Update of Head and Neck Cancer Staging in the 8th Edition Cancer Staging Manual of the American Joint Committee on Cancer (두경부암 병기 설정의 최신 변화: AJCC 암 병기설정 매뉴얼8판)

  • The recently released the $8^{th}$ edition of the American Joint Committee on Cancer (AJCC) Staging Manual introduces significant modifications from the prior $7^{th}$ edition. In this paper, the contents of the new changes in the decision of cancer of the head and neck is summarized except changes in staging of skin and thyroid cancer. In addition to the 8th edition, 1) Addition of extracapsular involvement in metastatic lymph nodes (N category) 2) Oral cancer T classification change, 3) Staging of the pharyngeal cancer was divided into 3 chapters: high-risk human papilloma virus (HR-HPV) associated oropharyngeal cancer (OPC), non HR-HPV associated OPC and hypopharynx cancer (HPC), and nasopharynx cancer (NPC) 4) Changes in T and N classification in NPC, 5) In the case of cancer of unknown primary, P16-positive case is defined as HR-HPV related OPC, and EBV-positive case is defined as NPC. The process that led to these changes highlights the need to collect high-fidelity cancer registry-level data that can be used to confirm prognostic observations identified in institutional data sets. Clinicians will continue to use the latest information for patient care, including scientific content of the 8th Edition Manual. All newly diagnosed cases through December $31^{st}$ 2017 should be staged with the 7th edition. The time extension will allow all partners to develop and update protocols and guidelines and for software vendors to develop, test, and deploy their products in time for the data collection and implementation of the 8th edition in 2018. The 8th edition strikes a balance between a personalized, complex system and a more general, simpler one that maintains the user-friendliness and worldwide acceptability of the traditional TNM staging paradigm.

EXPRESSION OF MATRIX METALLOPROTEINASE-1 AND -2 MRNA IN RETRODISCAL TISSUE OF THE TEMPOROMANDIBULAR JOINT (측두하악 관절원판 후조직의 MMP(matrix metalloproteinase)-1과 MMP-2 mRNA의 발현)

  • Matrix metalloproteinases (MMPs) play an important role in the normal morphogenesis, maintenance, and repair of matrix and also have important functions in pathologic conditions characterized by excessive degradation of extracellular matrix, such as rheumatoid arthritis, osteoarthritis, periodontitis and in tumor invasion and metastasis. In this study, expression of MMP-1 and -2 mRNA in retrodiscal tissue of the temporomandibular joint (TMJ) was examined and compared with magnetic resonance imaging (MRI) and surgical findings. MMP mRNAs in the retrodiscal tissue samples were detected by reverse transcription - polymerase chain reaction. TMJ internal derangement (ID) was categorized as normal disc position, disc displacement with reduction, early stage of disc displacement without reduction (DDsR) and late stage of DDsR. TMJ osteoarthrosis (OA) was classified with normal, mild and advanced OA. The amount of synovial fluid collection was divided into not detected, small, large and extremely large amount on MR T2-weighted imaging. Perforation and adhesion were examined during open surgery of the TMJ. Six out of 37 samples were excluded because of little amount of extracted total mRNA. MMP-2 mRNA was detected whole joints, and so the MMP-2 mRNA seems to be expressed normally in retrodiscal tissue. However, MMP-1 mRNA was expressed in 8 of 31 joints. Frequencies of MMP-1 mRNA expression according to the TMJ IDs, amount of synovial fluid and surgical findings made no significant difference. MMP-1 mRNA was detected more frequently in OA groups (7/16 joints, 43.8%) than in normal bony structure group (1/15 joints, 6.7%). Expression of MMP-1 mRNA in retrodiscal tissue might be related with OA of the TMJ.

A Study on the Relationship between the Meaning of life and Spiritual distress in Arthritis (관절염 환자의 삶의 의미와 영적고통과의 관계)

  • The arthritis patients suffer from psychological, social and spiritual problems as well as physical problems because the arthritis is not curable and has chronic pain, joint deformity, limitation of activity and physical dysfunction for all of his life. Especially if they do not find the meaning in their lives, they will experience spiritual distress seriously. Therefore, it is important that nurses help the patients to find the meaning in their lives and to reduce spiritual distress. The purpose of this study is to provide a basis for nursing intervention strategies to minimize the arthritis patients' spiritual distress and understand the relationship between the meaning of life and the spiritual distress in arthritis patients. The samples were composed of 157 arthritis patients. Data collection was carried out from October 1, 1998 to February 28, 1999. Data were analyzed using a SAS program for descriptive statistic, Pearson correlation, t-test, ANOVA, linear regression. The results were as follow; 1. The scores on the meaning of life scale ranged from 51 to 130 with a mean of 93. 2. The scores on the spiritual distress scale ranged from 26 to 91 with a mean of 60. 3. There were significant correlations between the meaning of life and the spiritual distress(r=.53, p=.00). 4. The linear regression analysis showed that the meaning of life explained 13% of the spiritual distress. 5. In the degree of the meaning of life and the spiritual distress according to the general characteristics, the level of the meaning of life in arthritis patients was different by the duration of incidence(F=2.71, p=.03). In conclusion, the nursing intervention strategies to reduce the spiritual distress in arthritis patients must take into account the meaning of life.

