족근중족관절이 탈구된 환자에서 원심성 등속성운동의 효과: 사례연구 (The Effect of Eccentric Isokinetic Exercise in Dislocation of the Tarsometatarsal (Lisfranc's) Joint: A Case Study)

  • Eccentric muscle contraction is more effective than concentric and iosmetric muscle contraction in increasing muscle strength. Also, eccentric or concentric-eccentric training has greater effective in neural activation and muscle hypertrophy than concentric training. In some study, eccentric exercises have been shown to reduce pain and improve function on Achilles tendinopathy. The purpose of this study was to evaluate the effect of eccentric isokinetic exercise in a patient with dislocation of the tarsometatarsal joint by traffic accident. After eccentric isokinetic training, peak torque, average work, and average power were increased. Also, the patient was fully weightbearing with a pain free normal gait thus making good recovery.

Isokinetic Muscle Strength and Muscle Endurance by the Types and Size of Rotator Cuff Tear in Men

  • Background: Our study was to determine the effect on shoulder isokinetic muscle strength and muscle endurance in isolated full-thickness supraspinatus tendon tear and combined other rotator cuff tear. Methods: Total of 81 male patients (mean age $57.8{\pm}7.4$ years) who were diagnosed as a full-thickness supraspinatus tendon tear were included. They were classified into isolated or combined tear. The isokinetic muscle strength and muscle endurance were measured using the Biodex multi-joint system $PRO^{(R)}$ (Biodex Medical Systems, Shirley, NY, USA) in following movements: shoulder abduction, adduction, flexion, extension, external rotation, and internal rotation. Then, the difference in muscle function according to the type of tears were assessed. Fifty-seven patients had isolated supraspinatus tendon (mean age $56.9{\pm}7.3$ years). They were classified into either anteroposterior tear or modified mediolateral tear. The size were measured using T2-weighted magnetic resonance imaging scans in sagittal plane. Results: Between subjects categorized into the type of tear, we found significant inter-categorical differences in isokinetic muscle strength during abduction, adduction, flexion, extension, and internal rotation, and in muscle endurance during flexion, extension, and internal rotation. Anteroposterior diameter tear, we did not show significant differences in either isokinetic muscle strength or muscle endurance during any movements. However, with modified mediolateral diameter, we found significant differences with isokinetic muscle strength during adduction, and in muscle endurance the external rotation and internal rotation. Conclusions: We found that a supraspinatus tendon tear associated with more numbers of rotator cuff tears has lower isokinetic muscle strength and muscle endurance than a tear found alone.

핸드볼 선수들의 12주간 플라이휠 운동을 활용한 웨이트 트레이닝이 체력요인, 무산소성파워 및 등속성근기능에 미치는 영향 (The effect of 12-week Weight training with Flywheel Exercise on Physical Fitness Factors, Anaerobic Power, and Isokinetic Muscle Function in Professional Handball Players)

  • 본 연구에서는 남자 실업팀 핸드볼 선수들을 대상으로 플라이휠 운동을 적용한 웨이트 트레이닝 프로그램을 개발하여 상해예방과 경기력 향상을 위한 기초자료를 제공하는데 그 목적이 있다. 12주간 복합 플라이휠 운동 프로그램을 적용하여 신체조성, 체력요인, 무산소성파워, 등속성근기능에 미치는 영향을 분석한 결과는 다음과 같다. 팔굽혀펴기는 그룹 간 유의한 차이가 나타났다. 체지방률, 무산소성 파워는 시기별 상호작용에서 유의한 차이가 나타났다. 제자리멀리뛰기, 제자리높이뛰기, 악력, 배근력, 무산소성평균파워 그리고 등속성근기능검사는 좌굴근과 우굴근에서 시기별로 유의한 차이가 나타났다. 따라서 12주간 플라이휠 운동을 활용한 웨이트 트레이닝 결과는 상해예방과 경기력 향상을 위한 기초자료로 제공할 수 있을 것으로 판단된다.

