종합병원의 비용절감과 업무개선을 위한 중앙공급실의 활동 (Case Report on the Activities for Cost Reduction and Performance Improvement in the Central Supply Room in A University Hospital)

  • Background : Because hospitals had to faced with financial hardship, we have to have more effective hospital management. In this study, we tried to improve job performance and to reduce cost maintaining safety in fields of patients care. Methods : Central Supply Room(CSR) staffs taught strategy of material cost reduction to middle level managers and staffs 14 times. All staffs and voluntary service workers, working in 33 nursing units, participated in this activity. We developed questionnaire to check the degree of participate in cost reduction activity and job performance in CSR, and quality improvement in the wards. Two hundred questionnaires were distributed and 197 returned. Results : Because CSR staffs developed new items and every item was managed at each department, stocks of each department were reduced. Overall, by comparing before and after cost reduction activity, almost 1.2 billion won reduced for one year. Staffs' loyalty and spirits of cost reduction were improved by changing clothes and duty environment. Furthermore, these activity improved patients' satisfaction. Conclusion : We suggest that CSR activity in a university hospital contributes to improve job performance and to reduce cost without any problem in patient care.

Assessment of Nicotine Dependence among Smokers in a Selected Rural Population in Kerala, India

  • Objectives: An attempt was made to understand the nicotine dependence of smokers selected for an ongoing smoking cessation intervention programme in rural Kerala, India. Methods: Data were collected from resident males in the age group of 18 to 60 years from 4 randomly allocated community development blocks of rural Thiruvananthapuram district (2 intervention and 2 control groups). Trained accredited social health activist workers were utilised to collect data from all groups through face to face interview. Nicotine dependence among participants was assessed by means of the six-item Fagerstrom Test for Nicotine Dependence (FTND) translated into the local language. The internal consistency of FTND was computed using Cronbach's alpha coefficient. Criterion validity (concurrent) was assessed by correlations of nicotine dependence scores with age at initiation of smoking and cumulative smoking volume in pack-years. Results: Among the 928 smokers identified, 474 subjects were in the intervention area (mean age = 44.6 years, SD = 9.66 years) and 454 in the control area (mean age = 44.5 years, SD = 10.30 years). The overall FTND score among current daily smokers was 5.04 (SD: 5.05). FTND scores in the control and intervention areas were 4.75 (SD: 2.57) and 4.92 (SD: 2.51) respectively. The FTND scores increased with age and decreased with higher literacy and socioeconomic status. The average FTND score was high among smokers using both bidi and cigarettes (mean 6.10, SD 2.17). Internal consistency analysis yielded a Cronbach's alpha coefficient of 0.70 in a subsample of 150 subjects, a moderate result. The association of the scale was strongest, with the number of pack-years smoked (rho = 0.677, p < 0.001). Conclusion: A moderate level of nicotine dependence was observed among smokers in the current study. Tobacco cessation strategies could be made more cost effective and productive if a baseline assessment of nicotine dependence is completed before any intervention.

Dentists' Perception of the Role they Play in Early Detection of Oral Cancer

  • Background: Dentists are typically the first professionals who are approached to treat ailments within the oral cavity. Therefore they should be well-equipped in detecting suspicious lesions during routine clinical practice. This study determined the levels of knowledge on early signs and risk factors associated with oral cancer and identified which factors influenced dentist participation in prevention and early detection of oral cancer. Materials and Methods: A survey on dentists' knowledge and their practices in prevention and early detection of oral cancer was conducted using a 26-item self-administered questionnaire. Results and Conclusions: A response rate of 41.7% was achieved. The level of knowledge on early signs and risk habits associated with oral cancer was high and the majority reported to have conducted opportunistic screening and advised patients on risk habit cessation. Factors that influenced the dentist in practising prevention and early detection of oral cancer were continuous education on oral cancer, age, nature of practice and recent graduation. Notably, dentists were receptive to further training in the area of oral cancer detection and cessation of risk habits. Taken together, the study demonstrated that the dental clinic is a good avenue to conduct programs on opportunistic screening, and continuous education in these areas is necessary to adequately equip dentists in running these programs. Further, this study also highlighted knowledge deficits and practice shortcomings which will help in planning and developing programs that further encourage better participation of dentists in prevention and early detection of oral cancer.

Quality of Life among Breast Cancer Patients Undergoing Treatment in National Cancer Centers in Nepal