  • As the era of e-navigation begins, the significance of harmonized collection, exchange, and integration of marine information conventional "ship-to-ship" and "ship-to-shore"/"shore-to-ship" has become more significant in ensuring safety and security at sea. This research aims to provide an internationally applicable list of Standard e-Navigation Phrases (SENP ) for communication in the e-Navigation environment, along with practical approaches to this. For this purpose, the importance of standardized communication phrases specific to e-Navigation will first be illustrated. In order to approach the e-Navigation phrases in a practical and pragmatic manner, means of compiling and processing authentic VHF linguistic data accumulated from one of the Korean ports, Ulsan, will be detailed. In addition, major outcomes to be considered in designing SENP will be suggested. Finally, future actions and a joint effort for the standardization of e-Navigation phrases among stakeholders will be sought.

Marine Molluscan Fauna of Jindo Island

  • As a part of the commemorative joint faunal survey for the 30th anniversary of the Korean Society of Systematic Zoology, the molluscan fauna of Jindo Island was investigated based on sample collection from 6 localities from the 6 to the 8 of Jul 2016. A total of 114 molluscan species from 47 families were collected and identified. Among these, 42 species from 11 families are newly reported from Jindo Island and combining the previous records with the present study totals 157 species from 57 families. Distribution of species records indicates that marine biogeography of Jindo Island represent an overlapping zone for marine organisms which dwell in the Yellow sea and the southern sea areas of Korean waters.

  • Pantograph, which collects current from cartenary system, is one of the important parts of high-speed train. Kinematic analysis is one of the key technologies for pantograph design and includes joint trajectories, reaction forces, and the required moment of main shaft calculations. The kinematic analysis, however, is very complex and time-consuming process. In this study PC based pantograph kinematic analysis software using graphical user interface tool was developed for the easy evaluations of kinematic characteristics necessary in pantograph design process.

  • With the objective of selecting suitable breeding resource material, 10 polyvoltine and 10 bivoltine breeds were drawn from the germplasm collection of Central Sericultural Research and Training Institute, Mysore, and evaluated for 3 seasons comprising one year (6 trials). Data were collected on seven traits of economic importance such as fecundity, pupation rate, cocoon yield, cocoon weight, cocoon shell weight, cocoon shell ratio and filament length, and statistically analysed with two-way classification, Joint scoring method and evaluation index. Significant seasonal variations (P < 0.01) were observed in both polyvoltine and bivoltine breeds. Polyvoltines BL27, BL36 and BL54 and bivoltines CSR2, CSR4 and Daizo scored highest ranking values in all the three methods. Hybridization was initiated based on larval markings and cocoon shapes. Seasonal variations were discussed.

Effects of Stretching Exercise Included Health Education on Physical Health Index, Self-reported Symptoms, and Self-efficacy of Exercise in Elderly (건강교육을 포함한 신장운동이 노인의 생리적인 지표와 자가증상 보고 및 운동 자기효능에 미치는 효과)

  • This was one experimental study, conducted to identify the effects of stretching exercise included health education on physical health index, self-reported symptoms, and self-efficacy of exercise in Elderly. Subjects included 28 women elderly who were residents of H Dong, Dondae moon-Gu, Seoul. The treatment intervention was applied during total 8 weeks as 5 times/week for stretching exercise with 2 times/weeks for health education. Data collection were from February to April, 2004. SPSS Window program was used by aims of this study for data analysis. The results were as following: 1. 75 old age above (39.3%) was the most of age in subjects. None education (42.9%) was the most of subjects in this study. Perceived health state was the most as 71.4% in moderate and good health state. 2. Stretching exercise included health education, significantly effected on the total Cholesterol, HDL-cholesterol, triglyceride (p=.009*; p=.043*; p=.006*). However, the body fat weight was no significantly on the effect of stretching exercise included health education (p=.991). Also, stretching exercise included health education, significantly effected on total self-reported symptoms (p=.001*) with joint pain, joint rigidity, fatigue, indigestion, elimination disorder, and sleep disturbance. However, indigestion (p=.129) was no significantly on the effect of stretching exercise included health education. Self efficacy of exercise (p=.000*) was significantly on the effect. As according to this results, physical health index (total Cholesterol, HDL-cholesterol, triglyceride) and self-reported symptoms (joint pain, joint rigidity, fatigue, elimination disorder, and sleep disturbance) were reduced through stretching exercise included health education. Self efficacy was improved by this exercise program. Therefore, it is confirmed that stretching exercise included health education is an effective nursing intervention for physical, mental, and psychological health management in elderly. Accordingly, authors are proposing that variously effective health management exercise programs must be developed for elderly, at the same time, the application and following up on the programs will be more important in the future.
