조기유방암환자의 이차원치료계획과 삼차원치료계획의 방사선조사범위의 차이 (Difference in the Set-up Margin between 2D Conventional and 3D CT Based Planning in Patients with Early Breast Cancer)

  • 목 적: 유방암 치료를 위한 방사선치료계획에 전산화단층촬영을 이용한 모의치료가 널리 이용되고 있다. 이는 환자 개개인의 해부학적 특징에 기반하여 종양 표적과 정상 조직을 정의할 수 있다. 본 연구는 일반적으로 유방 크기가 작은 한국인 유방암 환자에서 적용하는 방사선치료계획 방법에 따라 치료용적 및 치료범위에 차이가 있는지를 알아보기 위해 시행하였다. 대상 및 방법: 2008년 11월부터 2009년 1월까지 조기유방암으로 유방보존술을 시행 받은 25명의 환자를 대상으로 통상적인 접면조사의 이차원치료 계획과 전산화단층촬영을 기반으로 한 삼차원치료계획을 시행하였다. 이차원 치료계획에서 방사선조사영역의 경계는 유방실질을 촉지하여 결정하였고 위쪽으로는 쇄골의 바로 아래, 내측은 몸의 정중선, 외측은 중심 액와선 그리고 아래쪽은 유방밑주름에서 2 cm 아래에 표시하였다. 삼차원치료계획에서 임상표적체적은 모든 유선 조직을 포함하였고 계획용 표적체적은 임상표적체적에서 피부 쪽을 제외한 모든 방향으로 1 cm의 여유를 두어 결정하였다. 이차원과 삼차원치료계획 간의 조사영역의 경계 차이 및 동등조사영역의 차이를 확인하고 신체질량지수, 폐경 여부 및 유방크기의 관련성을 평가하였다. 그리고 선량분포로 지정한 유방조사량과 삼차원 선량분포에 기반하여 측정된 조사된 폐용적과 심장용적을 평가하였다. 결 과: 2명의 환자를 제외하고 계획용표적체적(PTV)은 삼차원치료계획이 이차원치료계획보다 모든 방향에서 표시한 방사선조사영역을 감소시켰고 위쪽 경계에서 그 차이값이 가장 컸다. 동등조사영역크기는 한 명을 제외하고 삼차원치료계획에서 이차원치료계획에서보다 작았으며(평균 0.9 cm), 신체질량지수, 폐경 여부 및 유방크기(브래지어 사이즈)와의 관련성은 보이지 않았다. 조사된 폐용적은 삼차원치료계획에서 유의하게 감소하였고, 심장용적 또한 감소하였지만 통계적으로 유의한 차이는 보이지 않았다. 결 론: 통상적인 치료계획과 비교하여 전산화단층촬영을 기반으로 한 삼차원치료계획이 작은 유방을 가진 환자에서 방사선조사영역 및 손상위험장기의 조사량을 줄이면서 수술 후 변화를 포함하여 더 정확한 종양치료가 가능할 수 있음을 보여주었다. 그러나 앞으로 임상표적체적의 정의에 대한 추가적인 연구 및 일반적인 합의가 필요하다.

형질 전환 Aspergillus oryzae의 첨가가 산란계의 생산성, 계란 품질 및 장내 미생물 변화에 미치는 영향 (The Effects of the Transgenic Aspergillus oryzae Supplementation on Performance, Egg Quality and Intestinal Microflora of Layers)

  • 본 연구는 메주에서 순수 분리된 메주곰팡이의 대표적 균종인 황국균(Aspergillus oryzae; AO)으로 만든 AO culture와 Salmonella 병원특이 유전자를 삽입한 형질전환 AO(TAO) culture가 산란계의 생산성, 계란 품질 및 장내 미생물 균총에 미치는 영향을 규명하고자 실시하였다. 39주령 산란계 Hy-line Brown 840수를 공시하여 대조구, AO culture 0.2%와 0.5%, TAO culture 0.2%와 0.5%, UV를 조사하여 단백질 분해효소를 감소시킨 mutant에 Salmonella 병원 특이 유전자를 삽입한 형질전환 AO(TMAO) culture 0.2%와 0.5% 첨가구들을 비교하였다. 각 첨가구는 6반복, 반복당 20수씩, 한 케이지 당 2수씩 배치하여 8주간 사양시험을 실시하였다. 사양시험 결과 모든 산란 생산성 및 계란 품질 관련 조사항목에서 처리간에 유의한(P<0.05) 차이가 있었다. TAO culture 0.2% 첨가구가 산란 생산성에 있어서 유의적으로 가장 높았으며, 난중은 모든 AO 첨가구들이 대조구에 비해 유의적으로 낮거나 낮아지는 경향을 나타내었다. 연파란율은 TMAO culture 0.5% 첨가구가 가장 낮았다. 사료섭취량과 사료요구율은 대조구와 모든 AO 첨가구들간에 유의적 차이가 나타나지 않았다. 난각 강도는 대조구 보다 모든 AO 첨가구들에서 유의적으로 높게 나타났으며, 난황 색도는 TMAO culture 0.5% 첨가구에서 가장 높았다. 난각 색도와 Haugh unit은 대조구와 모든 AO 첨가구들간에 유의적 차이가 나타나지 않았다. 장내 미생물 균총(Salmo- nella spp., E. coli. Lactobacilli spp.)에서는 유의적(P<0.05) 차이가 있었다. AO culture 첨가에 의해서 Lactobacilli spp.의 수는 증가되고, E. coli 및 Salmonella spp.의 수는 감소되었다. 특히 TAO와 TMAO culture 첨가구에서는 AO culture 첨가구보다 Salmonella spp. 및 E. coli 억제효과가 컸으며 첨가수준(0.5% vs 0.2%) 간에는 유의한 차이가 없었다. 결론적으로 TAO culture 0.2% 첨가는 산란 생산성 증가에 효과가 있었으며 TAO 및 TMAO culture 0.2% 첨가는 장내 E. coli 및 Salmonella spp.의 감소에 유의한 효과가 있었다.