  • Purpose: To study the quality of life and to identify associated factors among breast cancer patients undergoing treatment in national cancer centers in Nepal. Materials and Methods: One hundred breast cancer patients were selected and interviewed using a structured questionnaire. European Organization of Research and Treatment of Cancer EORTC-QLQ-C30 and EORTC-QLQ-BR23 were used to assess quality of life and modified Medical Outcome Study -Social Support survey(mMOS-SS) was used to assess social support. Only multi-item scales of EORTC C30 and BR23 were analyzed for relationships. Independent sample T-tests and ANOVA were applied to analyze differences in mean scores. Results: The score of global health status/quality of life (GHS/GQoL) was marginally above average (mean=52.8). The worst performed scales in C-30 were emotional and social function while best performed scales were physical and role function. In BR-23, most of the patients fell into the problematic group regarding sexual function and enjoyment. Almost 90% had financial difficulties. Symptom scales did not demonstrate many problems. Older individuals, patients with stage I breast cancer and thosewith good social support were found to have good GHS/GQoL. Of all the influencing factors, social support was established to have strong statistical associations with most of the functional scales: GHS/GQoL (0.003), emotional function (<0.001), cognitive function (0.020), social function (<0.001) and body image function (0.011). Body image was significantly associated with most of the influencing factors: monthly family income (0.003), type of treatment (<0.001), type of surgery (<0.001), stage of cancer (0.017) and social support (0.011). Conclusions: Strategies to improve social support of the patients undergoing treatment should be given priority and financial difficulties faced by breast cancer patients should be well addressed from a policy making level by initiating health financing system.

공동주택 소방시설 적법성 검토를 위한 화재안전규정 체크리스트 및 매뉴얼 개발 (A Checklist and Manual Developed to Review Fire Safety Facilities' Compliance with Fire Safety Requirements for Apartment Buildings)

  • 고층건물의 적법하지 않은 소방시설로 인한 화재 증가로 안전성 확보를 위한 법규 검토의 필요성이 증가하고 있지만 국내 공동주택의 경우 소방시설 화재안전규정에 대한 적법성 검토는 잘 이루어지지 않고 있다. 구조적 특성상 화재 시 인명피해의 위험이 큰 건축물임에도 검토가 미흡한 이유는 건축법과 소방법 혼재에 따른 법규 해석 오류에 있으며 이로 인한 불필요한 소방시설의 설치 및 소방시설이 누락된 상태로 이루어지는 준공검사는 막대한 경제적, 시간적 손실을 발생시킨다. 따라서 본 연구에서는 소방시설에 대하여 혼재되어 있는 법규를 한 번에 검토할 수 있는 체크리스트를 도출하고 IPA 분석을 통해 항목별 중요도와 현수준을 확인하였다. 이를 바탕으로 실무 활용성을 고려한 매뉴얼을 개발하여 건설사의 소방 준공 리스크 저감에 기여하고자 한다.

BIM 저작도구를 활용한 도로공사 수량산출 자동화 활용성 분석 (Analysis for the Applicability of Automated Quantity Take-off Technique Using BIM Authoring Tools on Road Construction)

  • 최근 BIM저작도구를 활용한 자동수량산출에 대한 관심이 많아지고 있으며, 단순한 부피, 개수, 면적 등의 단위계산 뿐만 아니라, BIM 기술을 이용한 물량산출의 정확성과 효율성에 대한 연구가 수행되고 있다. BIM 저작도구를 통한 자동수량산출은 매우 유용한 기능이나, 내역서 항목별 수량산출을 위해서는 모델링의 상세 수준을 높이는 등 많은 시간과 비용이 투입되어야 한다. 이에 본 연구에서는 2D 기반으로 설계된 도로건설공사 도면을 바탕으로 BIM 저작도구 이용하여 토공 및 도로공과 구조물을 3D 모델링 하고 BIM 저작도구에서 제공하는 자동물량산출기능을 통해 활용성을 살펴본다. 그 결과 토공 및 도로공이나 구조물공에 대하여 산출되는 자동수량산출의 항목들은 매우 제한적이나, 철근 모델은 수량산출서 항목의 대부분을 지원함을 알 수 있다.

종합건강진단시 위장검사의 선택 실태에 관한 분석 (Analysis on Actual State of Selective Upper Gastrointestinal Study in Medical Examination)

  • The purpose of this study is to present controversial point and reform measurements by analysing factors haying important effect on selection of upper gastrointestinal study in total health promotion. We examined 200 persons for this study, who visited for upper gastrointestinal study from January to February in 1999. We classified this group into Endoscopy, Upper gastrointestinal series, and sleeping endoscopy. We also investigated standard of satisfaction and factors having effect on selection of each study. As a results, in the motive of selection, Item of 'making accurate observation' and 'without pain' was 39.3% and 34.7%, respectively. In this study, sleeping endoscopy was 45.7%, but on the other side upper gastrointestinal series was low 22.6%(P<0.05). In the standard of preference of study, the man was 55.7% in the endoscopy, and the woman was 61.8% in the upper gastrointestinal series(P<0.05). The standard of preference of upper gastrointestinal series show that it was satisfied on the whole irrespective of sex, dwelling place, age, occupation, and level of education. In the selection of study, one's own will was showed the highest frequency, and family inducement was showed second(P<0.05). Persons over 60% were examined before the same study. Selection of upper gastrointestinal series was 47.9% of person with normal findings, and endoscopy and sleeping endoscopy was over 70% with gastritis, gastric and duodenal(P<0.01). For one's accurate selection of examination, it is important that objective and credible information should be given to a recipient for examination.

