We present an elderly patient with unilateral foraminal stenosis treated by isthmus resection. An 83-year-old female could not walk due to severe leg pain along right L5 sensory dermatome. Despite the laminotomy for spinal stenosis on the right side at the L4-5 level, her leg pain did not improve. Careful review of computed tomography scans and coronal source images of magnetic resonance myelography revealed foraminal stenosis on the right side at the L5 vertebra. Because of medical problem, she underwent isthmus resection on the right side at the L5 level instead of total facetectomy and fusion. After surgery, her leg pain was markedly improved. Isthmus resection showed successful result for this medically compromised elderly patient with unilateral foraminal stenosis.
Restorative procedures can lead to tooth fracture due to the relatively small amount of the remaining tooth structure. It is essential to prevent fractures by having a clear concept of the designs for cavity preparations. Among the several parameters in cavity designs, profound understanding of isthmus width factor would facilitate selection of the appropriate cavity preparation for a specific clinical situation. In this study, MO amalgam cavity were prepared on maxillary first premolar and filled with amalgam. Three dimensional, model with 1365 8-node brick elements was made by serial photographic method. In this model, isthmus was varied in width at 1/4, 1/3, 1/2 and 2/3 of intercuspal width and material properties were given for three element groups, i.e., enamel, dentin and amalgam. A load of 500 N was applied vertically on amalgam and enamel. In case of enamel loading, 2 model (with and without amalgam) was compared to consider the possibility of play at the interface between tooth material and amalgam. These models were analyzed with three dimensional finite element method. The results were as follows: 1. The stress was concentrated on the facio-pulpal line angle and distal marginal ridge of the cavity. 2. With the increase of the isthmus width, the stress spread around the facio-pulpal line angle and the area of stress concentration moved toward the proximal box. 3. In case of narrow isthmus width, the initiation point of crack would be in the area of isthmus corner of the cavity, and with the increase of the isthmus width, it would move toward the proximal box and at the same time the possibility of crack increase at the distal marginal ridge. 4. The direction of crack progressed outward and downward from the facio-pulpal line angle, and with the increase of the isthmus width, it approximated vertical direction. At the marginal ridge, it occurred in vertical direction. 5. It would be favorable to make the isthmus width narrower than a third of the intercuspal width, and to cover the cusp if isthmus width were wider than half of the intercuspal width. 6. It is necessary to apply the possibility of play to the finite element analysis.
Objectives: The aim of this study was to compare the cleaning efficacy of different final irrigation regimens in canal and isthmus of mandibular molars, and to evaluate the influence of related variables on cleaning efficacy of the irrigation systems. Materials and Methods: Mesial root canals from 60 mandibular molars were prepared and divided into 4 experimental groups according to the final irrigation technique: Group C, syringe irrigation; Group U, ultrasonics activation; Group SC, VPro StreamClean irrigation; Group EV, EndoVac irrigation. Cross-sections at 1, 3 and 5 mm levels from the apex were examined to calculate remaining debris area in the canal and isthmus spaces. Statistical analysis was completed by using Kruskal-Wallis test and Mann-Whitney U test for comparison among groups, and multivariate linear analysis to identify the significant variables (regular replenishment of irrigant, vapor lock management, and ultrasonic activation of irrigant) affecting the cleaning efficacy of the experimental groups. Results: Group SC and EV showed significantly higher canal cleanliness values than group C and U at 1 mm level (p < 0.05), and higher isthmus cleanliness values than group U at 3 mm and all levels of group C (p < 0.05). Multivariate linear regression analysis demonstrated that all variables had independent positive correlation at 1 mm level of canal and at all levels of isthmus with statistical significances. Conclusions: Both VPro StreamClean and EndoVac system showed favorable result as final irrigation regimens for cleaning debris in the complicated root canal system having curved canal and/or isthmus. The debridement of the isthmi significantly depends on the variables rather than the canals.