키네시오 테이핑이 고령자의 슬관절 등속성 근 기능 및 통증에 미치는 영향 (Effect of Kinesio Taping on Isokinetic Muscular Function and Pain of Knee Joint in Elderly Women)

  • 본 연구는 테이핑 요법이 슬관절의 등속성 기능 및 통증에 미치는 영향을 알아보기 위하여 슬관절 부위에 통증을 호소하는 $65{\sim}70$세 여성 노인 12명을 대상으로 슬건근, 대퇴사두근, 슬개골 부위에 폭 5cm의 테이프를 8주 동안 주당 3회씩 총 24회를 적용시킨 후 등속성 근 기능인 최대근력, 상대근력, 근 지구력(총 운동량), 근 지구력(운동피로도), 동측 근력 비율, 주관적 통증정도인 시각적 상사 척도와 구술적 평정 척도의 변화를 분석한 결과 다음과 같은 결론을 얻었다. 즉, 8주간의 테이핑 요법 후 최대근력, 상대근력, 근지구력(총 운동량), 동측근력비율은 각각 유의하게 증가하였고 (p<.05; p<.01), 근지구력(운동피로도) 및 시각적 상사 척도와 구술적 평정 척도 점수가 각각 유의하게 감소하였다(p<.05).

등속성 운동과 복합 운동이 퇴행성 슬관절염 여성의 근기능, 균형능력, 기능적 능력 및 관절기능상태에 미치는 영향 (The Isokinetic exercise and effects of combined exercises on the change of postural control, functional status in patients with osteoarthritis of the knee)

  • Objective: The purpose of this study was to investigate the effects of combined exercise on the change of postural control, functional status in patients with osteoarthritis of the knee. Methods : The subjects were consisted of 24 women patients fifties and sixties with knee osteoarthritis. Each group had a exercise for 30 minutes per day and three times a week during 8 weeks period. Was used to measure muscular function(60, $180^{\circ}/sec$), postural control. functional capacity(15 M walking test, Rising form a chair test, stair climbing and descending test) and the functional state of a joint(WOMAC). Results: This study results in following 1. In case of muscular function have significantly increased in both groups. 2. There was no significant difference in the postural control of the bilateral between two groups. However, the postural control of a isokinetic exercise program group showed a significant difference in the bilateral side(overall, $M{\cdot}L$ stability index). Especially, the change of postural control in the isokinetic plus postural control exercise program group decreased more significantly in the bilateral(overall, $A{\cdot}P$ stability index). 3 It was significantly decreased in climbing and descending the stairs, and rising from the chair, although functional capacity decreased in lam walking in both groups. 4. The functional state of a joint(WOMAC) showed a significant decrease in both groups. Conclusion : In order to improve the functional status and postural control of patients with osteoarthritis of the knee, performing exercise programming of isokinetic plus balance will be more effective than the exercise program composed of only the isokinetic exercise program.

Difference in Muscle Activity of Deltoid Muscle according to Isokinetic Range of Motion Using Proprioceptive Neuromuscular Facilitation Pattern of Upper Extremity

  • Purpose: This study sought to compare the muscle activity of the deltoid muscle according to the range of motion during the proprioceptive neuromuscular facilitation (PNF) upper extremity D2 pattern exercise performed with an isokinetic exercise device. The aim was to provide basic data for selecting an exercise for the relevant segment of the range of motion to enhance function in clinical practice. Methods: In this study, the relevant measurements of the anterior and middle trapezius of 25 healthy adults were taken using a surface electromyography system. The upper extremity pattern exercise was performed in three ranges (0% to 50%, 50% to 100%, and 25% to 75%) using the upper extremity isokinetic device. Results: There was a statistically significant difference in the muscle activity ratios of the anterior and middle deltoid muscles according to the measurement conditions (p<0.05). There was a statistically significant difference in the activity ratio of the middle deltoid muscle according to the measurement conditions in the ratio from the start range to the end range (p<0.05). There was no significant difference in the muscle onset time difference according to the measurement conditions (p>0.05). Conclusion: The muscle activity of the anterior and middle deltoid muscles was analyzed according to the range of motion during the PNF upper extremity pattern exercise performed with an isokinetic device. The results could be used as a basis for selecting exercises for the relevant segment of the range of motion according to the function to be emphasized.

엘리트 남녀역도선수들의 등속성 허리 근기능 및 유연성과 요통과의 관계 (The Relationships between Isokinetic Muscular Function and Flexibility of the Lower Back Pain(LBP) in Elite Weight Lifter)

  • Purpose. This study of purpose was to compare and analyze the relationship among the isokinetic trunk muscular functions, flexibility and low back pain of elite weight lifter with regard of sex. Methods. we measured the level of low back pain, isokinetic muscular functions according to gender, then analyzed the relationship between isokinetic functions and the level of low back pain, between flexibility and the level of pain, between Athletic Career and the level. Results. In this study, the gender, the VAS point was $2.6{\pm}2.3$ and the VRS point was $2.3{\pm}1.3$ in males. The other side, in females the VAS points was $3.6{\pm}1.7$ and the VRS was $3.2{\pm}1.1$. There was significant negative correlation(r=-0.826) between the VAS point and the maximal flexion muscular strength per kilogram of $30^{\circ}/sec$ isokinetic exercise in female. also there was negative correlation between the muscular flexion strength per kilogram and the VRS point in female, but there was no significant relationship in male. Conclusions. In current study, these results suggested that the higher muscular flexion strength per kilogram is, the lower the level of low back pain is in female athletes. this is caused by the imbalance between Abdominal Muscles and Back Extensor in weight lifter. Therefore, there is the need to apply the program to improve the balance of trunk.