산란전자선을 이용한 강내측방조사기구의 제작과 특성 (Fabrication of Backscatter Electron Cones for Radiation Therapy)

  • 목적 : 강내에 발생된 종양치료용 원통형 전자선 조사기구(Electron cone)는 기하학적으로 강내벽에 위치한 종양치료에 부적당하므로 후방 또는 측면방향으로 산란되는 전자선을 이용하여 체강 내벽점막 등에 발생된 종양을 효과적으로 치료할 수 있는 산란전자선 치료방법을 개발하고자 한다. 강내조사기구내에 전자선 입사방향에 수직 또는 일정한 각도의 산란판을 배치하여 측면방향으로 산란전자선을 방출시키는 강내 측면조사기구를 제작하고 산란판의 제원과 전자선 에너지에 따라 산란방출된 산란선의 특성과 조직내 선량분포를 측정 평가하였다. 새상 미 방법 : 외부조사용 전자선조사기구(Electron cone) 대신에 강내 삽입용 전자산란선 조사통(Intracavitary backscatter electron cone)과 이를 콜리메이터와 연결시킬 수 있는 차폐연결기구(Shielded electron device)를 고안하였다. 산란전자선 조사기구는 직경이 $2\~3\;cm$이고 길이가 25 cm인 금속(내식강)원통을 이용하였으며 입구에서 20 cm위치에 산란판을 부착시키고 원통 측면에 직경 $1\~2\;cm$의 산란선 방출구를 제작하였다. 산란판은 $2\~10\;mm$의 연판을 사용하였으며, 오제전자와 특성 엑스선을 제거하기 위하여 주석, 구리, 알루미늄판 등을 부착시켰으며 종양위치를 관찰할 수 있도록 표면을 처리하였다. 고에너지 방사선치료용 선형가속기(Clinac 2100C/D)에서 발생된 $6\~12\;MeV$ 에너지의 전자선을 이용하였으며 선량측정은 평행평판형 전리상(Markus chamber, PTW 23343)을 조직등가 팬텀(Polystyrene)에 삽입하여 측정하였다. 전자산란선의 에너지분포는 Monte Carlo (EGS4) 계산으로 예측하였으며 조직내 선량분포는 필름 흑화도(X-Omat V, Wellhofer 700i)에 의하여 측정하였다. 결과 : 전자선 입사에너지가 6 MeV일 때 전자산란선의 평균 에너지는 약 1.5 MeV 이었으며 산란각이 클수록 에너지는 줄어들었다. 입사 전자선 에너지 6 MeV 에서 산란판의 각도 $30^{\circ},\;45^{\circ}$ 에 따른 최대선량지점은 산란선 방출구의 중심에서 각각 5 mm 및 -10 mm지점의 표면에서 발생되며 입사전자선에 대한 전자산란선의 선량비는 약 $8.5\%$ 내외로 측정되었다. 입사전자선에너지 6 MeV에서 산란판각도 $45^{\circ},\;60^{\circ}$에 의한 $50\%$의 심부선량분포는 각각 6 mm와 7 mm 깊이에 도달하였으며 입사에너지 증가에 비례하였다. 결론 : 전자선 후방산란의 특성을 연구하고 이를 인체 강내 측방 점막부위에 발생한 종양을 효과적으로 치료할 수 있는 강내 전자산란선 조사통을 고안 제작 하였다. 시험용으로 제작한 전자산란선 조사기구를 이용하여 전자선 에너지와 산란판의 각도에 따른 산란선의 선량비율과 심부율을 측정하였다. 구강, 자궁, 직장 등 강내측벽 점막 등에 발생된 악성종양의 모양과 깊이에 가장 적당한 입사 에너지, 산란판의 각도, 산란창구 및 조사각도를 선택함으로서 방사선치료방법을 향상시킬 수 있을 것이라고 기대된다.