A case of distal aortopulmonary septal defect associated with aortic origin of right pulmonary ar- tery, patent ductus arteriosus and hypoplasia of aortic isthmus in a 50-day-old female infant is presented. Ligation of patent ductus arteriosus, resection and end-to-end anastomosis of hypoplasia of aortic isthmus, implantation of rlght pulmonary artery to main pulmonary artery and autologous peri- cardial patch repair of aortopulmonary septal defect were performed under cardiopulmonary bypass as one-stage approach. Deep hypothermic total circulatory arrest was applied during repair of hypoplasia of aortic isthmus. The p stoperative course was uneventful.
The purpose of this study was to evaluate the fracture resistance of tooth restored with gold inlay. A profound understanding of the isthmus width factor, which is one of the several parameters of cavity designs, would facilitate the appropriate cavity preparation in a specific clinical situation. In this study, the cavities for gold inlay were prepared in maxillary left first premolar. A three-dimensional model was designed using I-DEAS program. The model was composed of 2515- nodes and 2172 isoparametric brick elements. In the model isthmus width was varied into 1/4, 1/3 and 1/2 of intercuspal width respectively, and numeric values of the material properties of enamel, dentin and gold was set. Three types of load : concentrated load, divided load and distributed load was 500N. The empty cavities in the model were also examined using divided load and distributed load. The three - dimensional Finite Element Method was used to analysis the displacement and stress distribution. The results were as follows : 1. All of the experimental models which were filled with gold inlay revealed similar direction of displacement to that of the natural tooth model under the same load type. But in the models with empty cavities, as the isthmus width increased, the degree of displacement increased in the case of divided load type. 2. All experimental models which were filled with gold inlay showed stress concentration at load points, but in the models with empty cavities at divided load type, as isthmus width increased, stress was concentrated at the comer of the pulpal floor. 3. In the models with empty cavities at divided load type, tooth fracture was expected regardless of isthmus width, but all experimental models which were filled with gold inlay after cavity preparation were not susceptible to fracture. 4. In all experimental models which were filled with gold inlay after cavity preparation, displacement patterns were similar under both concentrated and divided load types. In the models with empty cavities, a divided load resulted in a bucco-lingual cuspal displacemenat in both sides, but a distributed load resulted in a lingual displacement of the tooth.
This study was carried out to investigate sperm storage, and the fate of spermatozoa in the female reproductive tract during hibernation in Korean greater horseshoe bat, Rhinolophus ferrumequinum korai. (1) Numerous sperm occurring in uterine lumen and glands were engulfed, and disappeared by the polymorphouclear leucocytes during the hibernation. (2) The stored sperm present in caudal isthmus of oviduct only, the heads of sperm toward the oviductal epithelial cells. Therefore, the projected sperm during the mating season are only alive in the caudal isthmus of oviduct in the long hibernation. The present result suggests that the caudal isthmus of oviduct may play an important role as the principal storage site in capacitation of sperm. (3) In March, the sperm do not occur in the caudal isthmus of oviduct. It suggests that the stored sperm in the caudal isthmus of oviduct should migrate to the ampulla of the site of fertilization to meet ovum in the period of ovulation. The results of this experiment consider that prolonged sperm storage, fate of sperm and sperm migration in the long hibernation have a kind of mechanism for the fertilization.

The 500N occlusal load varied in direction and it was examined using three types of load : concentrated load, divided load and distributed load. The models were also examined with empty cavities using the devided load and distributed load. These models were analyzed the displacement and strees distribution by the two - dimensional Finite Element Method. The results were as follows : 1. All experimental models which filled with gold inlay after cavity preparation were similar direction of displacement with control model under same load type. But in the models with empty cavities, as isthmus width was wider, the degree of displacement was increased at same load type. 2. Among the experimental models which were filled with gold inaly after cavity preparation, the model II showed the least stress concentration under concentrated load and divided load. But in the models with empty cavities, the model III showed the largest stress concentration and tooth fracture is expected regardless isthmus width. 3. All experimental models showed similar displacement pattern beneath restorative material under a concentrated load. In the models with empty cavities, a divided load resulted in a lingual displacement of the lingual cusp, but a distributed load resulted in a buccal displacement of the lingual cusp. In regard to the above results, the restored models were stronger than empty models in respect to the bending moment and tensile stress. The empty models are expected to fracture regardless isthmus width. The safest isthmus width was 1/3 of intercuspal distance, which showed the least stress concentration in respect to the effect of stress distribution.