당뇨교육이 당뇨환자의 역할행위 이행에 미치는 영향 (The Effect of Patient Teaching on Compliance with Sick Role Behavior of diabetic Patients)

  • This study examined the effect of patient teaching on Compliance with sick role behavior in diabetic patients. The purpose was to improve diabetic patients Compliance by D. M. patient teaching. The study objectives were to determine the effect of patient teaching on Compliance with sick role behavior, and factors influencing compliance with sick role behavior of diabetic patients. The Subjects, consisting of 52 diabtic patients diagnosed in the C. and Other hospitals in K. city were divided into experimental and Control groups. Data were gathered from July 25th to September 3rd, 1988 through interviews by questionnaires, measurment of blood sugar level by Reflolux. D. M. Patient teaching was defined as informational intervention of social support by the nurse. A booklet representing patient education and questionnairs were developed by the investigator, and were tested for Content validity, and reliability by Item Analysis: Cronbachs $\alpha$ for any instrument to measure variables was patient Compliance .83, perceived health belief .65, diabetic knowledge .70. Analysis of data were done by paired t-test, t-test, Anova, Pearson correlation and Stepwise multiple regression, The results of the study may be summarized as follows : 1. The effect of patient teaching on Compliance with sick role behavior, on diabetic Knowledge and health belief was Confirmed by significant differences between the experimental and the control group before and after the experiments. (P=000 P=006, P=004). 2. Factors influencing compliance with sick role behavior of diabetic patient were diabetic patient teaching (P<.005), perceived health belief(r=.5597, P<.005), blood sugar(r=.3205, P<.01), diabetic knowledge(r=.2876, P<.05).

초산모의 이유실천에 관한 조사연구 (A Survey on doing Weaning Activities of First-time Mother)

  • It is an important task to give adequate nutritions to infants and to wean properly for rapid growth. This study was conducted to survey on doing the weaning activities of the first-time mother. This study was to provide basic data of health teaching and nursing intervention for the promotion of ideal weaning activities. Survey was done from January to February in 2001. Total 444 mothers who have first-time baby under twelve month old were interviewed in five community health center around contry (Seoul, Choung-ju, Asan, Cheon-an, Jeju). The results are summarized as follows; Starting time of weaning was common in four month old(40.4%). Most common daily weaning times by age showed once(under 3 months), twice(4 to 7 months), three times(8 to 12 months). The younger, the more commercial weaning products(p<0.01). Education level showed relatively positive relation with home-made weaning food(p<0.05). Items showed low scores in 4 to 7 months group were starting new food item, waiting at least one week interval to new food, feeding by milk bottle, allowing grasping spoon, consulting pediatrician for problems after weaning, starting from morning, regular schedule. Items showed low scores in 8 to 12 months group were feeding whole egg rather than yolk only up to 12 months, keeping weaning interval were increased, feeding by milk bottle, trying various cooking method, not giving commerical cooking and instant food and unproper raw milk feeding. In weaning activities by general factors, mother age, health status, marriage duration, monthly income were statistically significant (p<0.05). To improve good weening activities, targetting on low maternal age and poor health status, and low income group, developing teaching materials and training program for items showed low scores and proper weaning time, type of easy home-made food and easy cooking method should be provided. Further study on effect of nursing intervention to improve weaning activities are needed.

고등학생의 일탈행동 영향요인 분석 (Affecting Factors of Deviant Behaviors of Korean High School Students)

  • Purpose: The purpose of this study is to analyze the factors affecting Deviant Behaviors of Korean High school Students. Method: Data was collected from October 8 to 31, 2002. The subjects for this study were 697 Korean High school Students(boys 347, girls 350), recruited from two High School located in Seoul. Data collection was conducted through the use of 6 Questionnaire that modified by the investigator. The data was analyzed by the SPSS win 10.0 program using Descriptive statistics, Pearson Correlation coefficient and stepwise multiple regression. Result: 1) The mean of total item score the Deviant Behaviors scales was 1.59, which was slightly low. 2) There was a significant correlation between Deviant Behaviors, Type A Personality, Aggression, Impulsivity, Stress and Social Support(γ= .11 ~ .65, p<.001), but It was no significant correlation Type A Personality and Stress(γ= -.01). 3) Stepwise multiple regression analysis showed that (1) Impulsivity, Social Support and Type A Personality were the predictors of Deviant Behaviors and account for 18.6% of the variance in Deviant Behaviors. (2) Impulsivity account for 3.6% of the variance in Deviant Behaviors in Subjects with a lower degree of score the Deviant Behaviors. Impulsivity and Social Support account for 23.2% of the variance in Deviant Behaviors in subjects with higher degree of score the Deviant Behaviors. (3) Impulsivity account for 18.3% of the variance in Deviant Behaviors in High school girls (n=350). Impulsivity and Social Support account for 20.1% of the variance in Deviant Behaviors in High school boys (n=347). Conclusion: Impulsivity and Social Support account for Deviant Behaviors of High school Students. Therefore it is necessary to develop nursing intervention to reduce the level of Impulsivity, to increase the Social Support in order to decrease the Deviant Behaviors.
