근거로 본 종의 형태적 특징을 상세하게 기재하였다. 갈기베도라치는 두부 정중선을 따라 부채살 모양의 피변이 있고, 눈의 상부와 비공에 분지된 1개의 피변이 있으며, 양턱에 견치가 없는 특징이 있다.

  • Ehlers-Danlos syndrome is an autosomal dominant hereditary disorder of connective tissue, while familial gigantiform cementoma is a condition that usually manifests as multiple radiopaque cementum-like masses throughout the jaws. This case report discusses the oral management and prosthetic rehabilitation of two patients presenting familial gigantiform cementoma with Ehlers-Danlos Syndrome.