In this experiments, the fracture loads were measured and analysed by using of Instron universal testing machine (Model 1132). With 80 upper and lower deciduous molars that were free of dental caries. Isthmus of teeth were prepared to 1/3 and 1/4 of intercuspal width, and internal line angles with round and sharp shape, and then amalgam restoration were performed. The results of this study were as follows; 1. Resistance to fracture loads were greater with isthmus of 1/3 intercuspal width than of 1/4 intercuspal width, especially, statistically significant difference on upper and lower second deciduous molars. 2. Resistance to fracture loads were greater with rounded internal line angle than sharp internal line angle. The above results were as follows, the fracture loads had greater resistance on isthmus of 1/3 intercuspal width, and rounded internal line angle, but normal physiological occlusal forces could permit isthmus of 1/4 intercuspal width and rounded internal line angle.
Because of its embryonic origin, the thyroid gland is predisposed to multiple anatomical variations and developmental anomalies. These include the pyramidal lobe, the origin of levator glandular thyroidae, the absence of the isthmus, ectopic thyroid, accessory thyroid tissues, etc. These anatomical variations are clinically significant to surgeons, anatomists, and researchers. The present study was designed to report anatomical variations and developmental anomalies of the thyroid gland in Ethiopian population. The study was conducted on 40 cadavers used for routine dissection classes. The thyroid gland was exposed and observed for any variations and developmental anomalies. The length, width, and thickness of the lobes were measured using a vernier caliper. Differences in the incidence of pyramidal lobe and absence of the isthmus between sexes were tested using a Pearson chi-square test. The mean length, width, and thickness of the right lobe were 4.24 cm, 1.8 cm, and 1.6 cm, respectively, whereas it was 4.08 cm, 1.8 cm, and 1.6 cm, respectively for that of the left lobe. The pyramidal lobe was noted in 52.5% of the cadavers. The levator glandulae thyroidae were prevalent in 40% of the cadavers. The isthmus mainly overlies the 2nd to 4th tracheal rings and was absent in 7.5% of the cadavers. Accessory thyroid tissue and double pyramidal lobes were noted in 2.5% of the cadavers. Most of the variations of the thyroid gland were seen frequently in female but it was not statically significant. Different clinically important and rare variations of the thyroid gland were found.
Fracture of cusp, on posterior teeth, especially those carious or restored, is major cause of tooth loss. Inappropriate treatments, such as unnecessarily wide cavity preparations, increase the potential of further trauma and possible fracture of the remaining tooth structures. Fracture potential may be directly related to the stresses exerted upon the tooth during masticatory function. The purpose of this study is to evaluate the fracture resistance of tooth, restored with composite resin inlay. In this study, MOD inlay cavity prepared on maxillary first premolar and restored with composite resin inlay. Three dimensional finite element models with eight nodes isoparametric solid element, developed by serial grinding-photographing technique. These models have various occlusal isthmus and depth of cavity, 1/2, 1/3 and 1/4 of isthmus width and 0.7, 0.85 and 1.0 of depth of cavity. The magnitude of load was 474 N and 172 N as presented to maximal biting force and normal chewing force. These loads applied onto ridges of buccal and lingual cusp. These models analyzed with three dimensional finite element method. The results of this study were as follows : 1. There is no difference of displacement between width of occlusal isthmus and depth of cavity. 2. The stress concentrated at bucco-mesial comer, bucco-disal comer, pulpal line angle and the interface area between internal slopes of cusp and resin inlay. 3. The vector of stress direct to buccal and lingual side from center of cavity, to tooth surface going on to enamel. The magnitude of vector increase from occlusal surface to cervix. 4. The crack of tooth start interface area, between internal slop of buccal cusp and resin inlay. It progresses through buccopulpal line angle to cervix at buccomesial and buccodistal comer. 5. The influence with depth of cavity to fracture of tooth was more than width of isthmus. 6. It would be favorable to make the isthmus width narrower than a third of the intercuspal distance and depth of cavity is below 1 : 0.7.