부인과암에서 조직내 삽입 방사선치료 - Applicator의 고안 및 제작- (Interstitial Vaginal Needle Implantation in Gynecological Tumors : Design and Construction of Applicator)

  • 목적 : 원발성 혹은 재발한 부인과암(자궁경부암, 질암, 및 외음부암)에서 질부위의 병소에 추가선량를 조사하기 위한 기존의 방법들 즉, 실린더(cylinder)를 이용한 강내치료와 바늘(needle)을 사용한 조직내 평면삽입(Single plane Implant)만으로는 이상적인 선량분포를 얻기 힘든 경우가 많다. 저자들은 방광 및 직장에 대한 조사선량을 최소화하면서 질내의 병소에 국한하여 고선량의 방사선을 효과적으로 조사하기 위해 체적삽입(Volume Implant)이 가능한 기구(applicator 혹은 template)를 고안, 자체 제작하여 사용하고 있으며 이를 소개하고자 한다. 대상 및 방법 : 1994년 8월부터 1998년 2월까지 9명의 환자를 대상으로 아주대학교병원에서 조직내 방사선치료를 시행하였다 이들은 질벽에 재발한 자궁경부암(5명), 원발성 자궁경부암(3명), 원발성 질암(1명)으로 치료받았다. 첫 3명의 환자는 기구없이 바늘을 평면삽입하였고 이후에는 병변의 위치 및 크기 등을 고려할 때 주위정상조직에 대한 선량을 줄이면서 종양에는 균등한 선량 분포로 고선량의 방사선을 조사하기 위해서는 체적삽입이 유리할 것으로 판단되어 기구를 고안, 자체 제작하였다. 바늘은 일반적인 조직내 삽입용(Microselectron, Nucletron)을 사용하였고 이는 저선량율(LDR) 및 고선량율(HDR) 근접치료 모두에서 사용 가능하였다. 6명중 3명은 50cc 주사기를 이용하여 만든 초기 단계의 기구를 사용하였다(1차 형태). 이러한 초기 단계의 기구는 표면이 매끈하지 못하여 질점막을 손상시킬 수 있고 바늘 삽입후 기구가 움직이지 않도록 회음부에 고정시키는 날개부위가 없는 단점이 있었다. 나머지 3명의 환자는 이러한 단점을 보완하고 종양의 크기 및 위치에 따라 좀 더 많은 바늘을 삽입할 수 있도록 새롭게 고안한 기구를 사용하였다 (2차 형태). 재료는 투명 아크릴 혹은 치과용 몰드(mold) 소재($Provil^{(R)}$)를 사용하였으며 회음부에 기구를 고정시키기 위한 날개를 만들었다. 2차 형태의 기구를 사용한 환자중 마지막 1명은 종양의 위치를 고려하여 삽입할 수 있는 바늘의 갯수를 증가시켜서 새로이 제작하였다 실제로 환자에게 시술하기 전에 예비치료계획을 시행하여 필요한 동위원소의 수량 및 배열을 결정하였다. 결과 : 자체 제작한 기구를 사용함으로써 바늘을 일정간격으로 삽입하는 것이 용이해졌으며 시술시간을 단축시킬 수 있고 삽입방향이 잘못되어 여러번 바늘을 찌르는 경우가 적어져 조직에 대한 손상을 줄일 수 있었다. 손쉽게 구할 수 있는 재료를 사용하여 병소에 따라 개별화간 가능한 값싸고 유용한 기구의 제작이 가능함을 확인하였으며 이를 통해 좀 더 이상적인 선량분포곡선을 얻을 수 있었다. 3명의 환자에서 바늘 삽입후 컴퓨터 단층촬영을 시행하여 배열의 정확성과 방광 및 직장에 조사되는 선량을 감소시킬 수 있음을 확인하였다. 마지막 추적관찰시 원발성 종양으로 치료받았던 3명의 환자 모두가 국소제어되었으나 재발하여 치료하였던 경우에는 모두 국소재발하였다. 결론 : 저자들은 부인과암에서 근접치료시 환자의 종양 특성에 따라 기구를 적절하게 고안하여 적은 비용으로 비교적 손쉽게 자체 제작하여 사용함으로써 효과적인 조직내 삽입 방사선치료를 시행할 수 있었다.

Sensory Information Processing

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The authors suggests that this system based on down-sized computer could provide a progressive approach to total LIS based on local area network, and the implemented system could serve as a model for other hospital's LIS for routine hematology. 30609 T00401030609 ^x To develop an artificial bone substitute that is gradually degraded and replaced by the regenerated natural bone, the authors designed a composite that is consisted of calcium phosphate and collagen. To use as the structural matrix of the composite, collagen was purified from human umbilical cord. The obtained collagen was treated by pepsin to remove telopeptides, and finally, the immune-free atelocollagen was produced: The cross linked atelocollagen was highly resistant to the collagenase induced collagenolysis.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