Effects of 8-week resistance exercise on gait-related isokinetic muscle function in males by age

  • The purpose of this study was to examine the effects of 8 weeks of resistance exercise on gait-related abilities and the differences in age groups to provide reference materials for application of training by age group. Subjects in their 30s, 40s, and 50s were recruited and subjected to a lower extremity exercise, in which the intensity gradually increased three times a week for eight weeks. The gait-related variables, torque at 0.18 sec, acceleration time, and torque at 30°, were assessed through isokinetic tests before and after the 8-week training. The torque at 0.18 sec showed a significant increase in the 50s group. There was no significant difference in acceleration time according to time and group. The torque at 30° showed a significant increase in the 30s group. In conclusion, 8 weeks resistance training can increase neuromuscular activity in adults in their 50s group, and changes in training variables are needed to increase torque at 30° that affects knee stabilization in adults over 40s group.

FES 치료 시 자극 주파수 차이가 퇴행성 슬관절염 환자의 대퇴근 기능에 미치는 영향 (Effects of Different Frequency on Muscle Function of the Thigh in Patients with Degenerative Knee Arthritis during the Functional Electrical Stimulation )

  • Objective: The purpose of this study was to investigate the effects of different frequency on muscle function of the thigh in patients with degenerative knee arthritis during the functional electrical stimulation (FES). Method: For this study, 16 male participants over 65 who patients with degenerative knee arthritis were recruited as research participants. In this research, isokinetic muscular function, EMG, and joint position sensation were performed after FES treatment was applied for three conditions (FES 20, FES 50, and Without FES). For each dependent variable, one-way ANOVA with repeated measures was to determine whether there were significant differences among three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed by using the contrast procedure. Results: When compared to FES 50 and without FES, FES 20 causes significant increase in isometric knee extension strength. No significant differences were found in EMG values across different EMS conditions. Conclusion: The present study examined isokinetic muscular function, EMG, and joint position sensation in order to investigate the effects of different frequency muscle function of knee extensors during the functional electrical stimulation. The results of this study showed that FES with 20 Hz frequency had positive effect on knee extensor. Based on the findings of the present study, FES with lower frequency may help the performer to focus on developing strength in knee extensor muscles.

운동형태에 따른 최대 운동이 심근 산소소비량과 혈중 젖산 농도의 변화에 미치는 영향 (The effects of different exercise type for maximal exercise on RPP and blood lactate)

  • The purpose of this study was to compare the effects of different exercise types including isokinetic, isometric, and istonic exercise of same exercise intensity on cardiopulmonary function and blood lactate level. The subjects of this study included 17 males college students. Each subjects after pretraining measurement performed isokinetic, isometric and isotonic exercise of 1 week interval using Cybex 6000 System. KBI-C and YSI 1500 were used to measure changes in cardiopulmonary function and blood lactate level respectively. First, comparing changes in RPP relative to exercise type, isokinetic group showed significant difference between before exercise ($7.08{\pm}1.12mg/kg/min$) and post exercise ($18.98{\pm}1.75mg/kg/min$). Isometric group showed that significant difference between before exercise($7.89{\pm}0.98mg/kg/min$) and post exercise($20.22{\pm}2.41mg/kg/min$). Isotonic group showed significant difference between before exercise ($8.14{\pm}0.11mg/kg/min$) and post exercise ($19.84{\pm}2.30\;mg/kg/min$). Second, comparing changes in blood lactate level relative to exercise type, isokineic group showed significant difference between before exercise ($2.99{\pm}0.65mmol$) and post exercise ($6.55{\pm}6.55mmol$). Isometric group showed significant difference between before exercise($1.71{\pm}5.48mmol$) and post exercise ($5.48{\pm}1.97mmol$). Isotonic group showed significant difference between before exercise($1.16{\pm}0.48mmol$) and post exercise($5.21{\pm}1.28mmol$). The results of this study indicate significant differences RPP in isometric exercise and significant differences blood lactate in isotonic exercise.
